1.Some biochemical parameters related to blood glucose mutation of total blood storage at the Central Institute of hematology and transfusion

The study was carried out in six stored blood by CPD-A1, at the first, 4th, 14th, 21st, 28th, 35th and 42nd days of the blood stored. The results showed that: The plasma glucose and pH; 2.3-DPG concentration were decreased, plasma lactat was increased gradually during blood stored by time, the difference was significantly than that in the first point of time with p<0.001. The plasma G-6-PD and PK activities were significantly decreased during blood stored by time.

3.Prenatal Serum Marker Screening in Korea: Survey Results.

BACKGROUND: Maternal serum triple marker screening has been covered by the medical insurance system in Korea since December 2004. The number of tests is on the increase, but an external quality control program and a basic survey have not been established yet. The aim of this study was to port the survey of prenatal screening tests. METHODS: Three different quality control specimens were prepared using the sera obtained from 100 women who were in the 15th to 20th week of pregnancy and visited Asan Medical Center during May 2005. We assumed that the three specimens belonged to the first day of 15 weeks, third day of 16 weeks, and second day of 19 weeks, respectively, and sent them to 10 laboratories. Nine laboratories replied to the survey. We analyzed concentrations, multiples of medians (MoMs), and risk estimates. RESULTS: The coefficients of variance of MoM were 32.1-32.6% for alpha-fetoprotein, 15.3-19.8% for unconjugated estriol, 6.3-12.5% for human chorionic gonadotropin, and 12.9-18.2% for inhibin-A. In Down syndrome risk estimation for specimen-2, six of the eight laboratories that used the triple test reported the screen positive, but two laboratories reported negative. Three of five laboratories using the quadruple test reported the screen positive, and two laboratories reported negative. In case of neural tube defect, all laboratories except one reported all specimens the screen negative. In case of Edward syndrome, all laboratories reported all specimens the screen negative. CONCLUSIONS: Since MoMs and risk estimates showed a wide variation among the participating laboratories in this survey, an external quality control and the standardization of the variables seemed warranted.

4.Is post-stroke hyperglycemia a marker of stroke severity and prognosis: A pilot study.

Various physiological parameters like blood pressure, temperature, blood sugar after onset of stroke have been proposed as possible marker of stroke prognosis to study the glycaemic status after acute stroke and assess the role of glycaemic status along with other clinical parameters in influencing stroke outcome. Forty-two confirmed stroke patients attending hospital within 6 hours of onset of stroke onset were included in the study. The time lag for hospitalization, blood pressure, blood sugar, HbA1c, stroke severity according to Toronto Scale, demographic factors, stroke onset type, type of stroke, past history of stroke, diabetes, and hypertension were recorded. The outcome was whether patient survived at the end of forth week. Twenty-one percent of patients who were not known diabetic found to be hyperglycemic though their HBA1C level was normal. Eighty-nine percent of such patients died. This rate was significantly higher than patients known to be diabetic with raised sugar and HBA1c level (26% patients, 12% mortality). There was strong and significant association between stroke severity and poor outcome. Strong and significant association was also found between stroke severity and blood sugar level. Modeling of stroke outcome using decision tree analysis (QUEST) found stroke severity as most important and significant predictor especially for severe stroke cases. In mild and moderately severe stroke, high sugar level was found to be a predictor, though not statistically significant.This study suggests that stroke severity is the most important predictor of stroke outcome, with high sugar level as a marker of stroke severity.

5.Detection of laminin in serum and ascites from patients with epithelial ovarian tumor.

The change in serum laminin (LN) level and its clinical significance in epithelial ovarian tumor were investigated. The LN levels in serum and ascites samples from 69 patients with epithelial ovarian tumor and 42 cases as control group before and after operation were analyzed by radioimmunoassay. The results showed that the serum LN levels in the patients with malignant tumors (157.85 +/- 14.37 ng/ml) were significantly higher than that in the control group (125.14 +/- 7.03 ng/ml) and in the patients with benign tumors (128.36 +/- 8.75 ng/ml) (both P < 0.01) before operation. The serum LN levels in the malignant group were decreased significantly after operation as compared with those before operation (P < 0.05). The serum LN levels in low-differentiated tumors was higher than those in moderate-differentiated tumors and high-differentiated tumors (P < 0.05). The LN levels in ascites (172.94 +/- 15.26 ng/ml) was significantly higher than in serum (161.34 +/- 6.59 ng/ml) (P < 0.05) in malignant tumors. The serum LN levels in the patients with lymph node metastasis (165.41 +/- 19.91 ng/ml) was obviously higher than those without lymph node metastasis (152.35 +/- 10.34 ng/ml) (P < 0.05). It was concluded that LN levels in serum and acistis were remarkably increased in malignant epithelial ovarian tumors, suggesting that LN might be one of important diameters reflecting tumor biological characteristics.

7.Increased Serum Level of P-Selectin in Patients with Lichen Planus.

