Purpose:To study the surgical treatment of the cases with cancer of esophageal and gastroesophageal conjunction by thoracotomy. Methods:Total of 43 patients were selected. Squamous cell carcinoma of esophagus were found in 26 patients, among them 23 received radical resection and 3 failed due to massive invasion of the tumor. The rest were diagnosed as adeno carcinoma of cardia,13 of them undergoing radical resection and 4 failed. Results:The results of the whole group were surgical resection 93.0 %,complication 41.9% and operative mortality 4.7%.Conclusions:The authors conclude that selecting patients with precise indications as well as treating complication correctly was very important for surgical procedures of elderly patients. The pre-operative preparation, nutrition and approach for resection are also important.

Objective To investigate the association between single nucleotide polymorphisms in the intron 1 of thyroid stimnulating hormone receptor gene (TSHR) and Graves' disease (GD) in the Chinese Han population from Linyi city,Shandong Province. Methods A total of 1759 GD patients and 1740 control subjects were recruited for genotyping in TSHR intron 1 with genome-wide association study (GWAS) and Taqman probe technique. At the same time, serum thyroid hormone and TSH receptor antibody (TRAb) levels of patients were determined. Results Five SNPs were selected for further replication. The rs12101261 _T was significantly associated with GD risk ( OR=1.257,95%CI 1.137-1.390,P =8.23 × 10-6 ). Logistic regression identified that rs12101261 was an independent susceptibility locus of GD ( P=1.61 × 10-3 ).Furthermore,rs12101261 _T was strongly associated with GD ( OR =1.317,95% CI 1.171-1.481,P=4.14× 10-4 ) in TRAb positive patients, but no association in TRAb negative patients ( OR=1.056,95% CI 0.892-1.251,P=0.524 ).Serum TRAb concentration showed remarkable difference among three genotype groups of rs12101261.Conclusions Five SNPs in TSHR intron 1 are associated with GD.rs12101261 contributes to increased GD risk independently and is associated with serum TRAb level.

Previous studies suggest that NGAL (neutro phil gelatinase-associated lipocalin) is involved in the transformation and development of esophageal carcinoma. Alteration of NGAL expression can trigger the change of cellular morphology in esophageal carcinoma cells. However, the mechanisms remain unclear. To get a better understanding of NGAL function in esophageal carcinoma, NGAL protein was expressed in methylotrophic yeast, Pichia pastoris, and purified by chromatography. EC1.71 cells expressed high levels of NGALR (NGAL receptor) and EC109 cells expressed low levels of NGALR were used as cells model. The trafficking and the possible function of NGAL protein were then analyzed in the esophageal carcinoma cells. The results showed that 5-FAM-labeled recombinant NGAL protein could internalize into the EC1.71 and EC109 cells. Furthermore, the internalized NGAL protein could induce the alteration of cellular morphology, resulting in generation of autophagosome, transcriptional up-regulation of genes associated with autophagy and increase of phospho-ERK1/2 (p-ERK1/2). Interestingly, the treatment with the NGAL protein did not affect the intracellular iron level. These data indicate that induced autophagy by exogenous NGAL protein is a mechanism that internalized NGAL plays important roles in esophageal carcinoma cells, independent with NGAL-mediated iron transport process, while ERK1/2 signal pathway is involved in activation of autophagy by exogenous NGAL protein.
