1.Treatment of femoral intertrochanteric instable fractures with cemented artificial bipolar arthroplasty for senile patients

Objective To discuss the feasibility and effects of cemented artificial bipolar arthroplasty for senile patients with femoral intertrochanteric instable fractures. Methods From January 2004 to June 2008,46 aged cases with femoral intertrochanteric instable fractures were cured with cemented artificial bipolar arthroplasty. According to improving-classfication of Evans,there were 19 cases with type Ⅲfractures and 27 cases with type Ⅳ fractures. Results All cases were successfully operated. The surgery time was 50-90 (68.00 ±20.43) minutes. Bleeding volume was 160-600 (210.00 ±40.26) ml. The time in hospital was 16-26 (19.30 ± 4.56) days. All cases were retrospectively reviewed for 6-39 (17.10 ± 4.33)months. Three cases died of other diseases, 43 cases were graded by the system of Harris. The Harris scale was 70-94 (86.70 ±5.16) scores,and the choiceness rate was 90.7%(39/43). No dislocation,loosening,subsidence and late infection occurred in all the implants. Conclusions Cemented artificial bipolar arthroplasty has good advantages of reduced laying up period and few complications. The short-term outcome is satisfactory.
2.Clinical application of stand-alone MC+PEEK cage in the anterior cervical fusion.

OBJECTIVETo explore the effect of clinical application of stand-alone MC+PEEK cage in anterior cervical fusion.
METHODSFrom January 2011 to January 2014,50 patients were treated with the MC+PEEK cage filled with autogenous cancellous illic-bone graft after anterior cervical discectomy. There were 22 patients with cervical spondylosis,26 patients with traumatic cervical disc herniation, 2 patients with cervical instability in these patients. There were 32 males and 18 females, aged from 30 to 79 years old with an average of 53.30 years old. There were 32 patients with single segment, 15 patients with double segments and 3 patients with three segments. Cervical AP and lateral and the flexion-extension X-rays were regularly taken in order to assess the cervical physiological curvature, the graft fusion and internal fixation related complications. Nerve function, clinical effect and bone fusion were respectively evaluated according to Japan Orthopedic Association (JOA), Otani grade and Suk method.
RESULTSAll patients were followed up from 6 to 36 months with an average of 20 months. No correlated surgical complications were found and all patients obtained bony fusion with an average time of 4.30 months. JOA score had significantly improvement after surgery (P < 0.05). The JOA score was 10.60 ± 3.00 before surgery and 16.10 ± 2.20, 16.40 ± 2.35 at one week and six months after surgery respectively. According to Otani grade,40 cases got excellent results, 9 good, 1 fair. No significant dysphagia and internal fixation related complications such as displacement of cages were found during the follow-up period.
CONCLUSIONUsing this cage in anterior cervical fusion can obtain satisfactory clinical effect with less operation injury and reduce the complications. It is a better fusion method in anterior cervical fusion.

3.Gene modification and high prokaryotic expression of porcine interferon alpha-1.

There are many E. coli rare codons in the gene of porcine interferon alpha-1. In order to obtain high expression of poIFN-alpha1 in E. coli, the cDNA encoded poIFN-alpha1 mature protein was synthesized using biased codons of E. coli without changing the original amino acid sequence and the terminator was changed as TAA. At the same time, Adenine and Thymine were used to the largest extent near the 5' terminus of poIFN-alpha1 mature protein gene. The synthesized gene was inserted into the Eco RI and Sal I site of the expression vector pRLC resulting pRLC-poIFN-alpha1. The poIFN-alpha1 is highly expressed in E. coli DH5alpha when the induction was carried out at 42 degrees C . The expressed poIFN-alpha1 account for 24.5% of the total cellular proteins and existed as inclusion body. The poIFN-alpha1 inclusion body was dissolved in 6mol/L guanidine chloride contained DTT and subsequently the denatured poIFN-alpha1 was re-natured by dilution in refolding buffer containing GSH and GSSH. In the present study it was found that the denatured poIFN-alpha1 was most efficiently re-natured in refolding buffer containing 1 mol/L guanidine chloride. In order to obtain pure protein, the concentrated re-natured poIFN-alpha1 was purified by Sephacryl S-200 chromatography. As a result, the purified poIFN-alpha1 is verified to be of high cytokine activity by inhibiting the cytopathic effect of vesicular stomatitis virus in MDBK cells, which is about 6.4 x 10(6) u/mg. This study paved the way for large-scale production of recombinant poIFN-alpha1 and its usage in virus disease control of pigs.

4.Effects of intensive insulin therapy on insulin resistance and serum proteins after radical gastrectomy.

