Vascular dementia,an acquired,persistent syndrome of intelligent disorder,mainly caused by cerebrovascular disease is one of the main types of senile dementia.Its pathogenesis remains unclear.Brain-derived neurotrophic factor sustains multiple neuronic survival,development, differentiation,and repair following injury.It also induces and maintains the long-term potentiation of hippocampus and cortex,and changes the morphologic plasticity of hippocampal neuron by regulating hippocanoal synaptic transmission and synaptic plasticity.It participates in the processes of hippocampus-dependent learning and memory,and it may play an important role in the onset and development of vascular dementia.

Objective To assess the clinical efficacy of electroanatomically guided mapping and radiofrequency ablation under CARTO system for premature ventricular contraction. Methods The CARTO electroanatomical mapping system displays real time three dimensional chamber structure with electrical information related to signal amplitude and activation time. Drugrefractory and frequent premature ventricular contractions were ablated under CARTO system. Results Frequent premature ventricular contractions were successfully ablated in all 8 patients with mean 2.2?1.7 radiofrequency applications under CARTO system. 6/8 frequent premature ventricular contractions occured in right ventricule, and 2/8 in left ventricule. After ablation, the premature ventricular contractions declined from 24 711?5 612 beats/24h to 0-5 beats/24h, and patient′s symptoms almost disappeared. No recurrent case was found during a period of 3-12 months following observation, and the premature ventricular contractions remained ≤10 beats/24h. Conclusions The CARTO electroanatomical mapping system, referred to the electrophysiologic data, may be applied in guiding the radiofrequency ablation of drug-refractory and frequent premature ventricular contractions in those patients who have no organic heart disease for its safety and accurate orientation.

Objective To observe the changes of osteoblasts and vascularization during osteogenesis by tissue engineering technique under the electron microscope and study the feasibility of improving vascularization of the tissue engineering bone by using the small intestine submucosa(SIS) as the scaffold. Methods The bone mesenchymal stem cells(BMSCs) were isolated by using the gradient centrifuge method.BMSCs were seeded in the SIS.The scaffold-cell constructs were cultured in vitro for two weeks.There were no cells on the SIS as control.They were implanted subcutaneously in the dorsa of the athymic mice.The implants were harvested after(in vivo) incubation for 4,8 and 12 weeks.The changes of osteoblasts and vascularization were observed under the transmission electron microscope and the scanning electron microscope. Results The BMSCs grew quite well.BMSCs differentiated on the surface of the SIS and secreted a great deal of extracellular matrices.The scaffold-cell constructs formed a lot of bone and vessels in vivo.The scaffold degraded after 12 weeks.No osteoblasts but vascularization and fibroblasts were observed as control. Conclusion SIS can be used as the scaffold for constructing tissue engineering bone as it can improve the formation of bone and vessels in vivo.

Objective To compare the time-course of relaxant effect of rocuronium between patients of Uygur and Han nationalities.Methods Thirty ASA Ⅰ or Ⅱ patients (15 Uygur nationality,15 Han nationality) of both sexes,aged 20-55 yr weighing 45-73 kg were divided into 2 groups (n=15 each):group U Uygur nationality and group H Han nationality.Anesthesia was induced with midazolam,propofol,fentanyl and suxamethonium chloride.Tracheal intubation was performed and the patients were mechanically ventilated.Recuronium 0.6mg/kg was then given iv.Neuromuscular function was monitored with acceleromyograph using single stimulation of the ulna nerve.The onset time (time from injection of recuronium to maximal paralysis),nonresponse time (duration of maximal twitch depression),clinical duration (time required for the twitch to return to 25% of control height after injection of initial dose),75%recovery time (time for twitch to return to 75%of control height after injection of initial dose) and recovery index (time for twitch height to return from 25% to 75% of control height) were recorded.Results There was no significant difference in the onset time between the two nationalities.The time of nonresponse period,clinical duration and 75% recovery time were significantly shorter in Uygur patients than in Han patients.Recovery index was significantly shorter in Uygur patients.Conclusion There is a significant difference in the time-course of muscle relaxant effect of recuronium between Uygur and Han nationalities.The influence of nationality on recommended dosage of recuronimn should be considered.

Objective To investigate the distribution and drug resistance of major pathogens for urinary tract infections in the First Affiliated Hospital of Nanjing Medical University.Methods Strains from midstream urine culture of patients with urinary tract infections were collected during January 1 and December 31,2011.All strains were identified by API system,and disk diffusion method was used for drug sensitivity test.Results Totally 1129 strains were isolated,in which 667 (59.1％ ) were Gram-negative strains,266 (23.5％) were Gram-positive strains,and 196 (17.4) were Candida.Among Gram-negative strains,Escherichia coli,Klebsiella pneumoniae and Proteus mirabilis were highly sensitive to carbapenem antibiotics; while Acinetobacter baumannii and Pseudomonas aeruginosa were highly resistant to most antibiotics including cephalosporins and penicillinase inhibitor,and the resistance rates were over 50％.Among Gram-positive strains,the major strains Enterococcus avium and Enterococcusfaecalis were completely sensitive to vancomycin and teicoplanin,and highly sensitive to linezolid (resistance rate below 10％ ).Candida albicans and Candida glabrata were highly resistant to voriconazole and fluconazole (with the resistance rates of 47.2％ - 60.0％ ), but were completely sensitive to amphotericin and nystatin.Conclusion Gram-negative strains account for most urinary tract infections in the First Affiliated Hospital of Nanjing Medical University with high drug resistance rates.

Objective To research the operation method for hypospadias by one-stage urethroplasty.Methods According to the length of urethra defect,18 cases were operated by using one-stage urethroplasty with perineal and preputial island flaps.Results 16 cases were successful in all patients(88.9%),except one case of urethrostenosis and urethral fistula respective.The case of urethrostenosis recovered by dilation of urethra after 5 months and the patient of urethral fistula was successfully repaired by operation after 8 months.All patients were followed up for 6～26 months with satisfactory penis and micturition.Conclusion The technique of one-stage urethroplasty by using perineal and preputial island flaps is the one of best choices for hypospadias.
