2.Study on the postmortem stabiliity of fibronectin for the diagnosis of myocardial infarction

Experimental acute myocardial ischemia model of dog was established, and the postmortem stability of fibronectin for the diagnosis of mycardial infarction was studied with immunohistochemistry and image analysis technique. The results showed that the positive reaction areas of fibronectin in ischemic myocardial tissues decreased along with the prolongation of postmortem interval, but positive reaction of fibronectin could still be found in ischemic myocardia kept for 4 weeks postmortem. No positive reaction for fibronectin could be found in normal myocardia when kept for different times. So fibronectin is a quite stable marker for the postmortem diagnosis of myocardial infarction, and is of practical value in the forensic practice.

For the purpose of establishment of a diagnostic method of mild viral myocarditis,an experimental mild viral myocarditis model was induced in Balb/c murine by coxsackie virus B 3.The cardiac tissue secfion were stained by LSAB-immunohistochemical method with anti fibronectin(Fn) antibody. The results demonstrated that the Fn deposition was found in the myocardium of mice with myocarditis and the mild degeneration of cardiomyocytes could be identified by Fn LSAB immunohistochemical staining.It is suggested that the Fn deposition in the myocardium tissue is one of the reliable marks of myocardium inflammation.
4.Immunohistochemical study of C5 complement in experimental early myocardial ischemia

Experimental acute myocardial ischemia model of rat was established, and the changes of C5 complement in the ischemic myocardia were studied with immunohistochemistry and image analysis technique. The results showed that the positive reaction of C5 could be observed in ischemic myocardia at 15 min after ischemia, and the positive reaction area increased along with the prolongation of the ischemic period. It is concluded that the positive reaction of C5 in cardiomyocytes is a quite sensitive marker of early myocardial ischaemia.The immunohistochemical detection of C5 in cardiomyocytes will be a meaningful tool for the postmortem diagnosis of early myocardial ischaemia.
5.Study on the fibronectin for the diagnosis of myocardial infarction

In order to explore the value of fibronectin (Fn) in the postmortem diagnosis of myocardial infarction.The changes of Fn staining in normal, infarcted and other non infarcted myocardial injuries resulted from myocarditis, mechanical asphyxia, electrocution, hemorrhagic shock, cardiac contusion and organophosphate poisoning were studied with immunohistochemistry and image analysis system. The results showed that positive Fn staining could only be observed in groups of myocardial infarction and myocarditis, but could not be found in groups of mechanical asphyxia, electrocution, hemorrhagic shock, cardiac contusion, organophosphate poisoning. It is indicated that positive reaction of Fn could be affected only by myocarditis, so it is quite specific for the diagnosis of myocardial infarction.
6.Experimental study on the cardiomyocyte apoptosis in the early myocardial ischemia

In order to explore the significance of cardiomyocyte apoptosis in the early myocardial ischemia,the rat model of acute myocardial ischemia was established,and the apoptotic cells were detected in the early phase of ischemia(within 6h)with TUNEL method.The results showed that scanty apoptotic cells could be observed in the ischemic region 1h after ischemia,and reached the peak 3h after ischemia,then decreased.No apoptotic cells were found in the normal region.In the peri ischemic region,apoptotic cells could also be observed 1h after ischemia,and reached the peak 5h after ischemia.It is indicated that apoptosis is the major form of early ischemic myocardial damage,and the detection of apoptotic cardiomyocyte may provide a new sensitive and objective method for the postmortem diagnosis of early myocardial infarction.
7.Introspection on the implementation of national essential drug system in three public community hospitals in Beijing

The national essential drug system is designed to solve the problem of covering hospital expenses with medicine revenue, and the problem of difficulty and high expense to see a doctor. This paper investigated whether the national essential drug system worked in three public community hospitals in Beijing. The result showed that the implementation of the program was now well underway, but left some problems. This paper put forward some suggestion for bidding of essential drug, improving the enthusiasm of the medical staff and improving the function of community hospitals.
8.Influence of personalized comprehensive nursing intervention on complications after coronary stent implantation in patients with acute cardiac infarction

