1.Evaluation of Clinical Effect of Rabeprazole Combined with Teprenone Capsules in Treatment of Gastric Ulcer by Marking Targeting Biopsy and Leptin

Objective To evaluate clinical effect of rabeprazole combined with teprenone capsules in treatment of gastric ulcer by marking targeting biopsy and leptin.Methods A total of 118 patients with active gastric ulcer confirmed by endoscopy were randomly divided into two groups.Patients in the treatment group (n=60) were given rabeprazole 10 mg,bid,and teprenone capsules 50 mg,tid.Patients in the control group (n=58) were given rabeprazole 10 mg,bid.Both groups were treated continuously for 56 days.Before and after treatment,2 groups were labeled with biopsy,the clinical efficacy and the healing rate of two groups were recorded,the quality of healing and the expression of leptin were compared.The level of leptin was tested after treatment.Results After 10 days,the difference of clinical curative effect was not statistically significant (P>0.05).After 56 days,the difference of clinical curative effect was statistically significant (P<0.05);ulcer healing rate (93.33%)in treatment group was higher than that of control group (72.41%);ulcer healing quality (93.33%) in treatment group was higher than that of control group (58.62%);leptin level of treatment group was lower than that of the control group;gastric ulcer recurrence rate (3.8%) in treatment group was lower than that of the control group (24.0%) (all P<0.05).Conclusion Rabeprazole combined with teprenone in the treatment of gastric ulcer is better than rabeprazole.Marking targeting biopsy and leptin can be used to evaluate the healing quality of gastric ulcer more accurately,which can be an evaluation index of the quality of gastric ulcer healing and used as an indicator of the quality of gastric ulcer healing.
2.Experimental Study on Differentiation of Human Cord Blood Stem Cells into Hepatocytes

Objective To investigate the feasibility of differentiating human umbilical cord blood stem cells into hepatocytes. Methods Thirty-six BALB/c nude mice were randomly divided into experimental group and control group(18 in each of the group), and experimental group was again randomly divided into group A, B and C (six in each of the group). The mice in experimental group and control group were exposed to 350 cGy radiation produced by 60Co. After 3 h, karyocytes at different concentrations in the fresh human umbilical cord blood were injected into the mice in experimental group A, B, C via their tail veins, and the equal volume of normal sodium (NS) was also injected into control group via tail veins. After one month, carbon tetrachloride (CCl4) was injected into experimental group A, B and control group via abdominal cavity, and the equal volume of normal sodium was injected into experimental group C. After two months, immunohistochemistry and reverse transcriptase polymerase chain reaction (RT-PCR) were used to detect the expressions of human cytokeratin-18 (CK18), cytokeratin-19 (CK19) and albumin (ALB) in liver tissues of all mice. Background Retinal development continues during the early postnatal period in mammals.Correct arrangement of layers and precise location of various cells in the retina are vital for forming normal visual function during critical period plasticity.Spectral-domain optical coherence tomography(SD-OCT)provides highquality in vivo retinal imaging and the possibility to measure retinal thickness longitudinally. Objective The present study was to investigate the changes of retinal thickness during critical period plasticity in rats. Methods In vivo consecutive scanning of retinal image was performed in 10 SPF Sprague-Dawley rats at postnatal day 14(P14),P18,P21,P24 and P42 with SD-OCT,and retinal histopathological examination was used to detect retinal morphologic changes at the same postnatal ages in 20 matched rats.The whole retinal thickness,the thickness from inner limiting membrane(ILM)to inner plexiform layer(IPL),the thickness of inner nuclear layer(INL)and the thickness from outer nuclear layer(ONL)to retinal pigment epithelium(RPE)were measured using Cirrus HD-OCT system and HMIAS-2000 Imaging System in retinal sections.The measurement parameters by Cirrus HD-OCT and those by hematoxylin-eosin staining were compared.The use of animals followed the Statement of National Institute of Health (USA). Results In vivo high-resolution images of rat retinas with SD-OCT compared well with histology,which enabled quantitative comparison of the SD-OCT and histological data during critical period plasticity in rats.From P14 to P42,the retinal thickness gradually decreased with the increase of rat ages(F=15.425,P=0.000),and so were the thickness from ILM to IPL,the thickness of INL and the thickness from ONL to RPE(F=3.973,P=0.007;F=17.529,P=0.000;F=7.038,P=0.000).The retinal thickness,thickness of INL.thickness from ONL to RPE measured by Cirrus HD-OCT were significantly correlated with those measured by retinal sections among P14,P18,P21,P24 and P42 rats(r=0.794,P=0.000;r=0.784,P=0.000;r=0.681,P=0.000). Conclusion SD-OCT is a demonstratably valuable technology to study the structure of retinas in rats.The retinal thickness is shown to reduce in thickness throughout the development of the retina during critical period plasticity due to the decrease in thickness of INL and the distance from the ONL to RPE,as illustrated by OCT scanning.
6.Selection of operation for old thoracolumbar fracture:anterior fixation versus posterior vertebral osteotomy

