1.Promoting interventional radiology in clinical practice of emergency medicine
Journal of Interventional Radiology 2001;0(06):-
Interventional radiology has lot of advantages in dealing with various emergencies. The technique is minimally-invasive,highly-effective and immediately-efficient,moreover,it integrates the diagnosis with the therapy perfectly. Besides, the interventional techniques applied in emergency medicine include not only the vascular interventions, such as embolization, embolectomy, etc, but also the nonvascular interventions, such as tracheal stent implantation, percutaneous vertebroplasty and so forth. However, importance has not been attached to the clinical use of interventional therapy in emergency medicine so far. It is imperative for us to promote the acceptance of interventional therapy in emergency medicine as well as to popularize the technique in clinical practice.

In this paper, the process performance indexes (PPIs) P(p) and P(pk) were introduced and applied to evaluate the process capability and quality consistence of Chinese medicine products. The historical quality analysis data of Qingkailing injection were collected and taken as the research object. The confidence intervals of P(p) and P(pk) were estimated based on the Bootstrap sampling methods. Results showed that the value and width of the confidence interval P(p) of were smaller that of P(pk) suggesting that the P(pk) index was more sensitive than P(p) in process capability analysis. Within the production period concerned, the P(pk) values estimated from different quality indexes of Qingkailing injection, such as baicalin, cholic acid, geniposide and total nitrogen, were 1.122, 2.055, 1.564 and 0.891, respectively. It could be found that the cholic acid had the highest process capability, followed by the geniposide and baicalin. The total nitrogen had the lowest process capability, indicating that it is necessary to reinforce the quality management of total nitrogen related manufacturing processes. The case studies demonstrate the effectiveness and feasibility of PPIs, which are convenient to be used in production practice of Chinese medicine.

Objective To explore the feasibility of echo planar imaging with short time inversion recovery(STIR-EPI)diffusion weighted imaging with background signal(DWIBS)suppression in breast cancer. Methods The diffusion weighted imaging(DWI)with background suppression(b=800 mm2/s)was performed in 26 patients with breast cancer.Apparent diffusion coefficient(ADC)of all lesions, vere measured and compared.3D maximum intensity projection(3D-MIP)and reverse black and white technique were used to show the lesions.DWI and DWIBS were performed and compared for the detection of breast cancer. Randomized blocks analysis of variance was used for the ADC values in different breast tissues.the ADC values in breast cancer and benign lesion were compared using t test. The paired chi square tes:was used for the detection rate of breast cancer in two different imaging methods. Results Most of the breast cancers were hyperintense on DWI(b:800 mm2/s).,rhe ADC value of caneer tissue was(0.93±0.25)×10-3mm2/s,tumor necrosis was(2.06±0.17)×10-3 mm2/s,normal breast tissue was (1.92±0.23)×10-3 mm2/s and metastatic lymph node was(1.10±0.14)× 10-3mm2/s and the differenees were statistically significant between two structures(P<0.01).The signal of the tissue8 around the breast cancer was decreased on DWIBS with 3 D-MIP and reverse black and white technique and similar to positron emission tomography(PET).DWIBS sequence detected more breast cancers(92.3/)and metastatic lymph nodes(88.4/)than DWI sequence did(57.6/and 42.3/,respectively,X2:8.307,P<0.05;X2=12.235,P<0.05).The ADC values in breast cancer(1.09±0.17)×10-3mm2/s and benign lesion (2.15±0.53)×10-3 mm2/s had significant statistical differences(t=8.626,P<0.05).conclusion Diffusion weighted MRI with background suppression can detect more lesions than DWI and can be potentially applied for the detection of the breast cancer eombining the ADC value.

