1.Construction of Eukaryotic Expression Vector Containing Human Vascular Endothelial Growth Factor 165 Gene

Objective To clone human vascular endothelial growth factor 165(hVEGF_ 165 ),construct its eukaryotic expression and to study the expression of hVEGF_ 165 . Methods Human vascular endothelial growth factor cDNA was amplified by PCR method from the HL60 cells and cloned to expression vector pcDNA_3,constructed pCD-hVGEF_ 165 recombinant plasmid, then transformed to E.coli BL21(DE3)cell.Results The cloned cDNA was confirmed to be VGEF_ 165 cDNA. It was observed that the expression of human VEGF gene was detected distinctly 72h after transferring.Conclusions We successfully cloned and expressed hGEF_ 165 gene, which provided the further foundation of the model of VEGF transgenic animal and makes a basis for the further study in retinal neovascularization.
2.Optimization of Extraction Technology of Panax Notoginseng and Scrophulariae Radix from Rupixiao Granule by Orthogonal Design

Objective: To optimize the extraction technology of Panax notoginseng and Scrophulariae radix from Rupixiao granule.Methods: With the dry extract rate and transfer rates of ginsenoside Rg1, ginsenoside Rb1 and harpagoside as the comprehensive index, the orthogonal design was adopted to investigate the effects of the amount and concentration of ethanol, extracting duration and times on the extraction technology.The contents of ginsenoside Rg1, ginsenoside Rb1 and harpagoside were determined by HPLC.Results: The optimal extraction technology was extracted twice with 8-fold amount of 60% ethanol with 2 h per time.The transfer rate of ginsenoside Rg1, ginsenoside Rb1 and harpagoside was (79.4%±1.56%), (42.62%±0.68%) and (44.89%±0.58%)(n=3), respectively.The dry extract rate was (20.99%±0.411%).Conclusion: The optimized extraction technology is stable and feasible, which can be used for extracting Panax notoginseng and Scrophulariae radix from Rupixiao granule.
3.Alterations of the proteasome in the CA1 neurons after transient ischemia

Objective To investigate the alteration of chaperonine hsp40 and its influence on delayed death of neurons in the CA1 region of hippocampus after transient cerebral ischemia. Method After transient global ischemia for 20 minutes, rat model was made. Following different lengths of reperfusion, all the 28 wistar rats were divided into sham-operation group,4 hour recovery group, 24 hour recovery group and 72 hour recovery group ( n = 7 rats in each group). Immunochemistry and laser scanning confocal microscopy were used to observe the distributional alteration of hsp40 in the neurons. Differential centrifuge and western blot assay were used to analyze the quantitative alteration of hsp40 and its redistribution in the neurons. Results Immunochemistry and laser scanning confocal microscopy showed the progressive reduction of hsp40 occurred at first in the cytosol, then in the nucleus until the death of all the neurons in the CA1 region died. Differential centrifuge and western blot assay showed the level of hsp40 decreased from 1.00 ± 0.21 to 0.23 ± 0.13 ( P < 0.01 ) 24 hours after reperfusion; the quantity of hsp40 in the protein aggregates increased from 1.00±0.18 to 8.61 ± 1.89 (P <0.01 =24 h after reperfusion.Conclusions The reduction of hsp40 in the neurons of hippocampus CA1 region is an important role in protein aggregates formation.
4.Comprehensive treatment of stage-lllb testicular non-seminomatous germ cell tumor: a case report and review of the literature.

OBJECTIVETo investigate the comprehensive treatment of stage-IIIb testicular non-seminomatous germ cell tumor (NSGCT) based on the chemotherapy with cisplatin, etoposide and bleomycin (BEP) and nerve-sparing laparoscopic retroperitoneal lymph node dissection (nsLRPLND).
METHODSWe reported a case of stage-IIIb testicular NSGCT, analyzed the clinical data and treatment methods and reviewed the relevant literature.
RESULTSThe patient underwent chemotherapy with etoposide (0. 18 g/d for the first 3 days), cisplatin (30 mg/d for the first 5 days), and bleomycin (30 mg/d on day 2, 9 and 16) for 3 cycles, followed by nsLRPLND. Both chemotherapy and surgery were successfully performed. The operation time was 175 min, with intraoperative blood loss of 50 ml, but no severe perioperative complications. No recurrence and distant metastasis were found during the 6-month follow-up after surgery.
CONCLUSIONThe comprehensive treatment based on BEP chemotherapy and nsRPLND can be used as an option for stage-IIIb testicular NSGCT.

