1.Endovascular stent-assisted angioplasty for basilar artery stenosis

ObjectiveTo probe the short-term efficacy of endovascu lar stent-as sisted angioplasty for basilar artery stenosis.MethodsTwenty p atients with s ymptomatic basilar artery stenosis were treated by angioplasty using a balloon-e xpandable coronary stent.ResultsThe basilar artery caliber r eturned to norma l size in 12 cases and restored by more than 80% in the remaining 8 cases. No tr ansient ischemic attack (TIA), stroke, or other peri-operative complications rec urred. Follow-up angiography in 13 patients revealed no re-stenosis.Co nclusionsShort-term efficacy of endovascular stenting for basilar arte ry stenosis is promising.
2.Clinical observation on plate on the metatarsal side to reconstruction of tarsometatarsal joint dislocations secondary to diabetic charcot foot.

OBJECTIVETo evaluate clinical results of plate on the metatarsal side to reconstruction of tarsometatarsal joint dislocations secondary to diabetic charcot foot.
METHODSSeven patients (9 feet) patients with tarsometatarsal joint dislocations secondary to diabetic charcot foot were treated with plating from April 2012 to December 2014. All patients were male, and 5 cases were on the unilateral side and 2 cases were on the bilateral sides. The age of patients ranged from 45 to 52 with an average of 48 years old. The history of diabetics was from 6 to 15 years. X-ray and CT were used to evaluate fractures healing,and AOFAS was applied to estimate recovery of joint function.
RESULTSAll patients were followed up from 12 to 24 months with an average of 19 months. All incisions were healed at stage I . No infection, loosening and breakage of internal fixation, bone nonunion were found after operation. According to postoperative X-ray and CT results, the time of fracture healing ranged from 10 to 20 weeks with an average of 16 weeks. Six feet got excellent results, 2 good and 1 moderate based on AOFAS scoring.
CONCLUSIONTarsometatarsal joint dislocations secondary to diabetic charcot foot treated with plate on the metatarsal side could obtain stable fixation and got satisfied early clinical results.

3.Construction of modified rat orthotopic models of renal transplantation

BACKGROUND: Rat model of renal transplantation is the best way for studying human rejection following renal transplantation. However, the utilization of this model is limited due to refined surgical technique.OBJECTIVE: To explore a microsurgery operation method for constructing a stable, reliable orthotopic renal transplantation model.DESIGN, TIME AND SETTING: The animal observation experiment was performed at the animal laboratory of Tianjin Union Medicine Centre from September to November 2008.MATERIALS: A total of 40 SD rats, weighing 250-300 g, were medially adopted as donators and recipients. METHODS: The renal vessels were separated by blunt separation. The kidneys of donators were infused, removed and conserved at low temperature. To the recipients, the left kidney was removed, and renal blood vessel was anastomosed by ext-ext anastomosis and reconstructed urinary passage by implanting ureter into bladder. 6-point acupuncture was used to suture wound. MAIN OUTCOME MEASURES: The rationality and the authenticity of this operation were evaluated through analyzing the operative time, postoperative death rate and the cause of death.RESULTS: The total time of operation was (150±17) min, with warm ischemia time of (15±2) s and (55±5) min cold ischemia time. Three rats died at the first week after operation, which caused by anesthetic accident, stomatorrhagia and ureterostenoma. The achievement ratio of operation was 85%. At day 8 after operation, exploratory laparotomy of survived rats showed that the transplanted kidney was recovered well without embolism or stenosis at the stoma.CONCLUSION: An orthotopic renal transplantation model of rat is constructed by ext-ext anastomosis and reconstructed urinary passage by implanting ureter into bladder.
4.Effects of Perinatal Bisphenol A Exposure on the Development of Brain of the F1 Generation Male SD Rats

Objective To investigate the effect of perinatal bisphenol A BPA exposure on brain development of F1 male offspring. Methods Pregnant SD rats were given BPA at 2 20 and 100 mg/kg body weight per day respectively from eleventh day of gestation to the whole lactation by gavage until their pups were weaned on postnatal day 21 the control group had no BPA exposure. Every six F1 male pups from each of the four groups were killed at differential time points on postnatal day 1510152130 and 45 respectively. Histopathological examination by HE stain was done on the brains. Results The results showed no abnormal change was found on postnatal day 1-10. Three dosage groups showed abnormal change of different degree on 15th 21th 30th postnatal day the mainly abnormal change was karyopyknosis of pyramidal cell in CA3 of hippocampus and cortical neuron in cerebral cortex. The cell numbers of pyramidal cell in CA3 of hippocampus and cerebral cortex were decreased on 45th postnatal day. Conclusion Perinatal BPA exposure may have an adverse effect on the brain developmnent of F1 male offspring.
5.Diagnosis and surgery for intramuscular hemangioma of skeletal muscle.

