1.Effect of rhodiola sacra SH Fu on heart hemodynamics and myocardial contractility

BACKGROUND: Rhodiola rosco L acts on anti-aging,anti-fatigue and improving immunity in human body and improves angina pectoris.OBJECTIVE: To study the effect of rhodiola root on heart hemodynamics and myocardial contractility and probe into its mechanism.DESIGN: Observational and controlled based on the experimental animals.SETTING: Department of physiology in a medical university.MATERIALS: The experiment was performed in Laboratory of Cardiac Vessels of Department of Physiology of Guangxi Medical University,from January to April 2001.Thirty New Zealand white rabbits of either sex,weighted varied from 1.9 to 2.3 kg,were randomized into two groups,named as the control(physiological saline 4 mL/kg) and rhodiola sacra SH Fu (nuodikang) group(0.5 g/4 mL · kg-1),15 rabbits in each group. A hundred common wild frogs were employed,weighted varied from 120 to 160 g,randomized into 4 groups, named as the control group of physiological saline,nuodikang of small dosage group(blood concentration 209 mg/L),nuodikang of moderate dosage group(blood concentration 418 mg/L) and nuodikang of large dosage group(blood concentration 627 mg/L),25 frogs in each group.METHODS: ms2000 multiple medium biological signal analysis system was applied,by Straub method,frog heart perfusion specimen in vitro was prepared and the cardiac activity was assayed. The relevant indexes of ventricle activity in rabbits were determined with hemodynamics.MAIN OUTCOME MEASURES: Comparison of hemodynamics changes in rabbits of two groups and the comparison of effects of nuodikang at various dosages on frog heart work in vitro.RESULTS: Myocardial contractility in every nuodikang group was strengthened remarkably compared with the control(F = 8.939, P<0.01 ), the myocardial contractility in nuodikang groups at the dosages of 209 mg/L,418 mg/L and 627 mg/L was in the tendency of increasing,strengthened by 52. 3%,70. 1% and 86.4% respectively. But the significant difference was not apparent in the comparison among experimental groups(P>0.05). The significant difference in frog heart rates among groups was not apparent(P>0.05). Concerning to hemodynamics in rabbits,in nuodikang groups,the maximum increasing rate of pressure in the left ventricle was increased by 22.1% (F = 6.259,P<0.05),the mean pressure of artery was decreased by 8.2% (F = 5.688, P<0.05) . The significant difference in other indexes was not apparent in the comparison between the control and nuodikang groups.CONCLUSION: Nuodikang strengthens myocardial contractility and speeds up myocardial contraction, but does not present remarkable influence on myocardial dilation; simultaneously it reduces the mean pressure of artery.
2.Gene function assembly analysis to screen key genes associated with deep vein thrombosis after trauma

Objective To screen key genes related to deep vein thrombosis (TDVT) after trauma using gene function assembly analysis. Methods Thirty Sprague Dawley rats were randomized into control, thrombosis and non-thrombosis groups. Traumatic limb DVT models were established in rats through quantitative beating on the bilateral posterior limbs. The Genechip Rat genome 430 2. 0 genechips were applied to detect changes in genes expressions on difference phases of DVT. On the basis of the differential gene expressions in the thrombosis and non-thrombosis groups, the gene function assembly analysis was conducted to define the most significant and concentrated gene functions leading to the biological characters of DVT.Results B factor (bf), complement 4 binding protein α (C4bpα), plasminogen activator inhibitor 1 (serpinel), urokinase-type plasminogen activator receptor (plaur) were screened to be the key genes related to DVT, because they were found to be involved in the functions like complement activation, development,growth, morphogenesis, primary metabolism, cell motility, protein metabolism, localization of cell, locomotion and localization. The abundance values of the genes expressed were 1.6, -0. 2, 2. 1, 5. 1 in the thrombosis group, and -0. 5, - 1.4, 2. 7, 3. 3 in the non-thrombosis group. Conclusion Bf, C4bpα,serpinel, plaur may be the key genes that play a role in the process of DVT.
4.Comparison of the impact of laparoscopic and open surgery on immune function in colon cancer patients

Objective To study the impact of laparoscopic radical resection and open radical resection on immune function in patients with colon cancer. Methods 110 cases received complete mesocolic excision surgery of the right colon cancer patients, according to the different surgical methods divided into observation group and control group, the observation group adopted laparoscopic surgery while the control group treated with open surgery, then compared the inflammatory stress response and related immunology index changes before and after operation. Results After op-eration, inflammatory stress indicators such as MCP-1, HMGB-1, Glucagon, BG in observation group were signifi-cantly better than that in control group. After operation, WBC, CRP, IL-6 index in the two groups were significantly improved. CRP, IL-6, CD4+, CD8+, NK cell index in observation group was significantly better than that in control group. There were significant differences between the two groups, < 0.05. Conclusion Laparoscopic CME surgery can help to relieve inflammatory stress response, and the immune function is less affected, it is worthy of promoting clinical application.
5.High Lumbar Disc Herniations.

