1.Surgical treatment of fibrous dysplasia of bone involving the femur with severe varus deformity

[Objective]To find an effective method of surgical treatment of fibrous dysplasia of bone involving the femur associated with severe varus deformity.[Method]Thirteen patients with fibrous dysplasia of bone involving 14 femurs with severe varus deformity were reviewed from Feb 2002 and Jul 2005.Medial displacement of valgus or valgus osteotomies,curettage and allograft,reconstructional nail have been adolted to treat fibrous dysplasia of bone involving the femur with severe varus deformity.[Result]All patients were followed up from 8 months to 41 months with an average of 21 months.Forteen femoral mechanical alignments(axis)had been corrected completely rdiologically;the preoperative average neck-shaft angle of 75?(range,55?~100?),corrected to the average 120?(range,95 ?~135?)of postoperative,the average shortening of the femur was 3.4 cm(range,2.0~4.5 cm)preoperatively,then enlongthened by 2.8 cm(range,1.8~3.6 cm)in postoperation.Ninteen location of osteotomies showed good union in 14 femur.All bone grafts were absorbed slightly beginning at 3 months and markedly at 10 to 14 months postoperatively.Preoperation,4 patients walked with bilateral crutches,2 used unilateral cane,5 can't walk due to pathological fracture,2 without support;after operation,11 patients walk without support,2 ambulated with the aid of unilateracane.Eleven patients were painless,2 moderate pain.All had no infections and recurrent fracture and progression of the deformity.[Conclusion](1)This method can correct varus deformity,improve function,as well as restore biomechanical axis of femur.(2)It is able to effectively eradicate lesions and prevent recurrence.(3)Impaction allograft is the key of prompting allograft incorporating fully and preventing pathological fracture.
2.A specific immune therapeutic effect of Der p2 T cell epitope vaccine on asthma mice

Objective To investigate the specific immune therapeutic effect of the T cell fusion peptide vaccine from group II allergens from Dermatophagoides pteronyssinus(Der p2). Methods Thirty mice were randomly divided into three groups, namely a negative control group(a PBS group),positive control group(an asthma group)and protein Der p2 T cell fusion epit?ope for specific immunotherapy(SIT)group(a Der p2 T group). The extract of house dust mites(HDM)was used to establish the asthmatic models in BALB/c mice,and the PBS group was always used with PBS buffer. Thirty minutes before spray inhala?tion from 25 to 27 days,the mice of the Der p2 T group were respectively injected subcutaneously with the therapeutic proteins for SIT,then the serum and bronchoalveolar lavage fluid(BALF)were collected. ELISA was used to assay the levels of IFN?γ, IL?4,and IL?13 in BALF,as well as serum levels of specific IgE and IgG2a. The lung tissue sections were stained with haematox?ylin and eosin(H&E)for pathological examinations. Results The ELISA detection revealed that the number of eosinophil in BALF of the asthmatic mice was(5.57 ± 0.64)× 105/ml,which was significantly higher than that in the PBS group[(0.50 ± 0.30)× 105/ml,P < 0.01],the number of eosinophil in the Der p2 T immunotherapy group decreased significantly[(3.45 ± 0.36)×105/ml,P<0.01]. The content of IFN?γin the PBS group,asthma group and Der p2 T group were(267.00 ± 21.98), (155.80 ± 20.53)pg/ml and(234.40 ± 24.46)pg/ml respectively. Compared with the asthma group,the mice with Der p2 T vac?cine specific immune treatment produced a high level of IFN?γ(P<0.01). The content of IL?4 in the PBS group,asthma group and Der p2 T group were(23.40 ± 5.96),(53.28 ± 8.26)pg/ml and(30.00 ± 5.50)pg/ml respectively. Compared with the asth?ma group,the content of IL?4 in the mice of the Der p2 T treatment group was significantly lower(P<0.01). Compared with the asthma group[(308.10 ± 28.32)pg/ml],the content of IL?13 in BALF of the mice in the Der p2 T treatment group was signifi?cantly decreased,which was[(174.50 ± 25.99)pg/ml,P<0.01]. The content of IL?13 in the PBS group was(95.99 ± 31.14) pg/ml. The lung tissue sections showed that the lung inflammation in the p2 T Der group was significantly less than that in the asthma group,and the inflammatory cell infiltration was significantly decreased,and airway epithelial construction remodeled. Conclusion The Der p2 T cell fusion epitope,which is as vaccines for specific immunotherapy with asthma models,can allevi?ate effectively allergic inflammation of airway and lung in the mice,and it may be used as a candidate vaccine for asthma.
3.Clinical and molecular characteristics of bloodstream infection caused by K. pneumonia

