1.The treatment of the intractable bile duct stones: analysis of 149 cases

Objective To investigate the treatment of intractable bile duct stones. Methods Of the total 149 cases of biliary stones, some are multiple bile duct stones,large stones,stones obstructed in the duodenal ampulla or residuary stones after T-tube surgery, others are associated with small papilla or papilla with neighoring diverticulum. All the stones were extracted with several endoscopic methods, such as choledochoscopy during or after operation, mechanical lithotripsy after EST(endoscopic sphincterotomy), biliary mother-baby endoscopy, ENBD(endoscopic nasobiliary drainage) or biliary-duodenum internal drainage. Results In general, extrahepatic bile duct stones were effectively removed in 94. 6% cases. In 19 out of 20 cases, stones were completely removed with choledochoscopy during operation; In 35 out of 40 cases stones were fully extracted through T-tube endoscopy, and in 72 of 76 cases stones were thoroughly removed with mechanical lithotripsy after EST. Conclusion The intractable bile duct stones can be effectively managed with combined endoscopic therapy, which is attributed to the high success rate in removing biliary stones and should be popularized in this field.
2.Clinical experiences of the reversed superficial peroneal neurocutaneous island flap based on the descending branch of the distal perforating branch of the pcroneal artery

Objective To report the operative technique and clinical experiences of the modified reversed superficial peroneal neurocutaneous island flaps for reconstruction of the ankle and foot.Methods According to the previous anatomical studies and our clinical experiences,we devised the reversed superficial peroneal neuroeutaneous island flap based on the descending branch of the distal perforator of the peronealrtery and its venae comitantes,and covered the soft defect of the ankle and foot with it.Results Twenty-one of the 23 flaps survived completely without complications,while the other two occurred marginal necrosis.The maximum surface of the flap in our series was 12 cm×13 cm.and the minimum one was 5 cm×4 cm.The length of the pedicle ranged from 5 cm to 10 cm.The texture of the flaps was good,while the cosmetic and function of them were evaluated as acceptable in all cased after 6 to 21 months follow-up.Conclusion The reversod superficial peroneal neurocutaneous island flaps is a versatile,reliable procedure useful in reconstruction of the ankle and foot.
3.Application of enhanced recovery after surgery in the perioperative nursing of cerebral glioma patients

Objective To explore the safety and effectiveness of enhanced recovery after surgery (ERAS) in the perioperative nursing of cerebral glioma patients. Methods ERAS optimized nursing was developed based on ERAS of other specialties combined with neurosurgical features. A total of 80 cerebral glioma patients recruited in the Department of Neurosurgery of Tang Du Hospital from June 2016 to April 2017 were investigated and divided into the experimental group (40 cases) and control group (40 cases) using the envelope random sampling with the registration number assigned when a patient was admitted to the hospital. Patients in the control group were treated with routine treatment, while patients in the experimental group received the ERAS nursing. The complications, nursing satisfaction rate, the time of taking food, removing urine tube, stopping the infusion and hospital stay were compared between two groups. Results There was no significant difference in the occurrence of complications between two groups (P>0.05). The nursing satisfaction rate was 82.5% in the experimental group and 62.5% in the control group with significant difference between two groups (P<0.05). The rate of removing urine tube within 24 hours after surgery was 85.0% in the experimental group and 25.0% in the control group; the bed activity rates within 3 days after surgery in the experimental and control groups were 95.0% and 30.0% respectively; the differences between two groups were statistically significant (P<0.05). The time of taking food, stopping the infusion and hospital stay in the experimental group were significantly shorter than those of the control group (P<0.05). Conclusions The application of ERAS in the perioperative nursing of cerebral glioma patients is safe and effective, which allows faster rehabilitation, and reduces postoperative hospital stay and hospitalization costs.
4.Impact of social support on posttraumatic growth in thyroid cancer patients: a chain mediating effect of positive psychological capital and general self-efficacy

Objective:To construct a chain mediating model based on two mediating variables of positive psychological capital and general self-efficacy, so as to explore the impact mechanism of social support on posttraumatic growth in thyroid cancer patients.Methods:From March to December 2023, convenience sampling was used to select 270 postoperative patients with thyroid cancer from the Chinese People's Liberation Army General Hospital as participants. The patients were surveyed using the General Information Questionnaire, Post Traumatic Growth Inventory, Social Support Rating Scale, Positive Psychological Questionnaire, and General Self-efficacy Scale. Spearman correlation was used to analyze the correlation between social support, positive psychological capital, general self-efficacy, and posttraumatic growth in thyroid cancer patients. SPSS PROCESS program was used to test for the chain mediating effect.Results:A total of 270 questionnaires were distributed, and 257 questionnaires were collected. After excluding questionnaires with missing items exceeding 10% and regular responses, 246 valid questionnaires were collected, with a valid response rate of 91.11% (246/270). Spearman correlation showed that there was positive correlations between social support, positive psychological capital, general self-efficacy, and posttraumatic growth in patients ( P<0.01). Mediating effect analysis showed that social support directly and positively predicted posttraumatic growth in thyroid cancer [ β=0.403, 95% CI (0.258, 0.547) ], and also affected patients' posttraumatic growth through three indirect pathways, namely social support → positive psychological capital → posttraumatic growth, with an effect value of 0.083 [95% CI (0.002, 0.173) ], accounting for 13.63% of the total effect, and social support → general self-efficacy → posttraumatic growth, with an effect value of 0.099 [95% CI (0.040, 0.166) ], accounting for 16.26% of the total effect, and social support → positive psychological capital → general self-efficacy → posttraumatic growth, with an effect value of 0.025 [95% CI (0.005, 0.051) ], accounting for 4.11% of the total effect. The total indirect effect accounted for 33.83% of the total effect. Conclusions:Social support can not only directly affect the post-traumatic growth of thyroid cancer patients, but also indirectly affect post-traumatic growth through the mediating effect of positive psychological capital and general self-efficacy.
5.Relationship between dietary patterns and sleep disorders among health screening population in Changsha.

OBJECTIVES:
Sleep disorders directly affect health-related quality of life, so it is of great significance to investigate the risk factors of sleep disorders and to actively intervene. This study aims to investigate the relationship between dietary patterns and associated factors and sleep disorders among the health screening populations in Changsha.
METHODS:
A cross-sectional study was carried out in 86 073 subjects aged 18-70 years old who underwent the health screening. The association between dietary patterns and sleep disorders was analyzed. The associated factors for sleep disorders were identified via by principal component analysis and classification tree model.
RESULTS:
The overall prevalence of reporting sleep disorders was 18.64%. Four major dietary patterns (healthy, snacks, whole-grain, and fried food patterns) were identified. In logistic regression, snacks and fried food patterns had higher risk of sleep disorders. The whole-grain pattern was a protective factor for sleep disorders. Nine associated factors including age, susceptibility to anxiety, snacking parterns, feelings of depression, chronic pain, physical activity, educational level, gender, and weight, and 9 groups at high risk for sleep disorders were identified by classification tree model.
CONCLUSIONS
Sleep disorders are prevalent in the health screening population of Changsha. There is a close association between snacks dietary patterns and sleep disorders. It is necessary to promote healthy and reasonable diet, and keep good lifestyle for the prevention and control of sleep disorders. Health management after physical examination should take different health interventions for high-risk groups with different characteristics of sleep disorders.
