1.Modified dacrocystorhinostomy with lacrimal duct intubation for chronic dacryocystitis and its complications

AlM: To discuss the efficacy of modified dacrocystorhinostomy with intubation in the treatment of chronic dacryocystitis combined with lacrimal duct narrow or obstruction.
METHODS: Retrospective analysis of 56 patients who underwent modified dacrocystorhinostomy with intubation in our hospital from July 2010 to September 2011.
RESULTS: All patients underwent the extubation of lacrimal and catheters irrigation, the patency rate was 95% at 2a after operation.
CONCLUSlON: Modified dacryocystorhinostomy combined with intubation is a effective method for chronic dacwocystitis combined with lacrimal duct obstruction, which has simple operative procedure, high success rate.
2.Study on the changing status of morphological development among minority students in China, from 1985 to 2005

Objective To identify the changes of morphological development status on minority students in China from 1985 to 2005. Methods We selected a total of 15 groups of the Chinese minority students as subjects of the study, including Mongolian, Hui, Uygur, Zhuang, Korean, Tibetan, Yao, Li, Qiang, Buyi, Dong, Miao, Tu, Salar, Kirgiz, with data from the Chinese national survey on students' physical fitness and health condition in 1985, 1995, 2000 and 2005. Height, weight and waist of the subjects were calculated and analyzed. Results From 1985 to 2005, the growth and characteristics of height in the Chinese minority students had a similar increase when comparing to the Han students, but with different degrees. However the growth rate was gradually decreasing. The average heights of Kirgiz, Korean, Salar and Mongolian schoolboys aged 18 years old were 170 cm, being 170.91 cm, 170.47 cm, 170.29 cm and 170.27 cm, respectively, which were close to that of the Hart students. Some minority students had a substantial increase of body weight. However, the waist of some minority students decreased. Only a few groups of minority students had increasing waist, such as Mongolian and Korean rural boys, Mongolian, Zhuang, and Korean rural girls, with the growth being 0.101 cm, 0.095 cm, 0.126 cm, 0.163 cm and 0.107 cm, respectively. Uygur, Mongolian, Kirgiz and Korean students had the morphological development similar to Han urban students, especially Uighur boys and girls. Conclusion From 1985 to 2005, The height, weight and waist of Chinese minority students had an overall increase at different degrees. In order to improve the physical fitness of minority students, awareness on nutrition and health education of both students and parents should be strengthened. Surveillance and programs on growth, development and health status of the minority children and adolescents should also be carried out continuously.
3.Diffusion tensor imaging and three-dimensional brain fiber tracking for the diagnosis of multiple sclerosis

Objective To demonstrate the diffusion tensor imaging(DTI)characteristics of multiple sclerosis(MS)plaques,periplaque white matter regions and normal appearing white matter (NAWM)regions in patients with MS,and to evaluate the clinical values of DTI and three-dimensional brain fiber tracking for the diagnosis of MS.Methods Conventional MRI and DTI were performed in 32 patients with MS and 32 age-matched control subjects.Fractional anisotropy(FA)and apparent diffusion coefficient(ADC)maps were generated and coregistered with T_2-weighted MR images,FA and ADC values were calculated in regions of interest in plaques,periplaque white matter regions,NAWM regions and white matter regions in control subjects.And three-dimensional brain fiber tracking maps were generated by using the DTI.Results TheADCwas(1.233?0.119)?10~(-3)mm~2/s in MS plaques,(0.973?0.098)?10~(-3) mm~2/s in periplaque white matter regions,(0.748?0.089)?10~(-3)mm~2/s in NAWM,and(0.620? 0.094)?10~(-3)mm~2/s in control subjects.The FA was 0.225?0.052 in MS plaques,0.311?0.050 in perip]aque white matter regions,0.421?0.070 in NAWM,and 0.476?0.069 in control subjects. 5.Effects of standardized three-stage rehabilitation treatment on neurological deficit scores and ADL performance of ischemic stroke patients