Objective To investigate the clinical features and molecular characteristics of bacteremia caused by K. pneumoniae and study on the differences between classical and hypervirulent K. pneumonia( hvKP) . Methods The clinical features of 70 cases of K. pneumoniae bacteremia collected from Beijing Chaoyang Hospital from 2008 to 2012 were retrospectively analyzed. The patients were identified according to the records from the Clinical Microbiology Department. Patients′ data were obtained from medical records. The molecular characteristics of the K. pneumoniae strains were also studied using PCR, multilocus sequence typing and pulsed field gel electrophoresis( PFGE) methods. Data were analyzed using the statistical package SPSS for windows version 17. 0. For categorical data, different groups were compared using the Chi-square test to analyze the quantitative variables. P≤0. 05 was considered to be statistically significant. All susceptibility data were analyzed using Whonet, version 5. 6. Results The hvKP was identified from 31. 4% of the patients with K. pneumoniae bacteremia, which displayed 4 serotypes ( K1, K2, K20, and K57). About 40. 9% (9/22) hypervirulent strains were isolated from patients without underlying diseases, while 95. 8% (46/48) of the classic K. pneumoniae infections were found in patients with the presence of one or more underlying diseases. More hvKP positive patients (95. 5%, 21/22) had community-acquired infections compared with classical K. pneumonia (cKP) infected patients (35. 4%, 17/48). There was a significant difference between these two groups (χ2 =21. 912,P <0. 001). Two ESBLs-producing hvKP strains were found. The 22 hvKP isolates had 14 different PFGE patterns, among which 6 isolates ( patients with primary liver abscess) shared similar PFGE patterns. Conclusions The
emerging hvKP was prevalent in patients with severe community-acquired infections in healthy individuals. Two ESBLs-producing hvKP strains were identified. Drug resistance had a rising trend, which called for more attention in clinical settings.
4.Experimental study of apoptosis in leukemic cell lines induced by combined use of Bevacizumab and chemotherapeutic drug

Objective To investigate the potential application of targeting at vascular endothelial growh factor (VEGF) induced apoptosis in leukemic cell lines by combined use of Bevacizumab and chemotherapeutic drug. Methods Leukemic cells were treated with several drugs at different concentrations in culture. The effect of VEGF, Bevacizumab and co-treated with Ara-C on leukemic cells proliferation were evaluated by CCK-8 and apoptosis and cell cycle were detected by flow cytometry (FCM). Results VEGF could enhance the proliferation of leukemic cells and caused a dose-dependent manner on U937 cell. It also increased the percentage of cells in S phase, tested by, and Bevacizumab group was decreased. Apoptotic rate of cells treated with Bevacizumab or co-treated with Bevacizumab and Ara-C for 48 h were significantly higher when compared with control or Ara-C group, respectively (P<0.05), but the apoptotic rate of VEGF group or VEGF and Ara-C group was lower (P>0.05). There was no significant difference in apoptotic rate between control and combined use of VEGF, Bevacizumab and Ara-C group(P>0.05). Conclusion VEGF could enhance the proliferation of some leukemic cells, and may contribute to leukemic cells survival and a resultant resistance to chemotherapy-triggered cell death. The study also showed that leukemic cells growth was significantly inhibited by Bevacizumab through directly against VEGF, and the sensitivity of leukemic cells for chemotherapeutic drug was increased.

Objective To find out technological evidence for the rapid propagation of Aloe vera L.var.chinensis (Haw.)Berber. Methods The technique of plant tissue culture was used to study the rapid propagation of Aloe vera L.Var.Chinesis(Haw.)Berger.The shoot growing from the underground stem of Aloe vera L.Var.chinensis(Haw.)Berger was taken as the explant to induce fascicular bud. Results The optimal culture medium was MT.MT culture medium added with 6-benzylaminopurine(BA) was effective to induce the shoot formation and increase the direct shooting rate.The optimal BA concentration for the induction and formation of shoot was 2 mg?L -1 ,the shooting rate being 100%.As for the induction of rooting,MT culture medium adding with 0.5 mg?L -1 naphthalene acid was optimal,the rooting rate being 100%. Conclusion The culture of Aloe shoot growing from the underground stem,proper culture medium and BA concentration are important for the rapid propagation of Aloe.
6.Three-dimensional finite element analysis and biomechanical study on reconstruction of the large defect of proximal femur with allograft prosthesis composite in clinical bone-healing phase.

