1.Advances in characterization of pharmaceutical polymorphisms by vibrational spectroscopy
Acta Pharmaceutica Sinica 2021;56(1):102-112
The determination and characterization of solid drug form polymorphisms plays an important role in drug quality control, selection of the production process and clinical efficacy evaluation. Vibrational spectroscopy is a powerful method for the characterization of drug polymorphisms. In this paper we review recent research and application advances in the polymorphic characterization of active pharmaceutical ingredients (APIs) and drug cocrystals/salts by using Fourier transform infrared (FTIR) and Raman spectroscopy to elucidate the characteristics of APIs and drug complexes. This may provide theoretical support for structural analysis during the development process for drugs.

Objective: To establish a method for the determination of paracetamol, caffeine and chlorphenamine maleate in Compound Ganmaoling Tablets. Methods: The RP-HPLC was conducted on a Phenomenex C 18 column(5?m,250mm?4.0mm) with methanol: water: acetic acid(35∶65∶1.5) as a mobile phase, detect wavelength at 270nm. Results: Paracetamol, caffeine and chlorphenamine maleate respectively showed good linear relationship in the following concentrations of ranges:50.4~117.6?g?mL -1,3.6~8.4?g?mL -1,0.84~1.96?g?mL -1; The average recoveries were obtained as 99.9%,99.8%,100.0%(n=6); respectively. Conclusion: The method was simple, sensitive, accurate and suitable for the determination of three constituents in Compound Gamaoling Tablets.

The optic nerve disease is a kind of diseases that seriously affect the visual function. In recent years, magnetic resonance diffusion tensor imaging ( DTI ) technology has been widely applied in the field of optic nerve diseases. Compared with the ophthalmic testing, such as optical coherence tomography imaging, visual evoked potential, field of vision, this method has obvious advantages. It not only can directly show the morphology changes of the optic nerve, visual pathway and visual cortex, but also can quantitatively analyze the morphological and pathological changes of the optic nerve, visual pathway and the visual cortex. This article reviews the imaging principle of diffusion tensor imaging, the progress and development prospect of diffusion tensor imaging in the study of the optic nerve diseases.

Objective To discuss the therapeutic options for treatment of subaxial cervical dislocation combined with facet locking. Methods There were 49 patients with cervical dislocations including 7 patients with dislocation at C3,4, 15 at C4,5, 14 at C5,6 and 13 at C6,7. Eleven patients were with old dislocation, with duration of dislocation ranging from 2 hours to 61 days. Neurologic status of the patients according to Frankel scale was graded A in 14 patients, grade B in nine, grade C in 10 and grade D in nine. All patients were treated surgically after closed reduction with skull traction. Results The successful reduction rate was 63% for fresh dislocation, with average improvement of 0.65 grade for spinal cord function. All bone grafts got fusion at four months after operation. Conclusion Therapeutic options are based on fresh or old dislocations, paraplegia or not, intervertebral disk injury severity, and reduction or not through traction for patients with lower cervical dislocations.

BACKGROUND:Studies have demonstrated that tetrandrine has protection on acute spinal cord injury, but the specific mechanism remains poorly understood.OBJECTIVE:To study the protection of tetrandrine on rat acute spinal cord injury and to study its mechanism from apoptosis pathway.METHODS:A total of 100 rats were randomly divided into 4 groups. All rats were prepared for spinal cord injury models using modified Allen method except that in the sham-surgery group.Methylprednisolone and tetrandrine was injected into rats in the methylprednisolone and tetrandrine groups by tail intravenous injection prior to and at 24,48 hours after model preparation. The same volume of physiological saline was injected in the sham-surgery and model groups.Basso-BeatUe-Bresnahan(BBB score)was recorded at 8 hours,1,3,7 and 14 days after model preparation. The morphological changes of spinal cord injury sites were observed by hematoxylin-eosin staining and the expressions of bcl-2 and bax were determined by immunohistochemistry.RESULTS AND CONCLUSION:The BBB score of methylpradnisolone and tetrandrine groups were significantly higher than that model group at 7 and 14 days(P<0.05),but there were no significant difference between the methylprednisolone group and tetrandrine group(P>0.05).Hematoxylin-eosin staining showed that the spinal cord injured severely at 3-7 days, the injury degree in the methylpradnisolone group and tetrandrine group was slighter than that of the model group, with smaller bax expression and greater bcl-2 expression(P<0.01).The findings demonstrated that, tetrandrine is able to protect neurons from apoptosis and promote the nerve function recovery by inhibiting the expression of Bax and promoting the expression of Bcl-2.Its effect is not inferior to methylprednisolone.

Objective To investigate the perfusion characteristics of intraductal breast lesion by real-time gray-scale contrast ultrasound and to determine the value of real contrast ultrasound in the diagnosis of breast masses. Methods A total of 30 breast lumps by ultrasound contrast enhancement were observed from the enhanced level. An enhanced mode and enhanced border were observed when the lesion was clear. The perfusion characteristics were compared between the benign and malignant lesions. Results Thirty breast lumps include 17 benign lumps and 13 benign lumps by pathological operation. After injected with the microbubble contrast medium, all breast lumps enhanced to varied extent. And malignant lesions showed significant enhancement for more than 3 grade(69.2%,9/13).The radial enhancement around lesion were mainly observed in the malignant lesions (P<0.05).Conclusion The microvascular perfusion of breast intraductal lesions can be clearly displayed by real-time gray-scale contrast-enhanced ultrasound. The feasibility of differentiation between benign and malignant lesions according to their perfusion characteristics appears to be promising.

Benzazepine is a kind of fused ring structure, which is composed of nitrogen-containing seven-membered ring and benzene ring. The introduction of benzazepine scaffolds into compounds can not only adjust the physicochemical properties, maintain or enhance the biological activities of the compounds, but also improve the pharmacokinetic properties, increase the brain permeability, and reduce the toxicity of hERG of the compounds, which is one of the privileged scaffolds for rational design and structural optimization of drug molecules. Benzazepine scaffolds can be constructed by different synthetic methods such as Dickmann condensation reaction, Mitsunobu reaction, Pictet-Spengler reaction, CMD reaction, multicomponent reactions (MCRs), metal catalysis reactions and asymmetric catalysis etc., which play an important role in enriching the structure diversity of drug molecules.
