1.Study of effects and its mechanism of sevoflurane preconditioning on cardiovascular function of coronary heart patients

Objective To investigate the effects of sevoflurane preconditioning on cardiovascular function in patients with coro -nary heart diseases , and explore its mechanism .Methods Eighty patients with coronary heart disease during 2009 January to 2012 December from our hospital patients were selected and randomly divided into the study group ( 41 cases ) and control group ( 39 ca-ses) .Patients in study group were given sevoflurane preconditioning , while the control group were given propofol .A series of parame-ters were recorded, including the patient's heart rate (HR), central venous pressure (CVP), mean arterial pressure (MAP), stroke volume (SV), cardiac output (CO), systolic time ratio (STR) and serum cardiac troponin T (cTnT) before induction of anesthesia (T0), 30min (T1) during the operation, incision suture (T2), 6h after operation (T3), 12h after operation (T4), and 24 h after op-eration (T5).Results HR, CO, SV, CVP, MAP, STR, and cTnT of the research group were relatively more stable than the control group in the preoperative, intraoperative, postoperative, and recovery.The study group at CO, T1, SV, STR, and cTnT were (3.48 ±0.40)L/min, (6.4 ±1.9)ml/(min· m2), (0.36 ±0.76), (0.227 ±0.112)ng/ml, CO, SV, STR in T2 and cTnT were (3.58 ±0.52)L/min, (6.6 ±2.3)ml/(min· m2), (0.36 ±0.63), (0.241 ±0.115)ng/ml, control group T1 at CO, SV, STR and cTnT were (3.11 ±0.53)L/min, (5.2 ±2.1)ml/(min· m2), (0.46 ±0.81), (0.351 ±0.106)ng/ml, CO, SV, STR in T2 and cTnT were (3.15 ±0.61)L/min, (5.7 ±1.5)ml/(min· m2), (0.44 ±0.90), (0.311 ±0.112) ng/ml.The patients in study group were in T1 CO, SV CO in T1 , SV, T2 times higher than that in control group .STR in T1 , T2 point was lower than that of the control group, cTnT in T1 , T2 point was lower than that of the control group , and the difference between the groups was statistically significant ( P <0.05 ) .Research group of patients with CO , SV higher than that in control group at in T 1 ,T2 , and STR lower than that at T 1 , T2 , cTnT lower than that of the control group at in T 1 , T2 , and the differences were statistically significant ( P <0.05 ) .Conclusions Sevoflurane preconditioning can make the hemodynamics more stable , more suitable for clinical anesthesia , and its mechanism may be related to the changes of cardiac troponin T .

3.Expansive Cervical Laminoplasty by Anterior and Posterior Cervical Decompression in the Treatment of Cervical Spondylosis Myelopathy

Objective To evaluate the clinical results of expansive cervical laminoplasty by anterior and posterior cervical decompression in the treatment of Cervical Spondylosis Myelopathy with developmental cervical stenosis.Methods Retrospective analyze 32 patients of cervical spondylosis myelopathy treated with expansive cervical laminoplasty by anterior and posterior cervical decompression from Mar 1995 to Aug 2000 years, there were 21 males and 11 females, developmental cervical stenosis (DCS) with cervical intervertebral disc protrusion in 25 cases, with the ossification of cervical posterior longitudinal ligament (OPLL) in 7 cases. Neural functionality (JOA Scoring System), cervical axial symptoms, and pre-and post-surgery dynamic cervical spine lateral x-ray films and cervical vertebra MRI were evaluated. Results 32 cases had been followed up for an average of 24.3 months, with an average age 58.9 years (range 38-72). The average rate of improvement of the third and sixth in month postoperative,the first and third postoperative year was 58.1%,65.9%,69.1% and 68.4% respectively;the excellent and good was 71.9%,81.3%,78.1%,77.3% respectivery. Cervical axial symptoms were reduced. Instability of the cervical spine were not found and decompression was confirmed on MRI. Conclusion Expansive cervical laminoplasty by anterior and posterior cervical decompression in the treatement of cervical spondylosis myelopathy with developmental cervical stenosis have good efficacy and it is a stable operative method.
4.Value of pre- and postoperative red cell distribution width-to-lymphocyte count ratio in judging the prognosis of patients with non-metastatic colorectal cancer

