Objective To study the effects of Compound Di Gui Capsule (CDGC) on the levels of glucose and lipid in diabetic rats.Methods Rats models with diabetes mellitus were induced by intraperitoneal injection of streptozotocin (50mg?kg-1).Then the diabetic rats were divided into different groups at random.CDGC groups were given CDGC in the dosages of 2.4,1.2,0.6 g?kg-1?d-1 respectively by gastric gavage for 90 d.Levels of whole blood glucose,glycosylated hemoglobin,total cholesterol (TC),triglyceride (TG),low-density lipoprotein cholesterol (LDL-C),high-density lipoprotein cholesterol (HDL-C),nitric oxide (NO),total antioxidation capacity (T-AOC) were determined.The insulin level and the pathological changes of pancreatic tissues were also observed.Results The glucose level of diabetic rat was decreased 2h after administration.After administration of CDGC for 90d,the glycosylated hemoglobin,TG,LDL-C and NO were markedly decreased,islet ?cells were protected and restored,and the insulin level and T-AOC were increased.Conclusion CDGC can regulate the metabolism of glucose and lipid in diabetic rats,and can protect and restore the pancreatic islet cells.

Objective To evaluate the effect of dexmedetomidine on the expression of nerve growth factor (NGF) in isolated hippocampal neurons of fetal rats incubated with propofol.Methods Hippocampal neurons derived from the fetal rats of pregnant Sprague-Dawley rats at 5-13 days of gestation were primarily cultured for 7 days,and were inoculated in the culture plate at a density of 5×105 cells/ml.The neurons were randomly divided into 3 groups (n =15 each) using a random number table:control group (group C),propofol group (group P),and dexmedetomidine + propofol group (group DP).In group P,propofol with the final concentration of 100 μmol/L was added to the culture medium,and the cells were incubated for 3 h.In group DP,dexmedetomidine with the final concentration of 1 μmol/L was added to the culture medium,the cells were incubated for 30 min,and then propofol with the final concentration of 100 μmol/L was added to the culture medium,and the cells were incubated for 3 h.The viability of hippocampal neurons was assessed by CCK-8 assay.NGF mRNA expression was detected by real-time reverse transcriptase polymerase chain reaction.NGF protein expression was detected by Western blot.Results Compared with group C,the viability of hippocampal neurons was significantly decreased,and the expression of NGF protein and mRNA was down-regulated in group P (P＜0.05).Compared with group P,the viability of hippocampal neurons was significantly increased,and the expression of NGF protein and mRNA was up-regulated in group DP (P＜0.05).Conclusion Dexmedetomidine improve propofol-induced decrease in the viability of isolated hippocampal neurons of fetal rats through up-regulating the expression of NGF.

OBJECTIVETo explore the clinical efficacy of the arthroscopic Mender II stapler for the treatment of patients with meniscus anterior horn injury needing meniscal suture repair.

METHODSAmong 47 patients with meniscus anterior horn injury, 29 patients were male and 18 patients were female, ranging in age from 12 to 31 years old, with a mean age of (20.53± 4.12) years old. The duration of disease ranged from 3 to 35 days, and the average duration was (12.43±5.74) days. The Mender II stapler was used to carry out arthroscopic suture from outside to inside. The Lysholm knee scoring system was used to evaluate and analyze preoperative and postoperative symptoms, such as pain, limping embolism and so on.

RESULTSForty-six patients were followed up, and the duration ranged from 12 to 48 months, with a mean of (20.53±4.12) years. The incision healed at the first stage without important vessels and nerves injuries. The symptoms of the locked knee joint disappeared, and symptoms such as pain, limp, swelling and limitation of activity improved. The Lysholm score increased from preoperative 52.33±7.31 to postoperative 86.74±6.92.

CONCLUSIONUsing Mender II stapler to treat patients with meniscus anterior horn injury who were treated with arthroscopic suture from outside to inside is effective to improve symptoms, and to obtain good short-term results.

