Medical Journal of Chinese People's Liberation Army 1981;0(06):-
Thirty three tibial fractures treated with interlocking intramedullary nail(IMN) were reviewed. Of all the patients, 16 were fixed with reamed IMN, and 17 with nonreamed IMN. There were 22 closed and 11 open fractures. Twenty seven patients were followed up for an average of 17.9 months (4 to 31 months). Of all the patients, 22 healed in 6 months, delayed healing occurred in 1,and in 1 the distal locking screw was broken. In the close fractures, the mean time from fracture to operation was 3.55 days (from the injury day to 27 days) , and in the open fractures operation was all done on the day of injury. Nonreamed IMN was used in 12 closed fractures (54.5%, 12/22 ) , and reamed IMN was used in 4 open fractures (36.3%, 4/11). There was no postoperative compartment syndrome or infection. The interlocking intramedullary pin is indicated for the tibia fractures, and early operation is feasible . The nonreamed interlocking intramedullary pin is suitable for open fractures and multi injuries.

Objective To define the three-dimensional relationship of the uncinate fasciculus, anterior commissure, inferior occipitofrontal fasciculus, and optic radiation, to determine the positioning landmarks of these white matter tracts by using the diffusion tensor tractography (DTT). Methods The anatomy was studied in 10 adult human brain specimens (20 hemispheres). DTT of the brain was performed on 10 healthy volunteers. DTT of the white matter tracts in the temporal stem was performed by using of the simple one regions-of-interest (ROI) and muhiple-ROl based on the anatomic knowledge and conventional magnetic resonance imaging (MRI). Results The inferior limiting sulcus averaged (46.3 ± 3.1)mm in length. The shortest distance from the inferior limiting sulcus to the superior floor of the temporal horn averaged (6.5 ± 1.8)mm. The posteroinferior insular point was the anterior extremity of intersection of the Heschl's gyrus and the inferior limiting sulcns. On the inferior limiting sulcns, this point was the posterior border of the optic radiation, and the temporal stem begined at the limen insulae and ended at the posteroinferior insular point. Its length averaged (33.0 ± 2. 9) mm. The distance from the limen insulae to the tip of the temporal horn averaged(10.9 ± 1.7)mm is just one thirds of the length of the temporal stem. The uncinate fasciculus and anterior commissure made up the core of the anterior temporal stem, with the anterior commissure located psoteriorly, and they occupied the anterior one third of the temporal stem together. The inferior occipitofrontal fasciculus passed through the entire temporal stem. The most anterior extension of Meyer's loop was located between the anterior tip of the temporal horn and the limen insulea. However, Most of the optic radiation crossed the postmedian two thirds of the temporal stem. Conclusion On the inferior limiting sulcus, the posteroinferior insular point is a reliable landmark of the posterior border of the optic radiations. The limen insulae, anterior tip of the temporal horn, and posteroinferior insular point may be used to localize the white matter fibers of the temporal stem in analyzing MR imaging or during surgery.

Objective To evaluate the role of delta and kappa opioid receptors in sufentanil preconditioning (SPC)-induced attenuation of myocardial ischemia-reperfusion (I/R) injury in rats. Methods Forty adult male Sprague-Dawley rats, aged 2-3 months, weighing 250-330 g,were randomly divided into 5 groups (n =8 each) using a random number table:sham operation group (group S),group I/R,group SPC,δ receptor antagonist naltrindole + SPC group (NTD + SPC group),and κ receptor antagonist nor-binaltorphimine (nor-BNI) + SPC group (BNI + SPC group).Myocardial I/R was induced by 30 min occlusion of left anterior descending branch of coronary artery followed by 120 min reperfusion. In group SPC,5 min infusion of sufentanil 3μg/kg was repeated 3 times at 5 min interval before myocardial ischemia. In NTD + SPC group, naltrindole 5 mg/kg was intravenously injected,at 10 before SPC.In BNI + SPC group,nor-BNI 5 mg/kg was injected intravenously at 15 min before SPC.Arterial blood samples were collected at 120 min of reperfusion for measurement of the serum cardiac troponin I (cTnI) and creatine kinase isoenzyme-MB (CK-MB) concentrations. The rats were then sacrificed and hearts removed for determination of myocardial infarct size (IS) and area at risk (AAR).IS/AAR ratio was calculated. Results Compared with S group, the serum cTnI and CK-MB concentrations were significantly increased in I/R,NTD + SPC and BNI + SPC groups, and the serum CK-MB concentrations were increased in SPC group, and IS/AAR ratio was increased in I/R group (P < 0.05 or 0.01).The serum cTnI and CK-MB concentrations were significantly lower in SPC,NTD + SPC and BNI + SPC groups and IS/AAR ratio was lower in SPC and BNI + SPC groups than in I/R group, and higher in NTD + SPC and BNI + SPC groups than in SPC group (P < 0.05 or 0.01).Conclusion Both κ and δ opioid receptors mediate SPC-induced attenuation of myocardial I/R injury in rats.
