1.Treatment of taeniasis with bithionol.

Ten cases infected with Taenia saginata were treated with Bithionol, 2, 2' thiobis (4,6-dichlorophenol) which is well known as an excellent anthelminthhic against paragonimiasis and their follow-up studies were carried on within 3 to 12 months after treatment. Bithionol, 50 mg dose per kg of body weight was divided into two equal dose and administered orally in 30 minutes intervals. Three hours later, this was followed with a sodium sulfate or magnesium sulfate purge. The strobilas without scolices were found in the stools after the medication and the follow-up studies have shown the complete deworming in all cases. It is well tolerated, toxic effects being uncommon and when present, limited to mild gastro-intestinal disturbances.

2.Immunoelectrophoresis for anthelmintics evaluation against experimental paragonimiasis.

Paragonimiasis is one of the most important endemic diseases in Korea. However, recognition of the limitation of microscopic examination of sputum and feces for Paragonimus eggs has led to the investigation of immunoserological techniques for paragonimiasis. The other hand, bithionol preparation has been used as a drug of choice in paragonimiasis but recently niclofolan preparation has introduced as a new agent. By the time, the authors attempted the evaluation of above both agents against experimental rat paragonimiasis by immunoelectrophoresis and Ouchterlony test. The immunoeletrophoresis and Ouchterlony tests were performed according to the method Tsuji and Grabar et Williams respectively, with antigen extracted from lyophilized worm of P. westermani with 0.1 per cemt saline solution. Meanwhile, rats were infected with per os 20 metacercariae of P. westermani above two kinds of serologic tests and were undertaken at biweekly intervals. Then, bithionol was administered every other day for 5 successively for 2 weeks or 6 weeks after infection. Another groups, niclofolan was administered per os single dose same as above. The sera from the rats infected with P. westermani, after treatment with bithionol or niclofolan preparations began to show the precipitin bands against P. westermani antigen 8 weeks after the infection in immunoelectrophoresis and Ouchterlony test, but from 10 or 12 weeks after infection, the number of bands were decreased or disappeared gradually. In genera1, the sera from the rats treated with bithionol or niclofolan showed the precipitin bands delaying 2 weeks than control rat groups. The sera from the rats administered with hydrocortisone showed precipitin bands neither in immunoelectrophoresis nor in Ouchterlony test.

3.An Analysis of 11 Cases of Paragonimus Brain Abscess.

Eleven cases of paragonimus brain abscess which were surgically treated from 1975 to 1978 were reviewed. All patients received supplementary bithionol therapy. Ten patient's clinical condition improved. Only one patient died due to post operative wound infections.

4.Cerebral Paragonimiasis Associated with Right Homonymous Hemianopsia.

A case of cerebral paragonimiasis associated with right homonymous hemianopsia was reported in view of its rarity and a review of the related literaturs was made. The patient was 14 years old man. He had been suffering from poor visual acuity for 2 months and headache for 7 years. He had hsitory of indigestation of crayfish during measle infection. Skull x-ray revealed calcification on temporo-occipital region. He had been treated with bithionol, daily doses of 20mg/kg of body wt. for 70 days. And the rsult was improvement.

5.Mass chemotherapy in the control of paragonimiasis.

Among 11,005 inhabitants(61% of total population) surveyed in 13 villages of Che Ju Island in 1964, were identified as paragonimiasis patients, of these 1,355 were treated with 40 mg/kg body weight of bithionol for 10-15 doses. The purpose of the study was to test whether mass chemotherapy of the human host may provie an answer for the control of paragonimiasis in an area where the human host is main source of infection to the intermediate hosts. The efficacy of the mass chemotherapy as control program was measured by the acceptance of the survey and treatment by the inhabitants, the efficacy and the toxicity of the drug applied, and the influence of the infection rates of the intermediate hosts. The acceptance of the survey and the treatment was not entirely satisfactory: an estimate based on data suggests that only fifty to sixty percent of all cases infected were detected, and 93% of detected cases were treated in the primary program. All cases detected during the follow up survey, which could not cover whole population, were treated. The efficacy of bithionol was excellent when the treatment was completed. 40 mg/kg body weight for 10-15 doses was sufficient to turn the sputum egg-negative except in a few cases infected heavily that needed 20 doses. Side reactions were generally mild; the most frequent reaction was gastrointestinal; which spontaneously subsided within a few to several days. The infection rate of intermediate hosts started to decrease from the third year of the mass treatment with a pattern of gradual drop. Follow up studies on general population in three and four years of the mass treatment also showed markedly decreased prevalence of paragonimiasis by intradermal test and sputum examination. It is concluded that a complete mass chemotherapy with bithionol in an area where human is the main source of infection to the intermediate hosts can be the answer for control of paragonimiasis.