Objective To evaluate the clinical significance of fetal nasal bone absence and thickened nuchal translucency ( NT) at 11-13 +6 weeks ultrasound screening .Methods A total of 4200 pregnant women with single fetus registered at Mother and Children ’ s Health Care Center in our hospital were examined at 11-13 +6 gestational weeks .Both fetal nasal bone and NT ultrasound evaluation were offered to assess whether nasal bone is absent and NT is thickened (>3.0 mm) in these cases.Particular attention was paid to the relationship between abnormal findings ,karyotype and pregnancy outcome .Results In all, 3492/4200 cases were included in the study with both NT measurement and nasal bone evaluation .Seven hundred and night cases were excluded because of unavailable clinical outcome .Among 3492 fetuses:(1) There were 3 cases absent of nasal bone .Among the 3 cases without nasal bone , 2 cases ( 1 case combined with thickened NT ) were trisomy 21(66.7%,2/3).(2) There were 351 cases with NT>3.0 mm (10.1%,351/3492).Among the 351 cases with thickened NT,there were 4 with trisomy 21 syndromes (1.14%,4/351,1 case combined with nasal bone absence ),1 with trisomy 18 syndrome,1 with Turner syndrome,6 with structural anomalies but normal karyotype (1.71%,6/351).(3)Among the 3139 cases with normal nasal bone and NT ,there were 8 cases with chromosomal or structural anomalies .Conclusions Absent nasal bone and thickened NT are important markers of trisomy 21 in the first trimester ultrasound screening .Thickened NT has significant correlation with other fetal chromosomal and structural anomalies .

Objective To evaluate the treatment of lateral tibial plateau fractures involving the posterolateral condyle with double plate fixation via the combined anterolateral and posterolateral approaches.Methods From January 2013 to August 2015,12 patients with lateral tibial plateau fracture involving the posterolateral condyle were treated by double plate fixation via the combined anterolateral and posterolateral approaches.They were 8 males and 4 females,with an average age of 36.9 years (range,from 23 to 58 years).According to the Schatzker classification,we had 8 ones of type Ⅱ,3 ones of type Ⅴ and one of type Ⅵ.Firstly,the posterolateral condylar fractures were stabilized temporarily by the Kirschner wire after reduction through the posterolateral approach.Secondly,the lateral tibial plateau fractures were dealt with via the anterolateral approach,and fixated with a proximal tibial locking plate.Finally,a T-shaped plate for the distal radius was used to support the posterolateral condyle.The clinical and radiographic results were evaluated using the Hospital for Special Surgery (HSS) knee score and the Rasmussen Score,respectively.Results The average operative time was 143.3 min (range,from 110 to 210 min);the average intraoperative blood loss was 190.0 mL (range,from 100 to 300 mL).The follow-ups for the 12 cases ranged from 12 to 24 months (average,16.6 months).All the cases obtained clinical healing after 12 to 24 weeks (average,16 weeks).At the last follow-up,the HSS knee-scores ranged from 65 to 98 points (average,84.6 points),giving 7 excellent cases,3 good ones and 2 fair ones.The Rasmussen radiological scores ranged from 13 to 18 points (average,17.1 points),giving 8 excellent and 4 good cases.Conclusions The combined anterolateral and posterolateral approaches can offer excellent exposure for articular reduction and fixation with double plates,achieving satisfactory radiological and functional results in lateral tibial plateau fractures involving the posterolateral condyle.First reduction of the posterior lateral condylar fractures,followed by reduction and fixation of the anterolateral plateau fractures and finally fixation of the lateral condylar fractures,can lead to effectively improved knee function for the patients.

Objective To evaluate image quality of 640-slice coronary CT angiography by combined anesthesia in Tibet minipigs. Methods Tibet minipigs underwent 640-slice coronary CT angiography after anesthesia with xylazine hydrochloride and pentobarbital sodium. The effect of anesthesia was observed and the image quality was evaluated.Results The anesthesia maintained in 40 minutes.The heart rate was (66.66±6.62)beat per minutes.The respiratory frequency was (15.62±1.53)beat per minutes.The revived time was 30 -60 minutes. All of images were good enough to be diagnosed.Conclusion Combined anesthesia with xylazine hydrochloride and pentobarbital sodium has excellent anesthetic effect.It is also simple,convenient and safe.Therefore,it is one of ideal anesthetic methods on such study for 640-slice coronary CT angiography in Tibet minipigs.