5.Application and Research of Clinical Laboratory Information Management System

Objective The application of laboratory information system (LIS) increases extensively. The objective is to expatiate how to widen the application of LIS so that it can be seamlessly combined with HIS, and play a greater role in quality control of clinical laboratory. Methods A consummate and compact working process was designed. A bidirectional communication from equipment to client was established. Module modes and prefect security systems were set up. Results With the existing HIS network environment, corresponding bar codes were generated through the examination applications from the doctor's workstation. At the same time, patients' basic information and test information were generated based on the bar code. In this way, the seamless combination of LIS and HIS were achieved. The switch of the functional module was simple, efficient and easy to maintain. The internet version Kaspersky antivirus software was used in the area of security. Database were backed up automatically and regularly. Authority was strictly defined for operators in different level. Conclusion Through the current application in our section, LIS has already been an essential part in the process to become a modern hospital.
6.Clinical application of percutaneous cholecystostomy in the treatment of high-risk patients with acute cholecystitis

Objective To discuss the therapeutic strategy and the clinical efficacy of percutaneous cholecystostomy in treating high-risk patients with acute cholecystitis. Methods During the period of Jan. 2006-June 2008,percutaneous cholecystostomy was performed in 27 high-risk patients with acute cholecystitis,consisting of lithic cholecystitis (n = 21) and non-lithic cholecystitis (n = 6). Of 27 patients,percutaneous cholecystostomy via transhepatic approach was performed in 22 and via transperitoneal approach in 5. The 7 F drainage catheter was used. Cholecystography was conducted before the drainage catheter was extracted. Results Percutaneous cholecystostomy was successfully accomplished in all 27 cases,with a technical success rate of 100%. Postoperative patency of gallbladder drainage was obtained in 25 patients,with the relieving or subsiding of abdominal pain and the restoring of temperature and leukocyte account to normal range within 72 hours. In one patient,as the abdominal pain relief was not obvious 72 hours after the procedure,cholecystography was employed and it revealed the obstruction of the drainage catheter. After reopening of the drainage catheter,the abdominal pain was relieved. In another case,cholecystography was carried out because the abdominal pain became worse after the procedure,and minor bile leak was demonstrated. After powerful anti-infective and symptomatic medication,the abdominal pain was alleviated. The drainage catheter was extracted in 25 patients 6-7 weeks after the treatment. Of these 25 patients,12 accepted selective cholecystectomy,7 received percutaneous cholecystolithotomy and 6 with non-lithic cholecystitis did not get any additional surgery. The remaining two patients were living with long-term retention of the indwelling drainage-catheter. Conclusion Percutaneous cholecystostomy is a simple,safe and effective treatment for acute cholecystitis in high-risk patients. This technique is of great value in clinical practice.
7.Cardiopulmonary Related Index Change of Diffusing the Lung and Promoting Urination on Cor Pulmonale Rats

This study was aimed to diffuse the lung and promote urination for the observation of cardiopulmonary re-lated index change of cor pulmonale rats to further explore the treatment effect on the pulmonary function, pulmonary arterial hypertension (PAH) and the influence of right heart hypertrophy of rats, in order to further illustrate the ef-fect of diffusing the lung and promoting urination for cor pulmonale. Sixty Wistar rats were randomly divided into the control group, model group and Xiao-Qing-Long decoction (XQLD) group with 20 rats in each group. The AniRes2003 animal lung function analysis system was applied to measure the pulmonary function of rats. And the multi-guide physiological recorder was used in the recording of the pulmonary artery pressure of rats. The conven-tional weighing method was applied to calculate and obtain the change of right heart hypertrophy. The results showed that compared to the control group, symptoms in the model group became severe obviously, which include reduced activity, slow movement and occasional airway sputum sound, and the right heart hypertrophy index of the model group increased obviously (P < 0.01). Compared to the model group, the pulmonary function and pulmonary artery pressure of the XQLD group have obvious difference (P< 0.05). It was concluded that to diffuse the lung and pro-mote urination can effectively improve the pulmonary function, PAH and the right heart hypertrophy index of rats with cor pulmonale (fluid retention). The effect of this method is definite in the treatment of cor pulmonale.

9.Low-dose tadalafil combined with Shuganyiyang capsules for mild-to-moderate erectile dysfunction.