OBJECTIVETo investigate the effects of intensive insulin therapy on insulin resistance(IR) and serum proteins after radical gastrectomy.
METHODSTwenty-two gastric cancer patients were randomly divided into the control (n=11) and intensive insulin therapy group (n=11), and underwent distal radical subtotal gastrectomy under epidural anesthesia. Fasting blood glucose (FBG), fasting insulin (FINS) and serum proteins were assayed preoperatively and at day 1, 3, 7 postoperatively. Insulin resistance index was calculated using homeostasis model assessment (HOMA). The length of hospital stay and postoperative complications were recorded respectively.
RESULTS(1)The levels of FBG, FINS, lnHOMA-IR (P<0.01,P<0.05) and the incidence of insulin resistance were remarkably decreased by intensive insulin therapy after the surgical procedure.(2)The levels of serum transferrin (TRF), prealbumin (PRE) and retinal binding protein (RBP) in the intensive insulin therapy group were significantly improved as compared to control group after operation(P<0.05). (3) The duration of fever, antibiotic use, passage of gas by anus, length of hospital stay and the occurrence of postoperative complications were also significantly lower than those in control group(P<0.01,P<0.05).
CONCLUSIONCompared to routine therapy, the intensive insulin therapy has more beneficial effects on the patients undergone distal radical subtotal gastrectomy in decreasing the insulin resistance, improving the status of nutrition and preventing postoperative complications.

5.Mechanism of alopecia in patients with paraneoplastic pemphigus.

OBJECTIVETo investigate the relationship between the levels of antidesmoglein (DSG) 1, 3 antibodies in the sera of patients with paraneoplastic pemphigus (PNP) and alopecia.
METHODSSera from PNP patients, bullous pemphigoid patients, and normal healthy subjects were collected and 2 tissue samples from 2 healthy scalps were resected. Anti-DSG 1, 3 antibodies in the sera of PNP patients were detected by enzyme-linked immunosorbent assay (ELISA). Indirect immunofluorescent assay was used to detect whether the antibodies in the sera of PNP patients binds with the follicular epithelium of normal healthy scalp.
RESULTSAnti-DSG3 autoantibody was strongly positive and anti-DSG1 weakly positive in one patient, while both two antibodies were negative in the other patient. Their sera could bind to keratinocytes and follicular epithelium in human scalp. Immunofluorescent signals were found on the intercellular epidermal cell surface and outer root sheath of the follicular epithelium. However, the immunofluorescent signals in the section incubating with serum of bullous pemphigoid were only found on basal membrane zone. No signals were found in the section incubating with normal healthy serum.
CONCLUSIONAlopecia in PNP patients are correlated with the anti-DSG3.

6.Relations between attitude and practice of smoking and the training program regarding tobacco control among community medical staff members in Hangzhou, Zhejiang province

Objective To investigate the relations between training and both the attitude and practice on smoking control among community medical staff members in Hangzhou,Zhejiang province.Methods Three representative districts including Xiacheng,Gongshu and Westlake were chosen from Hangzhou city.Questionnaire survey was applied to collect information from the related community medical staff members.The survey mainly contained three aspects:knowledge,attitude and practice regarding smoking control involved in the community medical activities.Availability and application of the resources on smoking cessation were also studied.Logistic regression analysis was applied to explore the factors associated with the smoking control training programs.Differences of rates between groups were assessed with chi-square statistics.Wilcoxon rank sum test was used to study the relationships among knowledge,attitude and practice related to smoking control programs,targeted to the community medical staff members.Results Eight hundred forty-six community medical workers were involved.Sixty-five percent of the community medical staff members had learned related knowledge on smoking control.Proportion of the community medical staff who had taken lessons on smoking control with 3-10 working years was 1.77 times more than the ones with experience less than two years (OR=1.77,95%CI:1.25-2.51).Eighty-eight point seven percent of the medical staff who had received smoking control training programs were identified with the consciousness that they should advise the patients to quit smoking,comparing to the proportion 81.60% (Z=-2.87,P=0.00) in the control group.In terms of the practice regarding smoking control,data showed that 21.62% of the medical staff who had received smoking control training programs would provide ‘how to quit smoking' to more than 90% of the smoking patients,while the proportion in the control group was 10.65% (Z=-5.68,P=0.00).The use of drugs,traditional Chinese medicine therapy and the smoking cessation hotline rate were all less than 30%.Conclusion The training programs being used on smoking control seemed useful in improving the consciousness and practice towards the smoking control programs during their medical activities among the community medical staff members.
7.Reparation role of recombinant human bone morphogenetic protein-4 mature peptide on hematopoietic system of mice with acute radiation injury

