1.Alterations of heart rate turbulence and immediate vagal reflex during left atrium circumferential pulmonary vein ablation

Objective To observe the alteration of heart rate turbulence(HRT) before and after left atrium circumferential ablation surrounding pulmonary veins(LACA) in patients with paroxysmal atrial fibrillation(PAF) in order to determine whether concealed vagal denervation occurred during the procedure.Methods Forty-four patients(26 male,aged 53?11 years) with paroxysmal atrial fibrillation who had received catheter ablation treatment from Mar 2006 to Dec 2006 were included in the study.The mean diameter of left atrium was 36?5 cm and the mean ejection fracion was 0.65?0.04 in these patients.The left atrium circumferential ablation surrounding pulmonary veins were performed in all the patients to achieve complete electrical isolation of both ipsilateral pulmonary veins.RS2 extrastimulus were delivered from right ventricular apex to obtain heart rate turbulence with 60% of basic sinus cycle length and at twice diastolic threshold before and after the procedure.Turbulence onset(TO)(%) and turbulence slope(TS)(msec/beat) were obtained using average five respective ventricular premature beats.Thirty-seven patients who had received catheter ablation of supraventricular tachycardia were included as control.Results There was no significant difference between AF and control group in HRT [TO:-0.021 6%?0.023 6% vs-0.023 6%?0.018 5%(P=0.67);TS: 7.331 8?6.983 5 vs 9.667 9?6.065 2(P=0.12)].TO and TS changed significantly after LACA [-0.021 6?0.023 6 to 0.000 9?0.013 3(P
2.Endoscopic Resection of Nasopharyngeal Angiofibroma after Superselective Embolization

Objective To explore the efficacy of endoscopic surgery for juvenile nasopharyngeal angiofibroma.MethodsDuring 27 years from March 1980 to July 2007,we performed endoscopic surgery on totally 19 patients with nasopharyngeal angiofibromas.According to Radkowski Stating,five of the patients were stage ⅠA,ten were stage ⅠB,and 4 were stage ⅡA.Before the endoscopy,superselective embolization was performed in all the cases via bilateral maxillary arteries.Results All tumors were successfully resected by endoscopy.The intraoperative blood loss was 200-600 ml.One of the stageⅡA patient developed cerebrospinal rhinorrhea during the endoscopic resection,and thus received repair of the middle turbinate by a mucosal patch immediately.In this series of patients,no massive epistaxis,intracerebral infection,or blindness occurred.Follow-up at three months after the surgery by nasoscopy showed epithelialisation of the nasal mucous membrane without residual tumor tissues.Meanwhile,otoscopy revealed normal light reflex of the eardrum,no fluid in the middle ear.Of the patients,eleven were followed up for one to five years,during which none of them had recurrent tumors confirmed by nasoscopy or CT scan.Conclusions Endoscopic resection of nasopharyngeal angiofibroma results in little trauma and few blood loss after superselective embolization.Experience and surgical skills are key points for the operation.

Objective To assess the safety and efficacy of domestic sirolimus-eluting stent(SES)compared with bare metal stent(BMS)in the primary percutaneous coronary intervention(PCI)for patients with ST-segment elevation AMI in a real-world scenario.Method From January 2005 to March 2006,a total of 143 patient with ST-segment elevation AMI were enrolled in this study,and all of them underwent primary percutaneous coronary intervention(PCI).Among the 143 patients,74 were treated with domestic SESs(Firebird stent)and 69 with BMSs.The incidence of major adverse cardiovascular events(MACE:death,reinfarction,and target vessel revascularization[TVR])was evaluated at 30 days and 180 days.Continuous variables were compared using Student's unpaired t test.Categorical variables were compared using Fisher's test.Cox proportional hazard survival models were used to assess risk reduction of adverse events.P value
7.Treatment of patellar fractures with cable through the bone and Kirschner with a hole in the tail.

