OBJECTIVE: To study the neuroprotective effect of facial nerve monitoring in canal wall down of mastoidectomy with tympanoplasty. METHOD: Twenty cases of chronic suppurative otitis media were performed canal wall down of mastoidectomy with tympanoplasty under general anesthesia. Facial nerve monitoring was used during surgery to locate the facial nerve. Threshold level of kinetic current and amplitude of synchronous reaction were recorded. RESULT: Seven cases with cholesteatoma were found facial nerve partial exposured in tympanic segment or mastoid segment. 13 cases had integrated facial never canal. Synchronous myopotential response of facial nerve was evoked successfully in all cases. Electroshock threshold had significant difference between facial nerve exposed group and non-exposed group (P<0.01). No facial palsy were found in all cases. CONCLUSION Facial nerve monitoring is benefit in locating facial nerve in canal wall down of mastoidectomy with tympanoplasty and could avoid facial nerve injury during surgery.

BACKGROUNDZengshengping (ZSP) tablets had inhibitory effects on oral precancerous lesions by reducing the incidence of oral cancer. However, the severe liver toxicity caused by systemic administration of ZSP limits the long-term use of this anti-cancer drug. The purpose of this study was to evaluate the tumor inhibitory effects due to the topical application of extracts from ZSP, a Chinese herbal drug, on 7, 12-dimethlbenz(a)anthracene (DMBA) induced oral tumors in hamsters. The study also investigated the anti-cancer mechanisms of the ZSP extracts on oral carcinogenesis.

METHODSDMBA (0.5%) was applied topically to the buccal pouches of Syrian golden hamsters (6 - 8 weeks old) three times per week for six weeks in order to induce the development of oral tumors. Different fractions of ZSP were either applied topically to the oral tumor lesions or fed orally at varying dosages to animals with oral tumors for 18 weeks. Tumor volume was measured by histopathological examination. Tumor cell proliferation was evaluated by counting BrdU labeled cells and by Western blotting for mitogen-activated protein kinase (MAPK) protein levels. The protein levels of apoptosis marker Caspase-3 and regulator Bcl-2 protein were also measured by Western blotting.

RESULTSTopical application of DMBA to the left pouch of hamsters induced oral tumor formation. Animals treated with DMBA showed a loss in body weight while animals treated with ZSP maintained normal body weights. Both the ZSP n-butanol fraction and water fraction significantly reduced tumor volume by 32.6% (P < 0.01) and 22.9% (P < 0.01) respectively. Topical application of ZSP also markedly decreased the BrdU-positive cell numbers in oral tumor lesions and reduced the expression level of MAPK. In addition, ZSP promoted tumor cell apoptosis by increasing Caspase-3 expression but decreasing Bcl-2 protein production.

CONCLUSIONThe n-butanol and water fractions of ZSP are effective at inhibiting tumor cell proliferation and stimulating apoptosis in oral cancer suggesting that these fractions have chemopreventive effects on DMBA induced oral carcinogenesis.

Objective To evaluate the clinical safety and efficacy of percutaneous interventional therapy in pediatric patients with secundum atrial septal defect(ASD).Methods Clinical data of 40 patients(age≤2 years)with secundum atrial septal defect treated in our hospital from February 2014 to December 2017 were analyzed retrospectively. There were 13 males and 27 females in these patients.Ultrasound of heart showed that there were 37 patients with single ASD,3 patients with multiple ASDs.One associated with pulmonary stenosis(PS),and 1 associated with patent ductus arteriosus.There were 6 patients with pulmonary hypertension, and the diameter of ASD was (10.6 ± 2.0) mm. All patients were proved to have secundum atrial septal defect before intervention.In the intervention,the transport system was delivered along the femoral vein,inferior vena cava and right atrium through atrial septal defect to the left atrium,and the occluder was released there. Results Of the 40 patients, 38 cases were successfully implanted, and the other two patients were not satisfied with the location of occlusion.The diameter of the ASD occluder was(12.0±2.1)mm and the transport sheath 7-9 F.Plug2 occluder was implanted in the patient with patent ductus arteriosus.To the patient with PS,pulmonary valve balloon angioplasty was performed,and then the pressure gradient reduced obviously, after that ASD occlusion was performed. The total follow up period was from 2 months to 3 years.No residual shunt and unsatisfactory device position were found during the follow up period.The pulmonary pressure reduced to normal,and the right atrium and right ventricle were smaller in a different degree. All patients had no arrhythmia and other complications.Conclusion Transcatheter closure of ASD is safe,reliable,and has fewer complications.It is worthy of popularization and application.Appropriate occluder should be selected according to the size and edge of ASD to reduce complications,such as residual shunt and valve injury.

