1.A Case of Successful Transabdominal Cervicoisthimic Cerclage in a Patient with Incompetent Internal as of Cervix.

Transvaginal cervical cerclage for the prevention of pregnancy loss in patients with cervical incompetence is well accepted. However, there is a small subject of patients with incompetence of cervix, congenital or surgical shortening of the cervix, previously failed transvaginal cerclage and deep cervical damage or tears in the fornices. We experienced a case of the transabdominal cervical cerclage. 'Ihe patient had a past history of deep cervical laceration due to vaginal delivery. Therefore, we present here a surgical technique of transabdominal cerdage and report it with brief review of literatures.

Uterine torsion is defined as the mtation of more than 45 degrees around the long axis of the uterus or a rotation which is severe enough to produce symptoms. Torsion of the human pregnant uterus is a very rare complication. The diagnosis is problematic and is often made during a cesrean section due to inhibited labor. Torsion of the human pregnant uterus can be caused by myoma or other uterine abnormalities, ovarian cysts, adhesions, abnormal fetal presentations, or fetal abnormalities ( though no cause can be found in about 20% of cases ). Uterine torsion produces symptoms of varying severity depending on the degree of rotation, and duration of the pregnancy. High mortality rates for both mother and infant have been reported. We experienced one case of the torsion of term pregnant uterus and report this case with a brief review of the concerned literature.

3.The Use of Cultured Allogenic Keratinocyte Grafting in a Patient with Epidermolysis Bullosa Simplex.

Epidermolysis bullosa (EB) is a rare genetic disease that is known for continuous skin blistering caused by minor trauma. The skin blisters and bullae that develop often cause skin defects. There is no definitive treatment for EB, only symptomatic relief. We report our experience with cultured allogenic keratinocyte grafting in a newborn patient with EB simplex who had unhealed raw surfaces and was not a skin grafting candidate. The skin lesions of the patient were covered with cultured allogenic keratinocyte grafts and re-epithelialized quickly with no scarring. Allogenic keratinocyte grafting reduced pain and produced noticeable improvements in the unhealed wounds. We think that allogenic keratinocyte grafting can play an important role in the management of patients with EB simplex.

4.Meningioma With Partial and Spontaneous Regression of Peritumoral Edema on Long-Term Follow Up

Spontaneous regression of meningioma is rarely observed. We report a one person of an incidentally diagnosed meningioma with a spontaneous regression. The 73-year-old female patient without symptoms showed the right sphenoid meningioma with peritumoral edema. The meningioma was incidentally diagnosed and followed up by MRI for 10 years. The tumor shrank with a decrease of edema on T2 MRI. The initial volume of 58.59 cm3 , regressed to 37.16 cm3 .

BACKGROUND: Myroides species are widely distributed in nature, but clinical infection by these organisms are extremely rare. We report herein prolonged outbreak of urinary tract infection by Myroides species. METHODS: Forty-four Myroides spp. were isolated from urine samples from 25 patients over a period of nine months, and random amplified polymorphic DNA (RAPD) method was performed to characterize the genotype of these isolates. RESULTS: All of the subjects were hospitalized patients with indwelling urinary catheter. Five of the patients showed concomitant pyuria, which could be considered as evidence of urinary tract infection, and isolation of these organisms in the remainder of the patients could be considered as simple colonization. All the isolates were resistant to antimicrobial agents tested. RAPD analysis showed identical DNA fingerprinting patterns in all the isolates. CONCLUSION: This study revealed that all the Myroides spp. isolated from urinary specimens of prolonged outbreak were genotypically the same. Because of its resistance to multiple antimicrobial agents, prevention of dissemination of this strain is clinically important.