Objective To discuss the influence of personalized comprehensive nursing intervention on complications after coronary stent implantation in patients with acute cardiac infarction,and provide an effective and feasible method for prevention and reduction of these complications.Methods 118 patients with acute cardiac infarction after coronary stent implantation were divided into the intervention group (51 cases)and the control group (67 cases),the puncture site complications,CRP level were compared,and the mastering of health knowledge were compared between two groups through questionnaires and telephone return visit as well.Results The incidence rate of puncture site complications in the intervention group was lower than that of the control group.Significant differences were seen in reasonable diet,medicine knowledge,emotional adjustment and preventive measures between two groups.CRP levels were compared between two groups and the result showed no statistical difference.Conclusions Through strengthening the personalized comprehensive nursing intervention,nurses can significantly reduce the rate of complications after coronary stent implantation and facilitate the recovery of patients.
9.Clinical analysis of 4 214 prepubertal girls with vulvovaginitis

Objective To explore the distribution of age,reasons for treatment,risk factors,and causes of vulvovaginitis in prepubertal girls.Methods A total of 4 214 prepubertal girls with vulvovaginitis who were admitted to the Girl's Sub-department,Zhongshan Boai Hospital from January 2010 to June 2013 was reviewed retrospectively.All clinical data were from medical records with files.Results Atotalof1 587 patients (37.7%) was0to1 years old,954 (22.6%) > 1 to3 years old,1 289 (30.6%) > 3 to 7 years old,and 384 (9.1%) > 7 years old.Many risk factors were associated with vaginitis,including poor hygiene (2 924 girls; 69.4%),allergies and exposure to allergens (875 girls; 20.8%).Most common reason for treatment was a referral from physical examination accounting for 919 girls (21.8%),followed by vaginal secretions 812 girls (19.3%).The causes of vulvovaginitis of all patients were evaluated,1 771 of which (42.0%) were nonspecific vulvovaginitis,1 309 (31.1%) labial adhesions,375 (8.9%) bacterial vulvovaginitis,266 (6.3%) allergic vulvovaginitis,and 266 (6.3%) affective leg rubbing action.Conclusions Prepubertal vulvovaginitis occurred mainly in infancy and preschool.Floating population was common.They were caused by many risk factors including poor hygiene,allergies,poor urination habits,etc.The most common causes of vulvovaginitis were nonspecific vulvovaginitis and labia adhesion,yet allergies and affective leg rubbing action were the more common causes of recurrent vulvovaginitis.We propose that focusing on girls' reproductive health,timely treating allergic and crossing rub legs and other diseases would help reduce the prevalence of vulvovaginitis in the prepubertal girls.
10.Expression of PPDK from Microbispora rosea subsp. aerata in Escherichia coli and its application in pyrosequencing.

Pyruvate phosphate dikinase (PPDK; EC 2.7.9.1) is found in certain microorganisms and plants, and catalyzes the conversion of AMP, PPi and phosphoenolpyruvate (PEP) to ATP, Pi and pyruvate. Using the genomic DNA of Microbispora rosea subsp. aerata as the template, a DNA fragment encoding the gene PPDK was amplified by PCR and inserted into the expression vector pET28a(+), yielding pET28a (+)-PPDK. The E. coli BL21 (DE3) was transformed with the pET28a (+)-PPDK. After inducing with IPTG, the E. coli BL21 (DE3) [pET28a (+)-PPDK] expressed recombinant PPDK fused to an N-terminal sequence of 6-His Tag. The molecular weight of PPDK was estimated to be 101 kD by SDS-PAGE. The PPDK was purified by His * Bind Resin affinity chromatography and ultrafiltration using 10 kD cut-off membrane. The successful application of PPDK in pyrosequencing was also demonstrated.