Objective To observe the effects of standardized three stage rehabilitation treatment on the neu- rological deficit scores (NDS) and ADL performance of ischemic stroke patients.Methods A total of 164 ischemic stroke patients were recruited and randomly divided into a rehabilitation group and a control group.The neurological function and ADL performance of the patients were assessed by using NDS and Modified Barthcl Index (MBI) at the admission,at the end of 1st,3rd and 6th months post stroke.Results No significant differences were found be- tween the rehabilitative and the control groups with regard to NDS and MBI at admission.The NDS demonstrated a decreasing tendency,while the MBI score an increasing tendency in both groups.In the control group,significant difference of NDS was found between admission and the end of 1st month as well as between the end of the 1 st and the 3rd months.In rehabilitation group,significant difference was revealed between all the time points with regard to NDS and MBI scores.At the end of the 1st,3rd and 6th months,the MBI scores of the rehabilitation group were signifi- cantly higher than those of the control group,indicating that the ADL performance of those treated with standardized three-stage rehabilitation protocol was improved quicker than those without the protocol.Conclusion Standardized three-stage rehabilitation treatment could improve the neurological function and ADL performance of the ischemic stroke patients.
6.Clinical significance of serum anti-microbial antibodies test in inflammatory bowel disease

Objective To investigate the clinical significance of serum anti-Saccharomyces cerevisias antibody (ASCA),anti-outer membrane porin C (anti-OmpC),antibody to Pseudomonas fluorescens-associated sequence I2 (anti-I2 )and antibody to bacterial flagellin (anti-CBirl )in the diagnosis and treatment of inflammatory bowel disease (IBD).Methods From 2011 to 2013,87 patients with IBD were enrolled and divided into Crohn′s disease (CD)group (66 cases)and ulcerative colitis (UC)group (21 cases).A total of 62 age and gender matched healthy individuals were enrolled as the control group. Fasting blood samples (2 mL)of the subjects were collected.The expression of ASCA,anti-OmpC,anti-I2 and anti-Cbirl antibodies was detected with enzyme-linked immunosorbent assay (ELISA)kits.The diagnosis value of each antibody in IBD and the differential diagnostic value of in UC and CD were compared by receiver operating characteristic (ROC)curve.Results The area under the curve (AUC)of ASCA between IBD and the healthy control group,between CD group and UC group was 0.580 and 0.512, respectively;the accuracy in diagnosis was low.The AUC of anti-CBirl between IBD and the healthy control group was 0.617.There was no differential diagnosis significance of the other antibodies.The positive rate of ASCA in IBD group was 62.1 % (54/87),which was significantly higher than that in the control group (38.7%,24/62).The positive rates of anti-OmpC and anti-I2 in IBD group was significantly lower than those in the control group and the differences were statistically significant (both P <0.05).No difference was observed in positive rates of serum antibodies among the others groups (all P >0.05).The specificity,sensitivity,positive predictive value (PPV)and negative predictive value (NPV)of ASCA in differential diagnosis of CD and UC was 52.4%,66.7%,81 .48% and 33.33%,respectively.The specificity and sensitivity of anti-OmpC,anti-I2 and anti-CBirl in differential diagnosis of CD and UC was 81 .0% to 100.0% and 9.1 % to 37.9%,respectively.The specificity,sensitivity,PPV and NPV of double-positive ASCA and anti-I2 in the diagnosis of CD was 57.1 %,86.4%,82.6% and 50.0%, respectively.The positive rate of ASCA and anti-I2 in CD group was significantly higher than that in UC group (84.8%(56/66)vs 57.1 % (12/21 );χ2 =5 .633,P =0.018 ).Conclusions Positive ASCA has some significance in the diagnosis of patients with IBD in our country.The detection of anti-I2 can help to diagnose ASCA negative CD.Because of low sensitivity and positive rate,anti-OmpC and anti-CBirl have limited value in the diagnosis of IBD and the differential diagnosis of UC and CD in our country.
7.Clinical application of conventional magnetic resonance imaging and diffusion-weighted imaging in differentiating histopathological types of small hepatocellular carcinoma