Objective To explore the reparation role of recombinant human bone morphogenetic protein-4 mature peptide (rhBMP-4m) on hematopoietic system injury induced by irradiation in mice.Methods Ninety BALB/c mice were randomly divided into normal control group,model group and rhBMP-4m treatment group,with 30 mice in each group.The mice in model group and rhBMP-4m treatment group were irradiated by 60Co γ-ray(7.5 Gy) for 200 s;while the mice in normal control group did not receive irradiation.After irradiation,the mice in model group were given physiological saline 1.0 mL by peritoneal injection per day for 6 days;the mice in rhBMP-4m treatment group were given rhBMP-4m 0.5 mg by peritoneal injection per day for 6 days.The peripheral white blood cell count and marrow mononuclear cells,the percent of CD34 + cells in marrow mononuclear cells of mice were detected on the first,third,fifth,seventh,ninth day after irradiation;the number of spleen nodus and the pleen weight/body weight ratio were detected at the ninth day after irradiation.Results There was no statistic difference in the number of peripheral white blood cells of mice among the three groups on the first day after irradiation(P > 0.05).The peripheral white blood cell count of mice in model group was significantly lower than that in the normal control group on the third,fifth,seventh,ninth day after irradiation (P < 0.01).There was no statistic difference in the number of peripheral white blood cells of mice between rhBMP-4m treatment group and model group on the third,fifth day after irradiation (P > 0.05);the peripheral white blood cell count of mice in rhBMP-4m treatment group was significantly higher than that in model group on the seventh,ninth day after irradiation (P < 0.05).The number of marrow mononuclear cells of mice in model group was significantly lower than that in the normal control group at each time piont after irradiation(P < 0.05).There was no statistic difference in the number of marrow mononuclear cells of mice between the rhBMP-4m treatment group and model group on the first,third day after irradiation (P > 0.05);the number of marrow mononuclear cells of mice in rhBMP-4m treatment group was significantly higher than that in the model group on the fifth,seventh,ninth day after irradiation(P < 0.05).The proportion of CD34 + cells in mononuclear cells of mice in model group was significantly lower than that in the normal control group at each time piont after irradiation(P <0.01).There was no statistic difference in the proportion of CD34 + cells in mononuclear cells of mice between the rhBMP-4m treatment group and model group on the first,third day after irradiation (P > 0.05);the proportion of CD34 + cells in mononuclear cells of mice in rhBMP-4m treatment group was significantly higher than that in the model group on the fifth,seventh,ninth day after irradiation(P < 0.05).Compared with the normal control group,the number of spleen nodus was increased and the spleen weight/body weight ratio of mice was decreased in model group on the ninth day after irradiation(P < 0.05);the number of spleen nodus and the spleen weight/body weight ratio of mice in rhBMP-4m treatment group were significantly higher than those in the model group on the ninth day after irradiation (P < 0.01).Conclusion rhBMP-4m can accelerate the reconstruction of bone marrow hematopoietic system of mice with hematopoietic system injury induced by irradiation.
8.Effect of TGF-β1 gene-modified dendritic cells on expressions of CD28/CTLA-4:B7 in peripheral blood mononuclear cells in rats with experimental autoimmune myasthenia gravis

Objective To explore the effect of dendritic cells (DC) modified with transforming growth factor β1 (TGF-β1) gene on the expressions of CD28/CTLA-4:B7 costimulatory molecules in peripheral blood mononuclear cells (PBMC) in the Lewis rats with experimental autoimmune myasthenia gravis (EAMG). Methods Thirty inbreeding line, healthy, female Lewis rats were divided randomly into 6 groups: normal group, EAMG group, DC treatment group, pcDNA3-TGF-β1-DCtreatment group, pcDNA3-DC control group and normal saline group. The rats were immunized with the AChR protein extracted from electric organ of Narcine timilei and CFA in the groups except normal group. 2×106 pcDNA3-TGF-β1-DCs/rat were injected subcutaneously into the backs of the rats which had been immunized 5 d earlier with AChR+CFA. The rats in DC treatment group, pcDNA3-DC control group and normal saline group were injected in parallel with untreated DC, pcDNA3-DC and normal saline, respectively. Seven weeks after the first immunization, the expressions of CD28 mRNA and CTLA-4 mRNA were detected by reverse transcription-polymerase chain reaction (RT-PCR), and the levels of B7-1 and B7-2 on the surface of PBMC were examined using flow cytometry. Results (1)The low expression of CD28 mRNA and rare expression of CTLA-4 mRNA were found in the normal rats, and both expressions increased markedly in EAMG rats (P<0.001). Compared to those in EAMG group, the expression of CD28 mRNA decreased and CTLA-4 mRNA was upregulated after the treatment with pcDNA3-TGF-β1-DC (P<0.05). There was no significant difference in the expressions of CD28 mRNA and CTLA-4 mRNA among the EAMG group, DC treatment group, pcDNA3-DC control group and normal saline group (P>0.05). (2) The expressions of CD28, CTLA-4, B7-1 and B7-2 on the surface of PBMC were rare in normal rats, which increased significantly in EAMG rats (P<0.001). The levels of CD28, B7-1 and B7-2 in pcDNA3-TGF-β1-DC group were lower than those in EAMG group (P<0.01), but the level of CTLA-4 was higher than that in EAMG group (P<0.05). They showed no statistically difference among the EAMG group, DC treatment group, pcDNA3-DC control group and normal saline group (P>0.05). Conclusions The expressions of CD28/CTLA-4:B7 costimulatory molecules are abnormal in the rats with EAMG. The regulation of CD28/CTLA-4:B7 costimulatory pathways may play a critical role in the mechanism of the treatment with DC transfected with pcDNA3-TGF-β1 in the incipient EAMG rats.
9.Effect of agonist CD40 monoclonal antibody on the tolerance of experimental autoimmune myasthenia gravis

