1.The relationship of pancreatic exocrine function with the staging and the size of tumor in the patients with pancreatic cancer

Objective To investigate the relationship of pancreatic exocrine function with the pancreatic cancer staging and the size of tumor. Methods N-benzoyl-tryrosyl para-aminobenzoic acid (NBT- PABA) test was used to measure the pancreatic exocrine function of 39 pancreatic cancer patients, 46 chronic pancreatitis patients and 20 normal subjects. In pancreatic cancer group, the relationship of pancreatic exocrine function with the size of tumor (TS) and JPS classification (T factor) was analysed. Results The rate of NBT-PABA excretion in normal subjects, chronic pancreatitis patients were (78.9±15.9)%, (58.6± 19.3)%, respectively; in mild, moderate, severe chronic pancreatitis patients, the values of NBT-PABA were (75.5±23.6)%, (57.9±21.5)%, (45.5±16.7)% ; respectively. The rate of NBT-PABA excretion in pancreatic cancer patients was (47.6±18.3)%, and the rate of NBT-PABA excretion in patients with T3+T4 stage was (42.2±21.70%, and was obviously less than (64.8±11.2)% of T1+T2 stage (P<0.05). Tthe rate of NBT-PABA excretion in patients with TS3+TS4 was (34.8±17.2)%, and was significantly lower than (55.6±23.5)% of TS1+TS2(P<0.05). The rate of NBT-PABA excretion in patients with pancreatic head cancer was (42.5±16.4)%, which was significantly lower than (71.8±9.6)% of the pancreatic body and tail (P<0.05). In the 33 patients with pancreatic head cancer, the rate of NBT-PABA excretion in patients with main pancreatic duct stricture was (54.2±14.1) %, which was significantly higher than (37.6± 14.1)% of patients with main pancreatic duct interruption (P<0.05). There was no significant difference in pancreatic exocrine function between pancreatic cancer and moderate or severe chronic pancreatitis.Conclusions The pancreatic exocrine function was related to the pancreatic cancer staging and the size of tumor to some degree,but it was affected by mass location,and it was also affected by degrees of the pancreatic duct obstruction. The pancreatic exocrine function can not be used to differentiate pancreatic cancer from moderate or severe chronic pancreatitis.
3.Quality Standard Study on Danqi Tablets

[Objective] To set up standard method of quality to Danqi Tablets.[Method] Take HPLC to make nature authentication,take reverse HPLC to test the content of salvianolc acid B in the preparation.[Result] The authentication is strong in attribute and has good repetition,the measurement has good linear relation,the recycle rate is 101.7%,RSD 1.77%.[Conclusion] The quality standard can effectively control the quality of Danqi Tablets.
4.Peer-assisted learning combined with micro-teaching for international undergraduates during anesthesia internship

Objective To explore the peer assisted learning approach combined with micro-teaching in anesthesia internship for oversea students.Methods 60 international medical students were randomly divided into two groups.Students in the control group have peer guidance before routine theatre observation,while students in experimental group discuss micro-teaching of anesthetic skills with each other by watching real time videos from smart phone.Students' subjective attitudes to new approach were evaluated and their final exam score were analyzed.SPSS 1 1.0 was used for data processing and the data was made descriptive analysis,t test and analysis of variance.Results Students in experimental group did better in both theory exam (46.8 ± 4.03) vs.(35.7 ±6.12) and practice exam (35.2 ± 3.20) vs.(27.6± 4.71) than those in control group,and the total scores of two groups were (80.5 ± 7.40) and (63.1 ± 6.81) respectively.These scores all had significant difference between two groups.More students from experimental group than those from control group feel interested in anesthesiology (P<0.01) and feel confident about exams (P<0.05);There was no difference between two groups in understanding clinical guideline (P=0.57).Conclusion Peer assisted learning combined with micro-teaching is proved to enhance the quality of anesthesia teaching during internship for oversea undergraduates.
5.Detection of 18 Antihypertensive Agents Illegally Added in Traditional Chinese Medicines and Healthy Care Products by HPLC-QTOF/MS