Objective To compare the surgical results between anterior fixation and posterior vertebral osteotomy in patients with old thoracolumbar fracture. Methods 39 cases of old thoracolumbar fracture with posttraumatic kyphosis and neurologic compromise underwent either anterior fixation (n=19) or posterior vertebral osteotomy(n=20). In the anterior group, the average patient age was 38.3 years (range:21 to 64), the mean time from injury to operation was 5.3 months (range:2 to 16), and the mean preoperative kyphotic angle was 25.2?(range:10?to 43?). In the posterior group, the average patient age was 39.9 years (range:18 to 68), the mean time from injury to operation was 5.6 years (range: 2months to 16 years), and the mean preoperative kyphotic angle was 27.6?(range:5?to 60?). Results In the anterior group, the mean operation time was 274 minutes (range:140 to 395) with a mean blood loss of 994 mL (range: 300 to 2000), the mean postoperative kyphotic angle was 14.7?(range: 0?to 35?), and the mean angle of correction was 10.5 ?(range: 5?to 16?). In the posterior group, the mean operation time was 283 minutes (range:190 to 390) with a mean blood loss of 1654 mL (range: 800 to 3800), the mean postoperative kyphotic angle was 4.4?(range:-10?to 35?), and the mean angle of correction was 23.2?(range: 7?to 40?). All the patients with incomplete neural injury had improvement of neurologic function. Conclusion The posterior vertebral osteotomy can produce better results in kyphotic correction, while it does not increase the trauma of operation.
7.Single-level surgery through anterior-posterior approach to treat dislocation of inferior cervical vertebra

Objective To introduce a single level fixation and reduction for cervical dislocation. Methods 15 patients who had old cervical dislocation were treated from May 2003 to April 2004. All the patients had facet locking that could not be reduced though traction. We used posterior approach to relax facet locking and thick Ethicon wire to fix spinal process. We used anterior poach to plug coralline hydroxyapatite (CHA) as the graft. AO plates and locking screws were used to fix the vertebral body. Results All the patients got satisfactory reduction and good alignment through lysis of facet locking and CHA plug. Conclusion The posterior decompression combined with anterior stabilization can achieve satisfactory reduction and alignment for dislocation of inferior cervical vertebra.
8.Effect of jiangtang bushen recipe in intervention treatment of patients with impaired glucose tolerance.

OBJECTIVETo evaluate the intervention effect of diet, exercise and Jiangtang Bushen Recipe (JBR, a Chinese herbal recipe) in preventing the progress of patients with impaired glucose tolerance (IGT) to diabetes mellitus (DM) type 2.
METHODSFifty-one patients with IGT, with their diagnosis conformed to the diagnosis standard of WHO, 1999, were randomly divided into the control group (n = 26) and the TCM group (n = 25). Patients in the control group attended to the educational course for DM and received dietotherapy and kinetotherapy, and to those in the TCM group, under these treatments, JBR was given additionally. Oral glucose tolerance test (OGTT), body weight index (BWI), levels of blood lipids and fasting insulin of all the patients were examined after 3 months, 6 months and 12 months of treatment. The total observation time was 1 year.
RESULTSExcept the 6 cases out of the 51 patients (11.7%), on whom the observation discontinued, in the control group, as compared with before treatment, levels of fasting insulin and fasting blood glucose after treatment were not changed significantly (P > 0.05), also insignificant difference was shown in levels of total cholesterol (TC) and triglyceride (TG), though the two indexes lowered slightly after treatment (P > 0.05), but significant difference was shown in comparison of OGTT/2 h, blood glucose and BWI (P < 0.05). While in the TCM group, fasting blood glucose was changed insignificantly (P > 0.05), but there was significant difference in comparison of fasting insulin, TC, BWI, OGTT/2 h and plasma glucose levels (P < 0.01) respectively before and after treatment. At the end of the study, the cumulative cases with conversion to diabetes were 3 (13.6%) in the control group, and 1 (4.3%) in the TCM group, chi 2 test showed insignificant difference in comparison of diabetes conversion rate between the two groups (P > 0.05), however, the TCM group showed a better year conversion rate of normal glucose tolerance than that in the control group (chi 2 = 8.31, P < 0.01).
CONCLUSIONTCM intervention is possibly effective in delaying the conversion of IGT to DM type 2, and plays integrative effeciency in impelling IGT patients to health. The favorable education and treatment of DM controlling, including dieto- and kineto-therapy may also be advantageous in IGT intervention, but could not be effective in blocking the advance of IGT.