Objective:To explore the value of pre- and postoperative red cell distribution width-to-lymphocyte count ratio (RLR) in predicting the disease-free survival (DFS) for non-metastatic colorectal cancer patients.Methods:The data of 108 patients pathologically diagnosed with non-metastatic colorectal adenocarcinoma after surgery from December 2012 to January 2020 in Hainan Hospital of PLA General Hospital were retrospectively analyzed. RLR was calculated and its differences in patients with varied clinicopathological characteristics were analyzed. The receiver operating characteristics (ROC) curve was applied to analyze the value of pre- and postoperative RLR in predicting DFS of patients, and the cut-off value was determined by Youden index. DFS of patients with different stratification stratified by a cut-off value of RLR was analyzed by using Kaplan-Meier method followed by log-rank test. Cox proportional hazards model was used to make univariate and multivariate analysis.Results:Youden index of pre- and postoperative RLR in predicting DFS was 8.86%, 9.15%; based on the above index, the patients were divided into the preoperative RLR<8.86% group (73 cases) and preoperative RLR≥8.86% group (35 cases), postoperative RLR<9.15% group (48 cases) and postoperative RLR≥9.15% group (60 cases). According to ROC curve, the area under the curve (AUC) of preoperative RLR in predicating DFS was 0.66 (95% CI 0.55-0.77, P = 0.01), and AUC of postoperative RLR in predicating DFS was 0.62 (95% CI 0.51-0.74, P = 0.04). Pre- and postoperative RLR cut-off values were used to predict 3-year DFS rate of patients with the sensitivity of 51.60% and 71.00%, respectively, and the specificity of 76.60% and 50.60%, respectively. Kaplan-Meier analysis showed that patients in preoperative RLR<8.86% group had a better DFS compared with those in preoperative RLR≥8.86% group ( χ2 = 7.35, P < 0.01); there was no statistical difference in DFS between postoperative RLR<9.15% group and postoperative RLR≥9.15% group ( χ2 = 3.69, P = 0.06). Preoperative RLR was an independent affecting factor for DFS of patients ( HR = 1.13, 95% CI 1.04-1.22, P <0.01). Conclusions:Pre- and postoperative RLR could be useful in predicating 3-year DFS for patients with non-metastatic colorectal cancer. Preoperative RLR is an independent affecting factor for DFS, and the patients with low RLR have a better prognosis.

Objective To observe the injury effect of ionizing radiation on bone marrow derived c-kit+ cells.Methods Via-bility of c-kit+ cells was measured by bioluminescence;the level of c-kit+ cells reactive oxygen species was measured by DCFH-DA, the ability of colony-forming units was reflected by CFU-GM;proliferation and apoptosis of c-kit+ cells were measured by flow cy-tometry;the variation of pathway was detected by arrays of gene chip.Results Compared to control group(0 Gy).It had a decrease of c-kit+ cells′cell viability and the ability of colony-forming units after the cells receipt irradiation with the dose of 1 Gy and 4 Gy;and the level of cell reactive oxygen species,ratio of apoptosis cells increased.After 1 Gy irradiation exposure,the ratio of prolifera-tion(S/G2/M phase)cells increased compared to control group.However,when the c-kit+ cells were receipt 4 Gy irradiation expo-sure,the ratio of proliferation(S/G2/M phase)cells decreased.After 4 Gy irradiation exposure,the up-regulate genes contained Srxn1,Psmb5,Cdkn1a,Smc1b,Bcl2l1,Lrdd and so on;the down-regulate genes contained Mpo,Mtf1,Chek1,Rcc1 Ebag9,Ciapin1 and so on.Conclusion There was injury effect of ionizing radiation on c-kit+ cells,and it could induce variation of many pathways.

7.Quality Control of Antenoron filiforme

Objective To establish the quality control method for Antenoron filiforme. Methods Antenoron filiforme was identified by TLC,and the content of gallic acid was determined by HPLC. The chromatographic conditions were as follows:Aichrom Reliasil C18 column (4. 6 mm×250 mm,5 μm) was used with mobile phase consisted of acetonitrle-0. 1% phosphoric acid(4∶96),the flow rate was 1. 0 mL·min-1 ,and the detection wavelength was 270 nm. Results TLC identification for An-tenoron filiforme was highly specific. Gallic acid content had a good linearity in the range of 0. 08-0. 56μg (r=1. 000 0). The av-erage recovery was 101. 12% and RSD was 2. 20% (n=6). Conclusion The method is simple, feasible,and reproducible, and can be used for the quality control of Antenoron filiforme.

Corneal endothelial cell(CEC)is the most critical part for the cornea, of which activity can influence the postoperative vision.It is very important for the clinical cornea preservation considering the function and its self-purification of donor cornea.There are a variety of classical methods, which can significantly prolong the saving time of donor cornea with its good quality of CEC.We reviewed the published papers about present preservation methods of cornea, which can give us many suggestions for the clinical cornea preservation.
10.Quantitative evaluation of common carotid arterial intima-media thickness and elasticity in healthy adults by ultrasound radio-frequency data technology

Objective To evaluate common carotid arterial intima-media thickness (IMT) and elasticity using ultrasound radio-frequency data (RF-data) technology, the normal values of the related parameters were determined in healthy adults for their potential clinical applications.Methods One hundred and sixty healthy adults were included and divided into 4 age groups with 20 males and 20 females in each group.The quantitative parameters of common carotid arterial IMT, common carotid arterial distensibility (CD),local pulse wave velocity(PWVβ) and stiffness(β) were calculated automatically.The correlations that the IMT, PWVβ and β were correlated positively with aging and CD was correlated negatively with between IMT and PWVβ or β, while no correlation between IMT and CD.The positive correlation was found correlation between IMT and age ( P<0.05) ;In 30~39 and 40~49 years old groups,PWVβ increased and CD decreased with aging;In 50~59 years old group,PWVβ and β increased with aging;But in 60~69 years these parameters changed with aging in subjects<60 years old.Conclusions RF-data technology could quantitativly evaluate common carotid arterial IMT and elasticity, the normal values of the related parameters were determined with aging,it may be a promising modality to assess the arterial condition.
