2.Effects of Various Intensity of Repetitive Transcranial Magnetic Stimulation on Upper Limbs Motor Function after Stroke

Objective To investigate the effects of various intensity of 1 Hz repetitive transcranial magnetic stimulation (rTMS) on up-per limbs motor function after stroke. Methods From December, 2014 to December, 2015, sixty patients with ischemic stroke were random-ly divided into high density group (HD, n=20), low density group (LD, n=20) and control group (n=20). They all received routine rehabilita-tion and medication. The HD group received rTMS over contralesional motor cortex twice a day, while the LD group received rTMS once a day, and the control group received sham stimulation, 10 minutes a time for 10 days. They were tested with motor-evoked potentials (MEPs), Fugl-Meyer Assessment (FMA) of upper limbs and modified Barthel Index (MBI) before and after treatment. Results The ampli-tude of MEPs of the affected cortex, the scores of FMA and MBI were more in HD and LD groups than in the control group after treatment (P<0.01), and were more in HD group than in LD group (P<0.01). No serious side-effect was found. Conclusion rTMS was feasible for pa-tients with stroke to improve the upper limbs motor function. It is more effective as more times a day.
3.Experimental studies on Fn, Mb and VEGF in cardiac conduction systems in acute myocardial infarction

In order to study the morphclogic changes of cardiac conduction system in six cases of acute myocardial infarctions fibronectin (Fn), myoglobin(Mb)and vascular endothelial growth factor (VEGF) were study by immunohistochemical method. It was observed that strong positive Fn staining were present in 3 cases, positive in 1 case, weak positive in 1 case; weak VEGF positive and depletion of Mb in all cases. It is indecated that the Fn staining is sensitive and was stable, and easily observed, and can be used as a good marker for diagnosis of the injury of the cardiac conduction system in acute myocardial infarction.
4.Introspection on the implementation of national essential drug system in three public community hospitals in Beijing

The national essential drug system is designed to solve the problem of covering hospital expenses with medicine revenue, and the problem of difficulty and high expense to see a doctor. This paper investigated whether the national essential drug system worked in three public community hospitals in Beijing. The result showed that the implementation of the program was now well underway, but left some problems. This paper put forward some suggestion for bidding of essential drug, improving the enthusiasm of the medical staff and improving the function of community hospitals.
5.Effects of Dysphagia on Nutrition in Stroke Patients

ObjectiveTo investigate the influence of dysphagia on nutritional status of stroke patients. MethodsSwallowing function of stroke patients were studied with videofluoroscopy. They were divided into study group and control group according to the results of videofluoroscopy (60 cases in each group). Their nutritional status were compared. ResultsThe triceps skin fold, mid-upper arm muscle circumference and serum albumin in the study group were lower significantly than those in the control group (P<0.05). ConclusionDysphagia is associated with poor nutrition in stroke patients.
6.A radiological study of dysphagia and rehabilitative outcome in post-stroke patients

Objective To explore the effects of videofluoroscopic swallowing study (VFSS) in the assessment of dysphagia in post-stroke patients. To study the representation and rehabilitative outcome of dysphagia following stroke. Methods Seventy post-stroke patients and 80 normal adults were assigned to study group and control group and underwent VFSS. Abnormal signs on VFSS were compared between 2 groups. Aspiration patients received swallowing therapy for 4 weeks followed by VFSS again. Abnormal signs on VFSS were compared with those of pre-treatment. Results Penetration/aspiration, oral retention and pharyngeal retention occurred respectively at a frequency of 5.0%, 13.4% and 25.3% in control group in mild status, 45.0%,46.5%, 48.9% in study group in moderate to severer status. 10 times of silent aspiration were seen, accounting for 24.4% of all aspiration. 7.Expression of Stat 3, GluT-1 and PCNA in condylomata acuminata

