Background The pathogenesis of myopia is currently unclear. Studies showed that retinoic acid regulates the development of myopia. Whether IRBP, a retinoid transport proteins, participates in the development of myopia or not is unknown. Objective The aim of this study was to investigate the expression of interphotoreceptor retinoid-binding protein (IRBP) in the retina of guinea pig with lens-induced myopia and study the relationship between IRBP expression and experimental myopia. Methods Fifty guinea pigs were randomized into the normal controls(n=10) ,defocus Ⅰ group (n=20) and defocus Ⅱ group (n=20). Guinea pigs in the defocus Ⅰ groups wore -10. 00 D lenses on the right eyes for 14 days and the defocus Ⅱ group for 28 days. The left eyes of both defocus groups did not wear any lens served as self-control. The refraction diopter and axial length were measured before and after wearing lenses. The protein expression of IRBP in the retina was detected by immunohistochemisty and Western blot. RT-PCR was used to observe the level of IRBP mRNA in the retina. Result Compared with the control eyes,the right eyes in the defocus Ⅰ and Ⅱ groups developed to ( -5. 53±1.93) D and (-8.69±2.46) D, and the ocular axial length elongated to (0. 31±0. 15 ) mm and (0.41 ±0. 13 ) mm, showing statistically significant differences between them ( group Ⅰ: t= 12. 57,29.63 ,P＜0.05 ;group Ⅱ :t= 15.82,44.35 ,P＜0.05 ) ,and the degree of myopia was lower and axial length was shorter in group Ⅰ than the eyes of group Ⅱ ( P＜0.05 ). There was no statistically significant differences found between the left eyes of defocus group Ⅰ and defocus group Ⅱ or eyes of the control group in diopter and axial length (P＞0.05). Immunochemistry revealed that the mean gray value of IRBP protein expression was 165.62t4. 93 and 171.00±4. 25 in defocus Ⅰ group and defocus Ⅱ group and was significantly higher than that of the self-control eyes and the normal control group ( 156. 31±4.00,155.26 ±3.49 and 158.61 ±4. 58 ) ( P＜0.05 ). Western blot demonstrated that the expressios of IRBP protein were down-regulated and RT-PCR showed the IRBP mRNA level of IRBP was significantly lower in defocusIgroup and defocusⅡgroup,compared with the self-control eyes and the normal control group (P＜0. 05). Conclusion Expression of IRBP in guinea pig may play a role in lens-induced myopia.

Objective:To analyze the sympathy,responsibility and rational emotion of clinical trainees and to construct the medical ethics emotion education system.Methods:A questionnaire survey was conducted among undergraduate clinical medical students.Of the 307 selected students,302 returned valid questionnaire.After eight months in the field trip,the educational outcome was assessed using questionnaire survey again,and the data was analyzed with descriptive statistics analysis.Result:According to the results of questionnaire survey,we came up with effective countermeasures to improve the medical ethics emotion of clinical medical students,and then strengthened students' sympathy,responsibility and rational emotion quality education.Conclusions:It suggests that cultivating the students' learning interests and motivating the subjective initiative of trainees on ethics learning with various teaching methods promoted the transformation from medical ethics emotion to behaviors and customs,and thus constructed a medical ethics emotion education model and improved quality medical ethics education.

Objective To investigate the application value of 3D arterial spin labeling (ASL) perfusion imaging for patients with unilateral carotid stenosis or occlusion.Methods Cervical MRA and cerebral 3D ASL were performed on 36 consecutive patients with unilateral carotid stenosis diagnosed by digital subtraction angiography (DSA).The consistency of degree of stenosis between cervical MRA and DSA was analyzed.36 patients were divided into mild,moderate and severe groups by degree of stenosis,which was based on the DSA result.Variance analysis of cerebral blood flow (CBF) unilateral and contralateral changes were performed on three different groups.Patients with severe stenosis,diagnosed by DSA,were divided into symptomatic group and asymptomatic group.Analysis was carried out on unilateral and contralateral change differences between symptomatic group and asymptomatic group.Results There was no statistical significance between MRA and DSA in the cervical artery stenosis degree (kappa＞ 0.75,P＜0.05).There was no statistical difference in CBF unilateral and contralateral changes between mild group and moderate group (P＞0.01).There was statistical difference in CBF unilateral and contralateral changes between severe group and other two groups (P＜0.01).There was statistical difference in CBF unilateral and contralateral changes between symptomatic group and asymptomatic group (P＜0.01).Conclusion 3D ASL as a safe,non-invasive,repeatable method,are meaningful for cerebral hemodynamics evaluation in patients with unilateral carotid stenosis.

Objective To evaluate anatomic hepatectomy (AH) vs lithotomy of liver parenchyma (LLP) for regional hepatolithiasis.Methods From January 2012 to June 2015,fifty-nine cases underwent LLP and 47 received AH.Clinical end-points included residual stones,infection of the raw surface of liver remnant,time to restoration of liver function,bile leakage,recurrent stones,morbidity and mortality.Results The time of liver function restoration was not statistically different between the two groups (2.96 d vs.2.82 d,P ＞0.05).Patients in AH group suffered from less residual stones (2 cases vs.13 cases,P ＜0.01),lower infection rate of the raw surface of liver remnant (1 case vs.21 cases,P ＜ 0.01),and less bile leakage rate (0 case vs.7 cases,P ＜ 0.05).With a median follow-up of 31 months (range 3-48 mon).AH group suffered from less recurrent hepatolithiasis (0 case vs.12 cases,P ＜ 0.01).There was no mortality in both groups.Conclusions Anatomic hepatectomy is a safe and effective treatment for hepatolithiasis,with a high stone clearance rate and low surgical complications.

