2.Arthroscopic Meniscal Repair
The Journal of the Korean Orthopaedic Association 1988;23(1):114-120
Peripheral longitudinal tears of the menisci in nine patients(eight men and one woman) were repaired by arthroscopic techniques from December 1985 to November 1986. Six lateral and three medial menisci were repaired. The results of the repairs were classified by Tapper and Hoover scale as excellent 2, good 4, fair 2 and poor 1. The aversge durstion of follow up was 17.6 months(range : twelve to twenty-four months). One patient had a rerupture and two patients had hemarthrosis. The repairs of peripheral meniscal tears hsve been shown to be an effective and successful procedure. Arthroscopic instruments and techniques have been improved to allow for the repair of some of these tears.

BACKGROUND: The purpose of this study was to assess the results of arthroscopic bridging repair using a human dermis allograft in the treatment of massive irreparable rotator cuff tears. METHODS: From November 2009 to April 2011, 12 patients underwent arthroscopic bridging repair using a human dermis allograft in the treatment of massive irreparable rotator cuff tears. Patients were followed for an average of 33.9 months. Clinical outcome was evaluated preoperatively and postoperatively using the mean University of California, Los Angeles (UCLA) score and the Korean Shoulder Scoring System (KSS). Magnetic resonance imaging (MRI) was performed postoperatively at an average of 6.5 months. RESULTS: At a mean follow-up of 33.9 months (range, 25 to 42 months), 11 out of 12 patients were satisfied with their procedure. Patients showed significant improvement in their mean modified UCLA score from 15.9 preoperatively to 29.4 postoperatively (p=0.001). The mean KSS score improved from 45.6 preoperatively to 80.5 postoperatively (p=0.002). In MRI studies, 9 out of 12 patients had full incorporation of the graft into the native rotator cuff remnant. To date, there has been no intraoperative or postoperative complication from the graft procedure, such as infection or allograft rejection, in any patient. CONCLUSIONS: Arthroscopic bridging repair using a human dermis allograft can be considered as an option in treatment of select cases of massive irreparable rotator cuff tears, resulting in high patient satisfaction.

Korean Journal of Dermatology 1981;19(2):207-213
Mercury had been an important constituent of the remedial drugs for last few centuries, but substantially replaced by more specific and effective mordern medicines. However, metalic mercury, tbe vestige of the last decades, has still been employed for the patients of scabies by the herbal doctors in Korea. Therefore, we have often experienced patients with such complications as exfoliative dermatitis following topical applications of metalic mercury or inhalation of the vapor. There are three major toxic chernical forms of mercury, such as metalic (ele- mental) mercury vapor, salts of the mercury, and organic mercur.'als. Metailc mercury is the most volatile of the inorganic forms of the metal, Inhalation of the vapor of the metalic mercury is very toxic, but ingestion of the globular forms is rarely harmful. Acute mercury poison.ng due to topical mercurials is very rare. This case was a such patient who showed the mercury poisoning can result from the percutaneous absorption of topically applied mercury. The patient was an 11 year-old schoolboy with scabies. Three days after topical applications on the whole body with the mixture of rneta, lic rnercury, phosphorus and perilla oil, as prescribed by a herbal doctor, he was suffered from headache, fever, facial flushing, sore throat, a,nd stomatitis for a few days, and then followed by exfoliative dermatitis, renal and bone marrow fa'.lures. However, bone marrow failure due to mercury was very rarely described in the literatures. Virtually, the whole body was covered by thick scales on the dirty greyish pigmented or erytheinatoas Cohfluent payules. It whs oozing on the external genital area, and all nails were spontaneously extracted. Tbe findings of renal failures included puffy face, ascites, BUN 92. 8 mg/dl, serum creatinine 7. 3 mg/dl. creat.nine clearance 12rnl/min., proteinuria, rnicro and gross hematuria, and non or poor visualizations of IVP. The peak in severity was at about 50 days after onset and bacame normal except mild proteinuria at about 70 days after onset. The findings of aplastic anemia were shown by pancytopenia(Hb 7. 6g/dl, WBC 1, 700, seg. neutro. 18%, lympho. 40% mono. 26%, eosino. 16%, Hct25%, thrombocytes 40, 000, RBC 3, 000, 000, reticulocytes 0. 1%) and about 20 per cent of the cellularity in bone marrow aspiration from about 70 to 90 days after onset. In fact, the aplastic anemia was coincided with nasal bleeding, staphylococcal septisema, and staphylococcal pustulation at the site of IM injection. However, the levels of onset 80 day-urine and blood rnercury were high: 208 ug/1. (norm.alless than 100 ug/l)in urine, and 126. 3 ug/100g (normal less than 5 ug/100g) in blood.