OBJECTIVETo explore effects of color Doppler ultrasound and magnetic resonance imaging (MRI) in diagnosis intramuscular hemangioma of skeletal muscle.
METHODSFrom December 2000 to January 2013, 54 patients treated by operation confirmed as intramuscular hemangioma of skeletal muscle by pathology postoperatively, there were 19 males and 35 females aged from 11 to 59 years old (averaged 33.6); the courses of disease ranged from 2.5 to 15 years with an average of 5.2 years. Thirty-eight patients were checked by color Doppler ultrasound, and 14 patients were inspected by MRI. All patients were treated by operation. Postoperative operative time, blood loss in operation, and complications and pathology postoperatively were observed, and IMH clinical effective evaluating standard were used to evaluate clinical outcomes.
RESULTSForty-three patients were followed up from 7 to 49 months with an average of 28.4 months. Operative time was (53 to 187) min with average of 76.3 min, blood loss was (70 to 350) ml with an average of 223.6 ml. No infections and death occurred. Thirty-five patients were diagnosed by color Doppler ultrasound and 13 patients were confirmed by MRI. Twenty patients were capillary type, 22 patients were spongy vascular type and 12 patients were mixed type according to Brown pathological type. In accordance with IMH clinical effective evaluating standard, 29 cases obtained excellent results, 8 moderate and 4 dissatisfaction and 2 poor.
CONCLUSIONColor doppler ultrasound and MRI get a high rate diagnosing patients with intramuscular hemangioma and have an significant valuable in clinical application, and surgical operation which has advantages of relieve symptoms obviously, improve life quality and reduce recurrence rate, could receive good curative effect.

6.Transfection of human umbilical vein endothelial cells with lentivirus containing enhanced green fluorescent protein

BACKGROUND:Human umbilical vein endothelial cells transfected with lentivirus containing enhanced green fluorescent protein can be easily traced. The optimal multiplicity of infection and time for producing strong fluorescence intensity can lay the foundation of tracing human umbilical vein endothelial cells in animal models.
OBJECTIVE:To observe expression of lentivirus containing enhanced green fluorescent protein in human umbilical vein endothelial cells, and thereby to find a stable method to label human umbilical vein endothelial cells.
METHODS:Using 0.1%col agenase perfusion digestion, we isolated human umbilical vein endothelial cells, which then were placed into a culture medium containing 20%fetal bovine serum and endothelial cellgrowth factor and observed under an inverted microscope. Fol owing digestion, centrifugation and suspension, the cells were counted and divided into four groups, 5.0×105 cells in each group. After cells were seededonto 24-wel plates, 10μL serum-free Dulbecco’s modified Eagle’s medium was added into the blank group, and lentiviruses containing enhanced green fluorescent protein were added into another three groups for celltransfection respectively at multiplicities of infection of 2, 3, 4. There were three dishes in each group.
RESULTS AND CONCLUSION:After cultured for 5-7 days, isolated cells grew into a single layer and exhibited a cobblestone-like arrangement under a light microscope. In addition, factor VIII related antigen test was positive. A green fluorescence was visible at 24 hours of transfection, and peaked at 72 hours. Transfection efficiency was in a linear growth with the multiplicity of infection. Up to the 21st day of transfection, the green fluorescence was stil visible. After 0, 7, 14, 21 days of transfection, the number of human umbilical vein endothelial cells showed no difference between the transfection group with the multiplicity of infection=3 and blank group, suggesting the proliferative ability of cells has no changes after transfection with lentivirus containing enhanced green fluorescent protein. These findings indicate that the lentivirus containing enhanced green fluorescent protein can highly transfect human umbilical vein endothelial cells, and green fluorescent protein can sustainably express for 21 days but cannot impact the cellproliferation.
7.Analysis on effect of small group learning in the course of introduction to general practice

Objective To introduce small group learning in the course of introduction to gen-eral practice to cultivate students' ability of active learning and innovation. Methods Totally 221 students of 2008 grade A and B classes were enrolled as teaching objects and were divided into 20 groups. Introduction to general practice was used as learning materials. One week before the class, teacher gave students learning task and A and B classes carried on class discussion respectively. Re-view speaking was conducted by the representatives of groups. Effectiveness was evaluated through the observation,interview,questionnaire survey and answer scoring points. Results Overall support per-centage to small group learning was 91%. Percentages of students who believed that small group learn-ing was beneficial to cultivating the ability of active learning and innovation were 88% and 73% re-spectively. Conclusions Introducing small group learning in the course of introduction to general practice is effective and conductive to training students' ability to study and innovate.
8.The application of optical coherence tomography to measure the thickness of rabbit knee joint cartilage