Objective To determine the values of gadoxetate disodium-enhanced MRI for quantitatively evaluating the liver function reserve.Methods Forty consecutive liver cirrhosis patients were enrolled in this retrospective study.All the patients underwent complete routine laboratory tests and the gadoxetate disodium-enhanced MR imaging with the same parameters.Liver volume and the relative enhancement index of the whole liver in hepatocellular phase were used to calculate liver relative uptake value,which is supposed to represent the total liver function reserve.And the function reserve of all liver segments was evaluated respectively by the relative enhancement index of each liver segment.One-way ANOVA was used for statistical analyses.Results The patient number in Child-Pugh A,B and C group was 23,10 and 7,respectively.Significant differences was observed in liver relative uptake value among patients in different Child-Pugh classes (F =122.05,P ＜ 0.01).Mean liver relative uptake value was highest in Child-Pugh A group(1212 ± 168),followed by B group and C group (695 ± 161,234 ±55).In Child-Pugh A group,the relative enhancement index of S1 to S8 was 1.13 ± 0.22,1.12 ± 0.50,0.81 ± 0.24,1.08 ± 0.32,1.41 ± 0.25,1.10 ± 0.30,1.16 ± 0.41 and 1.17 ± 0.23,respectively (F =5.93,P ＜ 0.01).There was no significant difference in B and C group (F =1.95,1.83 ; P ＞ 0.05).Conclusion Gadoxetate disodiumenhanced MRI can be used for evaluating whole liver function reserve quantitatively.This technique also has a potential value for evaluating of the segmental liver function reserve before partial hepatectomy.

Objective To evaluate the changes in the expression of aquaporin 1 (AQP1) and aquaporin4 (AQP4) in lung tissues during one-lung ventilation in patients undergoing pulmonary lobectomy.Methods Forty ASA physical status Ⅰ or Ⅱ patients,aged 30-64 yr,weighing 45-79kg,undergoing pulmonary lobectomy,were randomly divided into 2 groups(n=20 each):two-lung ventilation group (group T) and one-lung ventilation group (group O).Anesthesia was induced with iv injection of midazolam,fentanyl,vecuronium and propofol.In group T,the patients were intubated with a single-lumen endotracheal tube and two-lung ventilation was performed.In group O,double-lumen endobranchial tube was inserted,and two-lung ventilation was performed followed by onelung ventilation after chest opening.PET CO2 was maintained at 5.3-24.0 kPa and SpO2 was maintained ＞ 92％.Anesthesia was maintained with iv infusion of remifentanil,propofol and vecuronium.The normal lung tissues were obtained from the site near the pathologic changes for microscopic examination (with light microscope) and examination of the ultrastructure (with transmission electron microscope),and for determination of W/D lung weight ratio,the expression of AQP1 mRNA and AQP4 mRNA (by RT-PCR) and the expression of AQP1 protein and AQP4 protein (by Western blot).Results Compared with group T,W/D lung weight ratio was significantly increased,the expression of AQP1 mRNA and protein was down-regulated in group O (P ＜ 0.05),and no significant changes were found in the expression of AQP4 mRNA and protein in group O (P ＞ 0.05).Pathological changes were observed in group O.Conclusion Down-regulation of AQP1 expression may be involved in the acute lung injury induced by one-lung ventilation in patients undergoing pulmonary lobectomy,however,AQP4 has no such effect.

Methods: Using methylation sensitive restriction analysis followed by polymerase chain reaction (PCR),we studied 80 specimen that were obtained from surgical samples (including 40 gastric carcinoma and 40 matched adjacent normal gastric mucosae).Results: An demethylation state was identified in DNA from gastric carcinoma specimens.The demethvlation rate is 25%(10/40).In contrast,no demethylation state was identified in DNA from matched adjacent normal gastric mucosae. Objective To evaluate the value of stereotactic core needle biopsy(SCNB)in diagnosis of breast lesions.Methods Forty-seven cases were punctured with computer-assisted stereotactic system, spring-loaded biopsy guns and 16 G core needles.The record of each item was collected,including clinical manifestations,descriptions of the mammographic characteristics(such as calcification,mass and architectural distortion),the pathology of the SCNB and the surgical pathology or mammographic follow-up data.Then the results of SCNB were analyzed based on the comparison of SCNB pathology and the surgical pathology.The reason that SCNB failed and misdioagnosed was inferred from the relationship of SCNB accuracy and the X-ray characteristics.Results Forty-four cases were punctured successfully,3 cases failed.Thirty-one patients were operated soon after biopsy.The results of 27 SCNB cases agreed well with the final pathology but the other 4 did not.Conclusions SCNB as an accurate,time-saving and cost- effective method,is also minimally invasive and hardly changes the architecture of the breast.SCNB can diagnose breast lesions in advance,reduces the number of surgical biopsy,and is promising in clinical application.