STUDY DESIGN: This study is a retrospective evaluation of the high lumbar disc herniations. OBJECTIVE: The purposes of this study is to observe the clinical and radiological findings and out-comes of the high lumbar disc herniations, thus providing a guideline for making diagnosis and for proper treatment modality. SUMMARY OF LITERATURE REVIEW: High lumbar disc herniations at the L1 -2, L2-3 and L3-4 levels represent less than 5% of all disc herniations. The location of pain was highly complected. The represented symptoms and signs such as motor, sensory and reflex changes were variable and potentially misleading in suggesting a level of the disc herniation. MATERIAL AND METHODS: Thirty-one patients with high lumbar disc herniations were retrospectively evaluated. Seventeen patients were treated conservatively and fourteen patients underwent surgery. Review of results was undertaken by an independent observer using a proven outcome assessment measure. RESULTS: The incidence of high lumbar disc herniation is 4.7%(L1-2: 13%, L2-3: 29%, L3-4: 58%) with declining frequency as the level ascends. The peak age incidence is 6th decade in male and 5th decade in female. The positive rate of femoral nerve stretching test(74%) is higher than that of straight leg raising test(51%). In fourteen operative cases, the positive rate of straight leg raising test is 86%. The accuracy of MRI findings in predicting the type of herniated disc is 93%. In results of operation thirteen cases showed more than fair grade(93%). CONCLUSION: The age incidence of the high lumbar disc herniation is older than that of the lower lumbar disc herniation. The femoral nerve stretching test is more useful than straight leg raising test in making diagnosis of high lumbar disc herniations. The prognosis after treatment is comparable with high lumbar disc herniations.

6.Efficacy analysis of stenting in patients with internal carotid artery cavernous segment symptomatic stenosis

Objective To evaluate the safety,effectiveness,and middle or long-term efficacy of endovascular stenting of internal carotid artery stenosis at the cavernous segment. Methods Thirty-two patients underwent endovascular stenting at the cavernous segment of internal carotid artery from January 2012 to February 2015 were enrolled retrospectively. Angioplasty and stenting were conducted using Apollo or Winspan stent system. The improvement of internal carotid artery cavernous segment stenosis and perioperative safety and the results of the medium and long-term follow-up of the 2 kinds of stents were observed. Results All the 32 patients achieved technical success. The symptoms of cerebral ischemia of the patients were relieved significantly. The length of the stenosis at cavernous segment of the internal carotid artery was 4 to 13 mm (mean,7. 2 ±2. 9 mm). The stenosis rate from 82 ± 7% before treatment decreased to the 24 ± 7% . One patient had perioperative complication (4. 7%),26 of them were followed up with DSA,and 6 were lost to follow-up. The follow-up period ranged from 7 to 29 months (mean,16 ± 7 months). During the follow-up period,1 patient had intracerebral hemorrhage,1 had cerebral infarction,and none of them died. Four patients had in-stent restenosis,three of them used Winspan stents, and 1 used Apollo stents. Conclusion The patients should be screened strictly,particularly paying attention to the length of lesions. Endovascular stent angioplasty for the treatment of internal carotid artery cavernous segment stenosis is a safe and effective method.
7.Post-effect of acupuncture on brain functional connectivity

Objective To explore post-effect of acupuncturing ST36 (Zusanli) on brain functional connectivity. Methods Twelve healthy volunteers participated in this experiment. The fMRI data taken before and 25 minutes after removed acupuncturing stimulation were analyzed, while posterior cingulated cortex were chosen as seed points. Results At 25 minutes after removed acupuncturing stimulation, new increased functional connectivity were found in the left paracentral lobule, right superior parietal lobule and right postcentral gyrus. After acupuncture, there was intensity functional connectivity greater than in primary brain regions. Conclusion Post-effect of acupuncture can increase functional connectivity in healthy volunteer's brain.
8.Clinical analysis of retroperitoneal laparoscopic partial nephrectomy on patient with incidental renal cell carcinoma