Objective To investigate the effect of agonist CD40 monoclonal antibody (CD40mAb) on the tolerance of experimental autoimmune myasthenia gravis (EAMG) induced by immature dendritic cells (iDCs). Methods Lewis rats were equally randomized into normal group,EAMG group, tolerance group and CD40mAb treatment group (n=5). Rats in the tolerance group and CD40mAb treatment group were vaccinated with AChR pulsed iDCs; and rats in the CD40mAb treatment group were intraperitoneally injected CD40mAb at a dosage of 0.5 mg once when performing the vaccination and on the 2rd d of vaccination. One mL serum-free medium was given to the rats in the EAMG group; normal group did not receive any treatment. Three weeks after that, rats in the above 4 groups were immunized with AChR and complete Freund's adjuvant (CFA). Seven weeks after the immunization, the corresponding indexes of MG were observed: behavioral assessment was performed and electromyogram was employed to detect the repetitive nerve stimulation on these rats; enzyme-linked immunosorbent assay (ELISA) was used to determine the level of AChRab; the pathological changes of neuromuscular junction were also detected. Results Just as the rats in the normal group, the rats in the tolerance group did not have significant changes in any of the corresponding indexes of MG after being immunized with AChR and CFA. In contrast, rats in both EAMG group and CD40mAb treatment group showed typical changes in the corresponding indexes of MG: their electromyogram wave amplitude obviously attenuated; the level of serum AChRab increased and neuromuscular junction appeared as a typical damage of MG. Conclusion Agonist CD40mAb could abrogate the tolerance of EAMG induced by AChR pulsed iDCs, suggesting that the dysfunction of DCs is related to the priming of abnormal immune of MG.
10.Molecular epidemiological and clinical feature of human metapneumovirus in children with acute lower respiratory tract infection in Changsha area from 2007 to 2011.

OBJECTIVETo understand the epidemiological characteristics of human metapneumovirus (hMPV) infection in children with acute lower respiratory tract infection (ALRTI) in Changsha area, China.
METHODSA total of 2613 nasopharyngeal aspirate (NPA) specimens were collected from hospitalized children with ARTIs between September 2007 and February 2011 (a period of 3.5 years) in Changsha area, Hunan Province, China. Respiratory specimens were screened for hMPV M gene by reverse-transcription polymerase chain reaction (RT-PCR). All RT-PCR positive' amplification products were confirmed by sequencing or analyzed and compared with those in GenBank known hMPV reference strain.
RESULTShMPV was detected in 135 (5.2%) out of the 2613 children. The detected rate was significantly different between male and female (chi2 = 8.007, P = 0.003). The majority of the hMPV-positive patients (63.2%) were 0-1 year old infants. The majority of hMPV-positive patients were detected during spring season. Seasonal distribution showed that subtype A2b circulated predominantly in winter and spring, subtype B2 circulated predominantly in summer and spring. The most common genotype in 135 hMPV-positive patients were A and B in human subjects in Changsha. The predominant genotype of hMPV changed over the 3.5 year study period from genotype A2b to combined A2b and B2, and to B2. The 66 (48.9%) out of the 135 hMPV-positive patients were found to be co-infected with other respiratory viruses, and human bocavirus (HBoV) was the most common additional respiratory virus.
CONCLUSIONThese data suggest that some cases of acute lower respiratory infection in young children in Changsha area are related to hMPV and the majority of children infected with hMPV were less than 1 years old, the detected rate of boys was higher than that of girls. It circulated predominantly in spring. The subtype A2b genetic lineage and the subtype B1 genetic lineage of hMPV were revealed alternately in human subjects in Changsha area and the hMPV exhibited high co-infection rate with other respiratory viruses.
