Circulatory state and metabolic homeostasis could be more easily disturbed during perioperative periods for children compared with those of adults especially when receiving massive transfusions.Patient's age would be an important factor for clinical evaluation if certain amount of blood was lost.Component transfusions have now gained wide recognitions.Specific blood components should be considered according to different diseases,which would make transfusion more accurate and blood resources used more efficiently.This article focused on the indications,doses and several specific issues for perioperative component transfusions among pediatric patients.

We carried out a questionnaire survey on the career planning situation of pharmacy postgraduate students at Peking university, so as to collect information for career planning education of pharmacy postgraduate students, which is expected to promote their employment on completion of degree. by. The results revealed the status quo of career planning and the demand for guidance in this aspect. Recommendations for career planning education of pharmacy postgraduate students were made thereby.
4.XIAP modified adipose-derived stem cells improve cardiac function following myocardial infarction

Objective We want to study the therapeutic efficiency of autologous ADMSC transplantation in myocardial infarction.And we try to find out a good way to improve the therapeutic efficiency by using the combination of gene therapy and cell therapy.Anti-apoptotic protein XIAP was selected to fight against the ischemic environment of myocardic infaction.Methods ADMSC was isolated from rat inguinal fat tissue.ADMSC was cultivated with DMEM.XIAP experession plasmid was elertco-transduced to ADMSC.The anti-apototic function of XIAP was tested by serum stavation induced apotosis.The method of ligation of the left anterior descending artery was used to prepare the Myocardial infarction model.Then rats were randomly separated into three groups to receive direct epicardial injections of normal saline,or ADMSCs cell suspension or XIAP modified ADMSCs cell suspension at five sites in central zones of myocardial infarction and border zone.Cardiac function and the infarct size were evaluated 4 weeks after ADMSCs transplantation.Results West blotting suggest that,XIAP over-expression block serum starvation induced apotosis.It showed that there are significant statistic difference among XIAP modified ADMSC transplantation group,ADMSC transplantation group and control group 4 weeks after myocardial infarction (P < 0.05).Left ventricular ejection fraction(LVEF) showed a significant improvement in ADMSCs transplantation group compared to control group (P <0.05).Left ventricular end systolic diameter(LVDs) and left ventricular end diastolic diameter(LVDd) of ADMSCs group were smaller than control group(P < 0.05).The area of myocardial infarction was significantly reduced in the ADMSCs transplantation group compared to the saline group(P <0.05).Compared to ADMSCs transplantation group,effect of the XIAP modified ADMSC in rats with myocardial infarction is more obvious.The reduction of LVEF of XIAP modified ADMSCs group was signific antly lower(9%) than the ADMSCs group(16%) (P < 0.01).Infarction area in XIAP modified ADMSCs group(3.26 ±0.95)% was smaller than ADMSCs group(5.17 ±2.03)% (P <0.05).Conclusion Autologus ADMSC transplantation is an efficient therapeutic tool in myocardial infarction therapy.Over expression of XIAP can partly inhibit lowserum induced apotosis of ADMSC in vitro,and it can improve left ventricular function better in vivo.Over expression XIAP of ADMSC can improve the therapeutic efficiency compare to ADMSC transplantation.
5.Domain-specific physical activity among professionals with different occupational activity patterns

Objective To explore the profiles of physical activity domains among professionals with different occupational activity patterns.Methods A cluster random sampling method was used to recruit employed adults aged 30-60 years in Beijing as well as Zhejiang province.The information on the physical activity domains was collected with a revised Global Physical Activity Questionnaire (GPAQ).Results Of 3 326 professionals,about 30.79%,50.75% and 18.46% adults had Class Ⅰ (mostly sedentary reading or writing during work time) ,Class Ⅱ (mostly working with arms or legs with little effort) ,and Class Ⅲ (working with effort)occupational activity patterns,respectively.The median volume of leisure time activity and bicycling were all 0 Met-min/week,and no significant difference of domestic activity volume was detected among all the three classes of professionals.Of all the non-occupational activity volume among three classes of adults,domestic activity accounted for the most proportion (58.87%) performed by adults of Class Ⅱ,traffic activity accounted for the most proportion (34.2%) by those of Class Ⅲ,and leisure time activity accounted for the most proportion (12.7%) by those of Class Ⅰ.Most time was spent in sedentary behaviors by those of Class Ⅰ after hours.Conclusions Besides the generally low level of moderate activity,the diversity of the domains of physical activity among this population was evident.It is necessary to develop appropriate technology to improve physical activity for professionals with different occupational activity patterns.
6.Enlightenment of big-data era on clinical research of TCM and acupuncture.

