1.Microecological investigation of vaginal microflora in women with varying degree gynecologic symptoms in clinics

Objective To investigate the microecological status of vaginal microflora in the women with different vaginal symptoms. Methods From March 2006 to October 2007, 6982 cases with varying degree vaginal symptoms including pruritus, increaseed leucorrhea, the leucorrhea having unusual smell, in the gynecology outpatient department were studied. The vagina secretions were examined in terms of the pH value, the hydrogen peroxide test, and Gram dyeing inspection of vaginal bacteria and microecology appraisal for colony's density, the multiplicity, the superiority fungus, and the inflammatory response. Results Among 6982 patients, normal vaginal microecology was identified in 750 (10. 74%, 750/6982 );abnormal mieroeeology was found in 6232 ( 89. 26%, 6232/6982 ) ; bacterial vaginosis (BV) was detected in 729 (10. 44%, 729/6982);vulvovaginal candidiasis (VVC) was in 1527 ( 21.87%, 1527/6982 ). Ninety five patients (1.36% ,95/6982) were with both BV and VVC. Abnormal bacteria colonies were found in 1229(17. 60% ,1229/6982),and others were found in 2652(37. 98% ,2652/6982). Conclusions The vaginal microecology in the women with different vaginal symptoms can be either normal or abnormal. Microecology clinical evaluation system can assess the vaginal microecosystem.
2.Application of fluorescence PCR melting curve method in drug-resistance detection of Mycobacterium tuberculosis

Objective To evaluate the clinical application value of fluorescence PCR melting curve method in the drug-resistance detection of Mycobacterium tuberculosis.Methods With the Lowenstein-Jensen culture method and the absolute concentration method as the comparison,the fluorescent PCR melting curve method was used to detect the resistance to rifampin and isoniazid in 53 cases of pulmonary tuberculosis(TB)respectively.Results The single drug resistance rates and multidrug resistance (MDR) rate detected by the two methods had no significant statistical difference(P >0.05).The comparison between the two methods for detecting the same sample showed that compared with the absolute concentration method,the sensitivity and specificity of the melt-ing curve method for detecting rifampin-resistance,isoniazid-resistance were 80.0% and 81.0%,100.0% and 93.8% respectively, the accuracies were 92.2% and 86.8% respectively,the Kappa values in the consistency analysis of the two methods were 0.754 and 0.710 respectively,indicating the good consistency between them.Conclusion Compared with the traditional methods,the melting curve method has good sensitivity and good specificity with safety,high efficiency and good application value in the treat-ment of drug-resistant pulmonary TB.
3.Relationship between VEGF and ERK signaling pathway in spinal dorsal horns of rats with neuropathic pain

Objective To evaluate the relationship between vascular endothelial growth factor (VEGF) and extracellular signal-regulated kinase (ERK) signaling pathway in spinal dorsal horns of rats with neuropathic pain.Methods Twenty male Sprague-Dawley rats,aged 2 months,weighing 160-320 g,were randomly divided into 4 groups (n =5 each):sham operation group (group S); chronic constrictive injury (CCI) group; normal saline group (NS group); VEGF antibody group.Neuropathic pain was induced by CCI.The animals were anesthetized with intraperitoneal 10% chloral hydrate 350 mg/kg.The left sciatic nerve was exposed and 4 ligatures were placed on the sciatic nerve at 1 mm intervals.In group S,the left sciatic nerve was only exposed but not ligated.In VEGF antibody group,VEGF antibody 0.3μg/15 μl was injected intrathecally every 2 days for 4 times starting from 2 h after CCI,while the equal volume of normal saline was injected in NS group.Paw withdrawal threshold to von Frey filament stimulation (PWMT) and paw withdrawal latency to nociceptive thermal stimulation (PWTL) were measured at 1 day before CCI (T1),and 1,3,5 and 7 days after CCI (T2-5).The rats were sacrificed after PWMT and PWTL were measured and the L4-6 segments of the spinal cord were removed for determination of phosphorylated ERK (p-ERK) expression in spinal dorsal horns by immunohistochemistry and Western blot.Results Compared with group S,PWMT and PWTL were significantly decreased at T2-5,and the p-ERK expression in spinal dorsal horns was up-regulated in CCI and VEGF antibody groups (P < 0.05).Compared with CCI group,PWMT and PWTL were significantly increased at T3-5,and the p-ERK expression in spinal dorsal horns was down-regualted in VEGF antibody group (P < 0.05).Conclusion VEGF in the spinal dorsal horn is involved in the development and maintenance of neuropathic pain in rats through activating ERK signaling pathway.

