2. Insulin resistance and deficiency of post-receptor signal transduction after severe burns
Academic Journal of Second Military Medical University 2011;32(1):88-91
Severe burns and trauma can lead to insulin resistance (IR), which is associated with the metabolic disorder and hyperglycemia, and routine insulin treatment yield no noticeable outcome. However, long time big-dose insulin treatment can sometimes cause hypoglycemia, threatening the life of patients and making the condition more complicated to treat. By now, the mechanism of IR is not yet clearly understood. This article reviews the recent researches about the post-receptor signal transduction mechanism of IR after severe burns and its influencing factors.

Objective The coronary slow flow phenomenon (CSFP) is a coronary artery disease with a benign course, but its pathological mechanisms are not yet fully understood. The purpose of this controlled study was to investigate the cellular content of blood in patients diagnosed with CSFP and the relationship of this with coronary flow rates. Methods Coronary angiographies of 3368 patients were selected to assess thrombolysis in myocardial infarction (TIMI) frame count (TFC) values.Seventy eight of them had CSFP,and their demographic and laboratory findings were compared with 61 patients with normal coronary flow. Results Patients'demographic characteristics were similar in both two groups.Mean corrected TFC (cTFC) values were significantly elevated in CSFP patients (P < 0.001).Furthermore,hematocrit and hemoglobin values, and eosinophil and basophil counts of the CSFP patients were significantly elevated compared with the values obtained in the control group (P =0.005,P =0.047,P =0.001 and P =0.002).The increase observed in hematocrit and eosinophil levels showed significant correlations with increased TFC values (r =0.288 and r =0.217).Conclusion Significant changes have been observed in the cellular composition of blood in patients diagnosed with CSFP as compared to the patients with normal coronary blood flow. The increases inhematocrit levels and in the eosinophil and basophil counts may have direct or indirect effects on the rate of coronary blood flow.
4.A clinical study of total intravenous anaesthesia in laparoscopic operation
Chinese Journal of Postgraduates of Medicine 2008;31(z1):38-41
Objective To evaluate the recovery and hemedynamics of total intravenous anaesthesia (TIVA) with propofol and remifentanil. Methods Thirty ASA Ⅰ - Ⅱ patients undergoing hperoscopic oper-ation were randomly divided into the TIVA group(group A,n=15) and the combined intravenous with inhala-tion anaesthesia greup(group B,n=15). Anaesthesia was induced in the same manner in both greups:Midazo-lam 0.05mg/kg,vecurium 0.1mg/kg,propofol 1.5mg/kg and remifentanil 1.5 μg/kg.Remifentanil was infused 2.5 μg/ml) in group A or with sevoflurane( 1.3% end-tidal) in group B to maintain anaesthesia. With termi-nation of surgery, anaesthetic delivery was discontinued simuhaneously.Bp,HR at all stages and recovery profiles were recorded. Results (1)Between before inflation and after inflation or between at the end of op-eration and the basic values in group A,there were no significant differences in Bp,HR. (2) Times from stop-ping administration of anaesthetics until full spontaneous respiration and eye opening were similar for two groups.Time for trachcal extubation was significantly less in the group A than group B. (3)The rates of post-operative nausea or vomiting were similar. Conclusion The application of TIVA with propefol and remifen-tanil is associated with better hemodynamic stability and better recovery profile as compared to the combined intravenous with inhalation anaesthesia.

Objective To explore the therapeutic effect of liver transplantation on hepatic alveolar echinococcosis (HAE) in late stage. Methods Five HAE cases in late stage failed to be treated by hepatic lobectomy underwent liver transplantation, in which 4 cases were performed under veno-venous bypass and 1 without bypass. Three cases were subjected to veno-venous bypass prior to mobilization of the liver. The end-to-end anastomosis was made between the hepatic artery and hepatic artery, and between the bile duct and bile duct. Two cases received placement of T tube in the bile duct. The mean duration of surgery was 8.3 h. Results One patient was reoperated because of the T tube falling off on the postoperative day 10, and one because of the bile leakage. Four patients recovered completely in the postoperative period, one died of multiple organ failure (MOF) and septi-caemia caused by pneumonia, acute rejection and embolism of the liver artery. Four patients were followed up for 21 months to 37 months, showing a good quality of their life. Conclusion Liver transplantation can be applied in treatment of hepatic alveolar echinococcosis in end stage, and can ensure a better clinical result.
