1.Dosimetry of conformal radiotherapy, field-in-field intensity-modulated radiotherapy, and inverse-planned intensity-modulated radiotherapy after breast-conserving surgery for left-sided breast cancer: a comparative analysis

Objective To investigate the differences in dosimetry between conformal radiotherapy (CRT),field-in-field intensity-modulated radiotherapy (FIF-IMRT),and intensity-modulated radiotherapy (IMRT) after breast-conserving surgery for left-sided breast cancer.Methods A total of 31 patients who underwent breast-conserving surgery for left-sided breast cancer were randomly selected,and the plans for CRT,FIF-IMRT,and IMRT were developed.The dose-volume histogram (DVH) was used for self-control study,and the non-parametric test was used to compare the differences in target volume and doses to organs at risk (OARs).Results All the three methods met the requirements of the prescribed doses.The CRT group had a higher V105 of the target volume and higher heart V30 and Dmax (P=0.000,0.000,0.000).The IMRT group had higher V5 and Dmean (P=0.000,0.000),as well as a higher lung V5 and a lower lung V40 (P=0.000,0.000).The FIF-IMRT group had the lowest Dmean (P=0.000),and the IMRT group had significantly higher Dmean and Dmax of the right lung and the spinal cord than the other two groups (P=0.000,0.000,0.000,0.000).The FIF-IMRT group had a significantly lower single hop count than the other two groups (P=0.000).Conclusions CRT has a good dose distribution in the target volume,but greatly injures the surrounding tissues.FIF-IMRT can well protect OARs and cause less damage to the device.
2.Effect of motor imagery and facilitation techniques on the efficacy of rehabilitation for hemiplegic shoulder pain

目的探讨运动想像和促进技术对偏瘫肩痛的康复疗效。方法31例脑卒中后偏瘫患者被随机分成运动想像和促进技术组(治疗组)18例与一般康复治疗组(对照组)13例。采用Partridge方法进行疼痛程度的评估。结果治疗后两组疼痛程度积分均明显下降(P<0.01)。治疗组明显优于对照组(P<0.01)。结论运动想像和促进技术对偏瘫肩痛的康复治疗有一定的疗效。
3.Application and thoughts of team-based learning combined with flipped classroom in physiological teaching

Objective:To explore the application and effect of team-based learning (TBL) combined with flipped classroom in the teaching of physiology.Methods:A total of 70 medical students were selected and randomly divided into two groups, experimental group ( n=34) and control group ( n=36). Both groups studied digestion physiology by different method. In the experimental group, the students were taught by TBL combined with flipped classroom. The control group was given traditional teaching. The students studied and discussed problems in a team, and shared the answers in the flipped classroom. The effects of teaching were evaluated by the final test scores and the self-made questionnaire. SPSS 17.0 was used for t test on data comparison between the two groups. Results:The test scores of digestion physiology in the experimental group were (5.47±1.02) points, which were significantly higher than those of the control group (4.42±1.63) points, with significant differences ( P=0.020). A total of 34 questionnaires were issued and 34 valid questionnaires were collected, with an effective rate of 100%. The questionnaire results showed that TBL combined with flipped classroom was accepted and approved by about 82% of students in the experimental group who agreed that the teaching model helps enhance students' initiative and interest in learning, develop their sense of cooperation and comprehensive application ability. Conclusion:TBL combined with flipped classroom is feasible and effective in the physiological teaching, and it can be popularized in medical courses.
4.Pathogenic detection of infectious diarrhea in patients in a district of Beijing from 2011 to 2013

