1.Fluid balance and myocardial function relate to mortality of patients with septic shock

Objective To evaluate the correlation between fluid balance and myocardial function and the mortality in patients with septic shock.Methods Ninety two septic shock patients enrolled between January 2013 and January 2015 were divided into survival group and non-survival group based on 28-day mortality.Fluid balance,haemodynamic parameters and B-type natriuretic peptide (BNP) were collected on days1,3,7 after admission.Results Forty four patients survived the crisis while 48 died.The 7 day average fluid balance volume in survival group was lower than those in non-survival group [(55 ± 9)ml/kg vs.(187 ± 14)ml/kg,t =54.260,P <0.01].The initial left ventricle ejection fraction (LVEF) in survival group was lower than that in non-survival group [(42% ±4%) vs.(54% ±6%),t =10.100,P <0.01].Serum BNP level was significantly lower than those in non-survival group [(267 ± 54) ng/L vs.(681 ± 22) nig/L,t =48.580,P <0.01].Multivariate logistic regression analysis showed that fluid positive balance,LVEF and acute physiology and chronic health evaluation Ⅱ (APACHEⅡ) were the independent predictors for 28 day mortality (P < 0.05).Conclusions Fluid balance and myocardial function can serve as risk predictors for septic shock patients.
2.The early diagnostic value and prognostic significance of serum presepsin (sCD14-ST)in patients with sepsis

Objective To investigate the early diagnostic value and prognostic significance of serum presepsin in patients with sepsis.Methods A prospective observational study of 80 patients from ICU was carried out between January 2014 and June 2015.According to sepsis criteria,the cases were divided into sepsis group (n =50)and noninfectious systemic inflammation syndrome group (SIRS group,n =30), and another 25 healthy volunteers were collected as control group.Meanwhile,sepsis group was further divided into survival and death subgroups according to 28-day mortality.The dynamic changes of serum presepsin,procalcitonin (PCT),soluble myeloid cells expressing triggering receptor-1 (sTREM-1 ),C-reactive protein (CRP),interleukin 6 (IL-6)were monitored on the 1st,3rd and 7th day after admission. Results (1)The serum presepsin level of sepsis group was higher than SIRS group and control group on the 1st day,and the difference was statistically significant [912.0 (563.5,1514.8)pg/mL vs.462.0 (334.0,556.0)pg/mL vs.120.0 (70.2,320.1)pg/mL,P <0.05];(2)The area under the ROC curve (AUC)of presepsin for the sepsis diagnosis was greater than PCT,sTREM-1,CRP,IL-6.As cut-off point set at 672.5 pg/mL,presepsin measurements showed a sensitivity of 80%,specificity of 96.7%, combination of presepsin,PCT and sTREM-1 could offer better diagnostic confirmation;(3)The level of serum presepsin,PCT and sTREM-1 in death group were higher than those in survival group.The levels of serum presepsin,PCT,sTREM-1 and IL-6 in survival group gradually decreased to normal level following the therapy,but the levels of above markers in death group didn’t decrease significantly;(4)The AUC of presepsin in prognosis of sepsis prediction was greater than that of PCT,sTREM-1,and combination of the above three markers could offer better prognostic value.Conclusions Serum presepsin provided superior diagnostic and prognostic accuracy to PCT and sTREM-1,and the combination of presepsin,PCT and sTREM-1 can offer the reliable guide for sepsis diagnosis and death risk prediction.
3.Effects of Compound Di Gui Capsule on the Metabolic Disorders of Glucose and Lipid in Streptozotocin-induced Diabetic Rats

Objective To study the effects of Compound Di Gui Capsule (CDGC) on the levels of glucose and lipid in diabetic rats.Methods Rats models with diabetes mellitus were induced by intraperitoneal injection of streptozotocin (50mg?kg-1).Then the diabetic rats were divided into different groups at random.CDGC groups were given CDGC in the dosages of 2.4,1.2,0.6 g?kg-1?d-1 respectively by gastric gavage for 90 d.Levels of whole blood glucose,glycosylated hemoglobin,total cholesterol (TC),triglyceride (TG),low-density lipoprotein cholesterol (LDL-C),high-density lipoprotein cholesterol (HDL-C),nitric oxide (NO),total antioxidation capacity (T-AOC) were determined.The insulin level and the pathological changes of pancreatic tissues were also observed.Results The glucose level of diabetic rat was decreased 2h after administration.After administration of CDGC for 90d,the glycosylated hemoglobin,TG,LDL-C and NO were markedly decreased,islet ?cells were protected and restored,and the insulin level and T-AOC were increased.Conclusion CDGC can regulate the metabolism of glucose and lipid in diabetic rats,and can protect and restore the pancreatic islet cells.
4.Antagonistic action of epigallocatechin-3-gallate on apoptosis in rat PC12 cells induced by 1-methyl-4-phenylpyridinium ion