Journal of Korean Society of Endocrinology 1997;12(1):18-32
BACKGROUND: The Chinese hamster ovary cells transfected with human TSH receptor cDNA (hTSHR-CHO), expressing functional human TSH receptors, are known to be more sensitive in detection of thyroid stimulating antibodies than FRTL-5 cells. There has been no report on the usefulness of these cells to measure thyroid stimulation blocking antibody (TSBAb) activity which is frequently found in patients with primary myxedema, METHODS: We established the optimal assay condition of measurement of TSBAb using hTSHR-CHO cells, and simultaneously measured TSBAb activities with FRTL-5 cells and with hTSHR-CHO cells in 49 patients with primary myxedema, compared them with their thyrotropin binding inhibitor immunoglobulin (TBII) activities. RESULTS: 1) hTSHR-CHO cells specifically bound bTSH and were stimulated by bTSH in terms of cyclic AMP generation in a dose dependent manner. 2) Myxedema IgG suppressed TSH-stimulated cAMP production of hTSHR-CHO cells in a dose dependent manner reaching plateau at the concentration of I g/L. Normal pooled IgG has no suppressive action at the concentration of less than 1 g/L, but caused significant suppression at the concentration of greater than 1g/L. 3) TSBAb activities measured by hTSHR-CHO cells in 49 patients with primary myxedema were as follows: Four of 25 TBII-negative cases (16%) and 22 of 24 TBII-positive cases (92%) had TSBAb activities. Most of TSBAb positive patients (95%), especially in TBII positive cases, showed very high activities of more than 90%. 4) TSBAb activities measured by hTSHR-CHO cells and those by FRTL-5 cells were both positive in 24 patients (49%), both negative in 18 patients (37%), and were discrepant in 7 patients (14%). The TSBAb activities measured with hTSHR-CHO cells and those measured with FRTL-5 cells were significantly correlated (r=0.71, p< 0.01). 5) Forty five percent of patients with primary myxedema had all of 3 kinds of activities (TBII, hTSHR-CHO cell TSBAb, FRTL-5 cell TSBAb), 37% of them had none of 3 activities and 18% of them had 1 or 2 kinds of activities only. CONCLUSION: The usefulness of hTSHR-CHO cells in measurements of TSBAb activities were confirmed. The TSBAb activities of most patients with primary myxedema measured by hTSHR-CHO cells were concordant with those measured by FRTL-5 cells. However, a small subset of patients (18%) had discrepant results in assays of TSH receptor antibodies according to the differences in TSH receptors (rat, human and porcine) used in assay. Such discrepancy may be explained by heterogeneity in epitopes for blocking TSH receptor antibodies.

Journal of the Korean Surgical Society 1998;54(2):299-304
Takayasu's arteritis(arteritis syndrome) is an inflammatory disease process affecting primarily the aorta and its main branches. It's etiology and pathogenesis are unknown. Controversy exists in treatment. Segmental stenoses, occlusions, and aneurysmal degeneration of the aorta and the proximal arterial tree are frequent findings. Clinical presentations may include cerebrovascular ischemia, limb ischemia, aortic valvular insufficiency, heart failure, renovascular hypertension, or renal failure. Recently, we experienced a case of Takayasu's arteritis in a 24 years old female with severe dizziness, claudication in right upper extremity. Patient was treated by angioplasty of left vertebral artery and concomitant left subclavian-to-right axillary artery bypass surgery followed by percutaneous transluminal angioplasty with stent therapy of right vertebral artery to prevent cerebrovascular ischemia during the operation. Postoperative result was excellent and dizziness and claudication of right arm were completely relieved.

Yeungnam University Journal of Medicine 1989;6(1):43-46
Primary peptic ulcer disease in not known to be the result of underlying illness or trauma. These are most frequently duodenal or prepyloric. Since clinical features of peptic ulcer in children can easily be confused with many other disorders, the diagnosis is usually made when one of the more dramatic presentations, such as perforation, bleeding and obstruction. Recently, we experienced 2 cases of duodenal obstruction due to peptic ulcer in children. So, we report it with review of references.

Korean Journal of Dermatology 1982;20(5):641-646
Cimetidine, an H-histamine receptor antagonist, has been advocated to have enhancing effect on delayed type hypersensitivity. However, some recent reports have caused controversial opinion regarding the effect of cimetidine on delayed type hypersensitivity assessed in vivo. In this study the effect of cimetidine at the various periods of contact sersitivity reaction (CSR) to DNCB was assessed in guinea pigs. Three groups of five guinea pigs were sensitized with DNCB in the nuchal region and challenged with DNCB in the back on 16th day after sensitization, Cimetidine, 300mg/3ml/kg, was injected intraperitoneally once a day for the duration of different periods. For the first group, cimetidine was injected during the first 10 days after sensitization to assess its effect on the preparative and recognition phase of the induction period of CSR, For the second group, cimetidine was injected from the 6th to the 16th day to evaluate the enhancing effect of cimetidine on CSR at the period when suppressor T-cells were considered to be active. For the third group, cimetidine was injected for the last 3 days before elicitation test to assess the effect of short term treatment of cimetidine on the established CSR to DNCB. For the elicitation, two different consentrations of DNCB were applied on the three sites along both sides of the spine in the back. Total means of the degree of CSR elicited by DNCB were significanly ircreased by cimetidine in the second and third groups (p<0.05). These enhancing effects of cimetidine on CSR were more prominent at the uppermost back than other sites, eliclted by 0.05% DNCB rather than 0.01% DNCB.

Annals of Dermatology 1994;6(2):204-207
We report a patient with pachydermoperiostosis accompanied by hypertrophic gastritis. A 26-year-old man showed deep folds and furrows of the face and scalp, and terminal spade-like expansion of fingers and toes. Physical examination revealed no abnormalities except a grotesque appearance. Results of routine laboratory tests were unremarkable. Mild periosteal reactoins of both femurs and humeri were noted on long bone series, and hypertrophic gastritis on fibroscopic examination drew our attention. The relationship between pachydermoperiostosis and hypertrophic gastritis is uncertain. However it is probable that hypertrophic gastritis may be the endodermal counterpart of the ectodermal manifestations of pachydermoperiostosis.