5.Progress of screening, diagnosis and treatment strategies for mycoplasma infections in female genital tract

Healthcare professionals should be aware that genital mycoplasma infections are common in clinical practice, which are mainly caused by Mycoplasma hominis (Mh), Ureaplasma urealyticum (UU) and Mycoplasma genitalium (Mg). Mh and UU can colonize the urogenital tract of healthy individuals and are associated with bacterial vaginosis, urethritis, pelvic inflammatory disease, infertility and preterm birth; while Mg may cause urethritis, cervicitis, endometritis and pelvic inflammatory disease, bringing more threat to reproductive health in women. Mh and UU can be detected by culture on special media, while isolating Mg and subtyping UU require molecular biological techniques. Currently, there are no unified guidelines for the screening and treatment of genital mycoplasmas infections worldwide. The article reviews the progress of screening, diagnosis and treatment strategies for mycoplasma infections in female genital tract, aiming to provide a reference for clinical management of this conditon.
6.Myelin oligodendrocyte glycoprotein antibody-related diseases with onset with spinal cord injury(report of 2 cases)

Objective To investigate the clinical characteristics and immunotherapy effect of myelin oligodendrocyte glycoprotein antibody(MOG-Ab)-associated disorders(MOGAD)onset with spinal cord injury.Methods Clinical data of 2 children with MOGAD onset with spinal cord injury were retrospectively analyzed.Results The first symptoms of 2 children were both lower limbs fatigue.After testing the full-length MOG-Ab in both serum and CSF and MRI scanning,they were diagnosed as MOGAD onset with spinal cord injury.The clinical symptoms were relieved after hormone impact treatment,and the range of abnormal signals were reduced by reexamination.After discharge,the patient was followed up for more than 5 months,and the clinical symptoms were cured without recurrence.Conclusions MOGAD onset with spinal cord injury is a rare disease type.Its early clinical manifestations may be neck,back and waist discomfort,gradually aggravated limb weakness,urination and defecation dysfunction.Imaging examination shows abnormal signals of multi-segmental spinal cord,and MOG-Ab could be detected in CSF and peripheral blood.Early identification and diagnosis and timely application of hormone impact treatment can help to quickly relieve clinical symptoms and improve prognosis.
7.Evaluation of left ventricular systolic synchrony in patients with dilated cardiomyopathy by tissuemotion mitral annular displacement

Objective To investigate the value of tissue motion mitral annular displacement (TMAD) in assessment of left ventricular systolic synchrony in patients with dilated cardiomyopathy (DCM).Methods Thirty-eight DCM patients and 30 healthy subjects were enrolled in this study.The left ventricular end diastolic volume (LVEDV),left ventricular end systolic volume (LVESV),and left ventricular ejection fraction (LVEF) in two groups were investigated using conventional echocardiographyexamination.The time of systolic mitral annular peak displacement (TP) in two groups were measured by the technique of TMAD at six sites (anteroseptal,posteroseptal,anterior,lateral,posterior and inferior).The standard deviation of TP (TP-SD) and the maximal difference of TP (TP-DIF) were calculated.And the correlation between TP-SD and LVEF as well as TP-DIF and LVEF in the DCM group were analyzed.The standard deviation and maximal difference of the time to point with minimal systolic volume of 16 segments (Tmsv16-SD,Tmsv16-DIF) of the DCM group were measured by the technique of real-time three-dimensional echocardiography (RT-3D).And the correlation between Tmsv-16SD and TP-SD as well as Tmsv16-DIF and TP-DIF were analyzed.Results Compared with the healthy subjects,LVESV and LVEDV increased (t=14.023,16.643,P < 0.01) and the LVEF decreased significantly (t=26.112,P < 0.01) in the DCM group.TP,TP-SD and TP-DIF in the DCM group obviously increased (t=7.628,6.869,7.507,6.616,5.631,4.738,7.368,5.777,6.781,P < 0.01).TP-SD and TP-DIF were negatively correlated with LVEF (r=-0.645,-0.513,P < 0.05),while TP-SD and Tmsv1 6-SD,TP-Dif and Tmsv1 6-Dif were positively correlated(r=0.643,P < 0.01;r=0.563,P < 0.05).Conclusions In this study,the TMAD technique was used to measure the time of peak displacement of mitral annulus to evaluate the synchrony of left ventricle movement.It is expected to be a new method in evaluation of the synchrony of left ventricle,which is simple,feasible approach without angle dependence.
8. Primary study on the relationship between high-risk HPV infection and vaginal cervical microbiota