6.A follow-up study to evaluate the efficacy of mass chemotherapy for control of paragonimiasis.

In the survey of l1,005 subjects in l3 villages of Che Ju lsland, 1,450 persons were egg-positive by repeated sputum examinations. The response rate to intradermal test ranged 50-80 per cent of the population and to sputum examination for skin positive 60-70 per cent in the first survey done in l964. 93 per cent of all egg-positive was treated with 40 mg/kg body weight of bithionol on alternate days for 10-15 doses in l965. Follow-up studies were carried out in one year, three-year, five-year, and eight year after the mass treatment. The one year follow-up study covered 86.5 per cent of all treated cases in all villages but only a part of the population of a few villages in further follow-up studies. In this paper only the result obtained from the age group of 7-18 years, the common population studied in every follow-up for the purpose of comparison, is presented. Intermediate hosts of P. westermani were examined at the same time. In one year follow-up it was found that 91 per cent of egg-positive treated became negative. Three parameters, skin reaction, sputum examination, and infection status of intermediate hosts by area and year were used for comparison. Skin positive rate showed gradual decrease with more marked drop in Kang Chung where more thorough survey and mass treatment (about 43 per cent of all infected persons estimated among whole population) were carried out than other areas (only 20-30 per cent). The proportion of intermediate skin reation (60-100 mm(3) wheal size) increased markedly indicating decreased dose of infection which corresponds to the dropped infection rate and average number of metacercaria in crabs. Egg-positive rate in sputum examination done for skin positive reactors also presented similar pattern to skin reaction although it should be taken account that the result of 1964 was from repeated sputum examinations but that of other years from a single examination. Increased proportion of intermediate skin reaction which has much lower egg-positive rate probably played major role in decreased egg-positive rate. The infection rate of snail started to drop about one year earlier than crab; crab infection rate and intensity started to drop about four year after the mass treatment. The duration needed to show the change of infection status in intermediate hosts after the mass chemotherapy may correspond to the duration of survival of the parasite in the intermediate hosts. With the data available presently, it is difficult to determine on what point of the curve the present status is situated (on decreasing phase or increasing phase?), how far the rate will go down if it is on decreasing phase, and how long would it take in returning to the original status without further management. The result of follow-up study, however, strongly indicate that continous and intense chemotherapy may be able to put the paragonimiasis under control. Further follow-up study on total population including survey on changes of environment and eating habits is planned.

7.A Study on Photopatch Test in Photodermatoses.

It is believed that the photopatch test is a valuable screening procedure for the determination of responsible antigenic substances in photoallergic contact dermatitis. But the technigues used for this procedure are cumbersome and require expensive equipment. In the present study, we adapted an easy and inexpensive photopatch test met- hod to our need. We report the results of this test method which was performed. in 21 patients with photodermatitis and 9 patients with polymorphic light eruption. 1. The UVB sensitivity determined by minimal erytherna dose (mean+-standard deviation) was I1618mW sec/cm in photodermatitis group, 108+ROmW sec/cm in polymorphic light eruption group and 126-+32mW-sec/cm in control group. But there was no significant difference among them (p>0. 05). 2. The reaction to UVA were negative in all groups. R. The photopatch test using IO potential photosensitizers revealed 12 positive responses in 9 patients; including 7 patients in photodermatitis group and 2 patients in polymorphic light eruption group. And the number of positive photopatch responses obtaied with each photosensitizer in RO patients with photodermatoses was 5 in chlorhexidine, 2 in paraaminobenzoic acid, musk ambrette and bithionol and 1 in chlorpromazine, respectively.

