1.Medicine Ethics Education in Nosocomial Infection Management:Present Situation and Significance

OBJECTIVE To discuss the present situation of the medical ethics education and the significance to carry out in the nosocomial infection management. METHODS The method was retrieved the nosocomial infection management and the medical ethics education related monographs and publications in recent years,and to analyze and comment the environmental condition and necessity of the development of the medicine ethics education study. RESULTS Finally some hospitals developed the nosocomial infection medical ethics to educate,and carried on the preliminary ponderation over the nosocomial infection and the medicine ethics,but the infection management medical ethics standard was still at the vacuum stage,current the environment was in favor of the medical ethics education. CONCLUSIONS Through the development of the series medicine ethical education and the introduction of medical ethics appraisal and ethics analysis,from the macroscopic height and philosophy influence the medical personnel,promote the nosocomial infection management achievements,and guarantee to realize the infection foundation work. As the carrier of the achievement impelling the hospital culture,it has poured the soul into the nosocomial infection supervisory work.

Chronic myeloid leukemia is a kind of malignant cloning hyperplastic disease of hematopoietic stem cell .The treatments based on molecular biological and immunological techniques will become the new therapies .Gene silence can improve the effects of molecular targeted therapeutic drugs through two ways: one is the target mRNA can be digested by siRNA and the other is target gene lose the stability and reduce the generation of protein mediated by miRNA .Adoptive cel-lular immunotherapy is a treatment method through injecting immunocompetent cells such as CIK , NK, etc into the body of cancerous person .This can improve the immunity of body and the effects of molecular targeted therapeutic drugs .The fur-ther study about basic theory , molecular mechanism and clinical effects will be continued .
3.Clinical Value of Serum Pepsinogen and Gastrin-1 7 Levels in Patients with Atrophic Gastritis and Gastric Cancer

Objective To investigate the clinical value of serum pepsinogen(PG)Ⅰ,Ⅱ and gastrin-17(G-17)levels in early di-agnosis for atrophic gastritis and gastric cancer.Methods The study was performed on a case-control trial,and 430 subjects were recruited.Each of them underwent endoscopy biopsies before serum tests were performed.These patients were further divided into three groups based on endoscopic and histological findings:160 non-atrophic gastritis group,118 atrophic gastri-tis group and 152 gastric cancer group.Serum samples of PGⅠ,PGⅡ and G-17 were analyzed by enzyme-linked immunosor-bent assay(ELSIA),and calculated the values of PGⅠ/PGⅡratio.Results Compared with the non-atrophic gastritis group, serum PGⅠ and G-1 7 values in atrophic gastritis and serum PGⅠ,PGⅡ,PGⅠ/PGⅡratio and G-1 7 values in gastric cancer group decreased significantly (P<0.05);compared with the atrophic gastritis group,serum PGⅠ and G-17 values in gastric cancer group decreased significantly (P<0.05).By using receiver operating characteristic curves (ROC),the cut-off points of PGⅠ84.706μg/L,PGⅡ 10.873μg/L,PGⅠ/PGⅡ ratio 11.008 and G-178.265 pg/L in atrophic gastritis group,PGⅠ65.145μg/L,PGⅡ 10.089μg/L,PGⅠ/PGⅡ ratio 6.375 and G-17 4.971 pg/L in gastric cancer group.Conclusion Low serum PGⅠ,PGⅡ,PGⅠ/PGⅡ ratio and G-1 7 values are biomarkers of atrophic gastritis and gastric cancer and valuable for the early diagnosis of gastric cancer.
4.The effects of nicardipine induced controlled hypotension in patients underwent orthognathic surgery: A systematic review and meta-analysis