Objective To explore the expression and significance of signal transducer and activator of transcription 3 (Stat 3), glucose transporter protein 1 (GluT-1) and proliferation cell nuclear antigen (PC NA) in lesions of condylomata acuminata (CA). Methods SP immunohistochemistry method was used to measure the expression of Stat 3, GluT-1 and PCNA in tissue samples from 40 cases of CA and 20 normal skin controls. Results The positivity rates of Stat 3, GluT-1 and PCNA were 85.0% (34/40), 87.5% (35/40) and 85.0%(34/40), respectively in CA tissue, 35.0% (7/20), 30.0% (6/20)and 55.0% (11/20),respectively in the control tissue; statistical difference was observed in these rates between the two groups (all P < 0.05). The expression intensity of Stat 3, GluT-1 and PCNA was also higher in CA tissue than that in the controls. In addition, the expression intensity of PCNA was correlated with that of Stat 3 and GluT-1in CA tissue (both P< 0.05). Conclusions There is an overexpression of Star 3, GluT-1 and PCNA in CA tissue, and the overexpression of Stat 3 and GluT-1 may be associated with the over-proliferation of CA tissue.
8.Application of Gait Analysis for Stroke Patients with Hemiparesis

Objective To explore the characteristics of hemiparetic gait for stroke patients. Methods 49 stroke patients were allocated to two groups: experimental group (n=33) received 3D-video gait analysis, and control group (n=16) received the gait analysis with ankle-foot orthosis (AFO). The kinematic parameters were compared between two groups, and the experimental group with normal gaits. Results Compared with the normal gaits, the step, stride, step frequency, gait velocity, and the percentage single support in gait cycle decreased significantly,and gait width, and the percentage in gait cycle of double support increased significantly in the experimental group (P<0.01). There was no significant difference in kinematic parameters beween the experimental group and the control group (P>0.05). Conclusion The major kinematic characteristics of hemiparetic gait are slow velocity, long cycle, relative decrease of single support of weak limb, and increase of double support.
9.Analysis of cardiotoxicity of chemotherapy in 30 cases with gynecological cancer

Objective To investigate the occurrence of cardiotoxicity of chemotherapeutic drugs in gynecological cancer patients without heart disease,and patients with coronary heart disease or congenital heart disease for providing a basis for the clinical prevention of heart side effects during chemotherapy.Methods Thirty cases with gynecological cancer complication with or without coronary heart disease or congenital heart disease before or during chemotherapy admitted from Jan.2004 to Dec.2010 were retrospectively analyzed.Results For all 30 patients,there were heart failures in 3 cases ( 10%,3/30),myocardial infarction in 3 cases (10%,3/30),angina pectoris in 1 cases (3%,1/30),ST-T or T-wave changes in 9 cases (30%,9/30),and arrhythmia in 8 cases (27%,8/30).Conclusions Cancer chemotherapy drugs to the heart may produce an immediate or long-term toxicity,in which could significantly effects on the survival and prognosis of patients.It is very important to prevent the occurrence of cardiotoxicity of chemotherapeutic drugs in gynecological cancer patients with heart diseases during chemotherapy.
10.Diagnostic performance of HCV antibody and RNA Determination

Objective To explore changes of anti-HCV antibody and HCV-RNA in patients with HCV infection and its clinical significance in diagnosis and treatment of hepatitis C .Methods Serum samples from patients with HCV infection were collected . HCV antibodies were analyzed with a chemiluminescence micro-particle immunoassay method ,while a PCR-fluorescent probe meth-od was used to detect HCV-RNA .Concentrations of ALT and AST were also determined .Based on the concentrations of ALT , AST and HCV-RNA ,samples were divided into two groups respectively and the changes of different indicators were analyzed and compared .Meanwhile samples from 37 HCV-infected patients were collected continuously .Different indicators after treatment were compared with those before treatment .Results HCV-Ab and logarithm values of RNA load in the group with abnormal concentra-tions of ALT and AST were significantly higher than those in the normal group (P<0 .05) .HCV-Ab ,ALT and AST concentrations in HCV-RNA positive group were significantly higher than those in HCV-RNA negtive group(P<0 .05) .The areas under receiver operating characteristic curve of HCV-Ab and logarithm values of RNA load were 0 .990 and 0 .838 respectively .Concentrations of ALT ,AST and logarithm values of RNA load after treatment were significantly lower than those before treatment (P<0 .05) while there was no significance between HCV-Ab level after treatment and that before treatment (P>0 .05) .Conclusion The diagnostic performance of HCV-Ab is better than that of logarithm values of RNA load .Determination of ALT ,AST and HCV-RNA is of clinical importance in monitoring the effect of hepatitis C treatment .
