1.Eosinophilic Granuloma of the Lung.

Eosinophilic granuloma of the lung, first described by Farrinaci et al. in 1951, is rare. A 35-year-old male smoker presented with recurrent pneumothorax. Open thoracotomy with bleb resection and biopsy was performed. Microscopically there was histological changes consistent with typical eosinophilic granuloma and intertitial fibrosis. The Langerhans cells showed positive reaction for S-100 protein and typical Birbeck granules in their cytoplasm. A brief summary of histopathological aspect of this disease and a review of literature are presented.

5.Comparison of the retentive characteristics of two additional attachment used with an implant bar attachment

Purpose:
The purpose of this study was to evaluate the retentive characteristics of the additional attachments used with implant bar attachment under repeated insertion/removal cycles.
Materials and methods:
The newly developed attachment and the commercially available attachment were investigated: ADD-Lock (AL), Locator blue (LB). Two fixtures were placed parallel to each other on the custom lower mounting, and patrix of each attachment was fixed to the fixture. Also, the matrix of each attachment was placed on the opposing upper mounting. A universal testing machine was used to measure the retentive force during initial, 100, 250, 500, 1000, 2000, and 2500 repeated insertion/removal cycles. Wear and deformation of the attachment s were observed by scanning electron microscopy (SEM).Mann-Whitney U test (α=.05) and wilcoxon signed-rank test (α=.05) were performed to compare retentive force between each group and before and after 2500 repeated insertion/removal cycles.
Results:
In terms of initial retentive force and retentive force after 2500 repeated insertion/removal cycles, the AL group (15.24 ± 1.46 N and 9.74 ± 1.16 N) showed significantly smaller values than the LB group (43.53 ± 12.39 N and 22.99 ± 4.77 N) (P <.05). Also, in the loss of retentive force, the AL group (5.50 ± 1.08 N, 36.08%) showed a smaller value than the LB group (20.54 ± 11.89 N, 47.19%) (P <.05). Based on SEM analysis, The AL group showed noticeable wear and deformation in the patrix and the LB group in the matrix.
Conclusion
Locator showed a higher initial retentive force than newly developed attachment, while the loss of retentive force was also higher. Both additional attachments are considered to have sufficient retentive force after repeated insertion/removal cycles.

7.Genotyping cagA, vacA subtype, iceA1, and babA of Helicobacter pylori isolates from Korean patients, and their association with gastroduodenal diseases.

The genetic status of cagA, vacA subtype, iceA1, and babA, and the relationship to gastroduodenal diseases were assessed in Helicobacter pylori isolates in Korea. Seventy-six strains of H. pylori were isolated from the antrum and the corpus of 41 adult patients (22 with peptic ulcer and 19 with gastritis). The cagA, iceA1, and babA genes were assessed by polymerase chain reaction and the vacA subtypes were determined by reverse hybridization-line probe assay. The positive rates of 349-bp cagA, 208-bp cagA, iceA1, and babA genes were 97.4%, 96.1%, 84.2%, and 36.1%, respectively. The vacA s1a, s1b, s1c, and s2 variants were detected in 11.8%, 3.9%, 80.4%, and 1.3%, respectively. m1 (78.9%) is more prevalent than m2 (5.3%). The most common vacA genotype was s1c/m1 (61.9%), and 14 isolates (18.4%) contained mixed vacA genotypes from a single biopsy specimen. Twenty-one (60%) of 35 patients were infected with more than two strains of different cagA, iceA1, babA, and vacA genotypes. None of cagA, iceA1, babA, and vacA s1/m1 were associated with peptic ulcer. In conclusion, most H. pylori isolates in Korea carry cagA, iceA1, and vacA s1c/m1 genes, and reside with multiple strains. These genes do not correlate with the peptic ulcer in the Korean patients.

8.The Incidence Rate of Hypertension in Schizophrenic Patients Treated with Clozapine or Olanzapine.

OBJECTIVE: There were reports about weight gain, hyperglycemia, diabetes mellitus and hyperlipidemia associated with atypical antipsychotics. Moreover, these adverse effects of atypical agents, especially clozapine and olanzapine were reported to be able to precipitate cardiovascular disease. Accordingly, we investigated the incidence rate of hypertension in schizophrenic patients treated with clozapine or olanzapine by retrospective chart review. METHOD: We reviewed charts of patients with schizophrenia who admitted at St. Mary's Hospital, The Catholic University of Korea, and Naju National Hospital and selected records of patients treated with clozapine and olanzapine during at least 6 weeks. Patients treated with mood stabilizers, antidepressants, and antipsychotics other than clozapine and olanzapine were excluded. Finally, the records of 52 clozapine-treated patients and 76 olanzapine-treated patients were analyzed. Changes of systolic and diastolic blood pressure between before treatment and discharge time were analyzed by paired t-test. The incidence rates of hypertension before and after treatment were compared by McNemar test, and Cochran-Mantel-Haenszel test was used to compare incidence rates between clozapine and olanzapine treated group. Group difference of changes were analyzed by repeated measures ANOVA. RESULTS: The incidence rate of hypertension in clozapine treated group was increased after treatment. There was significant differences in the change of hypertension incidence rate in patients receiving clozapine versus olanzapine. However, there was no significant change in both systolic blood pressure and diastolic blood pressure in clozapine-treated patients. In olanzapine-treated patients, there was significant change in systolic and diastolic blood pressure. Repeated measures ANOVA indicated that antipsychotics medication induced decrease in systolic blood pressure, and diastolic blood pressure was decreased in olanzapine-treated patients. CONCLUSION: Our findings suggest that long-term clozapine treatment is associated with increased rates of hypertension, which may have a significant impact on medical morbidity and mortality.

9.Two Cases of Unusual Abscesses in Right Colon Cancer.

Two unusual presentations of carcinoma of the right colon are described. One of the two patients presented with huge abdominal abscess with adhesed to surrounding small bowels and the other presented with anterior abdominal wall abscess. Colonic carcinoma very rarely presents as abdominal wall abscess, retropertoneal abscess, groin inflammatory mass, subcutaneous thigh abscess, and obstruction with diastatic rupture of cecum. The appearance of the abscess may antecede any gastrointestinal symptoms, thus lessening the surgeon's suspicion of carcinoma. The literature has been reviewed and the pathology that characterizes such lesions and this management is discussed. The bulky colonic tumors with extensive local invasion and negative mesenteric lymph nodes have a relatively good prognosis if adequate resection is performed.

10.A Case of Pulmonary Embolism in a Patient with a Factor VII Gene Promoter -401G/A Polymorphism.

A factor VII gene -401 G/A polymorphism was identified in a patient with a pulmonary embolism. The patient was a 71-year-old woman who presented with acute-onset dyspnea. A chest CT scan revealed a pulmonary embolism. Despite the administration of low-dose warfarin as anticoagulation therapy, there was an excessively prolonged prothrombin time (PT). The blood tests revealed lower factor VII activity than normal. Full factor VII gene sequencing revealed a G to A substitution at ?401 in the promoter region. There were no other gene sequence anomalies. PCR-based analysis indicated lower factor VII gene expression in the patient than in a control subject. The data suggested the promoter polymorphism to be responsible for the lower transcription level. In conclusion, we encountered a case of Factor VII DNA polymorphism in a patient with a pulmonary embolism showing significantly reduced Factor VII activity.
