1.Relationship between gastric cancer and the expressions of IL-6,IL-6 receptor and proliferating cell nuclear antigen

Objective To detect the expression of IL-6 in peripheral blood of patients with gastric cancer,as well as the expressions of IL-6,IL-6 receptor(IL-6R) and proliferating cell nuclear antigen(PCNA) in the neoplastic tissue in order to investigate the clinical significance of such expressions in looking for a convenient and effective way for the diagnosis and therapy of gastric cancer.Methods Enzyme-linked immunosorbent assay(ELISA) was employed for the parallel semi-determination of IL-6 levels in the peripheral blood of 30 patients with gastric cancer,as well as the levels of IL-6 and IL-6R in the neoplastic and adjacent tissues.The expression of PCNA in the neoplastic tissues was determined with immuno-histochemistry technique.Moreover,the correlation was comprehensively analyzed among IL-6,IL-6R,PCNA and the clinicopathologic findings of gastric cancers.Results The IL-6 levels in both peripheral blood and neoplastic tissues of 30 patients were significantly higher than those of the normal controls.The IL-6 levels in peripheral blood decreased significantly after operation;the IL-6 and IL-6R levels in cancer tissues were significantly higher than those in their adjacent tissues with a positive correlation between IL-6 and IL-6R levels.The expressions of IL-6,IL-6R and PCNA in both peripheral blood and neoplastic tissues of patients were positively related to the clinical manifestation of gastric cancer,including the differentiation degree of cancer cells,tumor size,infiltration extent and the metastasis of cancer cells.Conclusions IL-6,IL-6R and PCNA are the important factors which affect the degree of malignancy of the gastric cancer concerned,and they also,reflect the possibility of infiltration and metastasis of cancer cells.The IL-6 level in peripheral blood and the levels of IL-6,IL-6R and PCNA in tumor tissues of patients with gastric cancer are obviously high.The expression levels of all these factors are correlated with the clinicopathologic findings of gastric cancer.An adequate determination of IL-6 and PCNA is of benefit to the diagnosis and treatment of gastric cancer,and to the judgment of the course and prognosis of gastric cancer.
2.Prognostic factors of survival for patients with carcinoma of the ampulla of Vater after pancreatoduodenectomy

Objective To investigate the clinical and pathological prognostic factors in patients with carcinoma of ampulla of Vater treated by pancreatoduodenectomy.Methods The clinical and pathological factors of patients with carcinoma of ampulla of Vater who were operated from February 1994 to February 2009 at the Urumqi General Hospital of Lanzhou Military Region were retrospectively analyzed.The survival curves of these patients were also analyzed.Results The post-operative 1-,3-,5-year survival rates were 90%,63%,55% respectively.The diameter of the majority of tumor (58.3%) was less than 2.0 cm.Of all the patients with carcinoma of ampulla of Vater,48.7% were in T1 stage,41.7% had high grade differentiation,73.0% had no lymphatic metastasis.Univariate analysis showed that gender,age,tumor diameter,biopsy or not,the type of operation,the value of pre-operative serum bilirubin and the value of serum CA19-9 were not related to survival.The gross type of tumor (P =0.018),histological type (P =0.000),grade of differentiation (P =0.003),depth of infiltration (P =0.003),lymphatic metastasis (P =0.014) and TNM stage (P =0.031) were significant impact factors of survival.Multivariate analysis showed histological type and depth of infiltration were independent prognostic factors of surviva1 (P =0.002,P =0.005).Conclusion Grossly ulcerated carcinoma,mucinous adenocarcinoma on histology,low degree of differentiation,T2/T3 stage,lymphatic metastasis and clinical stage of Ⅱb were all significantly associated with poor prognosis in patients with carcinoma of ampulla of Vater treated by pancreatoduodenectomy.Histological type and the depth of infiltration were the most important risk factors of survival.
3.Expression of interphotoreceptor retinoid-binding protein in lens-induced myopia in guinea pig