Objective To study the imaging apperances and the diagnostic value of conventional magnetic resonance imaging (MRI) and diffusion-weighted imaging (DWI) in differentiating histopathological types of small hepatocellular carcinoma (sHCC).Methods 40 sHCC confirmed by histopathology were classified into 4 groups according to their degree of differentiation:well (n=6),well-moderate (n=5),moderate (n=27) and moderate-poor (n =2).All patients received conventional MRI and DWI (1.5T,b =0 and 600 s/mm2) before the operation.The ADC values of the sHCC were measured and compared.Results On T1WI,32 lesions showed hypointensity,4 hyperintensity (well) and 4 isointensity (well-moderate =2,moderate =2).On T2WI,hyperintensity was observed in 39 lesions and isointensity in 1 lesion (well).Steatosis in the sHCC was seen in 17 of 40lesions (17/40,42.5 %,well=4,well-moderate=1 and moderate=12).A pseudocapsule was seen in 67.5 % sHCC (27/40,well=4,well-moderate=3,moderate=18 and moderate-poor=2).32 lesions showed hypervascularity on arterial phase,and 8 lesions showed hypovascularity (well=3,moderate =4,moderate-poor=1).On DWI,37 lesions showed hyperintensity,except for 3 lesions with welldifferentiated sHCC which showed isointensity (50%,3/6).The mean ADC values±S.D.of sHCC in the well,well-moderate,moderate and moderate-poor groups were (1.757 ± 0.337) × 10-3,(1.917±0.574)×103,(1.816±0.545)×103 and (1.723±0.217)×10-3,respectively.There were no significant differences among the 4 groups.Conclusion The imaging appearances of wellmoderate,moderate and moderate-poor sHCC on conventional MRI were classical which make diagnosis easy.Hyperintensity on DWI contributed to diagnosis.However,the imaging appearances of some well-differentiated sHCC were atypical.The lesions could be isointensity or hyperintensity on DWI.The combination of conventional MRI and DWI contributed to better diagnosis of sHCC,especial for atypical sHCC.

The etiology and pathogenic mechanism of autism spectrum disorders (ASD) are still unclear. The relationship between vitamin D and ASD has drawn attention in recent years due to common vitamin D deficiency in children with ASD. This article reviews the peripheral blood levels of vitamin D in children with ASD, the possible reasons for hypovitamin D and its possible roles in the etiology of ASD and the efficacy of vitamin D supplementation in ASD.

9.Magnetic resonance imaging-based diagnosis of spinal extradural angiolipoma

Objective To analyze the magnetic resonance imaging (MRI) features of spinal epidural angiolipoma to improve the diagnostic accuracy of this tumor. Methods The MRI findings in 2 patients with histopathologically confirmed spinal extradural angiolipoma were analyzed retrospectively. Results In both of the cases, the tumors were located in the extradural space of the thoracic intraspinal canal. The spindle-shaped tumor mass extended parallel to the longitudinal axis of the spine. The lesions presented with fat and vascular signal features. One of the tumors grew through the intervertebral foramen with a dumbbell appearance, invading the adjacent diaphragma, vertebral body, pedicle of the vertebral arch and the spinous process. The other tumor consisted of some tortuous and dilated blood vessels with flow void signals. Conclusion Spinal epidural angiolipoma presents characteristic features on MRI, which is considered the best diagnostic modality for this tumor.
10.Quantitative 3D simulation research of MRI diffusion tensor white matter fiber tractography in patients with multiple sclerosis

Objective To study the quantitative fiber tractography variations and 3D simulation patterns in patients with multiple sclerosis (MS),and assess the correlation between quantitative fiber tractography and scores of expanded disability status scale (EDSS).Methods MR diffusion tensor imaging was applied in 28 MS patients and 28 age-matched healthy volunteers.Then,quantitative tractography was generated in all subjects; and mean numbers of tracked lines and fiber density were measured.Paired-samples t tests were used to compare tracked lines and fiber density in MS patients with those in controls.Analysis of variance of ADC value and FA value was applied to MS patients having demyelinating plaque and normal appearing white matter,and controls.Linear regression model was used to determine the relationship between quantitative fiber tractography and scores of EDSS in MS patients.Results Tractography's tracked lines (2220±100) and fiber density (0.75±0.04) in MS patients were less than those in controls (2750±70) and (0.93±0.02,P<0.05).In MS patients having plaque and normal appearing white matter,and controls,the ADC values of ROI were (1.23±0.13)× 10-3 mm2/s,(0.76±0.09)×10-3 mm2/s and (0.63±0.10)×10-3 mm2/s,respectively,with a gradual declined trend; and the FA values were 0.24±0.04,0.42±0.07 and 0.48±0.06,respectively,with a gradual increased trend; significant differences were noted between each 2 groups (P<0.05).Both tracked lines and fiber density were found negative correlations with scores of EDSS in MS patients (P<0.05).Conclusion Damages of white matter tracts in MS patients can be detected by diffusion tensor fiber tractography,and it provide more space information than conventional MRI.