Objective:To establish a rapid, sensitive and accurate HPLC-QTOF/MS determination method for the illegally added antihypertensive drugs in traditional Chinese medicines and healthy care products .Methods:An Agilent Eclipse plus C 18 column ( 50 mm ×2.1 mm,1.8 μm) was adopted with the mobile phase of 0.5%formic acid and acetonitrile with gradient elution .The flow rate was 0.2 ml· min-1 .The electrospray ionization source was applied and operated in a positive ion mode .Results:The detection limit of 18 antihypertensive drugs was within the range of 0.2-2.5 ng· ml-1 .Reserpine was found in one sample .Conclusion:The method is selective and sensitive , which can be used for the detection of 18 chemical medicines illegally added in antihypertensive traditional Chinese medicines and health care products .
6.Prevalence of work-related musculoskeletal disorders in neck and shoulder among workers in pharmaceutical and chemical enterprises

Objective:
To investigate the prevalence and influencing factors of work-related musculoskeletal disorders (WMSDs) in neck and shoulder among workers in pharmaceutical and chemical enterprises, so as to provide the basis for prevention of WMSDs.
Methods:
Workers in 8 pharmaceutical and chemical enterprises in Jinhua City, Zhejiang Province from July to October 2023 were sampled using a stratified random sampling method, and demography, work conditions and WMSDs prevalence were collected using questionnaire surveys. Factors affecting WMSDs in neck and shoulder were analyzed using a multivariable logistic regression model.
Results:
A total of 1 343 workers were surveyed, including 496 males (36.93%) and 847 females (63.07%). The mean age of workers was 38 (15) years, and the length of service in current posts was 6 (8) years. There were 356 workers with WMSDs (26.51%), with 206 workers with neck WMSDs (15.34%) and 167 workers with shoulder WMSDs (12.43%). Multivariable logistic regression analysis showed that gender (female, OR=1.692, 95%CI: 1.203-2.381), sufficient rest time (OR=0.660, 95%CI: 0.471-0.926) and maintaining same neck posture for a long time (OR=1.690, 95%CI: 1.148-2.510) were factors affecting neck WMSDs; gender (female, OR=1.572, 95%CI: 1.060-2.311), sufficient rest time (OR=0.619, 95%CI: 0.422-0.906), repetitive daily work (OR=1.678, 95%CI: 1.064-2.647), uncomfortable work posture (OR=3.791, 95%CI: 1.330-10.803), multiple repetitive movements per minute (OR: 1.988-2.413, 95%CI: 1.060-4.161) and maintaining same neck posture for a long time (OR=1.926, 95%CI: 1.218-3.043) were factors affecting shoulder WMSDs.
Conclusion
The prevalence of WMSDs in neck and shoulder are high among workers in pharmaceutical and chemical enterprises in Jinhua City, and mainly affected by gender, rest time and adverse ergonomics.
7.The preliminary study of target uncertainty during fractionated radiotherapy for esophageal cancer

Objective To investigate the changes of target during fractionated radiotherapy for e-sophageal cancer, and to assess their impact on the implementation of radiotherapy. Methods Fourteen pa-tients with unresectable esophageal cancer were enrolled to receive full course conformal radiotherapy. CT scans were performed after every ten fractions. New targets (GTV) were delineated on repeated CT scans. Then the pretreatment radiotherapy plans were copied to the new targets to investigate the conformity between the new GTV and the plans. Results The majority of the GTVs decreased with the increasing fractions dur-ing radiotherapy. However, GTVs of 35.7% (5/14) patients increased by 2.0%-37.7% at the tenth frac-tion. The PTVs covered by 95% isodose curve at the time of pretreatment,tenth fraction,twentieth fraction and thirtieth fraction were 97.81%±1.53%, 91.95%±5.25%, 94.27%±4.23% and 94.03%±6.45%, respectively. Moreover, at tenth, twentieth and thirtieth fraction, there were 6,5, and 4 patients whose PTVs covered by 95% isodose curve were below 95%, respectively. Conclusions There are signifi-cant target changes during fractionated radiotherapy for esophageal cancer, which may result in uncertainties of radiotherapy implementation.