Objective To explore the operative methods for the first metatarsal fractures. Methods From January 2003 to January 2006, surgical operation was done on 17 patients with the first metatarsal fractures including 15 males and two females at average age of 40.5 years (11-65 years). There were four patients with the first metatarsal base fractures, seven with the first metatarsal shift frac-tures and six with the first metatarsal neck or head fractures, of whom four were with open fractures. The surgical treatment included open reduction, plate internal fixation and screw or K-wire fixation. Results Of all, 14 patients were followed up for average 14 months ( 12-24 months), which showed wound healing at one stage, without any complications. The bone union time was 8-16 weeks (mean 10.5 weeks). All patients could walk with weight loading after mean 13 weeks (9-18 weeks), without obvious pain or com-plaints. According to the midfoot and forefoot scale of American Orthopedic Foot and Ankle Society (AOFAS), the mean score was 86.3 points (78-100 points). Conclusion Anatomic reduction and stable internal fixation is the best solution for the first metatarsal fracture and plays important role in recovery of foot form and foot arch function.

To explore the effect of different processing methods and conditions of coumarin and volatile compounds in Angelica Dahuricae Radix and their change regularity, in order to optimize and establish appropriate drying methods and conditions. After being cleaned, fresh Angelica Dahuricae Radix herbs were baked, sun-dried, shade-dried, sun-dried after sulfur-fumigation, dried by quick-lime embedding, freeze-dried, microwave-dried. Finally, 24 groups of samples were obtained after being mashed and passing through the 60-mesh screen. The HPLC-PDA method was adopted to simultaneously determine the content of coumarin compounds. The GC-MS method was used to determine the content of volatile compounds. The principal component analysis (PCA) was made on the standardized analysis results for the 24 groups of samples processed with different drying methods. According to the PCA results, the comprehensive scores of coumarin and volatile compounds in Angelica Dahuricae Radix herbs processed with different methods in the order from high to low were that unpeeled and dried by quicklime embedding > unpeeled and dried with hot-air at 100 degrees C > unpeeled and dried with hot-air at 40 degrees C > peeled and infrared-dried > peeled and dried with hot-air at 60 degrees C > peeled and dried with hot-air at 40 degrees C > peeled and sun-dried > peeled and dried with hot-air at 60 degrees C > peeled and dried with hot-air at 100 degrees C > peeled and microwave-dried > peeled and dried with hot-air at 80 degrees C > unpeeled and sun-dried > unpeeled and dried with sulfur-fumigation > peeled and dried with sulfur-fumigation > unpeeled and dried with hot-air at 120 degrees C > unpeeled and freeze-dried > unpeeled and infrared-dried > peeled and dried with hot-air at 120 degrees C > peeled and freeze-dried > peeled and dried by quicklime embedding > unpeeled and dried with hot-air at 80 degrees C > peeled and shade-dried > unpeeled and shade-dried > unpeeled and microwave-dried. According to the findings, different drying processing methods have certain impacts on the content coumarin and volatile compounds in Angelica Dahuricae Radix herbs. The traditional method of drying by quicklime embedding is recommended as the optimum origin processing method of Angelica Dahuricae Radix, which is followed by the method for being peeled and dried with hot-air at 100 degrees C.

Chinese Pharmacological Bulletin 2017;33(3):312-316
Methotrexate (MTX)has dual effects of anti-inflam-matory and immune suppression, and its pharmacological mecha-nism is complex, diverse and synergistic. This paper summari-zes the main anti-inflammatory mechanism of low-dose MTX,in-cluding inhibition of JAK/STAT pathway, inhibition of inflam-matory reaction and immune response, increasing the accumula-tion of adenosine and the function of intracellular metabolites (methotrexate polyglutamate). In addition,low-dose MTX can inhibit oxidation by decreasing the level of lipid peroxidation, suppress the inflammatory response to secondary spinal cord in-jury,reduce spinal cord ischemia reperfusion injury and neuro-pathic pain, thus playing a neuroprotective role by a series of pharmacological mechanism. The anti-inflammatory mechanism of low-dose MTX and its application in spinal cord injury were reviewed,to guide the further research on the anti-inflammatory effect of MTX,and provide a theoretical basis for new drugs for clinical treatment of spinal cord injury.