Background The pathogenesis of myopia is currently unclear. Studies showed that retinoic acid regulates the development of myopia. Whether IRBP, a retinoid transport proteins, participates in the development of myopia or not is unknown. Objective The aim of this study was to investigate the expression of interphotoreceptor retinoid-binding protein (IRBP) in the retina of guinea pig with lens-induced myopia and study the relationship between IRBP expression and experimental myopia. Methods Fifty guinea pigs were randomized into the normal controls(n=10) ,defocus Ⅰ group (n=20) and defocus Ⅱ group (n=20). Guinea pigs in the defocus Ⅰ groups wore -10. 00 D lenses on the right eyes for 14 days and the defocus Ⅱ group for 28 days. The left eyes of both defocus groups did not wear any lens served as self-control. The refraction diopter and axial length were measured before and after wearing lenses. The protein expression of IRBP in the retina was detected by immunohistochemisty and Western blot. RT-PCR was used to observe the level of IRBP mRNA in the retina. Result Compared with the control eyes,the right eyes in the defocus Ⅰ and Ⅱ groups developed to ( -5. 53±1.93) D and (-8.69±2.46) D, and the ocular axial length elongated to (0. 31±0. 15 ) mm and (0.41 ±0. 13 ) mm, showing statistically significant differences between them ( group Ⅰ: t= 12. 57,29.63 ,P<0.05 ;group Ⅱ :t= 15.82,44.35 ,P<0.05 ) ,and the degree of myopia was lower and axial length was shorter in group Ⅰ than the eyes of group Ⅱ ( P<0.05 ). There was no statistically significant differences found between the left eyes of defocus group Ⅰ and defocus group Ⅱ or eyes of the control group in diopter and axial length (P>0.05). Immunochemistry revealed that the mean gray value of IRBP protein expression was 165.62t4. 93 and 171.00±4. 25 in defocus Ⅰ group and defocus Ⅱ group and was significantly higher than that of the self-control eyes and the normal control group ( 156. 31±4.00,155.26 ±3.49 and 158.61 ±4. 58 ) ( P<0.05 ). Western blot demonstrated that the expressios of IRBP protein were down-regulated and RT-PCR showed the IRBP mRNA level of IRBP was significantly lower in defocusIgroup and defocusⅡgroup,compared with the self-control eyes and the normal control group (P<0. 05). Conclusion Expression of IRBP in guinea pig may play a role in lens-induced myopia.
4.Clinical observation of jiont analyesia in labor

Objective To evaluate the effect of nitrous oxide-inhale combined with lidocaine-injection by Ciliao and Uterine points and single therapy in labor analgesia,and the impact on maternal and child.Methods 400 cases were randomly divided into four groups.Observation group:100 cases take the therapy of Nitrous oxide-inhale combined with lidocaine-injection in Ciliao and Uterine points in the time of active period.Control group 1:100 cases take the therapy of gas mixture-inhale( 50% Nitrous oxide and 50% Oxygen) in the time of active period.Control group 2:100 cases take the therapy of lidocaine-injection in Ciliao and Uterine points in the time of active period.Control group 3:100 cases do not take any analgesia.Analgesic effects,labor time,mode of delivery,neonatal conditions,the amount of bleeding in every group were observed.Results There was statistically significant differences between the observation group with the control group 1,2,3 on analgesic effects ( x2 =48.513,P < 0.05 ) labor time( F =782.88,1120.81,1104.57,all P < 0.05 ),mode of delivery ( x2 =85.715,P < 0.05 ),neonatal conditions,the amount of bleeding( F =422.66,P < 0.05 ).Conclusion Combination therapy was more effective than the single treatment.
5.Lentiviral vectors in cancer immunotherapy

Lentiviral vectors can transduce a wide range of different cell types.It can transduce not only dividing cells but also nondividing cells.It can be the immunotherapy tool for cancer therapy.Lentiviral yeators have several advantages,amongst which long-term expression,low toxicity,high capacity and low antivector immunity,they will play an important role and will attract more and more interest as an immunotherapeutic.Here,we discussed the clinical application and safety of the lentiviruses encoding tumor-associated antigen(TAA).
6.Treatment of otorhinolaryngologic diseases of deficient heat type with Professor XIE Qiang's Tihu Guanding needling method.

Professor XIE Qiang's Tihu Guanding needling method, a kind of acupuncture method which takes acupoints of the Conception Vessel as the primary and acupoints of the Governor Vessel as the secondary. Acupoints Lianquan (CV 23), Tiantu (CV 22), Qihai (CV 6), Zhongwan (CV 12), Baihui (GV 20) and Dazhui (GV 14) are adopted as the basic ones. Other points can be added according to various symptoms, for instance, Yingxiang (LI 20) and Yintang (GV 29) for rhinopathy, Tinggong (SI 19) and Yifeng (TE 17) for otopathy, Yan'an (Professor XIE's experience) and Shanglianquan (EX-HN 21) for pharyngopathy and Kaiyin 1 (Professor XIE's experience) and Kaiyin 2 (Professor XIE's experience) for laryngopathy. During the needle retention, rotation manipulation should be done every 5 min at Lianquan (CV 23). And the patient should be told to put the tip of one's tongue at the the palate as well as to do deep breathing to communicate the Conception Vessel and the Governor Vessel. Moxibustion is adopted at Yongquan (KI 1) to induce the up floating fire to mingmen (where the primary yang is stored). The therapeutic effect on treatment of persistent otorhinolaryngologic diseases with the above mentioned method is approve to be good.