Objective:To evaluate the clinical effects of nicardipine for induced controlled hypotension in patients underwent orthognathic surgery.Methods:The related trails were searched from English and Chinese literature databases.The quality of the RCTs was evaluated by 2 indepandent reviewers.The data were statistical analyzed using the Rev Man 5.3.3 software.Results:5 RCTs with 248 patients were included.Meta-analysis and descriptive analysis indicated that blood loss of nicardipine group was more than that of remifentanil group [WMD =43.85,95% CI(20.52,67.18)].There was no significant difference in blood loss between nicardipine group and dexmedetomidine group and nitroglycerin group.There was no significant difference in transfusion between nicardipine group and the control group.Nicardipine increased the heart rate during controlled hypotension and caused QT prolongation (P < 0.001).Nicardipine had no adverse effects on cerebral oxygen saturation and neurophysiological function.Urinary N-acetyl-1-b-D-glucosaminidase was lower in nicardipine group than that in remifentanil group (P < 0.05).Conclusion:Nicaridpine is effective in the induced controlled hypotension during orthognathic surgery,with potential renal protective effect.However,it is not better than the remifentanil on reducing the blood loss.Nicardipine can increase the heart rate and prolong the QT interval during the controlled hypotension.
5.Polypeptide from Chlamys farreri prevents UVB-induced apoptosis in the HaCaT cells through death receptor pathway and mitochondrial pathway

Aim To investigate whether the polypeptide from Chlamys farreri(PCF)protected HaCaT cells from UVB-induced apoptosis through Fas-caspase-3 and ROS-cytochrome C.Methods Experiment designs were divided into five groups:control group,UVB model group,UVB+5.69 mmol?L-1PCF group,UVB+2.84 mmol?L-1 PCF group,UVB+1.42 mmol?L-1 PCF group.SiRNA for Fas inhibited Fas expression of UVB-induced HaCaT cells.Using agarose gel electrophoresis,the effects of Fas siRNA and ROS scavenger NAC on UVB-induced apoptosis of HaCaT cells were investigated.Expression levels of cytochrome C andcaspase-3 after inhibitory Fas were determined by Western blot analysis.Intracellular ROS was detected by means of an oxidation-sensitive fluorescent probe(DCFH-DA).Results SiRNA for Fas had inhibitory effects on UVB-induced apoptosis of HaCaT cells and caspase-3 expression.NAC had inhibitory effects on UVB-induced apoptosis of HaCaT cells.PCF inhibited UVB-induced generation of ROS and cytochrome C release dose-dependently.Conclusions PCF protected HaCaT cells from UVB-induced apoptosis.Its inhibitory effect on apoptosis could be attributed to inhibition of Fas-caspase-3 and ROS-cytochrome C pathways.
6.Effect of Chronic Episodic Hypoxia on Liver CYP3A_2 and CYP2E_1 in Rats

OBJECTIVE: To investigate the effects of episodic hypoxia(EHYP) on liver CYP3A2 and CYP2E1. METHODS: Healthy masculinity rats were divided randomly into five groups: control group, 3 -day EHYP group, 7 -day EHYP group, 14 -day EHYP group, and 28 -day EHYP group. 24 h after the management of EHYP, the rats were anesthetized by intraperitoneal injection, and the eyeball blood 2ml was drawn to prepare serum. The activities of ALT and AST in the serum were determined by enzymatic methods. The activities of liver microsomal erythromycin demethylase(ERD) and aniline hydroxylase(ANH) were detected by spectrophotography. The levels of mRNA expression of CYP3A2,CYP2E1 were assayed with RT-PCR. RESULTS: The serum activities of ALT,AST showed no remarkable change. However, after 7 days of hypoxia, ERD and ANH activities were remarkably elevated, and at 28 days induction rates reached up to155.5% and 42.2%, respectively. Meanwhile, the levels of mRNA expression of CYP3A2,CYP2E1 also increased by 220.5% and 102.8%, respectively. CONCLUSION: Chronic EHYP can remarkably increase the activities of ERD and ANH, whose mechanism is probably related to up-regulating of CYP3A2 and CYP2E1 expression at the transcriptive level.
7.Human Interleukin-13 Affects Expression of Adhesive Molecules on Bovine Aortic Endothelial Cells