Lichen planus (LP) is a common, pruritic and inflammatory disease of the skin, hair follicles and mucous membranes. Immunologic mechanisms, especially cell-mediated immunity, play a major role in triggering the clinical expression of the disease. P-selectin is an adhesion molecule present within endothelial cells and mediates endothelial-leukocyte interactions. Therefore, it is considered that P-selectin plays an important role in LP. The aim of our study is to research the relation between P-selectin and LP. Serum P-selectin levels were determined with the enzyme- linked immunosorbent sandwich assay method in sera from 40 LP patients and 40 healthy controls. The serum levels of P-selectin were statistically significantly higher in the patients than in healthy controls (p < 0.05), in female patients (39.32 +/- 11.34 pg/ml) than in male patients (31.93 +/- 9.83 pg/ml) (p < 0.01), and in the patients with eruptive form (40.27 +/- 9.32 pg/ml) than in those with the localised (32.83 +/- 9.93 pg/ml) and hypertrophic (31.72 +/- 8.39 pg/ml) forms (both p < 0.01). In conclusion, we found a meaningful relation between LP and serum P-selectin levels.

8.The Role of Cardiac Biomarkers in the Diagnosis and Management of Patients Presenting with Suspected Acute Coronary Syndrome.

Myocardial infarction (MI) is the leading cause of death in the developed world. Biomarkers have an essential role in diagnosis, risk stratification, guiding management and clinical decision making in the setting of patients presenting with signs and symptoms of MI. Cardiac troponin (cTn) rose to prominence during the 1990s and has evolved to be the cornerstone for diagnosis of MI. The current criteria for MI diagnosis include a rise and/or fall in cTn with at least one value above the 99th percentile of the upper reference limit. Along with cTn, the natriuretic peptides B-type natriuretic peptide (BNP) and amino-terminal proBNP (NT-proBNP) have an important role in determining prognosis and guiding management. As assays for cTn have been evolved that are capable of reliably detecting smaller and smaller quantities in the blood, a dilemma has emerged as to how to use this new information. Several studies have attempted to answer this question and have shown that these lower concentrations of cTn have important prognostic significance and, more importantly, that intervention in these patients leads to improved clinical outcomes. New algorithms incorporating BNP, NT-proBNP, and more sensitive cTn assays hold promise for more rapid diagnosis or rule-out of MI, allowing for appropriate management steps to be initiated and more efficient and effective utilization of healthcare resources.

9.Peripheral serotoninergic response to physical exercise in athletic horses.

The purpose of this study was to evaluate the influence of exercise on plasma tryptophan (TRP) and free serotonin (f5-HT), whole blood-5-HT (WB-5-HT) and f5-HT/WB-5-HT ratio in Italian Saddle horses. Six clinically healthy Italian Saddle horses were subjected to a 450 meters obstacles course. Blood samples were collected from each horse by jugular venipuncture using vacutainer tubes with K3-EDTA at rest, immediately after exercise, and after 30 min. TRP, f5-HT and WB-5-HT were analyzed by HPLC. Immediately after exercise, statistically significant increases of f5-HT (p<0.001) and WB-5-HT (p<0.001) were observed. After 30 min, f5-HT and WB-5-HT decreased compared to immediately after exercise, but were still significantly higher than rest values (p<0.01 and p<0.05, respectively). A significant linear regression between f5-HT and WB-5-HT was observed during experimental conditions. f5-HT and WB-5-HT modifications after exercise suggest an important role of peripheral serotoninergic markers in response to physical activity. The possible source of extra serotonin detected after show jumping should be clarified by further investigation.

10.S100A8/A9 as a biomarker for synovial inflammation and joint damage in patients with rheumatoid arthritis.

S100A8 and S100A9 are major leukocyte proteins, known as damage-associated molecular patterns, found at high concentrations in the synovial fluid of patients with rheumatoid arthritis (RA). A heterodimeric complex of S100A8/A9 is secreted by activated leukocytes and binds to Toll-like receptor 4, which mediates downstream signaling and promotes inflammation and autoimmunity. Serum and synovial fluid levels of S100A8/A9 are markedly higher in patients with RA than in patients with osteoarthritis or miscellaneous inflammatory arthritis. Serum levels of S100A8/A9 are significantly correlated with clinical and laboratory markers of inflammation, such as C-reactive protein, erythrocyte sedimentation rate, rheumatoid factor, and the Disease Activity Score for 28 joints. Significant correlations have also been found between S100A8/A9 and radiographic and clinical assessments of joint damage, such as hand radiographs and the Rheumatoid Arthritis Articular Damage score. In addition, among known inflammatory markers, S100A8/A9 has the strongest correlation with total sum scores of ultrasonography assessment. Furthermore, baseline levels of S100A8/A9 are independently associated with progression of joint destruction in longitudinal studies and are responsive to change during conventional and biologic treatments. These findings suggest S100A8/A9 to be a valuable diagnostic and prognostic biomarker for RA.