OBJECTIVETo observe the clinical effect of low-dose once-daily tadalafil combined with Shuganyiyang Capsules in the treatment of mild-to-moderate erectile dysfunction (ED).
METHODSNinety patients with mild-to-moderate ED were equally randomized to groups A, B and C to receive Shuganyiyang Capsules, tadalafil, and tadalafil + Shuganyiyang Capsules, respectively. The scores of the patients on IIEF-5 and SF-PAIRS (15-Item Short Form of Psychological Interpersonal Relationship Scales) were recorded before and at 1 and 3 months after treatment.
RESULTSThe IIEF-5 scores of groups A, B and C were 10.13 +/- 1.55, 11.00 + 1.60 and 10.73 +/- 1.91 before treatment, and 13.77 +/- 2.11, 17.77 +/- 2.13 and 17.17 +/- 3.84 at 1 month after treatment, significantly higher in B and C than in A (P <0. 001) , but with no remarkable difference between B and C (P =0. 411). At 3 months after treatment, the IIEF-5 scores were 15.77 +/- 2.05, 18.07 +/- 2.24 and 19.37 +/- 3.76 in the three groups, dramatically higher in B and C than in A (P <0.001) as well as in C than in B (P<0.05). The scores on sexual self-confidence, sexual spontaneity and time concerns in SF-PAIRS were 3.90 +/-0.80, 8.67 +/- 1.94 and 14.43 +/- 1.92 before medication, 5.83 +/- 1.02, 9.90 +/- 1.75 and 11.17 +/- 1.68 at 1 month and 6.73 +/- 0.98, 11.07 +/- 2.08 and 10.67 +/-1.60 at 3 months after medication in group A; 4.17 +/- 0.87, 9.37 +/-1.43 and 14.47 +/-1.57 before medication, 6.47 +/-0.78, 10.83 +/- 2.18 and 10.20 +/-1.56 at 1 month and 6.83 +/-0.91, 11.30 +/- 1.88 and 9.47 +/- 1.57 at 3 months in group B; and 4.23 +/-0. 94, 9.50 +/- 1.89 and 14.67 +/- 2.91 before medication, 8.03 +/- 1.67, 13.43 +/-1.10 and 9.70 +/-1.21 at 1 month and 8.93 +/- 1.78, 14.70 +/- 1.26 and 8. 87 +/- 0. 97 at 3 months in group C. Compared with the baseline, the SF-PAIRS scores of the three groups were all significantly improved after treatment (P <0. 05) , and markedly higher in C than in the other two groups (P <0.05).
CONCLUSIONLow-dose once-daily tadalafil combined with Shuganyiyang Capsules is obviously effective in the treatment of mild-to-moderate ED, which not only improves the patients'erectile function, sexual self-confidence and sexual spontaneity, but also reduces their time concerns.

10.Effects of various compounds on efficacy of artemisinin in a yeast model.

Artemisinin is a key anti-malarial drug and few clinically meaningful resistant cases about its application have so far been reported. The World Health Organization (WHO) officially recommended the use of ACT (Artemisinin-based Combination Therapy) as the first line antimalarial application to increase its inhibitory efficacy and prevent the likely resistance development. Based on our current understanding of artemisinin, a set of compounds were selected to study their interaction with artemisinin by using the yeast (S. cerevisiae) model, in the hope that knowledge gained might provide some references for clinical investigations. In this research, yeast strain (BY4742) was cultured in the nonfermentable YPGE solid medium with 4 μmol · L(-1) artemisinin and one of the selected compounds for 48 hours, and the combined drug efficiency was evaluated by the inhibition of yeast growth. The compounds belong to the categories of oxygenants, antioxidants, metal ions, ion chelators and uncouplers. Among them, 0.2 mmol L(-1) FeCl3, 60 μmol · L(-1) BPS, 1 mmol · L(-1) CuCl2, 0.75 mmol · L(-1) VE and 1 mmol · L(-1) VC antagonized the action of artemisinin, while 40 μmol · L(-1) DNP, 0.1 μmol · L(-1) CCCP and 0.25 mmol · L(-1) H2O2 had synergistic effects. These results suggested that redox-active and mitochondria-dysfunctional compounds could affect artemisinin's potency, supporting our prior proposed mitochondrial model for artemisinin's action. This research in addition provided a convenient method to screen likely artemisinin-interacting compounds.
