2.Comparison of efficacy of PFNA internal fixation and hemiarthroplasty in treatment of elderly unstable inter-trochanteric fractures

Objective To compare the clinical efficacy of proximal femoral nail anti-rotation ( PFNA ) fixation and hemiarthroplasty in the treatment of elderly unstable intertrochanteric fractures .Methods 72 cases with unstable intertrochanteric fractures were divided into the two groups by random number table ,each group had 36 ca-ses,the control group was given implementation of hemiarthroplasty while the observation group was given PFNA fixa -tion,the clinical therapeutic effect were compared between the two groups .Results It showed no significant differ-ence in operative time between the two groups (t =2.58,P>0.05);the blood loss of the observation group was (187.52 ±10.83)mL,which was significantly less than that of the control group (t=9.56,P<0.05),the load time of the observation group was (33.52 ±3.28)d,which was significantly more than that of the control group (t=10.31, P<0.05);there were no significant differences in the hip function excellent rate after 1 year and postoperative com-plication rate between the two groups (94.44% or 91.67%,11.11% or 8.33%,χ2 =2.01,1.69,all P>0.05). Conclusion PFNA fixation and hemiarthroplasty for treatment in elderly unstable intertrochanteric fractures have good effect ,and the patients should be based on the specific clinical circumstances , under strict control of surgical contraindications choose the right surgery .
3.Pharmacokinetic Studies of Sinomenine by Blood Microdialysis Technique

[Objective] To explore the feasibility and advantages of blood microdialysis technique for the pharmacokinetic study of sinomenine. [Methods] In this study, rats served as the experimental subjects and sinomenine was administered to the rats by intravenous injection. With microdialysis technique for blood sampling, the blood concentration of sinomenine was determined by high performance liquid chromatography (HPLC) and the associated pharmacokinetic parameters were analyzed by 3p97 program. [Results] The pharmacokinetics process of sinomenine in rats presented as the two-compartment model. The half-life of distribution phase was 10.98min and that of elimination phase was 44.71 min. [Conclusion] It is feasible to study pharmacokinetic parameters of sinomenine by using blood microdialysis technique. In comparison with the traditional technique, blood microdialysis technique is superior to the examination of drug blood concentration .
5.Effects of vacuum sealing drainage on oxygen partial pressure and wound healing in rabbits

ObjectiveTo investigate the effect of vacuum sealing drainage (VSD) with different negative pressures on variation of oxygen partial pressure (PtO2 ) and wound healing in the rabbits.MethodsTwelve rabbit wound models were made and randomly (random number) divided into two groups, namely vacuum group ( n =6 )in which rabbits were treated with VSD by different negative pressures ( - 75 mmHg,- 125 mmHg,- 225 mmHg and - 350 mmHg) for 7 days, and routine treatment group ( n =6). At each interval of measurement, variation of PtO2 was measured by oxygen partial pressure admeasuring device, and area of VSD dressing and surface of wound were measured by vernier caliper, and growth of anaerobic bacteria was detected by bacterial culture, and morphological change and the course of wound healing were observed under by light microscope after HE tissue staining. Meanwhile anther two groups (n =6, in each) were set for comparing, including normal group, sham operation group. ResultsAverage PtO2 value of vacuum group was in the range of ( 1.87 +0. 19) kPa to ( 1.54 ±0. 21 ) kPa which was decreased gradually in 7 days under different negative pressures. Average PtO2 value of routine treatment group and normal group were ( 2. 82 ± 0. 37 ) kPa and ( 5.79 + 0. 50 ) kPa, respectively which weresignificant higher than that in vacuum group ( P < 0. 01 ). PtO2 was fell to 80. 94% of its original value after VSD for 5 seconds, and continued the downward trend with the increasing of negative pressure at the same interval of measuring. Area of VSD dressing significantly decreased to 65. 36% of its original area after VDS for5 minutes (P<0.01). Surface of wound was minimized to 62. 82% of its original area after VSD for 7 days ( P < 0. 01 ), and variations of those in - 350 mmHg group were significant greater than those in other groups ( P < 0. 01 ). There was no evidence of anaerobic bacteria growth in vacuum group during this experiment. ConclusionsPtO2 could be down-regulated by VSD significantly without growth of anaerobic bacteria, and minimization of VSD dressing at - 350 mmHg was significantly helpful to reduce the area of wound for promoting the healing.
6.Effect of data management quality on clinical research evidence evaluation.