ObjectiveTo study the correlation of urinary iodine and thyroid function in elderly men in Harbin,and to provide the basis for formulation of health measures for the elderly.MethodsSeventy five cases of clinically healthy elderly men were enrolled for check-up of urinary iodine,thyroid function and B-ultrasound in Geriatric Ward the Forth Affiliated Hospital of Harbin Medical University in 2010.The subjects of investigation were divided into iodine appropriate and iodine sufficient groups and thyroid function parameters and B-ultrasound results were compared.ResultsThe average age of the 75 cases of healthy elderly men was (79.07 ± 4.78) years old and the median of urinary iodine was 198.4 μg/L.There were 62.67％ (47/75) elderly males whose iodine nutritional status was appropriate,but there were still some individuals(6.67％,5/75) in the iodine excess state.The level of TSH of the iodine appropriate group [(1.91 ± 0.82)mU/L] was lower than that iodine sufficient group [(4.98 ±0.60)mU/L,t =12.58,P ＜ 0.05],while the level of FT3 of the iodine appropriate group[(4.71 ± 0.56)pmol/L]was higher than that iodine sufficient group[(3.31 ± 0.43)pmol/L,t =12.18,P ＜ 0.05].But the difference of FT4between the two groups [(14.91 ± 3.12),(14.06 ± 2.79)pmol/L] was not statistically significant (t =1.40,P ＞0.05].The thyroid volume of iodine sufficient group[(20.9 ± 6.1 )cm3] was higher than that iodine appropriate group [(17.9 ± 5.6)cm3,t =2.11,P ＜ 0.05].ConclusionsSufficient quantities of iodine intake may affect the thyroid of elderly people.Whether the quantity of iodine intake of the elderly population should be decreased or not need to be further studied.

This study was aimed to choose the best fixing method of electrocardiogram (ECG) measurement in mice by comparison of ECG index in three different fixing ways. Changes of ST amplitude and heart rate in ECG were measured in the self-made mouse sets, tape bundling or breathing anesthesia state. The amplitudes of the ST seg-ment in C57BL6 mice and β1/β2-AR double knockout mice were elevated in tape bundling or breathing anesthesia state in comparison with that in the self-made mouse sets (all P < 0.01). The heart rate under the tape bundling state in C57BL6 mice was significantly faster than that in the self-made mouse sets (P< 0.05 or P< 0.01). And the heart rate under anesthesia in β1/β2-AR double knockout mice was significantly slower than that in tape bundling state or the self-made mouse sets (both P < 0.01). It was concluded that relative to the binding and anesthesia method, fixing mouse in the self-made mouse sets, which can more truly reflect the physiological or pathological states, is a relatively good fixing method of ECG measurement in experimental study.

Objective To improve the success rate of selective cannulation of major papilla during endoscopic retrograde cholangiopancreatography (ERCP).Methods When cannulation failed with conventional methods,ultra-fine guide wire combined with taped cannulation,precut papillotomy,guide wire pancreatic occupation technique,and percutaneous transhepatic cholangial drainage (PTCD) assisted rendezvous technique was applied to improve the success rate of cannulation of major papilla.Results A total of 5743 patients received ERCP in our hospital during last 5 years,with a success rate of cannulation of major papilla at 98.6% (5664/5743).For 396 patients with difficult cannulation under conventional methods,ultra fine guide-wire technique was applied in 20,pancreatic occupation technique in 67,precut technique in 294 and PTCD assisted rendezvous technique in 15,which achieved a success rate of 80.0% (317/396).Conclusion Application of combined techniques according to the features of papilla can increase success rate of difficult cannulation.

OBJECTIVETo analyze the data of angle variation on traction based on a finite element model of complete cervical spine with straight physiological curvature, and try to give experimental reference and suggestion in treating cervical spondylosis.

METHODSA 43-year-old female patient with straight cervical spine was chosen and the CT scan data were collected. By using specially designed modeling system, a high quality finite element model of complete cervical spine with straight physiological curvature is generated,which included ligament and muscle according to anatomy. After the model was confirmed, traction was loaded with angle 0 degree, anterior 5 degrees, 10 degrees, 15 degrees, 20 degrees, 25 degrees, to observe the data of distance change on between adjacent intervertebral foramen, processus articularis, uncovertedral joint, intervertebral discs, and stress of anulus fibrosus and nucleus pulposus.

RESULTSWhen the angle was 0 degrees-15 degrees, the distance between intervertebral foramen, Luschka joint and processus articularis posterioris was enlarged, the tensile stress was adequate and compressive stress was small. It met the clinical requests.

CONCLUSION0 degree-15 degrees anterior position is suggested for the treatment of cervical spondylosis.

A comparable study were carried out by determination of trace elements on five marine-derived shell traditional Chinese medicine (TCM) (Ostreae Concha, Haliotidis Concha, Margaritifera Concha, Meretricis Concha, and Arcae Concha), which were recorded in the Chinese Pharmacopoeia (2010 version). Seven trace elements in 51 batches of this type of shell TCM were analyzed by Inductively Coupled Plasma Mass Spectrometry (ICP-MS), combined with principal component analysis (PCA) methods. The content of element Se, which exhibited significant differences among different drugs, could be used as a key element to distinguish this type of drugs. Meanwhile, the contents of elements Co, Cu, Mo, and Ba in Haliotidis Concha, Co and As in Margaritifera Concha, Mo and As in Meretricis Concha, Mo, As, and Ba in Arcae Concha, and Zn in Meretricis Concha were relatively stable. In the PCA plot, Arcae Concha and Meretricis Concha could be efficiently distinguished from Ostreae Concha together with Haliotidis Concha, and Margaritifera Concha. The results also showed a correlation with their medicinal function. In conclusion, trace elements in marine-derived shell TCM could not be neglected for their quality control.