9.Repeatability and reproducibility of retinal straylight measurement using C-Quant compensation comparison method

BackgroundIn an ideal eye there would be no light scattering at all,but the eye media is not optically ideal.Intraocular straylight causes a veil of light and reduction in the contrast of the retinal image and thus decrease the quality of vision.ObjectiveThe present study was to investigate the repeatability and reproducibility of C-Quant straylight meter( Oculus,Germany)in measuring retinal straylight of myopia and post-laser in situ keratomileusis(LASIK) corneas.MethodsThis is a prospective research.The consecutive 35 eyes of 21 myopic patients and 34 eyes of 22 patients who received LASIK were included in this trail.Retinal straylight was measured for 7 times at a period of time and analyzed quantificational to evaluate the repeatability of measurement.Thirty-eight eyes of 19 patients were measured again at 3-7 days for 3 times at a period of time to assess the reproducibility of C-Quant straylight meter.The mean standard difference (SD) and coefficient of variation (CV) were used as the credibility evaluation.This clinical study complied with Helsinki Declaration and the informed consent was obtained prior to the medical procedure.ResultsThe straylight Log(s) of 7 times measurement were all less than 0.95.The mean Log(s) were 0.92±0.12 and 0.93±0.17 respectively in myopia group and post-LASIK group,without statistically significant differences among 7 times measurement( F=0.335,P=0.812;F=1.000,P=0.409).The mean SD for the 7 times measurement was 0.07 Log units.SD and CV increased with the number of measurements.The differences of mean SD and CV between 3 times result and 6 times result were significant different (t =-2.080,P =0.045;t =-2.190,P =0.035 ).No difference was found between different time periods( t =-0.531,P=0.598 ).The difference of the results between two measurements from the same patient was 0.013.ConclusionsC-Quant is a noncontact,noninvade,rapid and convenient method for the measurement of straylight in myopia and post-LASIK eyes due to the high repeatability and reproducibility.
10.Observation of preliminary clinical effect and analysis of perioperative complications of radical prostatectomy for patients with oligometastatic prostate cancer

Background and purpose:It has been demonstrated that radical prostatectomy for patients with oligometastatic prostate cancer may contribute to improving local control of prostate cancer and overall survival by several retrospective studies. Perioperative complications play an important role in determining whether radical prostatectomy is appropriate for patients with oligometastatic prostate cancer. This study aimed to discuss the recurrence rate and the sever-ity of perioperative complications, and the primary curative effect of radical prostatectomy on oligometastatic prostate can-cer patients.Methods:A total number of 247 patients who received radical prostatectomy were recruited in the study from Jul. 2015 to Jan. 2016, including 25 patients with oligometastatic prostate cancer and 222 patients with localized prostate cancer. Patients with perioperative complications in both groups were graded with the Clavien-Dindo grading system. The proportion of PSA decline and the rates and severity of perioperative complications were analyzed in both groups.Results:The cases of prostate specific antigen (PSA) decline in the oligometastatic group were 21 (84.0%), lower than the localized group with 212 cases (95.5%). There were 6 cases (24.0%) with postoperative complications in the oligometastatic group, including serious complications (Ⅲ or above) 1 case (4.0%), and 49 cases (22.1%) with postoperative complications in the localized group, including serious complications (Ⅲ or above) 7 cases (3.2%). The differences between the groups reached no statistical significance (P>0.05).Conclusion:Radical prostatectomy for patients with oligometastatic prostate cancer could be safe, effective, and appropriate, the risk of perioperative complications should not be one of the limiting factors.