The big-data era has arrived, which involves all professions and trades. Its impact on the medical field has gradually revealed. With the characteristics of big data "4V" (volume, velocity, variety, value) and its theory generality with TCM (holistic thinking, correlativity) as well as regularity of disease development, combined with present status of acupuncture clinical research, the influences of big-data era on trial design, data collection, analysis and sharing of acupuncture clinical research are discussed in this paper, aiming to provide references for solving the difficulties of study design and determining the future research direction of acupuncture clinical research.

7.Expressions of GnRH and GnRH receptor mRNA in female rats with danazol-induced precocious puberty

Objective To ob se rve the effects of danazol on hypothalamus-pituitary-gonad axises and expressi ons of hypothalamic GnRH and pituitary GnRH receptor mRNA in female rats. Methods Female rats at 5 days of age were given a sin gle subcutaneous injection of 300 ?g danazol to induce true precocious puberty. Animals were divided into 3 groups: normal control, model group and vehicle gro up. 8.Outcomes and clinical prognostic factors for elderly patients with esophageal squamous cell carcinoma treated after definitive chemoradiotherapy

Background and purpose: With the increase of aging population, elderly patients (age ≥70 years) with esophageal squamous cell carcinoma occurred more and more. However, few studies have focused on elderly esophageal squamous cell carcinoma patients. This study aimed to assess the outcomes and prognostic factors for elderly patients with esophageal squamous cell carcinoma treated after deifnitive chemoradiotherapy. Methods:We retrospectively analyzed 53 patients (age≥70 years) with esophageal squamous cell carcinoma and treated with deifnitive radiotherapy with or without chemotherapy from Fudan University Shanghai Cancer Center from Mar. 2009 to Dec. 2011. Results:Median age was 74 years. Twenty-nine patients underwent radiotherapy, 24 patients underwent radiochemotherapy. 1-, 2-, 3-, and 5- year survival was 62%, 44%, 33% and 19% respectively. Grade 2 and above acute radiation-induced esophageal toxicity and radiation pneumonitis occur rate was 6% and 9% respectively. No treatment-related deaths occurred and no patients experienced any grade 4 and above toxicities. Multivariate analysis identiifed treatment modality, tumor site and smoking history as independent prognostic factors for overall survival. Conclusion:Radiotherapy may be an acceptable treatment option for elderly patients with esophageal squamous cell carcinoma. In appropriately selected patients, concurrent chemotherapy could bring a better overall survival.
9.The situation of brick tea type fluorosis in Inner Mongolia in 2009 and 2012

Objective To understand the trend of drinking-tea type fluorosis in the diseased areas in Inner Mongolia Autonomous Region,and to evaluate the effect of prevention and control measures.Methods In 2009 and 2012,6 monitored counties were selected in Inner Mongolia Autonomous Region; 5 diseased villages were selected in each monitored county,and 10 families were selected in each monitored village; fluorine content of brick tea was tested,and dental fluorosis conditions of all 8 to 12-year-old students and skeletal fluorosis conditions of all 36 to 45-year-old adults were examined.In 2012,in each monitored village,1 water sample of the drinking water source was collected respectively from five positions(east,south,west,north and the central of the village),and fluoride content of drinking water wastested.Results In 2009,the mean of brick tea fluorine content was 541.48 mg/kg in the monitored villages,and the fluoride content ≤ 300 mg/kg accounted for 17.62%.(49/278).In 2012,the mean of brick tea fluorine content was 536.06 mg/kg,and the fluoride content ≤300 mg/kg accounted for 11.00%(33/300).Total exceeding standard rate of the water fluoride content was 40.00%(12/30).In 2009,the detection rate of the children's dental fluorosis was 52.39% (274/523),very mild,mild,moderate and severe cases accounted for 9.56%(50/523),19.86%(104/523),21.03%(110/523) and 1.91%(10/523),respectively.In 2012,the detection rate of the children's dental fluorosis was 35.71%(200/560),very mild,mild,moderate and severe cases accounted for 14.11% (79/560),11.43% (64/560),9.64% (54/560),and 0.54% (3/560),respectively.Compared with 2009,the proportion of moderate and severe cases reduced in 2012(W =40 696.0,P < 0.01).In 2009,the detection rate of adult skeletal fluorosis was 23.52% (167/710),mild,moderate and severe cases accounted for 17.32%(123/710),4.51%(32/710) and 1.69%(12/710),respectively.In 2012,the detection rate of adult skeletal fluorosis was 20.12% (103/512),mild,moderate and severe cases accounted for 17.38% (89/512),2.54% (13/512) and 0.20% (1/512),respectively.Compared with 2009,the proportion of moderate and severe cases reduced in 2012(W =12 797.5,P < 0.01).Conclusion The drinking-tea type fluorosis in Inner Mongolia Autonomous Region has been controlled to some degree,but still serious in some areas;prevention and control measures need to be further strengthened.
10.Early diagnostic value of combined inflammatory cytokines in bloodstream infection with different organisms

