Yinchenwuling powder is a classic prescription of hygrosisdissipating and choloplania,which has been commonly used till now,with the further research and development of Yinchenwuling powder,It is not only nourish the liver and gallbladder,but also reducing blood lipid and anti-atherosclerosis,and has good effected in antiallergic,antiinflammation,analgesia and other aspects.Often used in jaundice,hepatitis,hyperlipidemia,gouty Arthritis,etc..Now we discussed the pharmacological action and clinical application of Yinchenwuling powder.

Objective To afford an new effective imaging technique for the morphological observation of the normal male pelvic floor by observing the normal male puborectalis with transrectal ultrasound and magnetic resonance imaging (MRI). Methods Thirty-six normal males were examined by transrectal ultrasound and MRI,data of puborectalis were recorded and compared including starting point, end point,appearance, structure, thickness and the relationship with peripheral organs. Results The results including the appearance,direction and internal echo of puborectalis and the relationship with peripheral organs can be clearly displayed by transrectal ultrasound compared with MRI. The thickness of right and left puborectalis at rest measured in ultrasound and in MRI were (6.22±0.57)mm, (6.19±0.59) mm, (6.20±0.61) mm and (6.17±0.53) mm, respectively. The thickness by two methods and two sides had no significant differences (P>0.05). The thickness of puborectalis measured by two methods did not correlate with age,height,body weight,the volume of prostate and interspinal diameter respectively. Conclusions Transrectal ultrasound is a new imaging method to observe male puborectalis. Identification for nomal puborectalis sonography is the foundation of diagnosis pelvic floor disease.

Objective To study the relationship between the job satisfaction and the work value among contract nurses.Methods A total of 1221 contract nurses in 3-Class general hospitals were investigated by using the short-form of Minnesota Satisfaction Questionnaire (MSQ) and the Work Value Questionnaire.Results Using stepwise regression analysis to determine the main effect factors of the job satisfaction,the result indicated that economic rewards,interpersonal relationship,intellectual stimulation,aesthetics,comfort,social status,altruism(β=-0.319,0.239,0.203,0.124,-0.095,0.077,0.073)significantly impacted on the job satisfaction.Conclusions The work value of contract nures affect their job satisfaction.Contact nurses paying more attention to interpersonal and intellectual stimulation are more satisfied with work while those payingmore attention on economic rewards are dissatisfied.

OBJECTIVETo research the application and clinical effect of titanium elastic nails (TEN) for the treatment of closed clavicular fracture in elder children.

METHODSFrom October 2010 to December 2012,16 elderly children with clavicular fracture of elder children were treated with internal fixation, including 9 boys and 7 girls with an average age of 14.2 yeats old ranging from 9 to 17 years. The mean time between injury and surgery was 2 to 6 days. Thirteen patients had a transverse fractures; 3 patients had a oblique fractures, excluding the long-oblique patients. Neer function score after 3 months follow-up and the shoulder activity degree after 3 weeks followed were compared respectively between the injured side and healthy side. Fracture reduction and healing were followed up by X-rays to analyze internal fixation with TEN technique.

RESULTSAll patients were healed well after surgery. There were 2 cases with soft tissue irritation. No cases of infection, TEN broken, delayed healing or skin bursting were observed. All patients were followed up for 3 to 10 months,with an average of 7.2 months. There were no significant differences (t=2.12, P>0.05) between the healthy side (98.75±2.70) and injured side(96.69±6.22)of Neer score at 3 months during followed up. The shoulder activity at 3 weeks during followed up was different between both sides (P<0.05). The intramedullary nail was removed after X-rays at 12-18 weeks during followed up showed good bone healing,and the shoulder function was recovered well.

CONCLUSIONInternal fixation with TEN for the treatment of clavicular fracture has advantages of a limited invasion, beauty, safety and reliability. This technique provides an ideal fixation option for the treatment of clavicular fracture in elder children.

Objective To evaluate the value of transperineal targeted biopsy with real-time fusion images of transrectal ultrasound (TRUS)and multiparametric magnetic resonance imaging (mpMRI)for the diagnosis of prostate cancer (PCa).Methods Clinical and imaging data of 62 consecutive patients suspected of PCa at the mpMRI scan and PSA>4.0 μg/L were retrospectively analyzed.Targeted biopsies (TB)were carried out for each cancer-suspicious lesion,and followed a systematic 12-core biopsy (SB) protocol.Pathological findings of TB and SB were analyzed.Results The age of the patients was (68.38± 6.57)years (range 5 1 -79 years).The preoperative PSA value was (10.21 ±5.57)μg/L (range 4.5 -30.1μg/L).Preoperative prostate volume was (34.05±9.86)ml (range 19-64 ml).The PCa patients detected by SB and/or TB were 34 (54.8%).Cancer-detected rates of SB and TB cores were 7.53% and 26.2%, respectively (P <0.001).The positive core length of SB and TB cores were (3.71 ±2.77)mm (range 1 -14 mm)and (5.00±3.04)mm (range 2-1 7 mm),respectively (P =0.016).The positive core percent of SB and TB cores were (28.77 ± 20.13 )% (range 7 - 100%)and (35.76 ± 1 8.73 )% (range 1 1 % - 100%), respectively (P =0.048).Moreover,clinically cores detected by the SB and TB for final diagnosis of PCa were 19 cores (2.6%)and 48 cores (1 8.5%),respectively (P < 0.001 ).Conclusions Transperineal TB using real-time TRUS and mpMRI fusion imaging can improve sampling quality and elevate clinically detection rate of PCa when application combined with SB.