OBJECTIVETo study clinical effects of a new internal fixation by using a cable through the bone and Kirschner with a hole in the tail, for the treatment of patellar fractures.
METHODSFrom May 2012 to July 2013, thirty-four patients with patellar fractures were treated with cable through the bone and Kirschner with a hole in the tail. All the patients had close fracture,including 12 transverse fractures and 22 comminuted fractures. There were 18 males and 16 females, ranging in age from 26 to 81 years old, with an average of (46.0 ± 3.0) years old. After open reduction, two appropriate length of Kirschner with a hole in the tail were driven into the patella as perpendicular to the fracture line or the major fragments as possible. A transverse bone tunnel was then drilled with a Kirschner at one side of the patella. Then the cable, which was successively pulled through the bone tunnel and the hole of Kirschner, was crossed in a figure-eight over the anterior of the patella, tightened and fixated by special instruments. The Kirschner was clipped off on the edge of the hole. If it was a comminuted fracture, another cable was used to fasten the patella with cerclage. Postoperative evaluation was based on Bostman.
RESULTSAll the patients were followed up, and the duration ranged from 12 to 26 months, with a mean of (16.0 ± 2.0) months. Fractures healed in all the cases without such complications as infection, loosening of Kirschner and cable loop, and skin irritation. According to the Böstman score system, 33 cases got an excellent result, and 1 good.
CONCLUSIONThe cable through the bone and Kirschner with a hole in the tail is a simple, stable and effective method for the treatment of patellar fractures, especially the transverse fractures, with earlier knee exercise and fewer complications.

8.Review on the role of Fu's subcutaneous needling (FSN) in pain relieving.

Through retrieval of all medical journals of CNKI and VIP from Jan. 1996 to Oct. 2012, ninety-nine articles were selected and analyzed. The result shows that reliable effect can be found in pain relieving, especially in pain of muscles and soft tissues. In mechanism research, action mechanism of Fu's subcutaneous needling (FSN) on pain relieving is expounded through theory of traditional medicine and modern medicine. Although the effect of FSN on pain relieving has been confirmed by numerous clinical trials, it is still lack of explanation on mechanism revealing. The further studies should focus on mechanism expounding, improving research methods and selecting more objective and reasonable evaluation system. Therefore, the effectiveness and scientificalness of FSN can be further enhanced.

9.Application of color Doppler ultrasound in examining hypothyroidism among radiation-exposed workers.

OBJECTIVETo explore the application of color Doppler ultrasound in examining hypothyroidism among radiation-exposed workers.
METHODSA total of 181 radiation-exposed workers who had clinical and laboratorial diagnoses of hypothyroidism were divided into X-ray diagnosis group, interventional radiology group, and radiotherapy group. Another 62 healthy people who underwent physical examinations were used as control group. All cases underwent color Doppler ultrasound; thyroid function, sonograms, and blood flow characteristics were analyzed.
RESULTSIn the X-ray diagnosis group, interventional radiology group, and radiotherapy group, significant differences in the levels of free thyroxine and thyroid-stimulating hormone were found across workers with different lengths of service (P < 0.05). In the X-ray diagnosis group, interventional radiology group, and radiotherapy group, the percentages of individuals having uneven internal echo intensity among clinical hypothyroidism cases were higher than the percentages among subclinical hypothyroidism cases (P < 0.05). The X-ray diagnosis group, interventional radiology group, and radiotherapy group had significantly higher constituent ratios of grade II and III blood flow signals than the control group (P < 0.05). The peak systolic velocity of the superior thyroid artery (V(max)) was significantly higher in the X-ray diagnosis group, interventional radiology group, and radiotherapy group than in the control group (P < 0.05); in each group, Vmax was significantly higher in clinical hypothyroidism cases than in subclinical hypothyroidism cases (P < 0.05).
CONCLUSIONAs a simple, non-invasive, and economic examination, color Doppler ultrasound has been demonstrated as a valuable method for evaluating occupational hypothyroidism and holds promise for clinical application.

The compound that distributes in the herbs with one common effect was named as "co-effect compound" (CEC). The CECs of three traditional Chinese medicine(TCM) effects, purgative, relieving pain and clearing heat, had been found and studied. A strong corresponding relationship was found between the pharmacological activities of CECs and the TCM effect they belong to. The study shows that it may be a feasible method to connect traditional effect of TCM with modem pharmacological activity.