Aim To investigate the antagonistic action of EGCG on apoptosis of rat PC12 cell induced by MPP+.Methods PC12 cells were cultured and the apoptosis induced by MPP+(900 ?mol?L-1)was observed.The cells were randomly divided into 6 groups:blank group without any treatment,MPP+ control group,vitamin E group and EGCG groups(10,50,100 ?mol?L-1).After treatment of drugs,cell viability,leakage of LDH,morphological changes of mitochondria and apoptosis were detected by MTT,Hoechst 33342 staining,transmission electron microscope and flow cytometry,respectively.Results After treatment of cultured PC12 cells with MPP+,cell viability was decreased,leakage of LDH and apoptotic rate were increased,and mitochondria swelling,vacuole and cristae breakage were observed.Vitamin E and EGCG en-hanced cell viability,reduced the leakage of LDH and apoptotic rate,and decreased the damage degree of mitochondria.Conclusions EGCG possesses the ability of inhibiting rat PC12 cell apoptosis induced by MPP+,and its protective action may relate to its function of keeping mitochondria integrality.
5.Effects of epigallocatechin-3-gallate on 1-methyl-4-phenylpyridinium ion-induced apoptosis in rat PC12 cells

AIM: To investigate the effects of epigallocatechin-3-gallate(EGCG) on 1-methyl-4-phenylpyridinium ion(MPP+)-induced apoptosis in rat pheochromocytoma(PC12) cells and to explore the relationships between its roles of anti-oxidation,intracellular calcium homeostasis and anti-apoptosis.METHODS: Rat PC12 cells were pretreated with vehicle control or EGCG(10,50,and 100 ?mol/L) for 30 min,then cultured with MPP+(900 ?mol/L) for 24 h.The cell viability and apoptosis were monitored by MTT assay and flow cytometry using Annexin V and PI.The activity of intracellular reactive oxygen species(ROS),contents of superoxide dismutase(SOD) and malondialdehyde(MDA),cytoplasmic Ca2+ density and apoptotic morphology of mitochondria were examined by fluorescent plate-based assays,confocal microscope,and transmission electron microscope,respectively.RESULTS: MPP+ impaired the PC12 cells in a concentration-dependent pattern and induced apoptosis of the cells(31% versus control).Compared with the control,the cells pretreated with EGCG showed markedly higher rate of viability and lower apoptosis.Meanwhile,EGCG pretreatment significantly increased the SOD activity and decreased the levels of MDA and ROS.Interestingly,EGCG also decreased the concentration of cytoplasmic Ca2+ and improved the morphology of mitochondria.CONCLUSION: EGCG exhibits inhibitory effects on MPP+-induced apoptosis in rat PC12 cells,which is possibly associated with increasing the cell ability of anti-oxidation and decreasing the concentration of cytoplasmic Ca2+.

Corneal endothelial cell(CEC)is the most critical part for the cornea, of which activity can influence the postoperative vision.It is very important for the clinical cornea preservation considering the function and its self-purification of donor cornea.There are a variety of classical methods, which can significantly prolong the saving time of donor cornea with its good quality of CEC.We reviewed the published papers about present preservation methods of cornea, which can give us many suggestions for the clinical cornea preservation.
7.Research on mRNA expression of leptin in different kinds of ovarian cancer

Objective:To explore the mRNA differential expression and clinical effect of leptin in different kinds of ovarian cancer.Methods: 129 patients with ovarian cancer and 42 controls were collected from Jun.2015 to Jun.2016 in our hospital.We got a small tissue by ovarian biopsy guided under the ultrasonic processing,detected the total RNA,purified mRNA and analyzed the correlation of the expression of leptin in different kinds and clinical stages of ovarian cancer.Results: Compared with the control groups,the mRNA expression of leptin in each group was increased(P<0.05),and the serous adenocarcinoma group increased obviously(P<0.05).Patients with clinical stage FIGO Ⅲ was significantly higher than other stages in serous adenocarcinoma group(P<0.05).Conclusion: Leptin could express in different kinds and clinical stages of ovarian cancer,and may be a new detection index for early diagnosis of ovarian cancer.
8.Low-dose tadalafil combined with Shuganyiyang capsules for mild-to-moderate erectile dysfunction.