Objective To analyze the distribution and pulsed-field gel electrophoresis (PFGE)of pathogenic bacteria causing infectious diarrhea in a district of Beijing from 2011 to 2013,and provide basis for tracing infection sources.Methods A total of 1 179 stool specimens of infectious diarrhea from patients in a diarrhea outpatient department from January 2011 to December 2013 were collected,all isolated pathogens were identified by serotyping and PFGE analysis.Results 330 enteric pathogens were isolated from 1 179 specimens,the top 4 bacteria were Shi-gella spp .(28.18%,n=93),Salmonella spp .(20.91 %,n=69),Vibrio parahaemolyticus (13.33%,n =44),and diarrheagenic Escherichia coli (3.33%,n = 11 ).18 Shigella sonnei isolates were identified as 8 PFGE patterns, clustering similarity was close to 88%;69 Salmonella spp .strains belonged to 18 serotypes and 41 PFGE patterns, Salmonella senftenberg and Salmonella enteritidis had dominant patterns;no dominant PFGE patterns were obviously identified among 23 strains ofVibrio parahaemolyticus .Conclusion The serotypes and PFGE patterns of pathogenic bacteria in infectious diarrhea in past three years showed a wide distribution characteristics,the dominant PFGE patterns of Salmonella spp .and Shigella spp .need to be paid more attention,and outbreak of infectious diarrhea caused by Salmonella spp .and Shigella spp .should be alerted.
5.Establishment of a multiplex real time quantitative PCR method for CMV promoter nucleic acid sequences detection

OBJECTIVE To establish and validate a multiplex real time quantitative PCR method for cyto megalovirus(CMV)pro moter nucleic acid sequence detection.METHODS Probes and primers were designed according to CMV pro moter sequence and mouse β-actin house-keeping gene,the a mpli-fication specificity was analyzed using SYBR Green I dissociation curve.The reaction syste m was opti-mized,the sensitivity,linearity and reproducibility of the method were validated.RESULTS Forward primer sequence for CMV pro moter sequence were 5′AGACTTGGAAATCCCCGTGAGT3′;reverse prim-er sequence were 5′CGTATTAGTCATCGCTATTACCATGGT3′;probe sequence were 5′AACCGC-TATCCACGCCCATTGATG3′. Forward primer sequence for β-actin gene were 5′CCTGAG-GCTCTTTTCCAGCC3′; reverse primer sequence were 5′TAGAGGTCTTTACGGATGTCAACGT3′;probe sequences were 5′TCCTTCTTGGGTATGGAATCCTGTGGC3′.Reaction efficiency of the CMV standard curve reached 100%, correlation coefficient reached 0.9978, quantification margin was between 1 .5 ×102 and 1 .5 ×107 copies,and sensitivity of the reaction reached 30 copies.CONCLUSION The multiplex method that could absolutely quantify the copies of CMV pro moter sequence is established.
6.A novel biodegradable polymer-coated cobalt-chromium alloy sirolimus-eluting stent:evaluation in a porcine model

Objective To assess the safety and efifcacy of a novel biodegradable polymer-coated Cobalt-Chromium alloy sirolimus-eluting stent in a porcine model. Methods Bare metal stents (BMS) (n=15), commercial available EXcellstents (n=21), and Cobalt-Chromium alloy PLAG-coated sirolimus-eluting stents (Co-P-SES) (n=21) were implanted in left anterior descending coronary (LAD, n=28)and left circumflex coronary (LCX, n=13), and right coronary artery (RCA, n=16) of 30 mini-pigs randomly. Coronary angiography was performed 28 days, 90 days and 180 days after the stenting procedure to evaluate luminal loss (LL), and then, histomorphology analysis was performed. Results 28 days and 91 days after the implantation, there were no significant differences in LL, neointimal area, Inflammation score and endothelialization score among the three groups. 28 days after the implantation, better endothelialization was observed in Co-P-SES group compared with EXcellgroup under scanning electron microscope. 182 days after the implantation, there were larger lumen area and less neointimal area in Co-P-SES group compared with EXcellgroup under similar internal elastic lamina area[(4.31±0.94) mm2 vs. (2.62±1.17) mm2, P=0.020];[(1.87±0.53) mm2 vs. (0.84±0.41) mm2, P=0.004]with statistical significance. Conclusions The novel biodegradable polymer-coated Cobalt-Chromium alloy sirolimus-eluting stents showed similar safety compared with commercial available EXcellstents. There would be a better potential of the novel stent on inhibiting the neointimal proliferation and endothelialization. However, further preclinical study including long term endpoints and clinical study should be conducted in order to evaluate the safety and effectiveness of the novel Co-P-SES.

