1.CT and MR Findings of Persistent Hyperplastic Primary Vitreous(PH PV).

PURPOSE: The purpose of this paper is to discuss the characteristic CT and MR findings in persistent hyperplastic primary vitreous(PHPV) and to compare the detectability of those findings in each modality. MATERIALS AND METHODS: We retrospectively evaluated CT and MR findings in 32 patients with PHPV. Twenty-five patients had CT, 13 patients had MR, and 6 patients had both CT and MR. RESULTS: Major findings of PHPV in 32 patients on both imaging modalities were lens deformity(78%), shallow anterior chamber(72%), heterogeneous vitreous opacity(72%), enhancing hyaloid artery or remnant of fibrotic hand(69%), and microophthalmos(67%). Minor findings were retinal detachment(22%), and vitreous hemorrhage(6%). In MRI, lens deformity(92%) and shallow anterior chamber(85%) were detected most commonly whereas in CT, opaque vitreous(80%) was the most common finding. Findings of enhancing hyaloid vessel or remnant of fibrotic band, considered characteristic of PHPV, were more commonly detectable in MR (85%) than CT(52%). CONCLUSION: Characteristic MR and CT findings of PHPV were lena deformity, shallow anterior chanber, heterogeneons vitreons opacity, enhanciny hgalind artery or remnant fibrotic band, and microphthalmos. MR seemed to be more useful than CT in detecting Globe pathology.

5.The effectiveness of glycosylated hemoglobin, fructosamine in the diagnosis of diabetes.

BACKGROUND: With respect to the risk of developing diabetic vascular complications, the central purpose of most screening and detection program, is to identify people with diabetes at early stage so they might have advantages of early treatment to prevent complication of the disease. Oral glucose tolerence test(OGTT) is widely used for diagnosis of diabetes and impaired glucose tolerence, bit the performance of a complete OGTT is not only time consuming and expensive but physically demanding on the individual being tested. Determination of HbA1 and more recently of glycosylated total serum proteins(fructosamine) has been proposed as an alternative method of screening and diagnosis. The aim of our cross-sectional study is to compare the values of fructosamine and HbA1, for the ourpose of diabetes diagnosis with the OGTT as reference method. METHODS: In the study, from January 1996 to August 1996, we included 55 consecutive subjects in Kyeungssang Hospital. Blood samples for HbA1 and fructosamone determination were drawn at the same time as a fasting plasma glucose sample, and then all subjects underwent a standard 2-hour OGTT according to the World Health Organization recom-mandations, The subjects were classified according to the American Diabetes Association classification. RESULTS: In our study, we observed fasting plasma glucose of 145.32+/-75.00mg/deciliter, two-hour plasma glucose of 245.83+/-155.22mg/deciliter, HbA1 of 7.06+/-2.77%, and fructosamine of 308.77+/-128.23 micromol/liter. The correlation coefficient between FPG and HbA1 was 0.9098(p<.05), between FPG and fructosamine 0.7953(p05). between two-hour plasma glucose and HbA1 0.7955-(p<.05), between 2h-PG and fructosamine 0.7770(p<.05), and between HbA1 and fructosamine with OGTT as a reference. After combination of FPG ir 2h-PG and HbA1 especially fructosamine, sensitivity was increased, Receiver operating characteristic curves showed the cutoff point of HbA1 7.0mg/deciliter, of fructosamine 290 micromol/liter. CONCLUSION: As observed in our study, HbA1 and fructosamine were highly correlated with FPG and 2h-PG. Combination of HbA1 or fructosamine and FPG or 2h-PG improve prediction over FPG or 2h-PG alone, especially fructosamine and FPG or 2h-PG combination. We conclude that measurement of HbA1 or fructosamine may be a useful diagnostic test for diabetes.

6.Recurrent Lipofibromatous Hamartoma of the Median Nerve.

