1.Observation on the efficacy of female obesity complicated with climacteric syndrome treated by acupuncture and moxibustion.

OBJECTIVETo observe the efficacy of acupuncture and moxibustion on female obesity complicated with climacteric syndrome and its impact factors.
METHODSOne hundred and fifty-one cases of female obesity complicated with climacteric syndrome were treated according to the basic principle as "pattern/syndrome differentiation". Accordingly, the reinforcing or reducing technique was applied at Quchi (LI 11), Zhongwan (CV 12), Tianshu (ST 25), Daheng (SP 15), Zusanli (ST 36), Shangjuxu (ST 37), Sanyinjiao (SP 6), Neiting (ST 44), etc. The auricular point sticking was applied at Hunger Point, Ear-shenmen, Sympathetic Nerve, Endocrine and Stomach. The treatment was given once every two days. The duration of treatment was 3 months. The changes in the symptoms, physical signs, obesity index, Kupperman index, the vegetative equilibrium index (Value Y), estradiol (E2) and follicle stimulating hormone (FSH) were observed before and after treatment.
RESULTSThe obesity patients complicated with climacteric syndrome presented unusually high level of obesity index, Kupperman index and FSH levels and unusually low level of E2. After treatment, the obesity index, Kupperman index and FSH levels were all reduced significantly (all P < 0.01) while E2 level was increased significantly (P < 0.05).
CONCLUSIONAcupuncture and moxibustion achieve the significant efficacy on obesity complicated with climacteric syndrome, which is closely related with the pattern/syndrome of the disorder and obesity grade. For the effective and curative rate of obesity index, the efficacy of liver qi stagnation pattern/syndrome is better than that at liver stagnation and spleen deficiency pattern/syndrome than spleen and kidney yang deficiency pattern/syndrome than liver and kidney yin deficiency pattern/syndrome than yin and yang deficiency pattern/syndrome; the efficacy of mild obesity is better than that at moderate obesity than severe obesity.

2.Overview of the two-way regulatory effect of acupuncture on gastrointestinal motility.

Relative literatures on the two-way regulatory eltect ot acupuncture on gastrointestInal muiiiity 01 inc recent 20 years were retrieVed in CNKI. Researches on effects of acupuncture on electrical activity of the gastric muscle, and mechanism of nerve and humor were summarized. And the result shows that the two-way regulatory effect of acupuncture on gastrointestinal motility function which eventually causes the recovery of gastrointestinal function, is a benign regulation reached by relative antagonism and inter-coordination of the neural, endocrine and immunity in all aspects. Meanwhile, the detailed conditions of the experiments are not strictly controlled, the intervention methods are not unanimous, the indices of observation are not unified, and the results are lack of scientificalness, regularity and corporeality. It is proposed that the future research should be carried out in the following 3 aspects, namely whether single points have the two-way regulatory effect or not; the application of single points in point prescriptions and the variety of quantity; and differences on the two-way regulatory effect and working processes between acupuncture and medication.

3.Mandibular distraction combined with orthognathic techniques for the correction of adult hemifacial microsomia.

OBJECTIVETo study the combination of Mandibular distraction and orthognathic techniques for the reconstruction of adult hemifacial microsomia.
METHODSThe three-dimensional CT reconstruction data was used with Mimics for preoperation design. The osteotomy location, distraction vector, distraction distance were decided before operation with a surgical guider. At the first stage, internal distractor was implanted after ostetomy through an extra-oral approach. The distraction begun 5-7 days after operation with a frequency of 1 mm/day. After distraction, the distractor was maintained for 3-6 months. At the second stage, the distractor was removed. Le Fort I osteotomy was performed in order to correct the cross-bite and improve the facial contour. Usually, bone graft was inserted into the gap after Le Fort I osteotomy. The genioplasty was also performed if necessary.
RESULTS9 cases of adult hemifacial microsomia with severe mandibular deviation were treated. The facial asymmetry were improved greatly. 1 patient suffered an wound infection in the maxillary region after Le Fort I osteotomy and healed uneventfully with wound irrigation.
CONCLUSIONSMandibular distraction combined with orthognathic surgery is an effective procedure for adult hemifacial microsomia with complicated mandibular hypoplasia.

4.Systematic evaluation of the effects of resistance training on the treatment and prevention of sarcopenia in the elderly

Objective:To evaluate the effects of different types of resistance training on the treatment and prevention of sarcopenia in the elderly.Methods:A computer search was conducted in seven databases including PubMed, Embase, Web of Science, Cochrane Library, CNKI, VIP, and Wanfang, to identify randomized controlled trials published from the inception of the databases to March 2023, for the assessment of the preventive effects of resistance training on sarcopenia in older adults.Two reviewers independently performed the literature search, selection, and quality assessment of the included studies.A Meta-analysis was conducted using RevMan 5.3.Results:A total of 12 studies with 1018 participants were included.Compared with the control group, resistance training significantly improved the skeletal muscle mass index( MD=0.34, 95% CI: 0.24~0.44, P<0.01), grip strength( MD=1.63, 95% CI: 0.73~2.53, P<0.01), and 5 times sit-to-stand test performance( MD=-1.99, 95% CI: -2.48--1.51, P<0.01).However, there was no significant improvement observed in muscle mass( MD=1.28, 95% CI: -0.38-2.94, P>0.05), knee extensor strength( SMD=1.71, 95% CI: -0.51-3.93, P>0.05), or gait speed( MD=0.03, 95% CI: -0.03-0.08, P>0.05). Conclusions:Resistance training improves the skeletal muscle mass index, grip strength and the time taken to perform the 5 times sit-to-stand test in the elderly, thus delaying degenerative skeletal muscle changes in older adults with sarcopenia and offering a preventive effect on sarcopenia in healthy senior adults.
5.Diagnosis and treatment of sellar germinoma.