Objective:
To understand characteristics of vaginal cervical microbiota in high-risk HPV (hrHPV) infected women and to uncover the relationship between hrHPV infection and vaginal cervical microbiota.
Methods:
All participants were randomly selected from Peking University First Hospital from September to October of 2017, including 5 subjects of control group, 5 cases of HPV16/18 group, 5 cases of other hrHPV infected group and 3 cases of cervical squamous carcinoma group. All subjects were required to fill in a questionnaire, and cervical and vaginal discharges were separately collected for microscopic examination and new generation sequencing targeting the variable region (V3-V4) of bacterial 16S rRNA gene.
Results:
Vaginal microbiota analysis: (1) 6 major phylum were found in vaginal microbiota:Firmicutes, Bacteroidetes, Fusobacteria, Actinobacteria, Tenericutes and Proteobacteria. Firmicutes contributed to the majority of normal vaginal flora, Bacteroidetes and Fusobacteria increased in hrHPV infected ones, while Fusobacteria showed significant difference in cervical carcinoma group. (2) Lactobacillus occupied most of normal vaginal flora while genus like Gardnella, Prevotella, Atopobium, Megasphaera and Sneathia increased in hrHPV infected subjects, Sneathia showed significant difference in cervical carcinoma group. 9.Correlation analysis of self-care contribution and binary coping among spouse caregivers of middle-aged and young stroke patients

Objective:To investigate the self-care contribution and binary coping level of spouse caregivers for middle-aged and young stroke patients and explore the relationship between the two, in order to improve the quality of self-care contribution of spouse caregivers to middle-aged and young stroke patients.Methods:A cross-sectional survey was conducted among 200 middle-aged and young stroke patients ′ spouse caregivers in the People ′s Hospital of Xinjiang Uygur Autonomous Region from January to June 2023 by the general data questionnaire, the Caregiver Contribution to Self-care of Stroke Patient Scale, the Dyadic Coping Inventory. The correlation between self-care contributions and binary coping strategies of spousal caregivers was analyzed, and the factors that affect their self-care contributions were screened. Results:Finally, 192 middle-aged and young stroke patients ′ spouse caregivers were included, including 70 males and 122 females, aged 18-59 years old. The total score of self-care contribution of spousal caregivers of middle-aged and young stroke patients was (75.43 ± 6.80) points, and the total score of binary coping was (117.12 ± 9.59) points, both of which were positively correlated ( r=0.691, P<0.05). Hierarchical regression analysis showed that after controlling for the influence of general data, binary coping could explain 28.4% variation in the self-care contribution of spouse caregivers of middle-aged and young stroke patients ( P<0.05). Conclusions:The duality coping level positively predicts the quality of spouse caregivers ′ self-care contribution to middle-aged and young stroke patients. Medical staff should assess the dual coping level of middle-aged and young stroke patients and their spouses, and carry out targeted dual intervention measures to improve the quality of caregivers' contribution to stroke patients ′ self-care.
10.Analysis on the nutritional composition of prepackaged foods commonly consumed by children aged 6-12 in Chengdu

Objective:
To analyze the current nutritional composition of commonly consumed prepackaged foods among children in Chengdu and to provide a scientific basis for health education among children and adolescents.
Methods:
Based on the 3 day and 24 hour dietary data of children aged 6-12 in Chengdu of the Southwest China Childhood Nutrition and Growth Cohort from 2021 to 2022, the nutritional information of prepackaged foods was collected by combining offline and online methods. All foods were classified step by step, and the nutrient content of each 100 g or 100 mL food was counted and graded.
Results:
A total of 1 902 children s prepackaged foods in 23 sub categories of 10 major categories were investigated. Nuts and seeds, snack foods, instant foods and other dairy products had higher total energy(2 476, 2 027, 1 728, 1 816 kJ/100 g), with the nutrient reference value percentage(NRV%) exceeding 20%. Fish, poultry, meat, eggs and their products had the highest protein content(22.8 g/ 100 g ) with an NRV% of 38%, nuts and seeds had the highest fat content(47.5 g/100 g) with an NRV% of 79%, confectionery and jelly had the highest carbohydrate content(82.1 g/100 g) with an NRV% of 27%, and seasoning flour products had the highest sodium content with an NRV% up to 118%. Seasoning flour products and instant foods were mostly high sodium, high fat and high carbohydrate food. Baked and puffed foods were almost high fat and high carbohydrate. Fish, poultry, meat, eggs, beans and their products were rich in protein but mostly high in sodium. Beverages and cold drinks were low in other nutrients except carbohydrate.
Conclusion
The nutrient content of various prepackaged foods commonly consumed by children in Chengdu are quite different. Most of the foods consumed by children are high sodium, high fat and/or high carbohydrate. Nutrition education should be strengthened to help children choose healthy foods.
