1.Research advances in children with lung transplantation at home and abroad

Lung transplantation is an effective treatment for end-stage lung diseases in children, and the only treatment for children with no response to conservative treatment.However, merely a few transplantation centers at home and abroad can provide lung transplantation at present.The mortality of pediatric lung transplantation is high, and it is difficult for donor matching.Besides, it has many complications.In recent years, pediatric lung transplantation has made positive progress and technological breakthroughs in many aspects.In this article, research progress in pediatric lung transplantation at home and abroad was reviewed.
2.CT Features of Extra-adrenal Ganglioneuroma:Comparison with Pathological Findings

Purpose To explore the CT features of extra-adrenal ganglioneuroma (GN), and to improve the diagnostic accuracy of the disease. Materials and Methods CT features of 13 patients with extra-adrenal GN confirmed by surgical pathology were retrospectively analyzed, and compared with pathological findings. Results Two of 13 lesions were located in neck, four in posterior mediastinum, four in retroperitoneal space, two in front of sacrum, one in the region of renal hilus. All these masses were well defined, two appeared as round, three as oval, 8 as irregular shape. Of eight cases with irregular shape, six lesions presented a tendency of wedging into the space encasing and lapsing the vessel, with no evidence of invasion of the surrounding organs or vessels. Two tumors grew along the vertebrae and invaded intervertebral foramen. On plain CT imagings, the attenuations of the tumors were less than that of muscle, the masses with homogeneous and nonhomogeneous density were found in four and nine cases, respectively. Two patients associated with scattered and patchy cafilication. After contrast administration, eight lesions showed inhomogeneous enhancement:slight enhancement were seen in the arterial phase and gradually more strong enhancement during the portal venous and delayed phase. Enhancing lines or patchy were observed inside these tumors. Pathologically, the tumors appeared on a large amount mucus background, a few of scattering or nesting ganglion cells were distributed in many mature spindle cells, which lined in weave or fasciculation. Conclusion CT manifestations of extra-adrenal GN have certain characteristics, which are correlated closely with pathological features, therefore is helpful for the diagnosis and differential diagnosis.
3.Climical perception for 100 cases valvular heart disease operated at the basic level hospital

Objective To summarize the experience of surgical treatment for 100 patients with valvular heart disease.Methods 100 valve patients were operated on from June 2002 to July 2006.The heart function(NYHA) was class Ⅱ in 24,class Ⅲ in 54,class Ⅳ in 22 including 7 patients with sever cardiac failure.74 patients had rheumatic disease,19 congenital disease,2 cases with SBE.7 aortic vavle degenerative disease including 1 case with ascending aortic dissection aneurysm,61 atrial fibrillation,10 left atrium thrombus and 3 coronary heart disease.Results There were low output syndrome in 14,including 2 death,4 reoperation for postoperative bleeding and 1 gasembolism in brain stem.1 case needed reoperation for peri-prosthetic leak half a year after discharge.In the follow-up period,91 cases of these patients were in NYHA class Ⅰ,4 cases were class Ⅱ,5 cases were recovering.Conclusions Result of vavle replacement surgical treatment is satisfactory.Pay attention to preparation before operation,accurate protection for myocardium and correct repair.Tricuspid repair should be alse considered actively.Eliminating gas in left heart before re-beat is important.

5.Clinical efficacy of Frey and Beger procedures for chronic pancreatitis

Objective To analyse clinical efficacy of Frey and Bger procedures for chronic pancreatitis. Methods Clinical data of seventy patients of chronic pancreatitis undergoing Frey (32 cases) and Beger Procedures (38 cases) in our hospital from January 1999 to June 2009 were retrospectively analyzed,the rate of pain relief and endocrine and exocrine function of the pancreas in longterm follow-up were compared with each other. Results Overall perioperative morbidity for Frey group and Beger group was 12. 5% and 7.9% respectively (P =0. 52); Total pain relief rate in long-term follow-up was 96. 8% and 94. 7% respectively (P =0. 57). Postoperative de novo diabetes mellitus was 12. 5% and 21% ( x2 = 0. 90, P = 0. 34), Rate of steatorrhea was 12. 5% and 18.6% respectively ( x2 = 0. 15, P =0. 70). Conclusions Frey and Beger procedures were equally effective in improving symptoms of chronic pancreatitis and there were no significant differences in perioperative morbidity, rate of pain relief and function of endocrine and exocrine of the pancreas.
6.Infuening factors and measures of evidence-based nursing on the success rate of venipuncture for children

