1.Sonographic features of follicular thyroid carcinoma th

Objective To determine sonographic features of thyroid follicular carcinoma ( FC) in comparison with thyroid follicular adenoma ( FA ).Methods This retrospective study included 36 pathologically proven FCs (5 widely invasive FCs and 31 minimally invasive FCs)and 52 FAs in 88 patients who underwent thyroid surgery .We analyzed clinical features of each patient ,including patient gender,age, and sonographic features of each tumor , including maximum diameter, peripheral halo, echogenicity, echotexture,calcifications and nodule number .These clinical and sonographic findings were compared by using t test ( age and diameter ) or the χ2 test (sex ratio,halo,echogenicity,echotexture,calcifications and nodularity)between FAs and FCs.Results For sonographic features,predominantly solid content(100.0%, 36/36),hypoechoic echogenicity (69.4%,25/36),inhomogeneous echotexture (83.9%,26/32),presence of calcifications(55.6%, 20/36) and without or with irregular halo (69.4%,25/36) were more commonly found in FCs.In comparison,FAs were more likely to present with mixed or predominantly cystic content (61.5%,32/52),isoechoic echogenicity (63.5%,33/52),homogeneous echotexture (80.8%,42/52), absence of calcifications (7.7%,4/52) and thin halo (86.5%,45/52) ( χ2 =34.813,28.596,35.256, 25.052,28.811,all P<0.01).The maximum diameter and mean age did not show statistically significant difference between FCs and FAs (t=0.222,-1.228,P=0.825,0.223).The proportions of tumor larger than 40 mm(22.2%,8/36 vs 17.3%,9/52),solitary nodule(31.3%,12/24 vs 21.2%,11/41),male sex (25.0%,9/27 vs 23.1%,12/40)and patient older than 45 years(66.7%,24/36 vs 71.2%,37/52)did not show statistically significant difference between FCs and FAs ( χ2 =0.330,1.635,0.043,0.201,all P>0.05).Conclusion Sonographic features,including predominantly solid content ,hypoechoic echogenicity, inhomogeneous echotexture ,presence of calcifications and without or with irregular halo were more commonly seen in FCs.
2.Permeability increase of prostate cancer cells membrane under low intensity ultrasound in vitro

Objective To investigate the permeability changes of prostate cancer cells membrane under low intensity ultrasound in vitro.Methods The culture of monolayer adherent LNCaP prostate cancer cells in six-well plate was exposed to continuous ultrasound at frequency of 1 MHz.The cells membrane permeability (stained with Calcein)and cells viability(stained with PI)were evaluated by fluorescent microscope (FM) and cells morphological changes were evaluated by scanning electron microscope (SEM) under ultrasound with acoustic intensity of 160 mW/cm2 for 5 s.The rate of cells with increased cells membrane permeability as function of acoustic intensity (80 mW/cm2,120 mW/cm2 and 160 mW/cm2,for 5 s) and exposure duration (5 s,10 s and 15 s,acoustic intensity of 120 mW/cm2) was evaluated by flow cytometry.ResultsAfter low intensity of ultrasound,the cells with increased cell membrane permeability could be clearly shown with Calcein uptake under FM while no cell showed Calcein uptake in the control group.The SEM showed less microvilli on the cells after low intensity of ultrasound exposure and few cells showed holes on the cell membrane.The rate of cells with increased membrane permeability increased with acoustic intensity and exposure duration.Conclusion Low intensity ultrasound alone could increase membrane permeability of prostate cancer cells and cells with increased membrane permeability showed surface plane,uncommon holes on the cells membrane.The rate of cells with increased membrane permeability positively correlated with acoustic intensity and exposure duration.
4.Analysis on tear film after LASIK by femtosecond laser with Oculus corneal topography

