1.Study on the relationship between the expression level of serum zinc α2 glycoprotein and type 2 diabetes mellitus complicated with hyperuricemia

Objective To investigate the relationship between the expression level of serum zinc α2 glycoprotein (ZAG) and type 2 diabetes mellitus (T2DM) complicated with hyperuricemia (HUA).Methods A total of 220 patients with T2DM and normal uric acid (group A),100 patients with T2DM complicated with hyperuricemia (group B),and 80 cases of normal healthy people (group C) were randomly selected.The clinical and biochemical indexes and expression levels of serum ZAG among three groups were determined and compared.The relationship between expression level of serum ZAG and related indexes was analyzed with Pearson correlation and multiple linear stepwise regression analysis.Results The serum ZAG levels showed significant differences among groups A,B and C [(58.89 ± 23.17)mg/L vs (86.42 ± 25.36) mg/L vs (40.65 ± 16.48) mg/L] (P < 0.05).Pearson correlation analysis showed that serum ZAG level was positively correlated with body mass index (BMI),triglyceride (TG),glycosylated hemoglobin (HbAlc),creatinine (Cr),urea nitrogen (BUN),and serum uric acid (SUA) (P < 0.05),and negatively correlated with high density lipoprotein cholesterol (HDL-C) (P < 0.05).Multiple linear stepwise regression analysis showed that BMI,TG and HDL-C were independent factors influencing serum ZAG level.Conclusions Serum ZAG level is significantly higher in patients with T2FM,especially in patients complicated with HUA.The increased level may be related to obesity and dyslipidemia,showing that serum ZAG may be involved in the pathogenesis of T2DM complicated with HUA.
2.Clinical efficacy of compound periploca liquid in treating condyloma acuminatum and its effect on human papilloma virus DNA expression in vitro.

OBJECTIVETo investigate the clinical efficacy of compound periploca liquid (CPL) in treating condyloma acuminatum (CA), and to explore its mechanisms at molecular level.
METHODSEighty-one patients with CA were randomly divided into three groups, 30 patients in Group A were treated with CPL, 21 in Group B were treated with periploca syrup and 30 in Group C were treated with Youtuoxin (YXG). The clinical efficacy and adverse reaction occurred in the three groups was evaluated respectively. Besides, change of human papilloma virus (HPV) DNA in two patients with CA was determined by polymerase chain reaction (PCR) in vitro after being treated with CPL and periploca, the monarchic drug of CPL.
RESULTSThe cure rate obtained in group A, B, and C was 56.67%, 42.86% and 63.33% respectively, the total effective rate in them was 83.33%, 71.43% and 86.67% respectively, the difference of therapeutic efficacy was insignificant among the three groups. But the adverse reaction occurrence in them (6.67%, 4.76% and 86.67%) was significantly different (P < 0.01). The recurrent rate in them was 14.29%, 12.5% and 47.1% respectively. PCR showed negative expression of HPV in the two samples of CA of same concentration after the verrucous homogenate suspension being treated with CPL or periploca syrup of different concentration, while it was positive after the suspension was treated with the group of normal saline without any drug.
CONCLUSIONThe therapeutic efficacies of CPL, periploca syrup and Youtuoxin are not significantly different, but the former two have advantages of less adverse effects and lower recurrent rate. And they are possibly having germicidal action on HPV-DNA in CA tissue in vitro.

3.Effects of delayed brain-derived neurotrophic factor application on cochlear pathology and auditory physiology in rats.

BACKGROUNDThe development and maintenance of spiral ganglion cells (SGCs) appear to be supported by neurotrophins. Removal of this support leads to their gradual degeneration. Intracochlear infusion with neurotrophins can provide trophic support to SGCs in animal deafness models if given shortly after deafening. However, it is not known whether delayed intervention will provide similar protection, which might be clinically relevant. The present research was conducted to determine the effects of brain-derived neurotrophic factor (BDNF) administration on the capacity of the peripheral processes to resprout.
METHODSThe left cochlea of 20 profoundly deafened rats, which were divided into 2 groups equally, was implanted with an electrode and drug-delivery system 30 days after deafening. Either BDNF or artificial perilymph (AP) was delivered continuously for 28 days. Electrically evoked auditory brainstem responses (EABRs) were recorded during the period. SGC body and peripheral process density were measured.
RESULTSThe EABR thresholds of AP increase continually. Those of BDNF increase slowly at the beginning then decrease, and were significantly less than those of the AP group from day 14 to 28 (P < 0.01). In terms of SGC and peripheral process density, the difference between the treated and control ears of BDNF group was clearly significant (P < 0.01), but not in AP group (P > 0.05). Analysis of the left cochlea between the two groups demonstrated that SGC/peripheral process density of the BDNF group was significantly greater than that of the AP group. Finally, a functional formula was developed relating the last EABR threshold and SGC density and process density, which was as follows: T = 466.184 - 2.71 (F.B.L).
CONCLUSIONSUnder the conditions of delayed intervention following 30 days after deafening in rats, it can be concluded that BDNF enhances SGC bodies and peripheral processes survival after differentiation and so improves auditory sensitivity. SGC peripheral processes influence the auditory sensitivity.

