2.Clinical efficacy comparison of breast-conserving surgery and modified radical mastectomy in treatment of early breast cancer

Objective To compare the effects of breast-conserving and modified radical mastectomy of early-stage breast cancer. Meth-ods 80 patients with early breast cancer were divided into two groups according to the different operation methods. The treatment group were given breast-conserving surgery while the control group were given modified radical mastectomy, and the clinical efficacy of two groups were observed. Results The operation time, intraoperatve blood soss, average drainage and the length of hospital stay of the breast-conserving group were significantly lower than those of the control group, and the excellent rate of breast shape in the breast-conserving group was signif-icantly higher with a statistically significant difference (P<0. 05). There was no significant difference between the two groups in the local re-currence rate, distant metastasis rate, 3-year survival rate or 5-year survival rate (P>0. 05). Conclusion The effect of breast-conserving surgery in treatment of early breast cancer is satisfactory, so it is recommending for wide application.
3.Anatomic studies on leaves from three plants of Goniothalamus (Bl.) Hook. f. et Thoms.

Object To study the botanic characteristics of leaves from three plants of Goniothalamus (Bl.) Hook. f. et Thoms. in order to correctly distinguish them from numerous plants of the genus, which are important resource of anticancer medicine.Methods The maceration method and paraffin method were used to study the epidermis and structures of leaves from G. griffithii Hook. f. et Thoms., G. leiocarpus (W. T. Wang) P. T. Li and G. yunnanensis W. T. Wang. Results Three leaves were morphologically similar in the structure, but there were some anatomical differences among them. For example, the absence of druses in the epidermis and the presence of fibrous sclereids in the lamina mesophyll of leaves from G. griffithii, while the presence of druses in epidermis and the absence of fibrous sclereids in lamina mesophyll of the leaves from G. griffithii and G. yunnanensis were observed. In addition, epidermal hairs of G. griffithii were composed of three cells, stomatas were always normal, there were seven oil cells and 25 mucilage cells per mm leaf width in lamina mesophyll and the vascular tissue of the midrib consisting of ten small bundles. However, epidermal hairs of G. yunnanensis were composed of two cells, many abortive stomatas were present at the distal surface, there were only four oil cells and 16 mucilage cells per mm leaf width and the vascular tissue of the midrib consisted of 12 small bundles.Conclusion Three species were easily identified on the basis of epidermal and structural characters of the leaves of them.
4.Interventional therapy for biliary restenosis occurring after surgical T-tube drainage: preliminary investigation

Objective To evaluate the curative effect,safety and feasibility of interventional therapy for biliary restenosis occurring after surgical T-tube drainage.Methods The clinical data of 25 patients with biliary restenosis that occurred after surgical T-tube drainage,who were admitted to authors' hospital during the period from June 2014 to March 2016,were retrospectively analyzed.The primary diseases included bile duct carcinoma (n=6),gallbladder carcinoma (n=3),biliary stone (n=13),hepatocellular carcinoma (n=2)and gastric cancer after surgery (n=1).Abnormal junction of pancreatic duct and biliary duct was observed in 4 patients.Interventional procedure via T-tube route was carried out in 22 patients,and T-tube radiography with subsequent percutaneous transhepatic cholangial drainage (PTCD) was conducted in 3 patients.Biliary balloon expansion combined with biliary drainage was performed in 21 patients,and biliary metal stent implantation was adopted in 4 patients.For patients with benign biliary stricture,the drainage tube was retained for 2-3 months before it was removed.All the patients were followed up for 3-24 months at outpatient clinic or by the telephone.The curative effect was evaluated with drainage-tube radiography.Results The interventional operation was successfully accomplished in all patients,no procedure-related complications occurred,the technical success rate was 100%.In 15 patients with benign biliary stricture,biliary plasty with balloon expansion via the T-tube fistula was conducted,then a 10.2-12 F drainage catheter was placed in the biliary tract and the T-tube was pulled out.During the follow-up period,one patient with anastomotic stricture of bile duct carcinoma died of pulmonary infection at 8 months after treatment.Of the 10 patients with malignant stricture,the biliary obstruction was located above the T-tube level in 3,and all the 3 patients received PTCD.Among the 3 patients,2 patients had hepatocellular carcinoma complicated by biliary invasion,as the extent of the cancerous thrombus was very large,both internal drainage tube and external drainage tube had to be implanted.After jaundice regression,the two patients died of hepatic failure at one month and 2.2 months after the operation respectively.One patient with gallbladder carcinoma complicated by invasion of bile duct received implantation of biliary stent,and the patient died of tumor deterioration at 2.5 months after the procedure.In 7 patients,the biliary obstruction was located below the T-tube level.hnplantation of internal drainage tube and external drainage tube via the Ttube fistula was performed in 4 patients,and implantation of metal stent was adopted in 3 patients.Among them,2 patients with gallbladder carcinoma died of tumor deterioration at 3.8 months and 5 months after the operation respectively.In 5 patients with cholangiocarcinoma,biliary stent restenosis occurred in 2 at 3 months after the treatment,and PTCD was adopted.Three patients died of tumor deterioration complicated by organ function failure at 3.6 months,5.2 months and 9.0 months after the operation respectively.Conclusion For the treatment of biliary restenosis occurring after surgical T-tube drainage,interventional therapy is safe and feasible with reliable curative effect,it can significantly improve the life quality of patients.
5.Application of"Hong's One Stitch Method"in pancreaticoduodenectomy