Objective To investigate the effects of human interlukin-13 (hIL-13) on the expression of E- selectin and intercellular adhesion molecule-1(ICAM-1) on bovine aortic endothelial cells(BAECs) stimulated by tumor necrosis factor alpha(TNF-?), and to provide experimental basis for hIL-13 inducing immunity endure and relieving the repulsion reaction of xenograft. Methods BAECs were co-cultured with different concentrations of hIL-13 for 2 h and followed by co-cultured with 4 ng/ml TNF-? for 6 h or 18 h. The expressions of E-selectin and ICAM-1 on BAECs were detected by Cell-ELISA. 8.ELECTRON MICROSCOPIC STUDY OF THE CHANGES OF CONNECTIVE TISSUE CELLS DURING THE SKIN WOUND HEALING IN RATS

Open skin wounds were made in 28 Wistar rats, and the changes of the connective tissue cells were observed by electron microscopy during wound healing. The fibroblasts and macrophages were increased in the early stage, later on the fibroblasts might be transformed into myofibroblasts which became preponderant cells in the wound gradually and led to wound contraction and scar formation. The intercellular substances were phagocytosed by fibroclasts simultaneously. The mast cells were closely apposed to the fibroblasts or myofibroblasts and represented active functions, such as degranulation. Considerably they released the granule contents to the extracellular space and effected the other cells functions. It shows that the cells mentioned above interplay and take part in the wound healing.
9.The success rate of ventilator weaning in acute exacerbation of chronic obstructive pulmonary disease patients influenced by bobbed endotracheal intubation

Objective To study the success rate of ventilator weaning in acute exacerbation of chronic obstructive pulmonary disease (AECOPD) patients influenced by bobbed endotracheal intubation. Methods One hundred and sixteen AECOPD patients who were given invasive mechanical ventilation and reached the standards of off-ventilator were divided into control group (general endotracheal intubation group) and experiment group bobbed (tracheal intubation) by random digits table method with 58 cases each. The respiratory rate (RR), heart rate (HR), rapid and shallow breathing index (RSBI), oxygenation index (OI), pulse blood oxygen saturation (SpO2), tidal volume and success rate of ventilator weaning were compared. Both groups were implemented of extubation if they reached the standards of the extubation time by observing their ability in spontaneous breathing test (SBT). Results The RR, HR and RSBI before extubation in experiment group were significantly lower than those in control group:(19.7 ± 2.3) times/min vs. (23.5 ± 2.3) times/min, (91.2 ± 6.3) times/min vs. (93.4 ± 8.1) times/min and 80.2 ± 6.7 vs. 90.5 ± 9.6, P<0.05, and the OI, SpO2, tidal volume and success rate of ventilator weaning were significantly higher than those in control group: (269 ± 9) mmHg (1 mmHg = 0.133 kPa) vs. (245 ± 16) mmHg, 0.929 ± 0.014 vs. 0.870 ± 0.037, (6.1 ± 1.2) ml/kg vs. (5.1 ± 0.8) ml/kg and 91.38%(53/58) vs. 77.59%(45/58), P<0.05. Conclusions The bobbed endotracheal intubations can improve the success rate of ventilator weaning in patients with AECOPD.
10.Application study on supervision system of hospital-university collaboration in class teaching of nursing undergraduates

Objective To evaluate the application effect of supervision system of hospital-university collaboration on class teaching of nursing undergraduates,so as to provide reference to promote the link-up between nursing theory and clinical practices.Methods The supervision group of hospital-university collaboration were formed by selecting 30 excellent clinical instructors from university-affiliated hospitals and 12 nursing teachers,they listened to the teachers in classes randomly and recorded,simultaneously filled in rating forms about the class with students.Supervision meetings were held and targeted strategies were put down regularly,their effects were evaluated through interview and investigation methods.Results Professional teachers accepted their suggestions and took some actions to make a change.The results of rating forms showed that the quality of classroom teaching were improved gradually.Conclusions The supervision system of hospital-university collaboration can enhance the sense of responsibility of teaching,contribute to the integration between the hospital and university.It should be continued as a long-run system.