4.Micro RNA differential expression in primary retroperitoneal liposarcoma

Objective:To explore the miRNA (micro RNA)differential expression profile between primary retroperitoneal liposarcoma tissues and normal fat tissues,and to provide the evidence that miRNA were involved in the molecular pathways of primary retroperitoneal liposarcoma tissues' occurrence.Methods:Collecting retroperitoneal liposarcoma tissues and normal fat tissues from 4 patients after radical surgery of retroperitoneal lipsarcoma.Using microarray analysis.The tissues' miRNA hybridizated with human's LC Sciences microRNA Microarray-Single (miRBase 21.0) expression profile gene chips,and got the date.Analyzing the differential expressing of the siginal date by LOWESS.Results:Total 38 differential expressed miRNA were found (P<0.05),including 23 over-expression and 15 low-expression miRNAs.10 of them(38 differential miRNAs) was significant deviation (P<0.01),including 4 over-expression and 6 low-expression.Date analysis revealed that some miRNAs were associated some different tumors,Conclusion:The number of over-expression were more than the low-expression in primary retroperitoneal liposarcoma compared with the normol fat tissue,which indicate that the genes expression are less abundant in primary retroperitoneal li-posarcoma;some of the miRNAs might involved in the molecular pa-thways of primary retroperitoneal liposarcoma tissues' occurrence and recurrence,they might become the target point of the targetedtherapy of the primary retroperitoneal liposarcoma,some of the over-expressed miRNAs can become new biomarkers in the following diagnosis of the primary retroperitoneal liposarcoma.

Renal angiomyolipoma is a type of benign tumor that occurs sporadically in addition to being associated with tuberous sclerosis. Preoperative embolization of large tumors is important to avoid excessive blood loss during surgery. We reported a patient with a 5505-g giant renal angiomyolipoma in a solitary kidney. The patient was treated with preoperative embolization and radical nephrectomy without complications. This type of treatment for an enormous angiomyolipoma can reduce the risk of uncontrolled hemorrhage caused by rupture of the tumor during the operation and should be considered for the treatment of similar tumors.

6.Percutaneous transluminal stenting versus directional atherectomy for lower limb artery TASC Ⅱ Class A and B superficial femoral artery lesions

Objective To compare the effect and safety of stenting versus directional atherectomy (DA) in the treatment of TASCⅡ A and B superficial femoral artery lesions.Methods 100 patients with TASC Ⅱ A and B lesions were divided into percutaneous transluminal stenting(PTS) group (n =50) and DA group (n =50).Patients were compared in terms of technical success rate,treatment success rate,first operation cost,postoperative ankle brachial index (ABI),limb salvage rate,survival,and patency.Results The technical success rate in both PTS and DA group was 100%.The treatment success rate was 98% vs.86%,P>0.05.Postoperative ABI:0.82 ±0.19 vs.0.80 ±0.27,P>0.05.First operation cost:(34 820 ± 1 051) yuan vs.(45 635 ± 1 358) yuan,P <0.001;All patients were followed-up for up to 2-year,the cumulative patency rate was 81.6% vs.72.9% (P>0.05).Limb salvage rate was 97.9% vs.93.8 %,P > 0.05.Conclusion There were no significant differences in the effect and safety of PTS versus DA in the treatment of TASCⅡ A and B superficial femoral artery lesions.
7.Association of polymorphism on HLA-DRB1*04 alleles with outcome of hepatitis B virus infection.

OBJECTIVETo investigate the relation between the alleles of HLA-DRB1*04 and outcome of HBV infection.
METHODSThe alleles of HLA-DRB1*04 were detected by polymerase chain reaction-sequence specific primer (PCR-SSP). The frequency of allele of HLA-DRB1*04 in four groups[106 asymptomatic HBsAg carriers (group ASC), 93 chronic hepatitis B patients (group CHB), 77 patients with hepatitis B cirrhosis and 102 cases of spontaneous recovery after HBV infection (control group)] were studied, and the frequency of that in different replication of HBV was also studied.
RESULTSThe frequency of allele of HLA-RB1*04 in groups ASC, CHB and hepatitis B cirrhosis was markedly higher than that of control group (25.94%, 26.34%, 27.92% respectively versus 14.22%, P< 0.01); the frequency of HLA-DRB1*0401 in groups ASC, CHB and hepatitis B cirrhosis was also higher than that of control group (20.91%, 24.49%, 22.09% respectively versus 8.62%, P< 0.05, P< 0.01,P< 0.05 respectively); the frequency of HLA-DRB1*0405 in groups ASC, CHB and hepatitis B cirrhosis was lower than that of control group (3.64%, 2.04%, 3.49% respectively versus 15.52%, P< 0.01, P< 0.01, P< 0.05 respectively ). There was no statistical significance in the allele frequency of HLA-DRB1*04 among groups ASC, CHB and hepatitis B cirrhosis (P> 0.05), and the same result was observed in different replication of HBV (P> 0.05).
CONCLUSIONHLA-DRB1*04 gene is one of the factors which determine the outcomes of HBV infection, while it has no influence on HBV replication.