OBJECTIVETo investigate the effects of maternally administered dexamethasone and ambroxol on the mRNA levels of surfactant proteins (SP-A, SP-B and SP-C) expression in fetal rat lungs at gestational age day 19.

METHODSA 19-day fetal rat lung model was employed. In situ hybridization was used to detect the expression of SP-B mRNA in alveolar type II cell, and the levels of SP-A, SP-B and SP-C mRNAs were detected by semi-quantitative reverse transcriptase-polymerase chain reaction (RT-PCR).

RESULTS(1) SP-B mRNA was detected in situ in alveolar type II cells in fetal rat lung of day 19 gestational age; (2) In the late developmental period of fetal rat lungs, alveolar type II cells were also found around bronchus; (3) Comparing to beta-actin mRNA, the relative values of SP-A, SP-B and SP-C mRNAs were 0.81 +/- 0.26, 0.97 +/- 0.20 and 0.88 +/- 0.11 in fetal lung in the control group. The relative values of mRNAs of SP-A, SP-B and SP-C to beta-actin were 1.04 +/- 0.16, 1.28 +/- 0.29, 1.09 +/- 0.25 in fetal lungs of the ambroxol injected rats, and were 1.08 +/- 0.25, 1.23 +/- 0.35, 1.21 +/- 0.25 in fetal lungs of the dexamethasone injected rats, respectively. Both ambroxol and dexamethasone-treated rats had significantly higher mRNA expression of surfactant proteins compared to the control saline injected animals (P < 0.05). (4) There were no significant differences between ambroxol and dexamethasone in the effects of increasing expressions of surfactant protein mRNAs (P > 0.05).

CONCLUSIONAntepartum administration of both ambroxol and dexamethasone can significantly increase fetal lung SP-A, SP-B and SP-C mRNAs expression.

The purpose of this study is to determine if paeonol can protect hippocampal neurons against injury due to oxygen-glucose deprivation (OGD) injury. The rat neurons were cultured in an OGD environment and the model of OGD injury was established. Paeonol and MK-801, a positive control drug, were added before deprivation. Neuron viability was measured by the reduction of MTT; glutamate was analyzed by amino acid analyzer; binding activity of NMDA receptor was evaluated by liquid scintillation counting and the expression of NMDA receptor NR1 subunit mRNA was semiquantitatively determined by RT-PCR. Compared with OGD injury group, paeonol treatment obviously increased cell survival rate and reduced the binding activity of NMDA receptors and the release of glutamate; and down-regulating the expression of NR1 subunit. These results suggest that paeonol may exhibit its protective effect against OGD injury by the action on NMDA receptor of rats.

OBJECTIVE: To investigate the clinical features, the CT, MRI features and therapeutic strategy of nasal neuroendocrine carcinoma (NEC). METHOD: A retrospective study was carried out on 6 cases of NEC. 2 were female and 4 were male. Routine preoperative CT and MRI examination were performed. In one case,the tumor had invaded the brain and then was treated in department of oncology. Five cases were undertaken tumor resection under nasal endoscope, and combined with postoperative chemoradiation therapy. RESULT: The postoperative pathological report of 1 cases was large cell type, the remaining 4 cases were small cell type. One case treated in department of oncology had lived for 2 month with tumor in his body. One cases relapsed and died at 4 month after postoperation, 1 case was lost and 3 case did not relapse till now. CONCLUSION NEC of paranasal sinuses is malignant, but early detection, early diagnosis and treatment can improve the prognosis of this disease. For limited lesion of NEC, nasal endoscopic operation is an effective, safe and minimally injury method.

OBJECTIVETo investigate the effects of inspiratory muscle training followed by non-invasive positive pressure ventilation in patients with severe chronic obstructive pulmonary disease (COPD).

METHODSThis investigator-initiated randomized, controlled trial recruited 88 patients with stable GOLD stage IV COPD, who were randomized into 4 equal groups to continue oxygen therapy (control group) or to receive inspiratory muscle training followed by non-invasive positive pressure ventilation (IMT-NPPV group), inspiratory muscle training only (IMT group), or noninvasive positive pressure ventilation only (NPPV group) for at least 8 weeks. The outcomes of the patients were assessed including the quality of life (SRI scores), maximum inspiratory pressure (MIP), maximum expiratory pressure (MEP), dyspnea (MRC scores), 6-min walking distance (6MWD) and lung function.

