4.Small Rice Bowl-Based Meal Plan versus Food Exchange-Based Meal Plan for Weight, Glucose and Lipid Control in Obese Type 2 Diabetic Patients.

BACKGROUND: The Korean National Health and Nutrition Examination Surveys reported 65% of daily energy intake (EI) as carbohydrate (CHO) in the Korean population and main source of CHO was cooked rice. We used a standardized-small sized rice bowl for diet education and investigated its effectiveness on body weight, glucose and lipid, compared to the conventional food exchange system in type 2 diabetes obese women. METHODS: Type 2 diabetic women with body mass index > or = 23 kg/m2 were randomly assigned to small rice bowl-based meal plan (BM) and food exchange-based meal plan (ExM) group. Both groups were asked to reduce their EI by 500 kcal/day for 12 weeks. The macronutrient composition was instructed: 55 to 60% of EI as CHO, 15 to 20% as protein, and 20 to 25% as fat. BM group received only a simple instruction for application of the rice bowl. Nutrient intake was estimated with the 3-day dietary records. RESULTS: Finally, 44 subjects finished the study. The percent reduction of body weight was significant both BM group (-5.1 +/- 2.6%) and ExM group (-4.8 +/- 2.8%) after 12 weeks (P < 0.001) but there was no difference between the groups. There was no difference in the proportional change of CHO, protein and fat in EI between the groups. Additionally, the change of HbA1c and low density lipoprotein-cholesterol were not significantly different between the two groups. CONCLUSION: The BM group was as effective as ExM for body weight and glucose control in type 2 diabetes obese women.

Metastases to the thyroid gland are not frequently observed in clinical practice, although an overall incidence of secondary thyroid malignant tumors has been reported to range from 1.25% to 24% in autopsy series. Generally, patients with metastatic thyroid cancer present with euthyroidism and they do not develop thyroid dysfunction. Thyroid dysfunctions, including hypothyroidism and hyperthyroidism, rarely occur in patients with metastatic thyroid cancer. We describe here a case of hypothyroidism induced by thyroid metastasis from cancer of an unknown primary site in a 53-year-old man and another case of thyrotoxicosis induced by thyroid metastasis from lung cancer in a 65-year-old man.

Esophageal perforation is a rare disease, which require emergent diagnosis and therapeutic procedure. In this paper, we present one case of esophageal perforation by traction of toothbrush which was swallowed during pharyngeal irritation. After the toothbrush was inserted to stomach by gastroscope, gastrostomy was performed for removal of toothbrush and the esophageal perforation was repaired by operation.

8.Effect of Estrogen on the Tumor Necrosis Factor-alpha-induced Apoptosis and Cytokine Gene Expression in MC3T3-E1 Osteoblast.

INTRODUCTION: Apoptosis is a naturally occurring cell suicide pathway. The absence of a survival factor, such as a particular hormone or growth factor, will induce a cell to initiate its own cell death. This process induced in osteoblast is thought to play a role in the pathophysiology of postmenopausal osteoporosis. Estrogen plays an important role and exerts direct effects on bone cells, but the role in the maintenance of cell survival and apoptosis is poorly understood. Based on the importance of survival factors and cytokines in bone remodeling, some studies suggest that estrogen acts on bone cell by modulation of cytokine production that increases bone resorption. However other studies have not investigated the effect of estrogen on the apoptosis of osteoblast cells. OBJECTIVE: To understand how estrogens regulate the apoptosis of osteoblast cells, the physiologically active etrogen metabolite 17 beta-estradiol was tested to determine its effects on the well characterized murine osteoblastic cell-line MC3T3-E1. METHODS: Experiments were designed to identify the effects of estrogen on TNF-alpha induced apoptosis and cytokine gene expression, and collagen synthesis. RESULTS: 1.Within 48 hours of exposure, recombinant murine at 10(-11)M - 10(-10)M TNF-alpha increased the rates of DNA synthesis and 10(-11)~M - 10(-9)M TNF-alpha reduces cell proliferation. 2.Estrogen treatment coordinately increased DNA contents, cell proliferation and collagen synthetic activity. 3.10(-10)M TNF-alpha increased the IL-1beta, IL-6 gene expression. 4.Estrogen attenuated the TNF-alpha- dependent increase in these gene expression. 5.Murine osteoblastic MC3T3-E1 cells underwent apoptosis following removal of serum, or addition of TNF-alpha, as indicated by terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling and DNA fragmentation studies. 6.Estrogen inhibited the apoptosis of osteoblast cells induced by 10(-10)M TNF-alpha but not to the levels observed in those control. CONCLUSION: This in vitro evidence suggests that estrogen might exert at least part of antiapoptotic inflence on the bone cells.

9.Comparison of two topographical airway length measurements in adults.

BACKGROUND: A correct estimate of the tracheal tube insertion depth can prevent complications, including endobronchial intubation and vocal cord trauma. We evaluated a new topographical method for endotracheal tube positioning relative to the carina, using a well-known prior topographical method for comparison. METHODS: One hundred adult (male 50, female 50) patients were studied. The comparison topographic length (in cm) was measured by adding the distance between the right mouth corner and the right mandibular angle to the distance between the right mandibular angle and the center of the sternal manubrium. The new endotracheal tube insertion depth (in cm) was determined by adding the distance between the right mouth corner and the vocal cords, measured with the endotracheal tube itself, to the distance between the thyroid prominence and the manubriosternal joint, and then subtracting 4 cm. After intubation, the endotracheal tube was positioned properly at the right mouth corner and the endotracheal tube tip was evaluated using a fiberoptic bronchoscope at the carina. RESULTS: The distances from the tip of the endotracheal tube to the carina were not significantly different between the methods in the same gender. However, our method allowed endotracheal tube tip placement between 3 cm and 5 cm, above the carina more frequently than the prior method in males. CONCLUSIONS: The new topographical method can be used as a guide to positioning the endotracheal tubes.

10.Cerebral Hemorrhage caused by Phenylpropanolamine.

Phenylpropanolamine (PPA) is the major ingredient of over-the-counter drugs, including diet pills and nasal decon-gestants. We present a case of ICH induced by PPA in diet pills in a 36-year-old woman. She didn't have any medical concerns when she began taking diet pills, containing PPA, since 10 days before admission. A brain CT showed large ICHs on the right basal ganglia and lateral ventricle. Physicians should be alerted to the potential fatal side effects of PPA, in commonly used OTC drugs.