7.Regression of asymptomatic intracranial arterial stenosis by aggressive medical management with a lipid-lowering agent

OBJECTIVE: The incidence rate of stroke as a result of intracranial arterial stenosis (ICAS) is higher in Asian countries than in the West. We aimed to analyze the regression, lack of change, or progression of asymptomatic ICAS after the administration of rosuvastatin and associated factors.METHODS: The patients who had undergone computed tomography angiography (CTA) at our hospital and had been diagnosed with ICAS with no ischemic event in the stenosed vascular territory were included in the study. They were administered 20mg of rosuvastatin per day. After a follow-up period of at least 6 months after treatment, the patients were examined using CTA again and the clinical information and imaging results were analyzed.RESULTS: In total, 48 patients were diagnosed with asymptomatic ICAS. During the final follow-up examination, it was found that the stenotic lesion regressed in 30 patients, whereas it remained unchanged or progressed without any adverse effects in 18 patients. In univariate analysis, the regressed group showed significantly higher differences in the levels of total cholesterol and low-density lipoprotein (LDL) between their initial and final values (both, p=0.031 for both). In the multivariate analysis, a significantly higher difference in the levels of LDL between its initial and final measurement was seen in the regressed group (p=0.035, odds ratio(OR) 3.9).CONCLUSIONS: Rosuvastatin was found to have better lipid-lowering effects for total cholesterol and particularly LDL in patients whose ICAS had regressed. We concluded that rosuvastatin administration can be recommended for the treatment of patients with asymptomatic ICAS.

8.Portal Vein Embolization with a New Embolic Material: Preliminary Study in Pigs.

PURPOSE: To evaluate the usefulness of new liquid embolic materials for portal vein embolization (PVE). MATERIALS AND METHODS: The liquid embolic material, Embol, was obtained by hydrolytic reaction of polyvinyl acetate (PVAc). Several Embols with a different hydrolysis ratio and polymer concentration were prepared and named, an example was Embol-78-12. These were percutaneously injected through a 5-Fr catheter into the left portal vein of six pigs, and radio-opacity under X-ray fluoroscopy and the possibility of reflux were evaluated. RESULTS: The radio-opacity of Embol-78-8, Embol-73-10, and Embol-73-12 was too low to recognize reflux, but was excellent when Embol-78-12 or (Embol-78-12 + Lipiodol) was used; it was still difficult to prevent reflux, however. With the use of a balloon catheter, however, complete embolization became possible. CONCLUSION: Embol-78-12 injection through a balloon catheter seems to be useful for clinical PVE.

9.Anterior Communicating Artery Aneurysms Associated with Multiplication of Anterior Communicating Artery.

The formation of intracranial arterial aneurysm is associated with vascular anomaly and accompanied hemodynamic abnormality. The authors report three cases of anterior communicating artery aneurysms associated with multiplicated anterior communicating artery, two duplicated, and one triplicated. It is not certain that multiplication of anterior communicating artery is responsible for development of anterior communicating artery aneurysm, but it would be interesting to remind the two factors affecting the formatin of arterial aneurysm, vascular anomaly and hemodynamic abnormality. Careful dissection is mandatory to avoid undue traction of aneurysm, to preserve small out important vessels, and to increase the room in which to operate within the subarachnoid space, espically if the anterior communicating artery is multipliated.

10.Incidence rate of Aneurysmal SAH in Gwangju City and Jeollanamdo Province in 2007.

OBJECTIVE: The incidence of subarachnoid hemorrhage (SAH) worldwide varies considerably. In spite of many reports about the incidence of SAH, there has been no report about the incidence of SAH on the basis of the Korean population. The purpose of this hospital-based study was to assess the actual incidence rates of aneurysmal SAH in Gwangju city and Jeollanamdo province. METHODS: All cases of SAH confirmed by computerized tomography (CT) between January 2007 and December 2007 were selected for analysis. For the data collection, three major training hospital and ten general hospitals working the CT in Gwangju city and four major general hospitals in Jeollanamdo province participate in this study. RESULTS: According to the official census of Korea, the population was 1,413,444 in Gwangju city and 1,929,836 in Jeollanamdo province in 2007. There were 163 patients in Gwangju city and 266 patients in Jeollanamdo province confirmed SAH by CT in 2007. The crude and the age- and sex-adjusted annual incidence rates per 100,000 population for all ages in Gwangju city were 11.5 and 12.4 for aneurysmal SAH and in Jeollanamdo province were 13.8 and 10.8. The incidence was higher in women and increased with age. The gender distribution varied with age. At young ages, the incidence was higher in men while after the age of 40 years, the incidence was higher in women. CONCLUSION: In the present study, the age- and sex-adjusted annual incidence rates is 11.8 in Gwangju city and Jeollanamdo province. The incidence was higher in women and increased with age.