Objective To explore the effect of evidence-based nursing of the success rate of venipuncture for children and summarize the strategies . Method Three hundred and sixty children receiving intravenous therapy were investigated by self-designed questionnaire to explore the influencing factors of success veinpuncture. Results The unsuccessful rate of veinpuncture was 22.2%(88/360). And the main influencing factors included children′factors which accounted for 42.1%, nurses′factors which accounted for 29.4%, parents′factors which accounted for 18.1%and environment factors 10.4%. Conclusion The following strategies can be effective in increasing the success rate of veinpuncture, such as choosing the appropriate veins, improving the veinpuncture skills, creating favorable treatment environment and doing psychological nursing well.
7.Effects of dietary fiber and inositol hexaphosphate on the onset of carcinoma of large intestine in rats induced by 1,2-dimethylhydrazine

BACKGROUND: Incidence rate of carcinoma of large intestine increases with the development of national living standard. Many epidemiologic surveys have showed that dietary factors have closely correlation with high risk of carcinoma of large intestine.OBJECTIVE: To observe the effect of dietary fiber and inositol hexaphosphate on the incidence of carcinoma of large intestine by establishing rat models with carcinoma of large intestine.DESIGN: Randomized block design.SETTING: Institute of Nutriology, Medical College, Qingdao University.MATERIALS: The experiment was performed at the Institute of Nutriology, Medical College, Qingdao University from March to December 2004.Totally 86 male Wistar rats of 4 weeks were randomly assigned based on body mass into cellulose group with 14 rats, pectin group with 14 rats, inositol hexaphosphate group with 15 rats, cellulose plus inositol hexaphosphate group with 14 rats, pectin plus inositol hexaphosphate group with 14rats and control group with 15 rats.METHODS: Basic feed without dietary fiber was given in the control group. 10% pectin was added in the pectin group. 10% cellulose was added in the cellulose group. 2% sodium phytate was added in the water of inositol hexaphosphate group. 10% pectin and 2% sodium phytate were added in the water of pectin plus inositol hexaphosphate group. 10% cellulose and 2% sodium phytate drinking water were added in the cellulose plus inositol hexaphosphate group. A total of 86 rats received 1,2-dimethylhydrazine (DMH) by subcutaneous injection to induce carcinoma of large intestine at week 4 after breeding. Incidence rate of large bowel neoplasm,number and volume of tumor were observed. Proliferating activity of large intestinal mucosa cells was determined (the ratio of number of positive cells of proliferating cell nuclear antigen to total count of nucleus).MAIN OUTCOME MEASURES: ①Incidence rate of large bowel neoplasm in each group, changes of mean number and mean volume of tumors of each rat, and ②proliferating activity of large intestinal mucosa cells of rats.RESULTS: ①Most of the rats in each group died during the 20-week trial. Oue rat died in the pectin group, the pectin plus inositol hexaphosphate group and the control group, respectively after 20 weeks. ②There was no significant difference in the incidence rate of large bowel neoplasm of rats compared with the control group (P > 0.05), but the mean number and mean volume of tumors of each rat in the inositol hexaphosphate group were significantly lower than those in the control group [quantity: (1.1±0.2),(4.1±1.2)rats,P < 0.01 ;volume: (176.1±65.5), (1 046.7±469.0)mm3,P < 0.05], and the mean number of tumors of each rat in the pectin plus inositol hexaphosphate group was obviously higher than that in the control group [(7.5±1.9), (7.2±1.0) rats,P < 0.05]. ③The proliferating activity of large intestinal mucosa cells in the inositol hexaphosphate group significantly decreased as compared with the control group. The proliferating activity of large intestinal mucosa cells in the pectin group increased as compared with the control group [(41.8±4.7)%, (83.6±2.9)%,(66.7±7.8)% ,P < 0.01 and 0.05].CONCLUSION: It may increase the incidence rate of carcinoma of large intestine when pectin is added in diet, on the contrary, the rate decreases if inositol hexaphophate is added in the water.
8.Relationship between endoplasmic reticulum stress and diabetes mellitus-caused influence on cardio-protection induced by remifentanil postconditioning in rats

