1.Two Cases of Familial Juvenile Hyperuricemic Nephropathy.

The accuracy of fine needle aspiration cytology (FNAC) of the lymph node was investigated through a review of 176 FNAC cases and the corresponding biopsies. We chose 157 FNAC cases after the exclusion of 19 inadequate ones. Sensitivity of malignancy was 94.0%, specificity 100%, false negativity 6.0%, and false positivity 0.0%. The overall diagnostic accuracy was 96.8%. Sensitivity of metastatic carcinoma was 98.0% and that of malignant lymphoma was 87.9%. False negative cases included one metastatic carcinoma and four malignant lymphomas. The aspirates of metastatic carcinoma with false negativity exhibited a diffuse smear of keratin debris without viable cells, which led to the difficulty in differentiation from benign epithelial cyst. The cases of malignant lymphoma with false negative diagnosis were two Hodgkin diseases, one Lennert's lymphoma, and one peripheral T cell lymphoma in the histologic sections. On the analysis of 39 cases of tuberculosis, 17 cases (43.6%) were diagnosed as tuberculosis, 4 (10.3%) as granulomatous lymphadenitis, 3 (7.7%) as necrotizing lymphadenitis, and 15 (38.5%) as reactive hyperplasia or pyogenic inflammation. Sensitivity of tuberculosis was 53.9%. In conclusion, lymph node FNAC is an excellent non-invasive diagnostic tool for the diagnosis of metastatic carcinoma. The diagnostic accuracy of malignant lymphoma could be improved with flow cytometry or polymerase chain reaction for antigen receptor genes. For the FNAC diagnosis of tuberculosis, AFB stain, culture, and PCR would be helpful as adjuvant techniques.

2.Human Sperm Morphology Comparison after Pre-and Post Percoll Gradient Centrifugation.

In order to assess the efficiency of Percoll gradient centrifugation(PGC) as a method of sperm selection, we have examined morphological characteristics of spermatozoa from 40 teratozoospermic patients attending the Infertility Clinic of Inchon Gil Gerneral Hospital. Patients were divided into three groups according to percentage normal morphology in the fresh sample : group A(n=5), < 5% normal morphology : group B(n=21), 5~14% normal morphology : and group C(n=14), > 14% normal morphology. Morphology slides were perpared using Diff-Quik staining techniques and evaulated by Kruger strict criteria, under oil immersion at a magnification of X 1000, specific defects, head, neck and tail were assesed individualy. The results were as follows. 1. Following PGC, sperm samples with enhanced morphology were recovered for all groups. 2. For group A, PGC did not select a sample with significantly improved morphological characteristics. 3. Usually, sperm defects affected by PGC was head and neck. No significant difference was found for tail abnormality. In conclusion, Percoll gradient centrifugation is an efficient sperm preparation technique when the semen sample exhibits teratozoospermia, especially head or neck abnormality. However, in sample with < 5% normal form or tail abnormality, There is not significant improvement following PGC.

3.Analysis on the Key Words related to Healthcare Issues of the Prevention and Control of COVID-19 in Major Korean Newspapers, 2020

Background:
s: This study was performed to analyze the main key words of newspaper articles related to COVID-19 in 2020 for each category of quarantine measures according to the epidemic period of COVID-19.
Methods:
We analyzed articles related to COVID-19 in three major newspapers of Korea between February 17 and December 31, 2020. We targeted the front page articles on mondays and thursdays. The analysis of the relationship between the two variables was confirmed through the chi-square test.
Results:
As a result of analyzing the main key words for each category of quarantine measures, non-pharmaceutical intervention were the most common at 54.3%, followed by 3Ts(test, tracing, treatment and vaccine) at 31.9%. In the category of non-pharmaceutical intervention, social distancing was the most common at 33.9%. In the categories such as 3Ts(test, tracing, treatment) and vaccine, diagnostic tests were the most common at 41.8%.
Conclusions
It was identified that non-pharmaceutical intervention were the most common, and there was a difference in the reporting of main key words by category of quarantine measures for each epidemic period related to COVID-19 in 2020.
4.A Choledochal Cyst Associated with Hemorrhagic Tendency and a Cerebral Hematoma.

Congenital dilatation of the common bile duct is relatively rare anomaly. Its pathogenesis has not been completely understood. Complications of the choledochal cyst are mainly suppurative cholangitis, liver cirrhosis, stone formation, malignant change, bile peritonitis due to spontaneous and traumatic rupture. We experienced one case of choledochal cyst associated with hemorrhagic tendency and a cerebral hematoma, which is extremely rare complication. The 3 months old male patient reported here was treated with complete excision of cyst and Roux-en-Y choledochojejunostomy after correction of bleeding tendency and removal of cerebral hematoma. Postoperative course was relatively uneventful. 11 days after operation, the patient was discharged with full improvement.

5.University Students' Cough Etiquette Knowledge and Practice to Protect Droplet Infection.

PURPOSE: This survey aimed to identify cough etiquette knowledge and practice level among university students. METHODS: 190 study subject were recruited from a university located in C city of Korea and data were collected utilizing self-reported questionnaires to evaluate cough etiquette knowledge and practice levels. RESULTS: The score of cough etiquette knowledge was 7.38 out of 12 and the correct answer rate was 61.5%. The respondents showed the highest right answer rate (85.3%) on 'cover with a paper tissue or a handkerchief while coughing' and lowest (20.7%) on 'cover with hand while coughing'. Practice level score was 27.28 out of 48. Among those items of practice, 'wearing a mask while coughing' was the lowest (1.40/48) level of practice. And the correlation between knowledge and practice was not significant. CONCLUSION: Cough etiquette knowledge and practice level was revealed to be rather low among university students. Education and social marketing are needed to be developed to encourage the practice level of cough etiquette followed by better droplet infection control and health promotion.

