1.The Electrophysiologic Diagnosis of Congenital Stationary Night Blindness.

An analysis of four patients with congenital stationary night blindness showed that all had essentially normal fundi. We studied electro physiologic examinations including electroretinogram(ERG), electrooculogram(EOG) and pattern visual evoked potential(VEP). We classified these patients into two groups, one case as Schubert-Bornschein type and three cases as Riggs type based on the evaluation of scotopic and single bright flash ERG. The oscillatory potentials were absent in one case of Schubert-Bornschein type and reduced in two cases of Riggs type. The hereditary mode of three patients in one family was autosomal dominant.

3.Clinical Aspects of 273 Patients with Visual Disability.

PURPOSE: To report clinical features of visually disabled people by newly revised legal blindness criteria. METHODS: A prospective evaluation of all blind registrations was conducted at Dongguk University Hospital from January 2000 to December 2000. Detailed ophthalmologic evaluations were performed and the review of the diagnosis was done in order to find out the cause of the visual disability. RESULTS: There were 273 patients (343 eyes) for newly registered blindness. Of these, 190 (69.6%, 190/273) were men and mean age was 53.8+/-15.48. The sixth degree visual disability was most common (74.0%, 202/273) according to the newly revised legal blindness criteria, and most common cause of blindness was trauma (38.2%, 131/343). Government office advised 122 patients (44.7%, 122/273) to register legal blindness . CONCLUSION: Further nationwide epidemiological survey for legal visual disability is required by standardized classification and criteria.

4.Unilateral Optic Neuropathy by Huge Sphenoid Sinus Mucocele: a Case Report to Emphasize the Importance of Early Detection and Treatment.

A mucocele of the sphenoid sinus is relatively rare, and it has nonspecific clinical features. Hence, it is hard to diagnose it with the history of the illness alone. However, if the sphenoid mucocele is not detected and treated in early stage, its sequalae could be very serious or permanent, such as intracranial complication or blindness. Recently, the authors experienced a case of a sphenoid mucocele with unilateral visual disturbance for a period of one year. The patient underwent endoscopic sinus surgery-marsupialization successfully, but remnant visual disturbance as a sequalae did not fully recover. Thus high index of suspicion, prompt diagnosis using imaging studies, and urgent surgical intervention are crucial for promising of complete recovery from the disease.

5.The Differences in Results of Short Wavelength Automated Perimetry According to Lens Opacities.

PURPOSE: Perimetric examination plays an important role in the diagnosis of glaucoma, which is among the leading causes of blindness worldwide. Several examination methods have been developed for early detection of glaucoma. Among those, short-wavelength automated perimetry (SWAP) is the most widely investigated. In this study, the effects of nuclear opacities on SWAP were examined. METHODS: Lens opacities were divided into cortical and nuclear opacity groups ; each group consisted of fifteen eyes. The eyes studied underwent standard white-on-white perimetry and SWAP exams before and after cataract operation. The differences in the results of the examinations were compared between the two opacity groups. The t test was used for comparing the data. RESULTS: In the cortical opacity group, the average difference between the mean deviation before and after the operation was 5.46dB in white-on-white perimetry and 5.02dB in SWAP. In the nuclear opacity group, the average difference between the mean deviation before and after the operation was 4.01dB in white-on-white perimetry and 10.82dB in SWAP. There was no statistically significant difference in the cortical opacity group (p=0.6575). However, there was a statistically significant difference in the nuclear opacity group (p<0.0001). CONCLUSIONS: In clinical situations, more caution should be taken when interpreting the SWAP results of nuclear cataract patients in order to make an accurate decision.

6.Two Cases of the Metastatic Endophthalmitis.

The endophthalmitis is an ophthalmic emergency and it usually takes a tragic course, so its early diagnosis and appropriate treatment are urgently mandatory to preserve the vision and the eyeball. The authors found metastatic endophthalmitis developed during the course of acute pyelonephritis of two diabetic patients. In spite of aggressive topical and systemic managements, the disesed eyes were brought about to enucleation. The pathogens were proved to be Escherichia coli by culture of the specimen obtained from the vitreous. In accordance with cases and book review, it is emphasized that early recognition and aggressive treatments are essential in prevention of phthisis and blindness.

8.Identification of Pathogenic Variants in the CHM Gene in Two Korean Patients With Choroideremia.

Choroideremia is a rare X-linked disorder causing progressive chorioretinal atrophy. Affected patients develop night blindness with progressive peripheral vision loss and eventual blindness. Herein, we report two Korean families with choroideremia. Multimodal imaging studies showed that the probands had progressive loss of visual field with characteristic chorioretinal atrophy, while electroretinography demonstrated nearly extinguished cone and rod responses compatible with choroideremia. Sanger sequencing of all coding exons and flanking intronic regions of the CHM gene revealed a novel small deletion at a splice site (c.184_189+3delTACCAGGTA) in one patient and a deletion of the entire exon 9 in the other. This is the first report on a molecular genetic diagnosis of choroideremia in Korean individuals. Molecular diagnosis of choroideremia should be widely adopted for proper diagnosis and the development of new treatment modalities including gene therapy.

9.A Case of Ocular Rosacea.

Ocular rosacea is the eye manifestations of rosacea, which involves the lids, conjunctiva, and cornea. A recent study revealed that all patients with cutaneous rosacea had some degree of ocular involvement which is often undiagnosed. It is difficult to make a diagnosis of ocular rosacea because it lacks clinical or pathological hallmarks. However, early diagnosis and treatment are emphasized to decrease morbidity and prevent potential blindness. We report a case of ocular rosacea in a 49 year-old man who showed blepharoconjunctivitis, keratitis, and cutaneous manifestations of persistent erythema, telangiectasia, and papules. The ocular symptoms and signs had improved significantly with tetracycline therapy.

10.Diabetic Retinopathy.

Retinopathy is the most common and vision threatening ocular complication of diabetes mellitus. Epidemiologic studies have described the natural history of diabetic retinopathy, and several well-designed clinical studies have demonstrated the benefits of laser photocoagulation, vitrectomy and tight metabolic control. New therapeutic trials are in progress. But it is one of the major causes of blindness in the adult population, which suggest the importance of early diagnosis and careful follow up examination as well as proper treatment in time.