In this study, UPLC-MS/MS was adopted to determine the contents of five ephedrine alkaloids (Norephedrine, Norpseudoephedrine, Ephedrine, Pseudoephedrine, Methylephedrine) in plasma and urine in rats after the combined administration of Ephedrae Herba-Gypsum Fibrosum and calculate relevant pharmacokinetic parameters, in order to discuss the effect of the combined administration of Ephedrae Herba-Gypsum Fibrosum on plasma pharmacokinetics and urinary excretion characteristics. According to the results, after being combined with Gypsum, the five ephedrine alkaloids showed similar pharmacokinetic changes, such as shortened t(max), accelerated absorption rate, but reduced AUC(0-t) and V(z)/F, which may be related to the increase in urine excretion. Besides, Gypsum was added to enhance C(max) of Pseudoephedrine and prolong MRT(0-t) of Methylephedrine, so as to enhance the anti-asthmatic effect of Ephedrae Herba and resist the toxic effect of Norephedrine and Ephedrine. This study proved the scientific compatibility of Ephedrae Herba-Gypsum Fibrosum and provided a reference for studies on the prescription compatibility regularity and relevant practices.

8.Determination of notoginsenoside R_1,ginsenoside Rg_1,ginsenoside Re and ginsenoside Rb_1 in Compound Danshen Tablets of different pharmaceutical factories by HPLC

AIM: To establish the method for determing effective components of notoginsenoside in Compound Danshen Tablets(Radix et Rhizoma Salviae miltiorrhizae,Radix et Rhizoma Notoginseng,Borneolum Syntheticum) by RP-HPLC. METHODS: The contents of notoginsenoside R_1,ginsenoside Rg_1,ginsenoside Re and ginsenoside Rb_1 were simultaneously determined by an HPLC system with Kromasil C_(18)(5 ?m,250 mm?4.60 mm),the mobile phase was CH_3CN-0.05%H_3PO_4(CH_3CN 0-12 min:22%;12-20 min:22%-28%;20-60 min:28%-43%).The flow rate was 1 mL/min.The detection wavelength was set at 203 nm. RESULTS: The linear ranges of notoginsenoside R_1,ginsenoside Rg_1,ginsenoside Re and ginsenoside Rb_1 were 0.326-3.260 ?g(r=(0.999 7),) 0.890-8.900 ?g(r=0.999 8),0.144-1.440 ?g(r=0.999 6),0.940-9.400 ?g(r=0.999 8),respectively.Their average recoveries(n=6) were 99.08%,98.36%,97.54% and 96.07%,corresponding RSD were 4.41%,1.64%,2.77% and 1.12%,respectgively. CONCLUSION: The results indicate that the HPLC method is simple,accurate,highly selective and reproducible,thus it could be used as quality control in the preparation of Compound Danshen Tablets.
9.Growth hormone treatment for hypopituitarism after sellar tumor operation

Objective To investigate the effect of growth hormone replacement therapy on life quality of patients with growth hormone deficiency after sellar tumor surgery.Methods The data of 13 patients undergoing sellar tumor resection and suffering postoperative hypopituitarism from Jun.2009 to Dec.2010.were collected.Each of them was given daily 0.5 units growth hormone (about 0.17 mg) as replacement therapy.The life quality of patients at different time points was evaluated with QoL-GDHA scale.SPSS 17.0 software was used for statistical analysis.Results 1,3,and 6 months after treatment,QoL-GDHA score of the patients was 3.91± 2.29,3.38 + 2.43,and 3.23 + 2.28 repectively,significantly lower than before ( 8.69 ± 2.66).The difference had statistical significance (P <0.05 ).Conclusion Growth hormone replacement therapy can significantly improve the life quality of patients with hypopituitarism after sellar tumor surgery.