AIM:To observe the changes of tear film on the patients after laser in situ keratomileusis ( LASIK) with corneal flap created by femtosecond laser with Oculus corneal topography.
METHODS:Totally 120 myopic patients (240 eyes) were collected who underwent femtosecond laser surgery LASIK from August to September 2013, and these patients can be followed up for 3mo. Tear break-up time ( BUT) and tear meniscus height ( TMH ) with Oculus corneal topography were recorded preoperatively and postoperatively at 1wk;1, 2 and 3mo.
RESULTS: Oculus BUT: there existed obvious differences (P=0. 012, 0. 000, 0. 023<0. 05) in 1wk, 1 and 2mo compared with the preoperative level. While no such obvious difference ( P = 0. 236 > 0. 05 ) existed in 3mo compared with the preoperative level. TMH:there existed obvious differences (P=0. 025, 0. 019, 0. 026<0. 05) in 1wk, 1 and 2mo compared with the preoperative level. No such obvious difference ( P = 0. 375>0. 05 ) existed in 3mo compared with the preoperative level.
CONCLUSION: Femtosecond laser surgery affects the stability of the tear film at a certain time and a certain extent. The mechanism related to many factors. It is temporary and lighted.

METHODS: The 120 myopic patients ( 240 eyes ) who underwent femtosecond laser surgery LASIK from August to September 2013 were collected, and these patients were followed up for 3mo. The patients were divided into two groups according to the gender, group A was male (110 eyes of 55 patients); group B was female (130 eyes of 65 patients). Dry eye symptom score, tear break-up time ( BUT ) , Schirmer Ⅰ test, corneal fluorescein staining were recorded preoperatively and postoperatively in 1wk,1,2,3mo.
RESULTS: Dry eye symptom score: it was statistically significant between two groups after operation in the 1wk, 1, 2mo(P = 0. 000,0. 023, 0. 030). It had no statistical significance between the two groups in 3mo(P=0. 283). BUT: it was statistical significance between two groups after operation in the 1wk, 1, 2, 3mo ( P= 0. 000, 0. 017, 0.026, 0. 032 ). Schirmer Ⅰ test: it was statistically significant between two groups after operation in the 1wk, 1, 2mo(P = 0. 012,0. 024, 0. 018). It had no statistical significance between the two groups in 3mo ( P=0. 206 ) Corneal fluorescein staining:it was statistically significant between two groups after operation in the 1wk, 1, 2, 3mo (P=0. 022,0. 015, 0. 036, 0.041).
CONCLUSION: The influence of tear film after femtosecond laser surgery for men less than that for women.
6.Changes of tear film after LASlK with corneal flap created by femtosecond laser and microkeratome

To observe the changes of tear film on the patients after laser in situ keratomileusis(LASlK)with corneal flap created by femtosecond laser and microkeratome.
●METHODS: Totally 150 patients (300 eyes) with myopia received operation of LASlK. Patients were divided into two groups according to the methods of making corneal flap. The patients of group one were assigned to receiving LASlK with corneal flap creation by lntralase femtosecond laser (190 eyes of 95 patients), group two were assigned to receiving LASlK with corneal flap creation by microkeratome ( 110 eyes of 55 patients ). Dry eye symptom score, tear break-up time (BUT), Schirmer Ⅰtest(Slt), corneal fluorescein staining(FL)were recorded preoperatively and postoperatively at 1wk; 1, 3 and 6mo.
●RESULTS: Dry eye symptom score: there existed obvious differences at 1wk; 1, 3mo between two groups(P<0. 05). While no such obvious differences existed in the 6mo between two groups(P>0. 05). BUT: there existed obvious differences at 1wk, 1, 3mo between two groups(P<0. 05). While no such obvious differences existed at 6mo between two groups(P>0. 05). SchirmerⅠ test: there existed obvious differences in the 1wk, 1, 3mo between two groups(P<0. 05). Whileno such obvious differences existed in the 6mo between 2 groups(P>0. 05). FL: there existed obvious differences in the 1wk, 1, 3mo between two groups(P<0. 05). While no such obvious differences existed in the 6mo between two groups(P>0. 05).
●CONCLUSlON: The early stability of tear film decrease after operation in both of the two groups. The dry eye symptoms are lighter and recover faster.