RESULTSs Compared to baseline values, SRI scores, 6MWT and MRC scores increased significantly after 8 weeks in IMT-NPPV, IMT and NPPV groups, and the improvements were significantly greater in IMT-NPPV group than in IMT and NPPV groups (P<0.05 for all). In IMT-NPPV and IMT groups, MIP and MEP increased significantly after the training (P<0.05), and the improvement was more prominent in IMT-NPPV group (P<0.05). No significant changes were found in pulmonary functions in the groups after 8 weeks of treatment (P>0.05).

CONCLUSIONInspiratory muscle training followed by non-invasive positive pressure ventilation, compared with inspiratory muscle training or non-invasive positive pressure ventilation alone, can better enhance the quality of life, strengthen the respiratory muscles, improve exercise tolerance and relieve the dyspnea in patients with COPD.

OBJECTIVE: To study molecular epidemiological basis of non-syndromic hearing loss in Yangzhou area. METHOD: The selected objects were 90 severe non-syndrome deafness students in special education schools in Yangzhou city, Jiangsu province. The deafness gene chip diagnostic kit was used for screening the nine hot spots mutations in four common deafness-related genes in Medical Testing Center of Northern Jiangsu People's Hospital. These nine hot spots gene mutations included GJB2 (35 delG, 176 del16, 235 delC and 299 delAT) GJB3 (538C > T), SLC26A4 (IVS7-2A > G, 2168A > G) and mtDNA 12S rRNA (A > G,1494C > T) mutation detection by line. RESULT: In 90 patients, 64 patients were found to carry deafness mutations by using gene chip diagnostic kit (the rate of mutation was 71.7%) GJB2 gene mutation in 40 cases (44.4%)which included 235 delC homozygous in 20 (22.2%) cases, 235 delC single heterozygous mutation in 4 cases (4.4%)and 235 delC and 299 delAT compound heterozygous mutations in 2 case (2.2%) separately. 299 delAT single heterozygous mutation in 2 case (2.2%), 299 delAT simple mutation in 2 case (2.2%). 176 del16 heterozygous mutations in 2 case, 176 del16 homozygous mutation in 2 case (2.2%). 176 del16 heterozygous mutations, and 235 del C heterozygous mutation in 6 cases (6.7%). SLC26A4 gene mutations in 22 cases (24.4%),which included IVS7-2A > G homozygous in 2 (2.22%) cases, IVS7-2A > G and 2168A > G compound heterozygous mutations in 2 cases (2.2%), IVS7-2A > G single heterozygous mutation in 18 cases (20.2%), and mtDNA 12S rRNA A > G mutation in 2 cases (2.2%), GJB3 mutations were not detected. CONCLUSION The deafness gene diagnostic techniques is worth applying for screening and diagnosis.

OBJECTIVETo study the expression of MUM-1/IRF4 and its significance in follicular lymphoma.

METHODSNinety-eight cases of follicular lymphoma were enrolled into the study. They were graded according to the 2008 WHO criteria. The expression of MUM-1/IRF4 protein and other markers (CD10, bcl-6, bcl-2 and Ki-67) was studied using tissue microarray and immunohistochemistry.

RESULTSAmongst the 98 cases studied, there were 24 grade 1 cases, 30 grade 2 cases, 26 grade 3A cases and 18 were grade 3B cases. The rates of expression of MUM-1/IRF4, CD10, bcl-6, bcl-2 and Ki-67 (≥ 25%) were 39.8% (39/98), 62.2% (61/98), 80.6% (79/98), 87.8% (86/98) and 50.0% (49/98), respectively. MUM-1/IRF4 predominantly expressed in high-grade follicular lymphoma and showed a significantly positive correlation with lymphoma grade (r = 0.628, P = 0.000) and Ki-67 index (r = 0.473, P = 0.000). MUM-1/IRF4 expression had a significantly negative correlation with CD10 expression (r = -0.597, P = 0.000), but no correlation with bcl-6 and bcl-2 expression.

CONCLUSIONSMUM-1/IRF4 expression is significantly higher in high-grade follicular lymphoma, indicating that these cases have a high proliferative activity, more aggressive behavior and poorer prognosis. MUM-1/IRF4, when strongly expressed, is another helpful marker for the diagnosis of high-grade follicular lymphoma.
