4.Bullous presentation of Acrodermatitis Enteropathica in three female siblings: A case series

Acrodermatitis enteropathica is a rare autosomal
recessive disease that results from a defect in zinc
metabolism. It is clinically characterized by a
phenotypic triad of periorificial and acral
dermatitis, diarrhea, and alopecia. Oral zinc
therapy gives a rapid excellent clinical response
and reduces mortality. We report three female
pediatric siblings who presented with periorificial
and acral dermatitis, diffuse alopecia, nail
dystrophy, irritable mood, and stunted growth. A
diagnosis of acrodermatitis enteropathicawas
confirmed with markedly decreased levels of
serum zinc. The patients were successfully treated
with oral zinc sulfate at a dose of 5mg/kg/day for
the first two weeks then maintained on a dose at
2mg/kg/day.

6.Amniotic Membrane Transplantation for Late-Onset Filtering Bleb Leaks.

Successful treatment of late-onset leakage of filtering bleb often requires conjunctival advancement or conjunctival graft with the risk of bleb failure. The authors performed amniotic membrane transplantation in two patients with Seidel-positive late-onset leakage of filtering bleb. In one patient, amniotic membrane transplantation(AMT)was performed twice. In the patient who underwent AMT twice, the bleb became Seidel-negative within 5 weeks of second AMT. However, it became Seidel-positive again within next 7 weeks. In the other patient, the bleb leak was partially resolved within 4 weeks, but became exuberant again within the next 4 weeks. In our experience, AMT had only a limited benefit in treating Seidel-positive bleb leaks. However, its initial success suggests that further studies are necessary to better understand the role of amniotic membrane in bleb leak and to develop a potential therapeutic modality for treating late-onset filtering bleb leak.

9.Low-dose and High-dose of Mitomycin-C in Trabeculectomy.

The purpose of this study is to compare the effects of three different concentrations of mitomycin-C(MMC) as an adjunct therapy to trabeculectomy. We reviewed 26 patients(36 eyes) who underwent trabeculectomy in which MMC was applied intraoperatively. The application time was 5 minute, and concentration of MMC was 0.1mg/ml(n=10), 0.2mg/ml(n=17) and 0.41mg/ml(n=9) respectively. The mean IOP at postoperative 3 months was significantly lower in 0.4mg/ml group (10.4 +/- 5.5mmHg) than in the 0.2mg/ml group(16.1 +/- 4.7mmHg)(p<0.05). There was no statistical difference among the three groups in the success rate regardless of medication, the size and longevity of bleb. The postoperative hypotony(2 eyes) was noted in the 0.4mg/ml group. In conclusions, It is desirable to use the lowest concentration of MMC that will facilitate bleb formation.

10.Immunoelectrophoretic Analysis of Blister Fluids.

The proteins, especially the 19S globulin (IgM) and 7S globulin (IgA) in the CO2 snow-induced blister fluids of 10 male healty volunteers were analyzed immunoelectrophoretically. The fnllawing results were obtained. 1) Compared with autologous serum, all blister fluids tested lacked some protein fraction lines. 2) The 19S globulin (lgM) line was absent in all of the blister fluids except one which was contaminated with blood. 3) The 7S globulin (IgA) line was present in all of the blister fluids when undiluted. In 1 : 2 dilution, however, it was absent, or of trace amount in a few cases.