Three-dimensional finite element models of the large defect of proximal femur were reconstructed with allograft prosthesis composite in clinical bone-healing phase; current model was under the given conditions of 138mm-intramedullary stem-length of host bone and 135mm defect-length of proximal femur. The femur was constructed with efilm software from CT data, then three-dimensional concrete models were created by using Proe-Wildfire software; the three-dimensional finite element models of allograft prosthesis composite were made in ANSYS11 software. Loads were simulated using the peaking values during stance walking. The stress on femur-cement-callus-prostheses and the influence of stress on the clinical bone-healing phase were analysed. The highest stress value of femur is on the medial side of the tip of the prostheses. The highest stress value of cement mantle is on the medical side of the cement mantle at the tip of the stem. The highest stress value of the prostheses is on the medial side near the upper 4cm of the stem tip. The highest stress value on the callus is at the medial side of the callus layer. The highest stress value on every part is under the corresponding fatigue strength. Clinical bone-healing phase model is well enough for stance walking.

7.Prenatal diagnosis of two families with megalencephalic leukoencephalopathy with subcortical cysts

Objectives To provide genetic counseling and prenatal molecular diagnosis for two families with megalencephalic leukoencephalopathy with subcortical cysts (MLC).Methods Two MLC patients (probands 1 and 2) were admitted to the Department of Pediatrics of Peking University First Hospital in June 2011 and June 2009,respectively.Peripheral blood was collected and DNA sequencing was performed for genetic analysis for the two MLC patients and their parents.Amniotic fluid and villus of two fetuses (fetus 1 and 2) were collected at 21+4 and 12+3 weeks of gestational age from their mothers when they were pregnant again.The genomic DNA of the two fetuses was extracted and corresponding sites of MLC1 gene were sequenced.Haplotype analysis using a combination of 3 microsatellite markers (AR,DXS6807 and DXS6797) on chromosome X and sex determining region of Y chromosome was performed to detect maternal cell contamination.Verification of the prenatal molecular diagnosis and follow up study after birth were conducted for both fetuses.Results Macrocephaly,motor development delay and typical findings on brain MRI were identified in the two probands,and were clinically diagnosed with MLC.Compound heterozygous mutations were detected in proband 1 [c.353C>T (p.T118M) and c.803C>G (p.T268R)] and proband 2 [c.353C>T (p.T118M) and c.836T>C(p.L279P)],respectively.MLC was genetically diagnosed.Heterozygous variation in c.353[c.353C>T (p.T118M)] and wild c.803C were identified in fetus 1,and both wild c.353C and c.836T were found in fetus 2.No maternal cell contamination was detected in both fetuses.Sequencing the corresponding sites after birth confirmed the prenatal diagnosis,and the head circumference and motor development were normal in fetus 1 at 5 months old.No macrocephaly was found and no DNA sequencing was done in fetus 2 at one month old.Conclusions Genetic counseling and prenatal molecular diagnosis for MLC families combined with clinical and genetic diagnosis are important in preventing MLC.Haplotype analysis with a combination of three microsatellite markers on chromosome X and sex determining region of Y chromosome is useful in detecting maternal cell contamination and avoiding its influence on prenatal diagnosis,and confirming the reliability of prenatal diagnosis.

9.Multimedia education form of congenital heart disease children of nursing research

Objective To observe the effect of multimedia education on congential heart disease children of nursing. Methods All 120 patients were divided into the control group (56 cases) and the multimedial group (64 cases)randomhy. In the multimedial group, nursing for postoperative children with congenital disease CD was used for guidance while the control group was guided by traditional oral health guidance, the appropriate nursing countermea-sures for all cases after guidance. The test score and the nursing incidence of postoperative complications were com-pared. Results Test score of multimedial group were significant higher than those in the control group after guidance(P<0.01), the nursing complications of mulitimedial group were significantly lower than those in the control group af-ter guidance (P<0.01). Conclusion Multimedia health education can significantly improve the patient's education ef-fect, improve nursing quality in patients with congenital heart disease and reduce complications.
10.Curative effect of ozone hydrotherapy for pemphigus

Objective:To determine clinical curative effects of ozone therapy for pemphigus vulgaris.Methods:Ozone hydrotherapy was used as an aid treatment for 32 patients with pemphigus vulgaris.The hydropathic compression of potassium permanganate solution for 34 patients with pemphigus vulgaris served as a control.The main treatment for both groups were glucocorticoids and immune inhibitors.The lesions of patients,bacterial infection,usage of antibiotics,patient's satisfaction,and clinical curative effect were evaluated in the 2 groups.Results:There was no significant difference in the curative effect and the average length of staying at hospital between the 2 groups (P>0.05).But rate for the usage of antibiotics was significantly reduced in the group of ozone hydrotherapy (P=0.039).The patients were more satisfied in using ozone hydrotherapy than the potassium permanganate solution after 7-day therapy (P>0.05).Conclusion:Ozone hydrotherapy is a safe and effective aid method for pemphigus vulgaris.It can reduce the usage of antibiotics.