Objective To study the feasibility of the measurement of the thickness of rabbit knee joint cartilage by comparing the optical coherence tomography (OCT)and the undecalcified frozen section method on respectively measure the thickness of New Zeal-and white rabbit’s knee cartilage.Methods 50 standardized cultivation,adult and male New Zealand white rabbits (100 knees) were selected in this study.The measurement point was at the knee weight-bearing area of the medial femoral condyle with a 2 milli-meter diameter trephine,and the cartilage thickness data at the same center point and ±0.5 mm from the center were obtained.OCT and freeze-sectioning method were adopted at each site,respectively.The difference between OCT and histological method was com-pared and Bland-Altman plot was constructed.Results The thickness results of the center,+0.5 mm,-0.5 mm points were (296.5± 1.6)μm,(302.6± 3.5)μm,(287.9±5.6)μm by OCT,(278.4±1.9)μm,(290.3±5.9)μm,(280.3±4.6)μm by freeze-sectio-ning method,respectively.In Bland-Altman diagram,mean difference and 95% confidence interval were 18.1 1 (1 6.65,1 9.56)μm, 12.4 (5.5,1 9.2)μm,7.4 (2.8,12.0)μm.Intra-class correlation coefficients(ICC)for resemblance between the 2 techniques were 0.93(95%CI:0.89-0.95,P <0.000 1),0.84(95%CI:0.77-0.89,P <0.000 1),0.91(95%CI:0.87 -0.94,P <0.000 1),respec-tively.Conclusion As compared with the measurement of undecalcified frozen section,OCT for the rabbits’knee joint cartilage thickness measurement is feasible with the advantages of noninvasiveness,good repeatability.OCT can provide data reference in ani-mal experiments of cartilage tissue engineering for articular cartilage defect repair.
9.Evaluation methods for small vascular network distribution and counting around the knee joint in rats

BACKGROUND:With the deepening of bone tissue engineering research and bone metabolism understanding, it is a hotspot to analyze the blood supply and nutritional status of tissue-engineered bone.
OBJECTIVE:To compare different methods for evaluating smal vascular network distribution around the knee joint in rats in order to provide a guideline for the study of microvascular network in tissue-engineered bone.
METHODS:Eighteen Sprague-Dawley rats were randomly divided into three groups, with six rats in each group. Three commonly methods were used to evaluate the smal vascular network around the knee joint in rats:immunohistochemistry analysis, angiography analysis, and CT scans and reconstruction analysis.
RESULTS AND CONCLUSION:The microstructure of vascular network could be observed by immunohistochemistry, but the spatial distribution of vessels could not be evaluated. The spatial distribution of vessels could be showed by angiography and CT scans. However, some of micro vessels were showed unclearly by CT scans. The number of blood vessels detected by immunohistochemistry was (26.50±3.02) vessels, significantly higher than those detected by angiography and CT scans that were (14.12±1.47) and (9.00±1.79) vessels, respectively. Combination of immunohistochemistry and angiography can evaluate the microvascular network at microscopic and macroscopic levels, which can provide the whole information of the vascular network.
10.Clinical significance of PCT, CRP, ESR, WBC count as predictors in postoperative early infectious complications with fever after posterior lumbar internal fixation.

OBJECTIVETo evaluate the role of serum level of procalcitonin (PCT), C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), and white blood cell count (WBC) as predictors in postoperative early infectious complications with fever after posterior lumbar internal fixation (PLIF).
METHODSA retrospective study was conducted from January 2012 to January 2014. Fifty-two patients with fever in the early stage(within 10 days) after the PLIF were collected in the study. They were divided into infection group and non-infection group (group A and group B) according to the results of postoperative blood culture. There were 26 patients in group A and 32 patients in group B. The values of PCT, CRP, ESR, and WBC count were compared and analyzed between two groups.
RESULTSThe values of PCT, CRP, and ESR in group A were higher than those of group B. Meanwhile, CRP and ESR in group B were still higher than the normal range. Among the 26 patients with infections (group A), PCT was superior to CRP and ESR, had a good ability in discriminating different kinds of postoperative infections. The area under the ROC curve of serum PCT levels was the largest (CI 95% was 0.81 to 0.98) in the indexs; and ROC curve of WBC count was no statistically significant. When the cut off points of each predictors were evaluated, the higher sensitive was CRP and reached at 90.27% and the higher specific was ESR and reached at 88.50%.
CONCLUSIONFor the patients with fever at the early stage after the PLIF should be paid attention and reasonable choosing predictors are helpful to identify postoperative infection in the early stage. The CRP and ESR may be influenced by the surgery, and the PCT level is helpful to differentiate infection type.