Objective Diabetic infrapopliteal artery occlusive disease Deep balloon angioplasty ( percutaneoustransluminal an-gioplasty, PTA) near the middle of the treatment effect and the Department of internal medicine ,conservative treatment .Methods Sixty patients with type II diabetes mellitus complicated with severe infrapopliteal arterial occlusive patients , divided into treatment group (41 cases) and Department of internal medicine treatment group (19 cases),all patients had CTA or MRA or DSA imaging;in-terventional treatment group using Deep balloonPTA ,3 days before operation and postoperative long-term Plavix and culture as anti platelet , blood glucose controltherapy;Department of internal medicine treatment group with control of blood glucose ,precedent in im-proving microcirculation ,debridement dressing and anti infection;observed in 2 week,1 month,3 months,6 months ,1 year,2 years,3 years, two groups were compared after treatment of clinical symptoms and changes signs ,ankle brachial index (ABI),the dorsal artery of foot diameter and blood flow velocity ,evaluation and the total efficiency of 6 months,1 year,2 years,3 years and cut rate .Results Interventional arterial blood flow improved significantly after operation in treatment group ,the clinical symptoms,signs were improved obviously, ankle brachial index improved significantly ,compared with the Department of internal medicine treatment group statistical significance ( P <0.05 ) ,and the total efficiency of 3 years the rate of limb salvage and Department of internal medicine therapy group with significant difference ( P <0.05).Conclusions Deep balloon PTA treating diabetic infrapopliteal artery occlusive disease cura-tive effect,can significantly improve the rate of limb salvage;Department of internal medicine treatment forelderly ,important organ dys-function patients .
9.Determination of Drug Loading and Encapsulation Efficiency of Epirubicin Hydrochloride-sorafenib PL-GA Embolic Microspheres by HPLC

OBJECTIVE:To establish a method for the determination of drug loading and encapsulation efficiency of Epirubi-cin hydrochloride-sorafenib-loaded Polylactic Acid-glycolic Acid Polymer(PLGA)embolic microspheres. METHODS:HPLC meth-od was adopted to determine the contents of epirubicin hydrochloride and sorafenib in the preparation,and then drug loading and encapsulation efficiency were calculated by formula. The determination was performed on Phenomenex Luna 5u C8(2) 100A col-umn with mobile phase consisted of methanol-water(containing 0.05% trifluoroacetic acid and 0.14% dium dodecyl sulfate)(75:25,V/V)at the flow rate of 1.0 mL/min. The detection wavelength was set at 252 nm,and the column temperature was maintained at 25℃. The injection volume was 10μL. RESULTS:The linear ranges were 2.020-101.00μg/mL for epirubicin hydrochloride(r=0.9998)and 2.048-102.40 μg/mL for sorafenib(r=0.9997),respectively. The limits of quantification were 3.2970,2.5468 μg/mL, respectively. The detection limits were 0.9891,0.7641 μg/mL,respectively. RSDs of precision,stability and repeatability tests were all less than 2.0%. The recoveries were 96.41%-101.80%(RSD=1.64%,n=9),99.46%-101.45%(RSD=0.70%,n=9),re-spectively. Drug loading of two components in 3 batches of samples were no lower than 1.17%,encapsulation efficiency no lower than 58%. CONCLUSIONS:The method is simple,accurate,can be used to determine drug loading and encapsulation efficiency of Epirubicin hydrochloride-sorafenib PLGA embolic microspheres.
10.Clinical significance expression of MMP-7 in patients with primary non-small cell lung cancer

Objective To study the relationship between expression of matrix metalloproteinases-7 (MMP-7) and clinicopathological characteristics in patients with primary non-smaU cell lung cancer(NSCLC). Methods MMP-7 in 20 normal people and 60 advanced NSCLC patiens were detected with reverse-transcription-polymerase-chain-reaction. Gelatum image analysator analyzed the result. Results The amount of MMP-7 was less in normal people (30.000) than in NSCLC patients(41.231) significantly(P<0.05); the level of MMP-7 was no correlated with gender, age, pathology pattern, tumor size, was inverse correlation with differentiation, and was positive correlation with clinical stages(P <0.05). Conclusion The level of MMP-7 is closely correlated with tissue differentiation and clinical stages of NSCLC, which may serve as a parameter for determining tumor invasion and metastatic.