That eleven patients with sellar germinomas were confirmed by pathological examinations were retrospectively reviewed. In the 11 patients, 2 were males and 9 were females. Mean age was 16.8 years old and mean history was 2.6 years. Among them, 11 patients had diabetes insipidus, 6 patients had visual disturbances, 5 patients had retarded growth, and 5 patients had hypopituitary function. The diameter of tumor was 1-3 cm in 8 cases and greater than 3 cm in 3 cases. MRI showed hypointensity or homogeneous on T1-weighted images in 8 cases. The tumors were fibrous, purple grey or reddish in color with rich blood supply. Germinomas were confirmed by postoperative pathological examinations. All patients received postoperative irradiation. Postoperative follow-up was from 3 months to 6 years. Diabetes insipidus was improved in 9 cases. Visual function were improved in 4 cases, but hypopituitarism and retarded growth were unchanged. In conclusion, when MRI shows sellar parenchyma tumor with unclear boundary in young patients with diabetes insipidus and hypopituitarism, sellar germinoma should be highly suspected. Otherwise, radiation therapy may profitably aid to confirm the diagnosis since irradiation is the main treatment of sellar germinomas.

6.Relationship between invasive pituitary adenomas and cavernus sinus and cariod artery and surgical outcomes.

OBJECTIVETo analyze the relationship between invasive pituitary adenomas and cavernus sinus and cariod artery and to predict their surgical outcomes.
METHODSTotally 270 patients with pituitary tumors were investigated in this retrospective study, including 113 men and 157 women, with a mean age of 40.8 years. The mean disease history was 3.6 years. Pituitary microadenomas were diagnosed in 56 cases, macroadenomas in 118 cases, and hugeadenomas in 96 cases. Adrenocorticotropic hormone-releasing adenomas (Cushing's diseases) were diagnosed in 40 cases, growth hormone-releasing adenomas in 58 cases, prolactinomas in 57 cases, and non-functional pituitary adenomas in 115 cases. Transsphenoidal microsurgery were performed on 260 patients, while transcranial microsurgery on 6 patients.
RESULTSThe percentage of invasive pituitary adenomas was about 3.6% in microadenomas, 20.4% in macroadenomas, and 61.4% in hugeadenomas. Rate of total removal was 94.1% in non-invasive pituitary adenomas, and was 58.8% in invasive pituitary adenomas.
CONCLUSIONSIt is important to analyze the grade of invasive pituitary adenomas to improve the removal of pituitary adenomas, avoid injuring cariod artery, and increase the rate of total removal.

7.An investigation of iodine nutrition and thyroid function in downtown residents of Longyan city, Fujian province

Objective To find out the iodine nutritional status of Longyan downtown residents,evaluate the effectiveness of control measures and provide a scientific basis for developing control strategies.Methods Infants aged 0 to 2 year-old,children aged 8 to 10,adults aged 18 to 45 and pregnant and lactating women were selected as survey subjects.Children goiter was detected with B ultrasound.Residents per capita daily salt intake was investigated by weighing method.Three urinary samples and a milk sample of lactating women were randomly collected.Urinary iodine and milk iodine content were determined by arsenic cerium catalytic spectrophotometric assay.Blood samples were collected and thyroid function (including serum TT3,FT3,TT4 and FT4) and thyroid stimulating hormone(TSH) were measured with direct chemiluminescence immunoassay,and thyroglobulin antibody(TgAb),thyromicrosome antibody(TMAb),and thyroglobulin (Tg) were measured with radioimmunoassay (RIA) in serum.ResultsThe goiter rate of children aged 8 to 10 was 1.8% (2/110),and median thyroid volume was 2.75 ml.Household iodized salt coverage rate was 100.00%(318/318),and qualified iodized salt was 94.03% (299/318).The daily per capita salt intake was (6.13 ± 3.56)g.The average medians of urinary iodine of the infants,children,adults,pregnant and lactating women were 181.8,315.2,196.6,158.7,136.4 μg/L,respectively.The median of milk iodine of lactating women was 155.6 μg/L.The proportions of serum TT3,FT3,TT4,FT4 and TSH which higher than normal were 3.6% (11/308),0.6% (2/309),23% (7/309),1.0% (3/313) and 1.3% (4/312),respectively.While the proportions of serum TT3,FT3,TT4,FT4 and TSH that lower than normal were 2.3% (7/308),11.7%(36/309),2.3%(7/309),12.8%(40/313),and 1.6%(5/312),respectively,of which 16 cases of both TgAb and TMAb were higher than normal.ConclusionsExisting salt iodine level is appropriate for 0 to 2 year-old infants and young children,18 to 45 year-old adults,pregnant and lactating women in downtown Longyan city.The iodine intake of children aged 8 to 10 is excessive.Thyroid function monitoring is recommended to be included in the routine monitoring.
8.Transsphenoidal microsurgical results of non-invasive prolactinomas.