6.Bilateral Ota Nevus and Bilateral Ito Nevus: 2 cases of extensive dermal melanocytic nevi associated with vascular nevus.

We present two cases, a 15 and a 24 year-old females, showing extensive involvement of dermal melanocytic nevi, so called bilateral Ota nevus and bilateral Ito nevus, and nevus anemicus. Both females had bilaterally brownish or slate-bluish pigmented patches on the eyelids, the scleras, the zygomatics, the frontal and temporal areas, the alae nasi, the nasal bridges (bilateral Ota nevus), and, the suprascapular regions(bilateral Ito nevus). Nevus amenicus was noticed on the left anterior neck in both females. In addition, a 15 year old female has had a diffush bluish pigmented patches on the nearly whole body (Mongolian spot), reticulate patches of nevus anemicus on the dorsal back and both arms and nevus flammeus on the right cheek. These cases are showing extensive melanocytic nevi associated with vascular anomalies, which are very rare in the literatures.

8.Treatment of Ipsilateral Fractures of the Femoral Neck and Shaft

Ipsilateral fractures of the femoral neck and shaft represent many difficulties in diagnosis and treatment. The femoral neck fracture of this injury is commonly missed on initial evaluation. The overlooked femoral neck fracture may lead to higher risk of complications such as avascular necrosis of the femoral head, nonunion and coax vara. The authors reviewed nine cases of ipsilateral femoral neck and shaft fracture treated in our clinic from September 1989 to May 1995, and average follow-up period was 33 months(range, 12 to 101 months). The results obtained were as follows : 1. The most common cause of injury was traffic accident, and three femoral neck fractures were missed initially. 2. The most common site of femoral neck fractures was basicervical in 7 cases, and that of shaft fracture was middle 1/3 shaft in 5 cases. 3. The most common associated injury was ipsilateral knee injury(6 cases). 4. Five cases were treated with femoral reconstruction nail, two with multiple neck-pinning and retrograde interlocking IM nail, one with multiple screws for neck fracture and long dynamic compression plate for shaft fracture, and the other, multiple neck-pinning and antegrade interlocking IM nail. 5. The complications were a refracture of the shaft after removal of interlocking nail and a delayed union of shaft fracture which had been treated by reconstruction nailing. Coxa vara, delayed union and metal loosening were found in femoral neck fracture site, but all that were clinically acceptable. In conclusion, the fracture of femoral neck should be kept in mind not to be lost in case of high velocity-femoral shaft fracture, and if found, it should be treated with anatomical and rigid internal fixation in femoral neck fracture having first priority. Recently reconstruction nail for the treatment of these fractures was widely used, but that is technically difficult and might destroy the neck fracture during shaft fracture fixation. We recommend retrograde interlocking nail followed by multiple neckpinning might be safe and stable fixation for the ipsilateral neck and shaft fracture.

9.Factors Influencing the Happiness of Late School-aged Children: A Focus on Family Strength and Self-control

PURPOSE: The purpose of this study was to obtain research-based evidence on the relationships among general characteristics, family strength, self-control, and happiness among late school-aged children using a correlational research design. METHODS: The participants were 172 fifth- and sixth-grade students from two public elementary schools. Data were collected by employing structured questionnaires, including the Korean Family Strengths Scale for Strengthening Family II, a self-control scale, and a happiness scale. Data analysis was conducted using SPSS version 23.0. RESULTS: The level of happiness of late school-aged students showed significant correlations with family strength (r=.78, p<.001), and self-control (r=.59, p<.001). Family strength had a significant positive correlation with self-control (r=.55, p<.001). The factors with a significant impact on participants' happiness were family strength (β=.63, p<.001), self-control (β=.21, p<.001), exercise frequency, and self-perceived health. The total explanatory power of the model was 69%, and the explanatory power of family strength for the level of happiness was 61%, showing that the family strength was the most important factor that promoted happiness in late school-aged students. CONCLUSION: These findings imply that improving family strength is an important aspect of promoting happiness among late school-aged children. Interventions to strengthen late school-aged children's self-control are also necessary.

10.Standardization of FEES Evaluation for the Accurate Diagnosis of Dysphagia

Objective:
Fiberoptic endoscopic evaluation of swallowing (FEES) is a standard diagnostic tool for swallowing disorders. However, it has not been used frequently in Korea because of the long test time, low cost, and the absence of a standard evaluation system. The purpose of this study was to suggest a standard fill-out form for the FEES result.
Methods:
From February 2019 to June 2020, a total of 98 FEES tests were performed by an otolaryngologist (JYJ) at the Wonkwang University Hospital. After the exclusion of 68 cases, 30 cases were analyzed twice by 4 raters with over 5 years of experience as otolaryngologists working in various hospitals. The results were measured for the rater’s test-retest reliability and inter-rater consistency.
Results:
Cohen’s kappa values for measuring the intra-rater consistency of the four raters were 0.984, 0.887, 0.848, and 0.930, respectively, meaning very good alignment of 0.8 or more, respectively. The Fleiss Kappa value for measuring inter-rater consistency was 0.276, meaning ‘fair’ for values of 0.2 or more. To examine consistency, an intraclass correlation coefficient (ICC) analysis conducted by assuming the grading score to be a constant continuous variable gave an ICC value of 0.729 (P<0.001), showing a very reliable tendency.
Conclusion
In this study, all the items of the fill-out form were rated using a three-step grading scale, so the degree of agreement was high when performed twice by the same rater, but the degree of agreement among raters was relatively low. Therefore, our fill-out form for FEES will be useful in evaluating the improvement of a patient over the course of clinical treatment.
