1.Pulmonary Lymphangioleiomyomatosis: A case report.

Lymphangioleiomyomatosis(LAM) is a rare disease of women of child-bearing age in which there is progressive hyperplasia of atypical smooth mucle along lymphatics in the lung, and/or axial lymphatics in the thorax and abdomen, resulting in honeycombing of lung. Interestingly there has been a speculation that it represents a forme furste or incomplete expression of tuberous sclerosis complex. This is based on the observation that patients with tuberous sclerosis can manifest pulmonary lesions indistinguishable from LAM. We report a case of LAM occuring in a 39-year-old female, who complained of recurrent pneumothorax, chest pain and shortness of breath. Three years ago, the patient had right nephrectomy under the diagnosis of ruptured angiomyolipoma. A X-ray film of the chest showed honeycombing with a diffusely reticulonodular pattern and cyst-like spaces. She had a characteristic facial appearance of adenoma sebaceum, which her father and uncle had. Microscopically, the lung showed a marked smooth muscle proliferation around the slit-like lymphatic spaces and also some respiratory bronchioles.

3.A Case of Tuberous Sclerosis Diagnosed by Computed Tomography.

Tuberous sclerosis is a rare heredofamilial disease involving multiple organs, classically associated with the clinical triad of adenoma sebaceum, epilepsy, and mental retardation. We have recently experienced a case of tuberous sclerosis, 18-year-old male patient, who had typical adenoma sebaceum and generalized epileptic seizure. Plain skull film showed multiple calcified paraventricular subependymal nodules and brain CT scan disclosed characteristic calcified tubers adjacent to the frontal horn and body of the lateral ventricle.

6.Clinical Study on the Effect of TRH in Patients with Prolonged Mild Disturbance of Consciousness.

Authors report the clinical study on the usefulness of thyrotropin-releasing hormone tartrate(TRH-T) in the treatment of mild disturbance of consciousness. 20 patients suffering head trauma, subarachnoid hemorrhage, and intracerebral hemorrhage were given TRH-T intravenously for ten days. TRH-T was effective for the patients in whom the consciousness disturbance was mild, the duration in fixed consciousness level was short, and the brain was not distorted on CT scan. These features were most prominent in patients with subarachnoid hemorrhage. Three was no significant side effect, and TRH-T turned out to be safe.

7.The Effect of Preceptorship on Clinical Education of Nursing Students.

This study examined effect of preceptorship on clinical education of senior nursing students of Y. University in Seoul. Quasi-experimental study design was used. Sixty students of pre-test and 80 of post-test participated in this study. Competency scores of the students before the clinical education(pre-competency scores) were compared with the competency scores after the clinical education(post-competency scores), and teacher evaluation scores between preceptors and clinical the data from students with preceptors(experimental group, N=22) and with clinical instructors(comparative group, N=58) were also examined. Study results indicated that the total post-competency scores were higher compared to the total pre-competency scores, and the difference was statistically significant. Among 7 sub-domains, differences were all statistically significant in 6 sub-domains except "self-esteem" domain. Competency scores of the students who had clinical education with had clinical education from clinical instructors. But the difference between those scores was not statistically significant. However, students gave higher evaluation scores to preceptors than clinical instructors. This study concluded that preceptorship did not influence much on clinical education at this time yet. However it is expected that when preceptors adjust their new roles and function them in expert education. This study recommended that for the preceptors to increase their motivation to teach students and to accept their roles, systemic rewards are needed.

8.A Case of Chopstick in the Left Optic Foramen without Injury to the Optic Nerve.

A 20-year-old, male patient was admitted to the Department of Neurosurgery, Seoul National University Hospital, complaining of painful swelling of the left orbit. Two days before admission, patient sustained piercing of an aluminium chopstick at his left upper eyelid by his friend accidentally. Fluttered friend tried to pull out the chopstick manually but only broke its proximal portion. On neurologic and ophthalmologic exam, he was fully conscious and free from neurologic deficits at all. His visual acuity was as usual and examination of fundi, extraocular muscle functions, visual field and pupillary light reflex were all within normal limit. Subconjuctival hemorrhage and chemosis were positive findings. Skull X-rays including optic foramen view revealed broken distal part of the chopstick, about 7cm in length, which passed through the left lateral orbit and left optic foramen to reach sellar region across the midline. Emergency Kronlein-Berke's lateral orbitotomy was performed and the chopstick was removed without difficulty. Postoperatively patient showed good recovery and presented quite normal and usual eye function.

10.A Case of Neonatal Altoimmune Thrombocytopenia due to Anti-HLA-B7 +1=160 +B61.

We encountered a case of neonatal altoimmune thrombocytopenia(NAIT) due to anti-HLA-B7+B60+B61. Bilateral cephal hematoma and umbilical hematoma were noted at the time of birth. Purpura developed at the third day. Platelet count was 110,000 at birth and decreased to 66,000/micro liter at the day 4. Prothrombin time and partial prothrombin time were within normal limit. The mother's platelet count was 220,000/micro liter and she had no history of abnormal bleeding. Platelet antibody tests empolying mixed passive hemagglutination and immunofluorescence revealed that the mother's serum was reactive against the platelets from the father and the neonate, but was not reactive with her own platelets. Platelets from eight volunteer group 0 donors were tested with the mother's serum; seven were reactive and one was negative. The positive reactions were lost after chloroquine treatment of platelets. Antigen capture ELISA(ACE) and modified antigen capture ELISA employing monoclonal antibodies against platelet glycoproteins In, IIa, IIb, and IIIa were negative. Mother's serum was tested for lymphocytotoxicity against 49 donor ]ymphocytes and the specificity was found to be anti-HLA-B7+B60+B61. At the 9th day, one unit of platelet concentrate from the mother was transfused and the platelet count of the neonate rose up to 340,000/micro liter. The neonate was discharged at the day of sixteenth and the platelet count remained high thereafter.