Objective To evaluate the relationship between endoplasmic reticulum stress and diabetes mellitus (DM)-caused influence on cardioprotection induced by remifentanil postconditioning in rats.Methods Adult male Sprague-Dawley rats,weighing 250-300 g,were used in the study.A model for type 1 DM was established by intraperitoneal streptozotocin 50 mg/kg and confirmed by blood glucose ≥ 16.7 mmol/L.Thirty six rats with type 1 DM were randomly divided into 3 groups (n =12 each) using a random number table:sham operation group (DM-S group),myocardial ischemia-reperfusion (I/R) group (DM-I/R group) and remifentanil postconditioning group (DM-R group).Another 36 normal rats were exposed to single intraperitoneal injection of sodium citrate-hydrochloric acid buffer solution and served as control group.Two weeks later 36 normal rats with nondiabetes mellitus were also randomly divided into 3 groups (n =12 each) using a random number table:sham operation group (NDM-S group),myocardial I/ R group (NDM-I/R group) and remifentanil postconditioning group (NDM-R group).Myocardial I/R was produced by 30 min occlusion of left anterior descending branch of coronary artery followed by 120 min reperfusion.Remifentanil postconditioning was induced by 10 min infusion of remifentanil 10 μg · kg-1 · min 1 via the femoral vein starting from 5 min before reperfusion.Before ischemia and at 30 and 120 min of ischemia,MAP,SP and HR were recorded and rate-pressure product (RPP) was calculated.At 120 min of reperfusion,arterial blood samples were collected for measurement of plasma cardiac troponin I (cTnI) concentration.The animals were then sacrificed and hearts were removed for determination of myocardial infarct size (IS).The left 6 rats from each group were sacrificed at 120 min of reperfusion,the specimens from their left ventricular apex were obtained to detect the expression of endoplasmic reticulum stress marker glucose-regulated protein 78 (GRP78),C/EBP homologous protein (CHOP) and caspase-12 by Western blot.Results MAP and RPP were significantly decreased,the plasma concentration of cTnI was increased,changes of cardiac infarction were found,and the expression of GRP78,CHOP and caspase-12 was up-regulated in diabetic and nondiabetic rats.Remifentanil postconditioning could inhibit the expression of GRP78,CHOP and caspase-12,increase MAP and RPP,decrease the plasma concentration of cTnI,and reduce myocardial infarct size in nondiabetic rats,but it had no such effects in the diabetic rats.The expression of GRP78,CHOP and caspase-12 was significantly higher after remifentanil postconditioning in diabetic rats than in nondiabetic rats.Conclusion Enhanced endoplasmic reticulum stress is involved in DM-caused loss of cardioprotection induced by remifentanil postconditioning in rats.
9.Correlation study on behavioral problems and self-concept of children with oppositional defiant disorder

Objective To investigate the behavioral problems and self-concept of children with oppositional defiant disorder (ODD) and the correlation between them. Methods The subjects came from a sample survey of primary and middle school students' mental status in 6 regions in Hunan province. Achenbach child behavior checklist (CBCL) was filled out by parents. The Children's self-concept scale (CSCS) was completed by children themselves. Results There were significant differences between the scores of ODD group and the normal control among all CBCL subscales except the activities scale, and the scores for the CSCS scales were all lower in patients with ODD than the control group except for the physical appearance and attributes (P<0.01). In addition, according to correlation analysis, the CBCL scores yielded small but significant correlations with the CSCS scores for children with ODD(P<0.05). Conclusion Children with ODD have generally behavioral problems, poor social competence, and lower self-concept, whose behaviors were correlated with their self-concept.
10.Quantification of Organ Fat Deposits in Patients with Type 2 Diabetes Mellitus by Magnetic Resonance Imaging

Purpose Lipotoxicity plays an important role in the progression of diabetes and its complications,and the gold standards for organs' lipid quantification are biopsy or histochemical staining,which are invasive and have their own limitations.Thus,our research was to identify the difference in organ lipid deposition between type 2 diabetic patients and healthy volunteers by using noninvasive three-point T2* corrected Dixon imaging.Materials and Methods Sixty-five type 2 diabetic patients and 34 healthy volunteers were included in this study.All participants underwent three-echo Dixon with 3.0T MR imager.Dixon imaging parameters [hepatic fat fraction (HFF),splenic fat fraction (SFF),pancreatic fat fraction (PFF)] were collected.All of the MRI parameters were compared.The relationship between HFF,SFF,PFF and BMI,age were analyzed.Results The hepatic and splenic lipid percentage in diabetic group [HFF=(5.4±4.3)%;SFF=(3.7± 1.4)%] was significantly higher than healthy volunteers group [HFF=(2.9± 1.3)%;SFF=(3.0± 0.9)%;P<0.05,respectively].However,there was no significant difference in pancreatic fat fraction between diabetic and healthy groups (P>0.05).There were positive correlations between HFF,SFF,PFF and BMI (r=0.379,0.305 and 0.306,P<0.05).Moreover,only the positive correlation between pancreatic fat fraction and age were observed (r=0.261,P<0.05).Conclusion The three-point T2* corrected Dixon revealed the abnormalities of hepar and spleen lipid accumulation in diabetic patients.Thus,the three-point Dixon imaging may potentially aid in evaluating the lipid deposition of abdominal organs.