Objective To evaluate the effects of Dermlin and Mepilex Border combined with oxygen therapy on bedsore. Methods 60 neoplastic hematologic disorder patients suffering from bedsores arising from various causes were randomly divided into observational group (n =30) and control group (n =30). The former group was treated by Dermlin and Mepilex Border combined with oxygen therapy, the latter one was treated by timing irradiation of infrared lamp and routine surgical methods. The recovery conditions of bedsores were observed. Results The effective rate of the observational group was significantly higher than those of the control group [( 100％ vs 60％ ) ;x2 =17.78 ,P ＜0. 01]. There were significant differences in healing time and times of dressing change between both groups [( 11.4 ± 1.13 ) h vs ( 3.9 ± 1.13 ) h, (21.2 ± 1.39) vs (2.8 ± 0. 71 ) ;t =3. 403,3.411;P ＜ 0. 01]. Conclusions Dermlin and Mepilex Border combined with oxygen therapy has satisfactory effects on treating bedsores arising from various causes.

Objective To find out metabolic markers for hepatic injury caused by α-naphthyl isothiocyanate (ANIT) in rats with jaundice,and to explore the mechanism of Yinchen Wuling powder.Methods According to the weight,SD rats were randomly divided into 6 groups,10 rats in each group:normal group,model group,control group and the low,middle and high doses of three experimental groups.The experimental group was treated with 8.7,17.4,25.5 g · kg-1 · d-1 Yinchen-Wuling powder water decoction;control group was treated with ursodeoxycholic acid for 10 mg · kg-1 · d-1;the other two groups were administered with 0.9％ NaCl 2.0 mL · d-1 for consective fifteen days.The ANIT olive oil solution was given orally in single dose to establish the model.The serums were obtained after 48 h later.The serum data was extracted by gas chromatography-mass spectrometry.The principal component analysis and sparse partial least squares discriminant analysis were combined to reduce the dimension of the data.The VIP values of the model group and the normal group were compared to find the different metabolites and their involved metabolic pathways were analyzed.Results Fourteen metabolic markers were identified.There were significant differences in the relative peak intensity of metabolic markers between the model group and the normal group(P ＜0.05,P ＜0.01,P ＜0.001).The relative peak intensity:linoleic acid (1.72 ±0.60)％,citric acid (0.58±0.16)％ in the model group decreased,and the arachidonic acid (3.07±0.80)％,serine (2.78 ± 0.74) ％ increased.After intervention by Yinehen Wuling power,the succinic acid was (0.43 ± 0.23) ％,citric acid was (2.72 ±2.02)％,linoleic acid was(7.44 ±4.54)％,glyceric acid was(0.31 ±0.14)％ and inositol was (4.40 ± 3.70) ％ in high dose experimental group,which have no significant differences with those in normal group (P ＞ 0.05).Conclusion The Yinchen Wuling powder has a certain treatment on the jaundice type hepatic jury induced by ANIT,its mechanism may be related to the regulations of oxidative stress metabolism,glyoxylate and dicarboxylate metabolism and amino acid metabolism.

OBJECTIVETo investigate the impact of early rapid virological response on the outcomes of hepatitis B associated acute on chronic liver failure during antiviral treatment.

METHODS106 acute on chronic liver failure patients in our hospital from January 2008 to July 2010 were enrolled in present study retrospectively. Besides internal medicine therapy, all patients received lamivudine (100 mg/d) or entecavir (0.5 mg/d) treatment. The profile of liver biochemistry, prothrombin time activity and viral load were detected at baseline and week 4, respectively. The patients were divided into HBV DNA negative group and HBV DNA positive group according to the viral load at week 4. The clinical features and treatment outcomes were compared between groups. Frequency variables were compared by x2 test or Fisher's exact test. Continuous variables were compared using independent samples T-test. The factors that impact on the treatment outcomes were determined using stepwise multivariate logistic regression analysis.

RESULTSAt the week 4, the TBil and PTA in HBV DNA positive group [(261.6+/-205.6)mumol/L and 44.7%+/-19.7%, respectively] were significantly different from those in HBV DNA negative group [(160.1+/-173.4) mumol/L and 56.8%+/-23.1%, respectively] ( t = 2.190 and -2.077, respectively, P less than 0.05). The non-effective rate of HBVDNA positive group (50%, 9/18) was significantly higher than that of HBV DNA negative group (14.8%, 13/88) (x2 = 9.235, P less than 0.01). By using stepwise multivariate logistic regression analysis, the disease stage and HBV DNA undetectable at week 4 were the independent factor. The OR values of disease stage and HBV DNA undetectable were 6.559 and 0.209, respectively, and 95% CI was 2.316~18.576 and 0.058~0.747, respectively.

CONCLUSIONThe rapid suppression of viral load by nucleotide analogue may improve the efficacy of hepatitis B associated acute on chronic liver failure treatment. The early rapid virological response within first 4 weeks may contribute to the prediction of the treatment outcomes.

To solve the issues of costly planting of facility cultivation method and inferior efficacy than wild herbs of Dendrobium officinale, the cliff epiphytic cultivation method was studied. To research the growth, agronomic traits, yield, polysaccharide and alcohol-soluble extract contents were measured on the D. officinale from different water regulation and cliff slope gradients treatments. The results showed that D. officinale epiphytic at 85 degrees-90 degrees cliff and sprayed water 1-2 h x d(-1) at the growing season can get better growth and obtain high yield, and the morphology has no different from wild cliff D. officinale, even in the environments without shade. The contents of polysaccharide and alcohol-soluble extract are closely related to the physiological ages, but significantly higher than the facility cultivation. It is possible that environmental stresses benefit the accumulation of polysaccharides, alcohol-soluble extract and other efficient ingredients.