ObjectiveTo compare the efficacy and side effects of ilioinguinal/iliohypogastric nerve blockades and rectal paracetamol after pediatric inguinal hernia repair.MethodsNinety children undergoing half inguinal hernia repair were randomly divided into 3 groups:nerve block group(n=30),paracetamol group(n=30) and control group(n=30).After basal anesthesia,ilioinguinal/iliohypogastric nerve blockades was administed in nerve block group,paracetamol group received rectal paracetamol,control group had not any medication.Every child was oberserved 1,3,6,8 h postoperatively for pain score,overall satisfaction were evaluated by parents,furthermore,evaluation of distress for children such as nausea,vomiting and delayed femoral nerve palsy was made.ResultsPain scores were significantly lower in nerve block group and paracetamol group during the postoperative follow-up 1,3 and 6 h.Overall satisfaction in nerve block group and paracetamol group were significantly higher than control group.The incidence of delayed femoral nerve palsy in nerve block group was higher than paracetamol group(F=4.22P

A comparable study were carried out by determination of trace elements on five marine-derived shell traditional Chinese medicine (TCM) (Ostreae Concha, Haliotidis Concha, Margaritifera Concha, Meretricis Concha, and Arcae Concha), which were recorded in the Chinese Pharmacopoeia (2010 version). Seven trace elements in 51 batches of this type of shell TCM were analyzed by Inductively Coupled Plasma Mass Spectrometry (ICP-MS), combined with principal component analysis (PCA) methods. The content of element Se, which exhibited significant differences among different drugs, could be used as a key element to distinguish this type of drugs. Meanwhile, the contents of elements Co, Cu, Mo, and Ba in Haliotidis Concha, Co and As in Margaritifera Concha, Mo and As in Meretricis Concha, Mo, As, and Ba in Arcae Concha, and Zn in Meretricis Concha were relatively stable. In the PCA plot, Arcae Concha and Meretricis Concha could be efficiently distinguished from Ostreae Concha together with Haliotidis Concha, and Margaritifera Concha. The results also showed a correlation with their medicinal function. In conclusion, trace elements in marine-derived shell TCM could not be neglected for their quality control.

10.A novel integrated patient specific instrumentation system and its application for total knee arthroplasty

Objective To study the accuracy of preoperative planning and postoperative satisfaction of prosthesis posi?tion by applying the personal surgical cutting guide (PSCG) to total knee arthroscopy (TKA) based on 3D printing technique. Meth?ods In this randomized study, 20 patients were selected from August 2014 to October 2015, who were randomly divided into two groups. In PSCG group, 4 males and 6 females were enrolled, with a mean age of 70.9±6.69 (from 59 to 78) years. In con?ventional instrumentation group, 2 males and 8 females were enrolled, with a mean age of 69.9 ± 6.35 (from 58 to 77) years. The conventional instrumentation group was treated with conventional TKA, while the PSCG group was treated with personal surgical cutting guides. Then the knee valgus angle of patients, the angle between prosthesis components on the coronal and the sagittal plane of two groups were evaluated. Subsequently the correlation of the actual osteotomy amount, the valgus angle, caster angle, external rotation angle between intra? and preoperative planning were compared in the PSCG group. Results Compared with the ideal value of each angle, the mean deviation of the hip?knee?ankle angle 0.77°±0.51°, frontal femoral compo?nent angle 0.37° ± 0.53° and frontal tibial component angle 0.11° ± 0.24° showed statistically significant difference between two groups (P<0.05). The mean deviation of the lateral femoral component angle and the lateral tibial component angle (respectively 8.26°±0.85°, 0.71°±0.77°) were smaller than that of the conventional instrumentation group, while the differences all had no statis?tically significant difference (P>0.05). The actual osteotomy amount, the valgus angle, caster angle, external rotation angle corre?lated well between intra?and preoperative planning (t=-2.547, 3.864, 0.537,-0.040,-1.290, P>0.05). Conclusion TKA assist?ed by PSCG can make lower extremity alignment and accuracy of prosthesis implantation more accurate compared with convention?al TKA, especially in hip?knee?ankle angle, frontal femoral component angle and frontal tibial component angle.