8.Changyanqing decoction produces anti-inflammatory effect by inhibiting the activation of nuclear factor-kappaB.

OBJECTIVETo detect the changes in intestinal mucosal permeation in rats with methotrexate-induced small intestinal damage and investigate the protective effects of Changyanqing decoction.
METHODSRat enteritis model was established by methotrexate (MTX) and sodium chloride. The rats were randomly divided into normal control group, model group, N-acetylcysteine (NAC) group and Changyanqing decoction group, and Changyanqing decoction (100 mg/kg) or saline was administered daily in the corresponding groups by gastric irrigation for 6 days. The disease activity index (DAI), colonic mucosal damage index (CMDI) and histological score (HS) of the rats were observed and evaluated. The levels of mRNA expressions of TNF-alpha and IL-1beta were detected by semi-quantitative RT-PCR. The expression of IL-10 was detected by enzyme linked immunosorbent assay, and IkappaB expression was determined with Western blotting.
RESULTSCompared with the normal control group, the model group showed significantly increased DAI, CMDI and HS. The DAI, CMDI, and HS in rats treated with Changyanqing decoction were significantly decreased in comparison with those in the model group (P<0.01). The expressions of TNF-alpha and IL-1beta were significantly higher in MTX-treated group than in the control group. The expression of TNF-alpha and IL-1beta mRNA in the Changyanqing group and NAC group were significantly lower, but IL-10 significantly higher than those of the MTX group. In MTX group, obvious NF-kappaB activation was observed, whose expression was significantly stronger in the cell nuclei, and the IkappaB in the cytoplasm was markedly degraded.
CONCLUSIONChangyanqing decoction offers protection on intestinal mucosa by inhibiting NF-kappaB activation to reduce TNF-alpha and IL-1beta mRNA expressions and increase IL-10 expression.

9.Effects of apolipoprotein A1 and B gene polymorphism on avascular necrosis of the femoral head in Chinese population.

OBJECTIVETo investigate the effects of Apo A1 and B gene polymorphism on avascular necrosis of the femoral head (ANFH) in north Chinese Han population.
METHODSPolymerase chain reaction-restriction fragment length polymorphisms (PCR-RFLP) technique was used in samples of 143 cases with documented ANFH and 92 healthy control matched by age and sex individuals selected from north Chinese Han nationality. The studied loci include promoter region (-75bp) and the intron 1 (+83 bp) of Apo A1 gene, Eco RI,Xba I of Apo B gene; polymerase chain reaction was used to study 3'-VNTR of Apo B gene.
RESULTSAt -75 bp in promoter, the frequency of A/A genotype in ANFH group was significantly higher than that in control group (P < 0.01), while the frequency of G/A genotype in ANFH group was significantly lower than that in control group (P < 0.01). No difference was found in the frequency of genotype at +83bp in intron 1 of Apo A1 gene, Eco RI, Xba I and 3'-VNTR loci of Apo B gene.
CONCLUSIONApolipoprotein A1 gene A/A substitution at position -75 in promotor is associated with ANFH, the mutation may be one of the sensitive genes of ANFH, first reported inside and abroad. But no evident relationship was found between gene polymorphism of +75 bp loci of Apo A1 gene, Eco RI loci of Apo B gene, Xba I loci of Apo B gene or 3'-VNTR of Apo B gene and ANFH.

10.Comparison of six-month follow-up results of primary percutaneous coronary intervention guided by optical coherence tomography or coronary angiography.

Objective: To compare the 6-month follow-up results of primary percutaneous coronary intervention (PPCI) guided by optical coherence tomography (OCT) or coronary angiography (CAG) alone in a larger ST-segment elevation myocardial infarction (STEMI) cohort. Methods: We enrolled 275 STEMI patients undergoing OCT-guided PPCI from March 2017 through December 2018. Two hundred and seventy-five propensity score matched STEMI patients undergoing CAG-guided PPCI served as control group. The 6-month clinical follow-up results were compared between the two groups. The demographic data, complications, coronary angiography and OCT characteristics were evaluated. Results: OCT evaluation showed that there were 151 patients (54.9%) with plaque prolapse and 113 patients (41.1%) with stent malposition. Proximal and/or distal dissection of stents occurred in 38 patients (13.8%), of which 3 patients (1.1%) had both proximal and distal dissection. Of the 38 patients, 2 patients received rescue stent implantation. Results of clinical follow-up at 6 months showed that there was no significant difference in cardiovascular death, repeat myocardial infarction, target vessel revascularization, stroke and hemorrhage endpoint events between OCT-guided PPCI patients and CAG-guided PPCI patients (P=0.682). Conclusion: Clinical events at 6 months are similar between OCT-guided PPCI and CAG-guided PPCI for STEMI patients.