Lipofibromatous hamartoma is a rare tumor of the peripheral nerves which is characterised by an excessive infiltration of the epineurium and perineurium by fibroadipose tissue and very few cases have been described and reported in the literature. Surgical treatments of lipofibromatous hamartoma include partial excision, debulking operation, nerve decompression and so on. We report a case of recurrent lipofibromatous hamartoma of the median nerve that partial excision was done previously but causing secondary carpal tunnel syndrome and a review of the literature regarding the etiology, pathogenesis and surgical management of lipofibromatous hamartoma.

7.The "Four-in-One" Procedure for Habitual Patellar Dislocation in Children with Formation Failure of Femoral Trochlea and Generalized Ligamentous Laxity : A Preliminary Report.

Purpose: To evaluate the clinical results of a "Four-in-One" procedure for a habitual dislocation of the patella in children with generalized ligamentous laxity and formation failure of the femoral trochlea. Materials and Methods: Five knees in 4 patients were included in this study. The average age of the patients at the time of surgery was 5.9 years and the subjects were followed up for an average of 41.6 months postoperatively. The clinical results were evaluated using the criteria of the Kujala's scoring system as well as a physical examination and radiological findings. Results: During the follow-up period, there were no recurrent dislocations, knee joint pain, limitations of motion or gait disturbances in any of the cases. The mean Kujala score was 96.8. Two cases had complications related to wound healing but they healed eventually. Conclusion: The "Four-in-One" procedure, which include the lateral retinacular release, medial vector augmentation, semitendinosus tenodesis, and patellar tendon transfer is recommended for a habitual dislocation of the patella in children with generalized ligament laxity and formation failure of the femoral trochlea.

8.Chemical Burn due to a Hair Bleaching Agent.

Several hair dressing procedures, for example, bleaching and dyeing of the hair, are currently being frequently performed for cosmetic purposes. While allergic contact dermatitis due to hair dressing products has often been described, only a few cases of burns caused by hair dressing chemicals have been reported. A 6-year-old girl presented with an 8x6 cm sized round ulcerative lesion on the occipital area after hair bleaching with ammonium persulfate mixed with hydrogen peroxide. The skin biopsy specimen showed epidermal necrosis and sclerosing change in the dermis. She was referred to another burn center and then she was treated with skin grafting by a plastic surgeon. To the best of our knowledge, this is the first reported case of chemical burn due to a hair bleaching agent in the Korean dermatologic literature.

9.Thermal Injury Complicating Improperly Reamed Intramedullary Nailing of the Tibia: A Case Report.

Endosteum and bone marrow thermal necrosis caused by reaming during tibial intramedullary nail insertion, and unskilled operation of soft tissue penestration by reamer resulted in chronic osteomyelitis and soft tissue defect. So, several times of free flaps were done but the result was unsuccessful. At last, the authors performed radical necrotic bone resection and internal bone transport using Ilizarov external fixator. The authors report case with literature review.

10.Analysis of Clinical Types and Retinal Nerve Fiber Layer According to Optic Disc Appearances.

PURPOSE: To evaluate the clinical characteristic of retinal nerve fiber layer (RNFL) and The clinical types of glaucoma according to optic disc appearances. METHODS: Sixty eyes in 60 patients with glaucoma were evaluated retrospectively. Complete ophthalmologic examination, optical coherent tomography (OCT), automated perimetry, stereoscopic optic disc photography and RNFL photography were performed. We classified optic disc pattern into focal, myopic, generalized enlargement, senile sclerotic, and normal appearance types. RESULTS: In 60 eyes with glaucoma, 15 eyes were adult-onset primary open-angle glaucoma (Adult-onset POAG), 22 eyes were normal-tension glaucoma (NTG), 10 eyes were juvenile-onset open-angle glaucoma (JPOAG), and 13 eyes were secondary glaucoma. JPOAG patients were significantly more detected in association with myopic optic disc type (P<0.05). OCT- measured temporal RNFL thickness in focal optic disc pattern were significantly lower than that in myopic optic disc pattern (P<0.05). Age was significantly correlated with temporal RNFL thickness in our study subjects (P<0.036, r=-0.272). CONCLUSIONS: Our study revealed that JPOAG patients may have a relationship with myopic optic disc type. OCT-measured RNFL thickness of glaucoma patients particularly in the temporal quadrant may be related to age.
