The National Health and Family Planning Commission of China together with State Administration of Traditional Chinese Medicine jointly issued Regulations on Cost Accounting for Public Hospitals at County Level in order to promote the comprehensive reforms of county level public hospitals and regulate cost accounting.The issu-ance of the Regulations provides operation guidance on cost accounting at county level public hospitals, specifies divi-sion of responsibility for cost accounting, lays out details of implementation, and emphasizes the importance of achie-ving cost accounting through the use of information technologies.The Regulations incorporates many impressive terms, forming the foundation for effective cost accounting at county level public hospitals.
2.Effect of extracellular zinc on osteoclastic resorption in dental mineralized tissues

Objective:To investigate the bone resorption caused by osteoclasts and modulating functions of zinc ion on dental slices.Methods:Osteoclasts were separated from long-limb bones of neonatal rabbits,cultured with de-activated human tooth slices and glass slices.The cells in the experiment group were treated with 1?10~(-14)mol/L-1?10~(-4)mol/L zinc+10%(volume fraction) fetal calf serum(FCS)+?MEM,while those in the control group were grown in 10%FCS+?MEM.Osteoclasts on glass slices were stained by TRAP staining.The absorption pits on tooth slices were observed by inverted phase contrast microscope.The resorbing activity was evaluated with the concentration of calcium in the supernatant liquid of osteoclasts.The ratio between the concentration of calcium in the experiment group and that of the control group was termed the resorption index.Results: The isolated cells were multinuclear and showed positive in cytoplasma by TRAP staining.Usually,osteoclasts resorbed tooth slices first on the cementum and dentin,which had lower content of mineralized tissue.Compared with those on bone slices,the lacunae on the dental slices appeared less in amount,less in area and shallower in depth.They often showed shallow pits in a large area.Microscopy showed that the number and area of absorption pits formed on treated tissues were less than those on the control tissues.The content of calcium in the supernatant liquid increased at the concentrations of 1?10~(-4)-1?10~(-14)mol/L zinc,especially in the group of 1?10~(-8)mol/L,1?10~(-10) mol/L,1?10~(-14)mol/L zinc on the 3~(rd) day(P
3.The effect of isometric contraction in opening femoral artery collateral circulation in rabbits

Objective To explore the effect of isometric contraction-induced ischemic burden for evoking maximal intrinsic femoral artery collateral circulation. Methods Twenty-four adult New Zealand rabbits weighing (2.0±0.5) kg were subjected to a model of isometric contraction-induced ischemia. An electrode was implanted in the sciatic nerve of the left hind limb for evoking isometric contraction of the gastrocnemius, which blocked local blood flow and induced local ischemia. Femoral artery collateral blood flow (FCBF) was measured using a micro-sphere technique. Results Local blood flow was the lowest at 40% of the maximal isometric contraction (IEmax), and reached plateau at 60% of IEmax. FCBF increased gradually and reached a plateau at 40% of IEmax in the 4th min. After 4 minutes at 40% of IEmax, FCBF had increased to higher than pre-stimulus, peaking in the 2nd minute, and then decreasing gradually to baseline at the 5th minute after reperfusion. It ceased to increase when reperfusion was prolonged to 8 or 10 min. Conclusions The minimal ischemic burden for full obstruction of the femoral artery is 40% of IEmax, and the shortest stimulus duration is 4 min. The maximal FCBF opening duration is no more than 5 min.

5.Comparison of two methods of isolation and culture of osteoclasts and the dynamic observation of bone resorption

Objective: To study the origin, morphological structure, and functional regulation of osteoclast(OC) for further investigation on the mechanism and regulation of bone resorption. Methods: The OCs were isolated by two kinds of traditional method. Osteoclasts were isoclated from neonatal rat long bones. The cytochemistry was observed. The osteoclast-like cells (OLC) were derived from the mouse bone marrow cells in the presence of 1,25(OH)2VitD3 in vitro. Results: Both morphological and functional studies showed that the isolated cells shared some of the typical characteristics of osteoclasts, that is A. multinuclearity; B. developing spreading and pseudopodial activity when cultured on glass; C. high tartrate-resistant acid phosphatase (TRAP); D. resorption lacunae could be found when the cells were cocultured with devitalized bone slices and the number was increased as the time followed. OLC had the same histological and structural traits as the OCs by the former method. The concentration of Ca 2+ and acid phosphatase (ACP) increased gradually. Conclusion: Different kinds of method fit different experiments. The OC obtained by the first method has more activity of bone resorption. The OLC by the second method has more in quantity and can be used in the study of cell differentiation.

7.Inhibitory effect of siRNA-mediated MSK1-silencing on the proliferation of CNE2 cells and its mechanisms