Objective To investigate the expression of procalcitonin (PCT) and C-reactive protein (CRP),and endotoxin in bloodstream infection with different microorganisms,so as to assess the value of these inflammatory cytokines in early diagnosis of sepsis in bloodstream infections patients.Methods Data of 152 septic bloodstream infected patients with 90 male and 62 female aged from 62 to 102 years and 79.2 ± 16.3 years in average admitted from January 2012 to December 2013 were analyzed retrospectively.According to the results of blood culture,the microorganisms could be categorized into gram-negative bacteria,gram-positive bacteria and fungus groups,and the levels of serum CRP,PCT,and endotoxin were compared among these groups of bloodstream infections patients within 24 hours after admission.Results (1) A total of 152 strains of microorganisms were surveyed including 92 gram-negative strains (61.18%),43 gram-positive strains (28.29%),and 16 fungal strains (10.53%).In the gram-negative strains,Klebsiella pneumoniae (n =29),Acinetobacter baumannii (n =24),Escherichia coli (n =23),Burkholderia cepacia (n =9) and Pseudomonas aeruginosa (n =4) were the most common isolates.In the Gram-positive strains,13 strains of Staphylococcus aureus were isolated.(2) In the gram-negative bacterial bloodstream infections group,there were 60 (64.52%) patients with endotoxin positive,and there were no endotoxin positive cases with detected gram-positive bacteria and fungal bloodstream infections.The median levels of PCT were significantly different among the three groups [gram-negative strains group:7.760 (3.365,28.585) ng/mL,gram-positive strains group:0.705 (0.265,3.225) ng/mL,fungal infection group:1.245 (0.543,1.998) ng/mL].In the fungal bloodstream infection group,the mean level of CRP was higher than that in other two groups [gram-negative strains group:(126.01 ± 66.53) mg/L,gram-positive strains group:(77.58 ±54.21) mg/L,fungal infection group:(140.14 ±71.21) mg/L].(3) The receiver operating characteristic (ROC) curve of inflammatory cytokines was made for the diagnostic value in bloodstream infections.ROC curve used to distinguish between gram-positive bacterial bloodstream infections group and fungal group showed that AUCPCT+CRP =0.791.When PCT cut-off value was 0.92 ng/mL,and CRP cut-off value was 68.00 mg/L,the sensitivity was 50% and the specificity was 95.5%.ROC curve used to distinguish between the gram-negative bacterial bloodstream infections group and fungal group showed that AUCPCT+CRP+LPS =0.947.When PCT cut-off value was 2.16 ng/mL and CRP cutoff value was 94.10 mg/L,and endotoxin was positive,the sensitivity was 82.8% and the specificity was 100%.ROC curve used to distinguish between gram-negative bacterial bloodstream infections group and gram-positive bacterial group showed that AUCPCT+CRP+LPS =0.947.When PCT cut-off value was 2.68 ng/ mL,CRP cut-off value was 106.5 mg/L,endotoxin was positive,the sensitivity was 74.2% and the specificity was 97.7%.Conclusions Gram-negative bacteria were the most common microorganisms in bloodstream infections in ICU patients.Compared with single inflammatory cytokine,the serum concentrations of PCT,CRP and endotoxin used together could provide more sensitivity and specificity for the early diagnosis of bloodstream infection with different microorganisms.