Current clinical evaluation of literature quality has various ways. Most of them lay special emphasis on the evaluation of the design quality, but the evaluation of the implementation process quality is not perfect. Especially data management is not fully emphasized during the enforcement of clinical trials. Data from clinical research were bases for evaluating clinical findings. Although strict specifications and requirements for data management might be strictly written clearly in research protocols, they were not embodied in current clinical research evidence evaluation system. Data management is an important part of implementing the whole clinical trial process, which is a comprehensive reflection of data collecting, logging, sorting, and managing. Its objective is to obtain high quality research data for statistical analysis, thereby coming to a true and reliable conclusion. In order to overall evaluating clinical design and implement, we suggest that present quality evaluation indicators of clinical trails should be completed, and add data management quality evaluation during the whole implement process. Data management plans, standards and requirements for data checking, and management regulations for disobeying data and exception data should be added in quality evaluation indicators for clinical research evidence. The effect of data management quality on clinical research evidence evaluation should be emphasized.

According to the common standard of ethic review for clinical research , it is equal to each kind of research program whether western medicine or Traditional Chinese Medicine .It should be reviewed the ethic and scientific issues in the same time .However, on account of the two majority characteristics of Traditional Chinese Medicine , it would be concerned more evidence , such as the safety data before clinic , the syndrome differentiation and treatment of target disease , drug combination , dosage and course of treatment , the methods of outcome meas-urement, admixture of the drug, the selection of control drug including placebo and so on .
8.Hip dysfunction-associated factors after total hip arthroplasty for ankylosing spondylitis

BACKGROUND: Total hip arthroplasy is an effective method to treat hip disease caused by ankylosing spondylitis. Long-term fol ow-up studies have shown that there have been hip function problems after total hip arthroplasy, and it stil lacks of perfect rehabilitation programs. OBJECTIVE: To systematical y review the influential factors for dysfunction after total hip arthroplasy in ankylosing spondylitis patients. METHODS: The first and second authors retrieved PubMed and Sciencedirect Databases, CNKI, and Wanfang digital database for relevant articles published from January 1970 to July 2015. The key words were “arthroplasty, replacement, hip, spondylitis, ankylosing, function”. Final y, relevant articles were included. RESULTS AND CONCLUSION: A total of 40 articles were summarized, including 8 Chinese articles and 32 English articles. After total hip arthroplasy, hip function problems were mainly associated with patient’s factors, disease, surgery, prosthesis, postoperative recovery and postoperative complications. Avoiding above relevant factors can improve joint function after total hip arthroplasy for ankylosing spondylitis patients, and improve patient’s quality of life.
9.Prevalence of fatty liver in a district of Shanghai detected by B-type ultrasonography and serum activity of alanine aminotransferase

Objective To study the prevalence of fatty liver and its risk factors in adult population of Pudong New District,Shanghai detected by combination of B-type uhrasonographic features and elevated serum activity of alanine aminotransferase (ALT).Methods A cross-sectional survey was performed in 2017 residents aged 16 years over recruited from four neighborhoods of Prdong New District of Shanghai with multi-phase cluster sampling,including interview with questionnaire,physical check-up,anthropometry, measurement of plasma glucose and lipid profile,ALT activity and real-time B-type ultrasnnography.Serum hepatitis B surface antigen (HBsAg) was further detected for those with elevated ALT activity.Results Prevalence of fatty liver was 21.32 percent (430/2017) in the residents of the District participated in this survey.Prevalence of abdominal obesity,hypertriglyceridemia,hyperlipoproteinemia (low-density lipoprotein-cholecterol),essential hypertension,impaired glucose tolerance,diabetes and metabolic syndrome were 71.16,71.16,11.86,66.74,35.58,24.40 and 47.21 percent in those with fatty liver, respectively,as compared to 26.34,12.73,4.79,39.57,24.01,6.81 and 11.28 percent in those without fatty liver (controls),respectively.Multivariate logistic regression analysis showed that body mass index, 2-h postprandial glucose level,diastolic blood pressure,serum level of triglyceride,abdominal obesity and diabetes all were independent risk factor for tatty liver,with odds ratio (OR) of 1.080,1.149,1.035, 1.526,1.960 and 1.391,respectively.Conclusions Prevalence of fatty liver was relatively high in Shanghai Pudong New District.Fatty liver closely associates with disturbance of carbohydrate and lipid metabolism.