Objective To evaluate the safety and clinical efficacy of retroperitoneal laparoscopic partial nephrectomy (LPN) in patients with incidental renal cell carcinoma.Methods Twenty-six patients with T1 incidental renal cell carcinoma treated with retroperitoneal LPN from Dec.2011 to Oct.2013 were analyzed retrospectively.The operation time,warm ischemia time,intraoperative blood loss,complications and prognosis of perioperative periods were collected.Results All 26 cases were operated successfully without conversion to open or other surgery.The operation period was 90-190 minutes.Blockage of renal artery was applied in all 26 cases,and the warm ischemia time was 15-30 minutes.The intraoperative blood loss was 100-360 ml.The drainage was removed 3-7 days postoperatively,and the hospitalization period was 7-12 days.All cases were clear cell renal cell carcinoma by pathological examinations,and showed negative surgical margins.No complications such as postoperative bleeding,inflammation and leakage of urine were occurred.All cases had normal renal function during the follow-up of 3-25 months without local recurrence or distant metastasis.Conclusion Retroperitoneal LPN for the treatment of T1 incidental renal cell carcinoma is worthy of application with the advantages of safety,minimal invasiveness,fewer complications,quick recovery and good nephron-sparing functional recovery.
9.Lipoic acid effects on electrophysiological changes of the sciatic nerve following ischemia/reperfusion injury

BACKGROUND:Lipoic acid, with a closed circle structure composed by sulphur and carbon atoms, exerts strong anti-oxidation, and has been extensively applied in the prevention and treatment of oxidative stress, diabetic cataract, diabetic neuropathy and cardiovascular diseases. OBJECTIVE:To investigate the protective effect of lipoic acid on peripheral nerve function during peripheral nerve ischemia/reperfusion injury. METHODS:Models of peripheral nerve ischemia/reperfusion injury were established in rabbits, and then rabbit models were then allotted to treatment and non-treatment groups. The treatment group was subdivided into experimental (injection of lippoic acid) and control groups according to the use of lipoic acid at 1, 3 and 6 hours after ischemia and before reperfusion. The ultrastructural changes of the sciatic nerve were observed under electron microscope, and the electrophysiological changes of the sciatic nerve were detected using evoked potential instrument. RESULTS AND CONCLUSION:With the ischemic time increasing, the number of vacuoles in the axon increased gradually, accompanied by axonal atrophy, and Waller's degeneration in the aggregated microfilaments. The myelin sheath thickening and dissolving were visible. All above phenomena became severest at 6 hours after ischemia. Compared with the control groups, lipoic acid reduced the number of the vacuoles in the axon and all eviated axonal atrophy, Waller's degeneration and demyelination. As the ischemic time increasing, the latency of sciatic nerve was significantly increased, and peaked at 6 hours of ischemia;while the amplitude was significantly decreased, and reached a minimum at 6 hours of ischemia. Compared with the control groups, in the experimental groups, the latency of sciatic nerve was significantly decreased, but the amplitude was significantly increased. These results suggest that lipoic acid provides neuroprotection against peripheral nerve ischemia/reperfusion injury.
10.Laparnscopic repair for adult inguinal hernia in 512 cases

Objective To summarize the experiences in laparoscopic inguinal hernia repairing for adult patients. Methods Clinical data of 512 hernia cases admitted in our center from March 2007 to Sep 2010 were retrospectively analyzed.There were 437 cases of single-sided hernia,including 281 indirect inguinal hernia,86 direct inguinal hernia,15 femoral hernia,16 combined inguinal hernia and 39 recurrent hernia.There were also 75 cases of double-sided inguinal hernia,including 3 recurrent hernia.There were 41 acute incarcerated hernia cases.The average postoperative follow up time was(29 ± 12) months. Results 507 cases underwent successful laparoscopic repair,and 5 cases were converted to open procedure.There were 238 TAPP and 269 TEP in laparoscopic operations.The average operative time for TAPP was (69 ±19) min,and (58 ±15) min for TEP.The average length of postoperative stay was (5.0 ± 1.5) days.The percentage of resuming normal activity after 2 weeks and 4 weeks were 95.7% (485/507) and 99.0%(502/507).The most common postoperative complications were seroma (9.7%,49/507),transient paresthesia (4.1%,21/507) and chronic pain (0.8%,4/507).The recurrence rate was 0.6% (3/507).Conclusions Laparoscopic repair of inguinal hernia has the advantage of less trauma,faster recovery,and lower recurrence rate.