Objective:To investigate the effect of Hong's One Stitch Method in pancreaticoduodenectomy(PD).Methods:A total of 40 patients who underwent PD in our hospital from Jan 2021 to Dec 2022 were divided into two groups according to random number table method,with 20 patients in each group.The control group was treated with end to end pancreatojejunal anastomosis,and the observation group was treated with"Hong's One Stitch Method".The perioperative indicators,complications,secondary surgery,mortality and quality of life were compared between the two groups.Results:The pancreatoenteroanastomosis time,operation time and hospitalization time in the observation group were shorter than those in the control group,and the incidence of pancreatic fistula was lower than that in the control group(P<0.05).There were no significant differences in intraoperative blood loss,pancreatic biochemical leakage,bile fistula,hemorrhage,localized abdominal infection,gastric emptying obstruction,pulmonary infection,secondary surgery and mortality between the two groups(P>0.05).The mental health score,emotional function score,social function score,energy score,general health status score,body pain score,and physiological function score in the observation group were higher than those in the control group(P<0.05).Conclusion:In PD surgery,the application of"Hong's One Stitch Method"to perform pancreatoenterostomy is beneficial to shorten the pancreatoenterostomy time,operation time and hospitalization time,accelerate the postoperative recovery,reduce the incidence of pancreatic fistula,and improve the quality of life of patients.
6.Evaluation of a multi-level quality control system in health management center setting

Objective:To explore the application and effect of multi-level quality control system (referred to as “quality control”) in health management center setting.Methods:The health management center of Hanzhong Central Hospital constructed a multi-level quality control system of “hospital-department-unit” and “department-unit-quality-controller” in August 2019. A total of 83 619 people who underwent physical examination in the Health Management Center of Hanzhong Central Hospital from August 2018 to July 2020 were selected as the subjects. 32 009 people who underwent physical examination from August 2018 to July 2019 were selected as the control group, and 51 610 people who underwent physical examination from August 2019 to July 2020 were selected as the experimental group. The timely notification of important abnormal results and the follow-up of the “four-high” population (hypertension, hyperglycemia, hyperlipidemia, and hyperuricemia) were observed. Two thousand satisfaction questionnaires and two thousand physical examination reports were collected from the institutions who had received both physical examination in the Health Management Center of Hanzhong Central Hospital for two consecutive years. The physical examination items of the above clients were basically the same. The satisfaction rate of the two groups of physical examination and the qualification rate of the physical examination reports were measured respectively.Results:The results showed that the timely notification rate of important abnormal results (99.4% vs 96.6%), follow-up rate of “four-high” population (hypertension 95.1% vs 91.2%, hyperglycemia 95.3% vs 91.6%, hyperlipidemia 94.6% vs 92.3%, hyperuricemia 92.7% vs 86.4%), satisfaction rate of physical examination (physical examination environment 94.0% vs 91.3%, service attitude 96.4% vs 91.9%, waiting time 97.6% vs 95.4%, physical examination process 98.3% vs 96.8%, professional level of medical staff 97.2% vs 95.1%), and qualified rate of physical examination report (accuracy of input information 99.5% vs 98.1%, accuracy of main examination conclusion 99.4% vs 97.3%, normative sorting 99.8% vs 98.8%, rationality of health advice 99.2% vs 96.8%) in the experimental group were significantly higher than those in the control group (all P<0.05). Conclusion:The establishment of a multi-level quality control system in health examination service can improve the timely notification rate of important abnormal results, the follow-up rate of the “four-high” population, the satisfaction of physical examinees, and the qualified rate of physical examination reports.
7.Genetic polymorphism of eight STR loci in the Han population in Henan province