OBJECTIVE To investigate the status of nosocomial infection in community health services and offer a scientific basis for nosocomial infection control.METHODS Totally 5738 outpatients were investigated retrospectively for nosocomial infection in community.RESULTS The most frequently nosocomial infection sites were lower respiratory tract,and next upper respiratory tract and urinary tract(2131,1045 and 934,respectively).The major reasons of causing the nosocomial infection were irrational usage of antibacterial drugs(39.0%),invasive procedures(31.0%) and using glucocorticoids(10.0%).Most of the pathogenic bacteria of the nosocomial infection were Gram-negative and the next were Gram-positive and fungi.74.4% of the pathogens were resistant to most of antibioties.CONCLUSIONS Alzeimer′s disease patients in the community health centers are popular and susceptible to infection.
9.Effect of 5-aminosalicylic acid on antioxidant mechanism and attenuation of proinflammatory cytokines in trinitrobenzene sulfonic acid-induced colitis in rats

AIM: To investigate the effect and mechanism of 5-aminosalicylic acid (5-ASA) in topical treatment on trinitrobenzene sulfonic acid (TNBS)-induced colitis in rats. METHODS: Colitis was induced by intracolonic administration of TNBS and was treated with 5-ASA at the dose of 100 mg?kg -1 for 2 weeks. Normal control group was administrated with normal saline and TNBS control group was treated with TNBS, not with 5-ASA. Macroscopic damage, histological changes and myeloperoxidase (MPO) activity were evaluated. Superoxide dismutase (SOD) activity and malondialdehyde (MDA) level in colonic mucosa were detected by kits. The expression of interleukin-1? (IL-1?) and tumor necrosis factor-? (TNF-?) mRNAs in colonic mucosa was determined by a reverse transcription and polymerase chain reaction method. RESULTS: Compared with TNBS control group, the macroscopic and histological changes and MPO activity in 5-ASA treated groups were improved. SOD activity was increased and the level of MDA in colonic mucosa was reduced significantly. The expression of IL-1? and TNF-? mRNAs in colonic mucosa was also decreased significantly. CONCLUSION: 5-ASA enema can significantly ameliorate TNBS-induced colitis in rats via antioxidant mechanism and attenuation of proinflammatory cytokine expression.
10.Comparative study on ordinary coordinates and standardized coordinates converted by general procrusts analyzation method in cluster analysis and discrimination analysis of AngleⅠ malocclusion

Objective: To classify the AngleⅠ malocclusion subgroups using lateral radiographic films according to their ordinary coordinates and standardized coordinates converted by general procrusts analysis (GPA), and to compare the two kinds of classifications and their valne in diagnosis of malocclusion. Methods: 946 pretreatment lateral radiographic films of patients with Angle'I malocclusion were chosen and their ordinary coordinates were acquired via soft ware. The ordinary coordinates were then converted into standardized coordinates by GPA. All of the films were classified by cluster analysis and discrimination analysis applying the upper two kinds of coordinates respectively. Results: (1) Twenty one subgroups were identified according to the ordinary coordinates of the chosen films with the total differentiate rate of 92.7% and leave-one-out differentiate rate of 68.4% (Classification A). Correspondingly, 20 subgroups were identified according to the standardized coordinates with the two differentiate rates of 87.8% and 71.9% (Classification B). (2) If the ordinary coordinates were discriminated by Classification B, the total differentiate rate and leave-one-out differentiate rate were 79.8% and 60.2 % respectively. If the standardized coordinates were discriminated by Classification A, the two differentiate rates were 79.8% and 60.2 % respectively. (3) There were some subgroups having the similar form in Classification A and their difference mainly arose from the difference of the patient age, while there were no such subgroups like that in Classification B. (4)The proportion of the largest subgroup in total subjects is 15.9% and there were 8 subgroups having the number of subjects over 40 in Classification A, whilethe corresponding proportion and number of subgroups were 74.7% and 2 in Classification B.(5)Classification A and Classification B were both suitable to classify a new subject, but Classification B was required to standalize all of the subjects onc again, which was very complex, while Classification A was more simplified. Conclusion: Classification A and Classification B are interelated. GPA could concentrate the subjects in cluster analysis, which reduces some influence of the age to the classification, but the process to classify a new subject is very complex. Thus if quick diagnosis is needed in clinics, Classification A should be recommended, but the influence of the age should be noted.