Objective Abnormal activation of mitogen-and stress-activated kinase (MSK1) plays an important role in the development of various cancers.This study was to explore the effect of small interfering RNA (siRNA)-mediated MSK1-silencing on the proliferation of human nasopharyngeal carcinoma (NPC) cells and its underlying mechanism.Methods The siRNA vector targeting MSK1 was constructed and transfected into CNE2 cells, and the NPC cell line stably expressing MSK1 was established.Then the cells were divided into a blank control (without transfection of the plasmid), a negative control (with stable transfection of the negative control plasmid), and an experimental group (with stable transfection of the positive recombinant plasmid).The expressions of MSK1 mRNA and protein were detected by real-time quantitative PCR and Western blot, respectively, the proliferation of the cells determined by CCK-8 and colony formation assays, the cell cycles analyzed by flow cytometry, the level of histone H3 phosphorylation at Ser10 examined by Western blot, and The transcriptional activity and expression of the c-jun protein measured by dual-luciferase reporter gene assay and Western blot.Results Compared with the blank control, the inhibition rates of cell proliferation at 48, 72 and 96 hours were significantly reduced in the experimental group (P<0.05), and so were the colony formation ability of the cells (P<0.01) and the expression and transcriptional activity of the c-jun protein (P<0.05).In comparison with the negative control, the experimental group showed significant decreases in the rate of cell growth after 24 hours, the inhibition rates of cell proliferation at 48, 72 and 96 hours (P<0.05), the number of formed colonies ([221.00±20.08] vs [99.67±15.57] / 300 cells, P<0.01), the proportion of S-phase cells (P<0.01), and the expression of the c-jun protein in the CNE2 cells ([100.00±0.00] vs [48.77±10.71] %, P<0.05), but a remarkable increase in the percentage of G0/G1-phase cells (P<0.01).Furthermore, histone H3 phosphorylation at Ser10 was markedly reduced (P<0.01) but no significant change was observed in the expression of the total c-jun protein in the experimental group.Conclusion Knockdown of MSK1 using siRNA can significantly inhibit the growth and proliferation of CNE2 cells, which may be closely related to the decreased phosphorylation of histone H3 and subsequently down-regulated transcriptional activity of c-jun.
8.Outcomes and prognostic factors of advanced squamous cervical cancer after concurrent chemoradiotherapy

Objective To evaluate the outcomes and the prognostic factors for advanced squamous cervical cancer after concurrent chemoradiotherapy (CCRT).Methods Totally 172 patients with International Federation of Gynecology and Obstetrics stage Ⅱ b-Ⅳ who were treated in Cancer Hospital,Chinese Academy of Medical Sciences between January 2007 and December 2008 were retrospectively analyzed.Patients were received external radiotherapy,high-dose rate brachytherapy and cisplatin-based chemotherapy concurrently.Results The median follow-up period was 54.5 months.The 2-year and 5-year overall survival (OS) were separately 81.5% and 68.8%.The 2-year and 5-year progress-free survival (PFS) were separately 69.2% and 63.1%.Using univariate analysis followed with multivariate analysis,the results showed that these clinicopathological factors including stage (Ⅲ and above versus Ⅱ b; P =0.021,HR =1.95 ; P =0.020,HR =1.86),maximum diameter of local tumor size (>4 versus ≤4 cm; P =0.009,HR =2.55 ; P =0.033,HR =1.94),squamous cell carcinoma antigen (SCC) level before treatment (>3 versus ≤3 μg/L; P =0.010,HR =2.47; P =0.013,HR =2.09) and retroperitoneal lymph node status on imaging (para-aortic lymph node positive versus negative,P =0.009,HR =3.00,P=0.010,HR =2.74; pelvic lymph node positive only versus negative,P =0.044,HR =1.98,P =0.033,HR =1.92) had the significant effect on OS and PFS.Patients with no above adverse prognostic factor were assigned to Group A (n =18),those with one factor were assigned to Group B (n =43),and those with no less than two factors were assigned to Group C (n =1 11).Among three groups,the 2-year OS were separately 94.1%,97.7% and 73.1%,the 5-year OS were separately 81.4%,90.1% and 58.6%,the 2-year PFS were separately 88.2%,90.4% and 57.9%,the 5-year PFS were 82.4%,87.9% and 50.0%.The results showed that group C was significant difference from Group A or B in OS and PFS (all P < 0.05),while Group A had no significant difference from Group B in OS and PFS (P > 0.05).Conclusions Stage Ⅲ or above,maximum diameter of local tumor size > 4 cm,SCC level > 3 μg/L before treatment and positive retroperitoneal lymph nodes on imaging are four independent adverse factors for prognosis of squamous cervical cancer of advanced stage after CCRT.The treatment of patients with no less than two adverse factors should be considered to be improved.
9.The value of serum intestinal fatty acid binding protein measurement in discriminating intestinal ischemia in patients with acute abdomen

ObjectiveTo assess the differential diagnostic value of serum intestinal fatty acid binding protein (I-FABP)in distinguishing intestinal ischemia patients from acute abdomen patients.MethodsA total of 151 patients with acute abdomen and 17 healthy controls from the PLA General Hospital were enrolled from November,2009 to August,2011. Serum I-FABP levels were measured by ELISA.According to the ROC curve,the cut-off value,sensitivity,specificity,positive likelihood ratio (PLR),negative likelihood ratio ( NLR),positive predietive value (PPV) and negative predictive value (NPV) were calculated. ResultsOf the 151 acute abdomen patients,there were 24 intestinal ischemia patients and 127 without intestinal ischemia.Serum I-FABP level in intestinal ischemia group [( 109.67 ±48.82) μg/L]was significantly higher than those in patients without intestinal ischemia [(36.78 ± 11.25) μg/L]and healthy controls[(8.33 ±6.25) μg/L]( all P values <0.01 ).The serum I-FABP cut-off value for the diagnosis of intestinal ischemia was 87.52 μg/L.Serum I-FABP was efficient in terms of sensitivity (0.762),NPV(0.963),PLR(3.05) and NLR (0.24) in the diagnosis of intestinal ischemia.ConclusionI-FABP is potentially useful for discriminating intestinal ischemia from acute abdomen.