OBJECTIVETo observe the clinical effect of low-dose once-daily tadalafil combined with Shuganyiyang Capsules in the treatment of mild-to-moderate erectile dysfunction (ED).
METHODSNinety patients with mild-to-moderate ED were equally randomized to groups A, B and C to receive Shuganyiyang Capsules, tadalafil, and tadalafil + Shuganyiyang Capsules, respectively. The scores of the patients on IIEF-5 and SF-PAIRS (15-Item Short Form of Psychological Interpersonal Relationship Scales) were recorded before and at 1 and 3 months after treatment.
RESULTSThe IIEF-5 scores of groups A, B and C were 10.13 +/- 1.55, 11.00 + 1.60 and 10.73 +/- 1.91 before treatment, and 13.77 +/- 2.11, 17.77 +/- 2.13 and 17.17 +/- 3.84 at 1 month after treatment, significantly higher in B and C than in A (P <0. 001) , but with no remarkable difference between B and C (P =0. 411). At 3 months after treatment, the IIEF-5 scores were 15.77 +/- 2.05, 18.07 +/- 2.24 and 19.37 +/- 3.76 in the three groups, dramatically higher in B and C than in A (P <0.001) as well as in C than in B (P<0.05). The scores on sexual self-confidence, sexual spontaneity and time concerns in SF-PAIRS were 3.90 +/-0.80, 8.67 +/- 1.94 and 14.43 +/- 1.92 before medication, 5.83 +/- 1.02, 9.90 +/- 1.75 and 11.17 +/- 1.68 at 1 month and 6.73 +/- 0.98, 11.07 +/- 2.08 and 10.67 +/-1.60 at 3 months after medication in group A; 4.17 +/- 0.87, 9.37 +/-1.43 and 14.47 +/-1.57 before medication, 6.47 +/-0.78, 10.83 +/- 2.18 and 10.20 +/-1.56 at 1 month and 6.83 +/-0.91, 11.30 +/- 1.88 and 9.47 +/- 1.57 at 3 months in group B; and 4.23 +/-0. 94, 9.50 +/- 1.89 and 14.67 +/- 2.91 before medication, 8.03 +/- 1.67, 13.43 +/-1.10 and 9.70 +/-1.21 at 1 month and 8.93 +/- 1.78, 14.70 +/- 1.26 and 8. 87 +/- 0. 97 at 3 months in group C. Compared with the baseline, the SF-PAIRS scores of the three groups were all significantly improved after treatment (P <0. 05) , and markedly higher in C than in the other two groups (P <0.05).
CONCLUSIONLow-dose once-daily tadalafil combined with Shuganyiyang Capsules is obviously effective in the treatment of mild-to-moderate ED, which not only improves the patients'erectile function, sexual self-confidence and sexual spontaneity, but also reduces their time concerns.

Objective To demonstrate the diffusion tensor imaging(DTI)characteristics of multiple sclerosis(MS)plaques,periplaque white matter regions and normal appearing white matter (NAWM)regions in patients with MS,and to evaluate the clinical values of DTI and three-dimensional brain fiber tracking for the diagnosis of MS.Methods Conventional MRI and DTI were performed in 32 patients with MS and 32 age-matched control subjects.Fractional anisotropy(FA)and apparent diffusion coefficient(ADC)maps were generated and coregistered with T_2-weighted MR images,FA and ADC values were calculated in regions of interest in plaques,periplaque white matter regions,NAWM regions and white matter regions in control subjects.And three-dimensional brain fiber tracking maps were generated by using the DTI.Results TheADCwas(1.233?0.119)?10~(-3)mm~2/s in MS plaques,(0.973?0.098)?10~(-3) mm~2/s in periplaque white matter regions,(0.748?0.089)?10~(-3)mm~2/s in NAWM,and(0.620? 0.094)?10~(-3)mm~2/s in control subjects.The FA was 0.225?0.052 in MS plaques,0.311?0.050 in perip]aque white matter regions,0.421?0.070 in NAWM,and 0.476?0.069 in control subjects. 10.Effect of taurine on thyroid hormone and second messenger in myocardium of rats after exhaustive exercise.

AIMProbe into protective action of taurine against exercise-induced myocardial damage in rats after exhaustive exercise.
METHODSUsing the model of exhaustive exercise, the present study researches into effects of taurine on the content of triiodothyronine (T3) and tetraiodothyronine (T4) in serum and myocardium as well as the activity of T(4) 5' deiodinase (T(4)5'-DI) and second messenger cAMP in myocardium of rats.
RESULTSThe content of T3 in serum and myocardium, T(4)5'-DI activity, second messenger cAMP in myocardium increased significantly (P < 0.01) while that of T4 in serum and myocardium did not change significantly after exhaustive swimming. The supplement of taurine might significantly inhibit above changes caused by exhaustive exercise.
CONCLUSIONTaurine could protect myocardium from exhaustive exercise induced damage by inhibiting hypermetabolism of thyroid hormone and change of second messenger.
