Sepsis is a life-threatening organ dysfunction caused by a dysregulated host response to infection. Septic shock is the primary cause of mortality in sepsis, with its core pathophysiological mechanism being severe ischemia and hypoxia in critical units—composed of microcirculation and the mitochondria of functional cells—resulting from disruptions in blood flow and oxygen flow following a dysregulated host response. Due to the systemically convergent yet clinically heterogeneous nature of the host response, current understanding and management strategies for hemodynamics remain inconsistent, often leading to inadequate resuscitation or overtreatment. To improve the quality of care, based on a systematic review of the "blood flow-oxygen flow" theory, an expert panel emphasizes reevaluating septic shock from an integrated perspective of blood flow and oxygen flow, and has formulated the

With the rapid advancement of hemodynamic indices and monitoring technologies, their classification methods and application processes have become increasingly complex. Currently, no unified standard hasbeen established, making it difficult to fully meet the clinical requirements for hemodynamic management. To assist in hemodynamic monitoring assessment and therapeutic decision-making in critically ill patients, the Critical Hemodynamic Therapy Collaborative Group, in conjunction with the Critical Ultrasound Study Group, has jointly developed the Standard for the Application of Hemodynamic Monitoring Techniques in Critical Care. The first part of this standard systematically categorizes hemodynamic indicators into flow indicators, pressure and its derivative indicators, and tissue perfusion indicators, while elaborating on the clinical application of each. The second part establishes a standardized clinical implementation pathway for hemodynamic monitoring. It proposes a tiered monitoring strategy-comprising basic, advanced, indication-specific, and special scenario monitoring-tailored to different clinical settings. It emphasizes the central role of critical care ultrasound across all levels of monitoring and establishes hemodynamic assessment standards for organs such as the brain, kidneys, and gastrointestinal tract. This standard aims to provide a unified framework for clinical practice, teaching, training, and research in critical care medicine, thereby promoting standardized development within the discipline.

Paclitaxel(PTX)is a first-line chemotherapy drug for breast cancer, but its resistance issues significantly impact clinical treatment efficacy.Fucosylation, especially core fucosylation,is closely related to tumor chemoresistance, resulting in poor chemotherapy responses and poor prognosis in patients. In this study,we investigated the effect and mechanism of the fucosylation inhibitor 2-fluorofucose(2-F-Fuc)on the chemosensitivity of paclitaxel-resistant breast cancer MCF-7/PTX cells. The drug resistanceindex(RI)of MCF-7/PTX cells was 8.49 by MTT assays.Western blotting,real-time PCR,enzyme-linked immu-nosorbent assay(ELISA)and Lens Culinaris Agglutinin(LCA)lectin imprinting showed that compared with MCF-7 cells, the expression of FUT8,MDR1and core fucosylation in MCF-7/PTX cells was high.Western blotting showed that 2-F-Fuc had a significant inhibitory effect on the growth of MCF-7/PTX cells, and the expression levels of FUT8 and MDR1 were significantly down-regulated after 2-F-Fuc treatment, and the down-regulation was more pronounced in the PTX and 2-F-Fuc combination group(P<0.05).Compared to the control, expression of PCNA in MCF-7/PTX cells in the PTX and the 2-F-Fuc group were down-regulated,and the apoptosis-related proteins, such as cleaved caspase-3 and Bax/Bcl-2 were in-creased.The level of p-PI3K and p-AKT were down-regulated,and the changes in the combination of 2-F-Fuc and PTX were more robust(P<0.05).The above results showed that the core fucosylation level of MCF-7/PTX cells was significantly increased, and 2-F-Fuc could reduce the core fucosylation level of MCF-7/PTX cells by inhibiting the expression of FUT8,and enhance the sensitivity of drug-resistant cells to PTX,which may correlate with the downregulation of PI3K/AKT signaling pathway proteins.

Objective To explore the changes in neural activity in patients with type 2 diabetes mellitus(T2DM)and their corre-lation with executive function, and to analyze the neural mechanisms underlying the decline in executive function in T2DM patients. Methods Thirty-one T2DM patients(T2DM group)and thirty-two healthy controls(HC)(HC group)matched for body mass index(BMI)underwent resting-state functional magnetic resonance imaging(rs-fMRI)scans and N-back task tests were included.Differ-ences in the amplitude of low-frequency fluctuation(ALFF),regional homogeneity(ReHo),and seed-based functional connectivity(FC)between the two groups were compared, and partial correlation analyses were performed between the difference results and N-back task performance. Results The T2DM group showed prolonged reaction time(RT)in the 1-back and 2-back tasks.T2DM patients exhibited increased ALFF in the bilateral caudate nucleus, left medial superior frontal gyrus, and right postcentral gyrus,as well as elevated ReHo in the right putamen.FC analysis revealed significant alterations in FC between the caudate nucleus, putamen, and multiple brain regions in T2DM patients, with some of these FC changes significantly correlated with RT and accuracy(ACC)in the N-back task. Conclusion The decline in executive function in T2DM patients may be associated with abnormal neural activity in brain regions such as the striatum, salience network, and frontoparietal control network.FC further decreases under increased cognitive load. These findings provide evidence for the study of the neural mechanisms of executive function impairment in T2DM patients.