OBJECTIVETo analyze the transsphenoidal microsurgical results of non-invasive prolactinomas, in order to provide reference for their treatment choice.
METHODSTo review the transsphenoidal microsurgical results of 234 non-invasive prolactinomas treated in our department in recent 10 years, and to analyze the prognostic factors. There were 18 males and 216 females, aged between 13 and 58 years, averaged (31.1 +/- 8.5) years. The course ranged from half a month to 20 years, averaged (47.3 +/- 44.9) months. The preoperative serum PRL level ranged between 41 and 8406 ng/ml, averaged (400.5 +/- 888.0) ng/ml, with a median of 164.1 ng/ml. The primary symptoms were amenorrhea, galactorrhea and/or infertility in 211 cases. The tumor size was small (< 1 cm) in 100, large (> or = 1 cm) in 116 and giant (> or = 3 cm) in 18 cases. All the patients received transsphenoidal microsurgery and were followed-up for 12 to 132 months, averaged (43.8 +/- 35.0) months.
RESULTSThere was no mortality. One hundred and twenty-seven (54.3%) cases had transient postoperative imbalance of water and electrolytes. One hundred and eighty-eight cases (80.3%) were cured, 12 (5.1%) experienced clinical remission, 20 (8.5%) were improved, and 14 (6.0%) were ineffective. The male patient, the giant prolactinomas and those with higher preoperative serum PRL level had a relative poor postoperative prognosis. While the other factors had no influence on prognosis, including the course, preoperative bromocriptine intake, tumor texture, tumor apoplexy and intraoperative descending extent of the diaphragm of sella. The overall operative expense for transsphenoidal microsurgery ranged from 8323.8 to 22898.5 yuan, averaged (12912.0 +/- 2361.2) yuan.
CONCLUSIONSTranssphenoidal microsurgery may be chosen as the primary therapy for non-invasive prolactinomas, with the purposes of therapeutical efficacy, facilitating the patients, re-establishing the patients' self-confidence and reducing the overall expense.

9.Relationship between tissue type plasminogen activator and coronary vulnerable plaque in patients with acute coronary syndrome: virtual histological study.

BACKGROUNDThe association between vulnerability of plaque assessed with intravascular ultrasound (IVUS) and plasma levels of fibrinolytic biomarkers was determined in patients with acute coronary syndrome (ACS). However, few data are available on the relationship between the levels of tissue type plasminogen activator (t-PA) and virtual histological intravascular ultrasound (VH-IVUS) signs of plaque instability.
METHODSEighty-nine patients with ACS were enrolled in the study. Blood was collected to measure t-PA levels by liquid phase bead flow cytometry. Eighty-nine nonbifurcate lesions (identified by coronary angiography and ECG) were investigated using IVUS before catheterization. IVUS radiofrequency data obtained with a 20 MHz catheter were analyzed with IVUS virtual histological software. The areas of plaque and media were calculated and lesions were classified into two groups: VH-IVUS derived thin cap fibroatheroma (VH-TCFA) and non-VH-TCFA plaque.
RESULTSPlasma t-PA level in the patients with TCFA was significantly lower than that with non-TCFA ((1489+/-715) pg/ml vs (2163+/-1004) pg/ml). Decreased plasma levels of t-PA were associated with plaque vulnerability. Plasma levels of t-PA correlated negatively with plaque plus media and necrotic core in plaque in patients with ACS.
CONCLUSIONSt-PA is an independent risk factor and a powerful predictor of vulnerable plaques. Decreased levels of t-PA may reflect instability of atherosclerotic plaques and might therefore serve as noninvasive determinants of those at high risk for consequent adverse events.

10.Identification of a novel candidate gene KLRC1 within the putative susceptibility locus for systemic lupus erythematosus at 12p12.3-13.2 in a Chinese cohort.

OBJECTIVETo identify the candidate genes within the putative susceptibility locus for systemic lupus erythematosus (SLE) at 12p12.3-13.2.
METHODSKLRC1 was selected as the candidate gene according to the results of previous gene chip studies. TaqMan real-time quantitative PCR was performed for detecting KLRC1 mRNA expression in 55 SLE patients and 30 controls.
RESULTS AND CONCLUSIONKLRC1 mRNA expression was significantly higher in the mononuclear cells and T cells of SLE patients than in the healthy controls (P<0.01), but showed no significant difference in the B cells. No obvious correlation was found between the SLE disease activity index (SLEDAI) and KLRC1expression level, suggesting that KLRC1 can be a probable candidate gene for SLE on 12p12.3-13.2, but which is not associated with the disease activity.