8.Anthelmintic effects of various drugs against metagonimiasis.

The infection of Metagonimus yokogawai has been known to be present nationwidely and endemic in some parts in Korea. In order to select a highly effective compound against Metagonimus infections, the therapeutic effects were observed in 156 cases who were treated with several anthelmintic i.e. bithionol, niclosamide, niclofolan and praziquantel. All of the above drugs showed highly effective to the treatment of metagonimiasis by a single or two medication days. The egg reduction rates were in the range of 80.2 to 100 per cent. However, relatively low cure rates(8.3 to 37.5 percent) were observed in the groups treated by two doses of 30 mg/kg of bithionol, a single day treatment with 100 mg/kg of niclosamide and two doses of 1.0 mg/kg of niclofolan. On the other hand, the high cure rates were observed at the following dose rates of each drugs, i.e. the cure rate of 66.7 percent was obtained at two doses of 100 mg/kg of niclosamide, 88.9 percent of cure rate was observed at two doses of 2.0 mg/kg of niclofolan and a single dose of 10 and 20 mg/kg of praziquantel were shown as 85.0 percent and 88.0 percent of cure rates respectively. But the complete radical cure was obtained in the group treated by two doses of 20 mg/kg of praziquantel in 10 metagonimiasis cases. From the above results praziquantel was shown as the most highly effective against metagonimiasis and it was well tolerated without any side effect. Therefore, praziquantel is a very promising drug for mass treatment against metagonimiasis.

9.Five cases of Diphyllobothrium latum infection.

Five cases of Diphyllobothrium latum infection were proved by collection of worms after treatment with bithionol, niclosamide or praziquantel, during 1975-1983. The patients were 4 males and one female aged 10-64 years residing in Seoul or Wando-gun, Chollanam-do, Korea. All of them remembered the history of eating several kinds of raw marine fish and 2 persons said they ate the raw flesh of perch, Lateolabrax japonicus. Three among them experienced abdominal pain, 2 natural discharge of a chain of segments, but none revealed any sign of anemia. The average egg size in stool varied 59-67 x 41-48 micrometer according to cases. The eggs were characterized by ovoid to elliptical shape, terminal operculum, and distinct abopercular protuberance. One naturally discharged segment (30 cm) and 4 complete strobilae (320~680 cm) with scolices obtained after treatments were examined. The morphological characters of proglottids such as rosette-form uterus with 3-6 loops, vaginal pore included in the cirrus sac, separated seminal vesicle from cirrus sac etc. were all compatible with D. latum. These are the 7th~11th cases of D. latum infection proved by worms in Korea.

10.A Study on the chemotherapy in clonorchiasis. Report 1. An experimental study on chemotherapy with Dithiazanine iodide and Bithionol sulfoxide in clonorchiasis.

Authors carried out experimental study on chemotherapy with oral administration of Dithiazanine iodide (D.I.) and Bithionol sulfoxide(B.S.) in rabbit clonorchiasis. And the following result was obtained. In change of the E.P.G (eggs per gram feces) by D.I. administration, it was rather increased in early stage of the administration than prior to administration, and thereafter decreased gradually. In the change of the C.S. worm body by D.I. administration, there was not only prominent change of supporting tissue but also the change of reproductive organ was found. In considering the wormicidal effect of D.I. from detecting rate of survival worms, the effect was slight in group of 100 mg(80 mg/kg) per day dosage, but the effects were very excellent and almost complete by proper times of administration in groups of 200 mg (130 mg/kg) per day or higher dosage. But the side effect and intoxication sign of D.I. were appearent in groups of 200 mg or higher dosage. By B.S. administration, E.P.G. was decreased gradually. In changes of the C.S. worm body by B.S. administration, prominent inhibitory chnnge was seen in egg formation ability. Slight wormicidal effect of B.S. was observed in groups of 140 mg (100 mg/kg) per day or higher dosage. Side effect and intoxication sign of B.S. were found little in groups of 140 mg or lesser dosage.