The objective of this study was to develop a rapid and precise detection technique for por-cine Senecavirus A(SVA)employing reverse transcription recombinase polymerase amplification(RT-RAA)in conjunction with CRISPR Cas13a technology.Additionally, the study aimed to opti-mize the assay's reaction conditions to enhance amplification efficiency.Eight RT-RAA primer sets were designed based on the conserved gene sequence of porcine SVA,and a series of reaction condi-tions were evaluated to refine the RT-RAA reaction system.Subsequently,CRISPR-derived RNA(crRNA)sequences were developed and selected to construct the RT-RAA-CRISPR reaction sys-tem.The method's specificity was determined by examining six prevalent porcine pathogenic nucleic acids, while its sensitivity was assessed using SVA cRNA standards quantified by digital PCR.The method's stability and the consistency of clinical sample analysis were also evaluated. The findings revealed that the optimized RT-RAA and CRISPR reaction systems exhibited the highest amplifi-cation efficiency at a reaction temperature of 37 ℃.Among the eight crRNAs,five were identified as exhibiting the strongest detection signals. The formulated RT-RAA-CRISPR Cas13a method demonstrated exceptional specificity, showing no cross-reactivity with other common porcine disea-ses,including ASFV,PRRSV,PEDV,PCV2,CSFV,and PRV.The method achieved high sensitivi-ty,detecting as low as 0.86 copies/μL of SVA,and exhibited stable fluorescence output, robust re-producibility,and the ability to complete clinical sample analysis within 50 minutes.Consistency e-valuation with six positive and 58 negative samples indicated 100%agreement in outcomes. These results substantiate that the study successfully established a rapid and specific RT-RAA-CRISPR Cas13a detection method for the on-site identification of porcine Senecavirus A,demonstrating high specificity and sensitivity, and holds promise for application in SVA monitoring and control initia-tives.

Objective To discuss the application value of coronary CT angiography(CCTA)evaluation of morphological parameters of coronary chronic total occlusion(CTO)in guiding interventional therapy. Methods A total of 300 patients with coronary angiography-confirmed CTO in Jinhua Municipal People's Hospital from January 2021 to December 2023 were selected. All patients received CCTA examination before surgery. The morphological parameters of CCTA,including proximal morphology of occlusion segment, length of occlusion vessel, length of linear enhancement in occlusion segment, length of linear enhancement in occlusion segment/length of occlusion vessel, vascular linear enhancement in occlusion segment, vascular calcification in occlusion segment, vascular calcification area in occlusion segment ≥50%on cross section image, tortuous path of lesion(>45°),collateral vessels and lesion of vascular opening, were recorded. The correlation between the above parameters and therapeutic results of percutaneous coronary intervention(PCI)was analyzed. Results There were 325 lesions in 300 patients with CTO,and 227 lesions(69.85%)were successfully treated by PCI and PCI treatment failed in 98 lesions(30.15%).The proximal blunt shape of occlusion segment, length of occlusion vessel, vascular calcification area of occlusion segment ≥50%on cross section image, and tortuous path of lesion(>45°)in the failure group were significantly higher than those in the successful group(all P<0.05),while the length of linear enhancement in occlusion segment, length of linear enhancement in occlusion segment/length of occlusion vessel and vascular linear enhancement in occlusion segment in the failure group were strikingly lower than those in the successful group(all P<0.05).No obvious significant differences in other parameters existed between the two groups(all P>0.05).Multivariate logistic regression analysis showed that the length of linear enhancement in occlusion segment(OR=1.975,95%CI:1.306-2.988),the length of linear enhancement in occlusion segment/length of occlusion vessel(OR=3.831,95%CI:1.332-11.017),and the linear enhancement in occlusion segment(OR=1.702,95%CI:1.007-2.879)were the valuable factors for predicting the success of PCI treatment(all P<0.05).Conclusion CCTA evaluation of coronary CTO morphological parameters has a certain value in guiding interventional therapy. The length of linear enhancement in occlusion segment, the length of linear enhancement in occlusion segment/length of occlusion vessel, and the linear enhancement in occlusion segment are the useful factors for predicting the success of PCI treatment.