Objective To investigate the allele frequencies of eight short tandem repeats(STR) loci:TH01,FES, D19S400,D7S820,D16S539,D20S161,D3S1545 and D5S818 in Han population in Henan province.Methods DNA was extracted with phenol-chloroform from EDTA-blood samples of the unrelated individuals in Henan province and amplified with PCR technique. The PCR product was analyzed with the undenatured PAGE vertical electrophoresis and silver-stain.Results The authors got the frequencies of the eight loci. The heterozygosities of the eight loci are 0.66, 0.67, 0.80, 0.76, 0.79, 0.79, 0.78 and 0.78; the discrimination powers are 0.83, 0.83, 0.94, 0.91, 0.93, 0.93, 0.92 and 0.92.Conclusion The heterozygosities of the eight loci are high and the frequencies are in good agreement with Hardy-Weinberg equilibrium, so the eight loci can be used in idividual identification testing.
8.Porphyromonas gingivalis FimA genotype distribution among periodontitis patients with type 2 diabetes

Objective To investigate the distribution differences of Porphyromonas gingivalis(Pg) FimA genotypes between periodontitis patients with and without type 2 diabetes for a better understanding of the relationship between diabetes and periodontitis.Methods Questionnaires and detailed periodontal examinations(6 sites per tooth) were conducted in 80 subjects with moderate-severe chronic periodontitis in the Department of Periodontology, Peking University School and Hospital of Stomatology.There were 40 type 2 diabetic patients and 40 systemicly healthy patients enrolled respectively.The periodontal parameters including plaque index(PLI), probing depth(PD), bleeding index(BI), attachment loss(AL) by 6 sites per tooth and numbers of missing teeth were also recorded.Pooled subgingival plaque samples using pocket method with Whatman No3 filterpaper were collectedat each of the 6 sites from one incisor and one molar.Pg and its FimA genotype distributions were investigated using DNA extrctedfrom plaque samples by PCR.Results Diabetic patients had a significantly higher score of PLI[2.35(0.58) vs 1.64(0.76), P<0.05] , while rest of periodontal indexes observed(PD, BI and AL) did not differ significantly between diabetic patients and systemicly healthy controls(P>0.05).The detection rate of Pg did not show statistically significant difference between the two groups(50% vs 60%, P>0.05).However, the proportion of FimA Ⅱ was significantly higher in diabetic group than systemicly healthy group(80% vs 42%, P<0.05).Conclusions Type 2 diabetic patients were prone to be infected by highly virulent strains of Pg: FimA Ⅱ Pg.
9.Study of metabonomics on pharmacological action appraisal Rhizoma coptidis in rats.

OBJECTIVEHPLC-MS/MS-based metabonomics method was used to find the possible biomarker of Rhizoma Coptidis in rat urine.
METHODSprague-Dawley rats were successively administrated 7 g x kg(-1) aqueous extract of Rhizoma Coptidis for 30 days, urine were collected by metabolism cages and detected by using the HPLC-MS-MS. All dates were analyzed by the principal component analysis (PCA) through using the SIMCA-P 10.0 software.
RESULTThe PCA demonstrated that the metabolome between treated group and control group had difference in rat urine sample after of 22 days administrated, for treated group 169 kinds of biomarkers were found including oxalacetic acid, malic acid, 2-ketoglutaric acid, NE, arachidonic acid, 5-HIAA and other compounds, the result was consistent with pharmacological effects of R. coptidis, such as antiinflammatory, inhibiting biosynthesis of CA biosynthesis, anticentral nerve and energy metabolism inhibition.
CONCLUSIONMetabonomics may be available in pharmacological action evaluation of drugs.

10.Expression and clinical significance of SMYD3 in rectal cancer

Objective:To study the expression level and clinical significance of SET and MYND domain-containingprotein 3(SMYD3) in rectal cancer tissues.Methods:Immunohistochemistry and Western blot were used to detect the expression of SMYD3 in 64 cases of rectal cancer tissues and corresponding adjacent tissues. The relationship between SMYD3 expression and clinicopathological characteristics and prognosis of rectal cancer were analyzed.Results:The expression rate of SMYD3 in rectal cancer tissue was 60.94%, which was higher than that in adjacent tissues by 14.06%, and the difference was statistically significant (χ 2=30.00, P<0.05). The expression of SMYD3 in rectal cancer tissue has no correlation with the patient's age, gender, and degree of tissue differentiation (all P>0.05), but has correlation with TNM staging, depth of invasion and lymph node metastasis (all P<0.05). The prognosis of the rectal cancer patients with positive expression of SMYD3 was worse than that with negative expression ( P<0.05). The expression of SMYD3 protein in rectal cancer tissues (1.26±0.31) was significantly higher than that in the corresponding adjacent tissues (0.55±0.22) ( t=11.95, P<0.05). Conclusions:The increased expression of SMYD3 is related to the occurrence and development of rectal cancer, and its expression level can be used as an index for judging the malignancy of rectal cancer and evaluating its prognosis. The results of this study show that SMYD3 is involved in the occurrence and development of rectal cancer. The results of this study can provide a basis for SMYD3 as a new target for rectal cancer targeted therapy and a prognostic criterion.
