1.Investigation on epidemical factors of Toxoplasma gondii infection

Objective To understand the relationship between Toxoplasma gondii infection and pet-breeding and hygienic status. Methods One or two districts were chosen from Jining City, Za-ozhuang City and Linyi City as study areas and the questionnaire surveys were carried out for hygienic status, etc. The pet-breeders and non-pet-breeders were paired by sex, age, living place and occupation so on in a ratio of one to one. Blood samples taken from each were tested for antibodies against T. gondii with ELISA in a double-blind method. Results The total infection rate was 14. 25%, much higher than that investigated ten years ago (1. 53%). Objective To observe the inhibitation and radiosensitization effect of hematoporphyrin and radiotherapy on H22 liver cancer mice.Methods Mice models of liver cancer were established by subcutaneous injection of H22 cells,tumor-bearing mice were randomly divided into:control group, simple radiotherapy group, small dose hematoporphyrin with radiotherapy group (combined treatment group Ⅰ), middle dose hematoporphyrin with radiotherapy group(combined treatment group Ⅱ),and high dose hematoporphyrin with radiotherapy group (combined treatment group Ⅲ).Tumor volumes,inhibition rate,sensitizing factor,the survival time of mice,pathology,apoptosis and other indicators in five groups were observed after14 days. Results Hematoporphyrin and radiotherapy can inhibit tumor growth in mice,The q values in three combined treatment groups were all above 1.0 at different time points ,and combined treatment groupⅢwas the highest.The three combined treatment groups had higher apoptosis rate than control group and simple radiotherapy group,but the difference within groups were not statistically significant. The survival time in three combined treatment groups were higher than control group,the combined treatment group Ⅱ was the longest and simple radiotherapy group was the shortest.Conclusion Hematoporphyrin has tumor inhibitition and radiation sensitizing effect on H22 liver cancer mice,and the effect was positive correlation with drug dose. Hematoporphyrin combined with radiotherapy can extend the lifespan of mice in H22 liver cancer,its sensitizing mechanism may be association with induction of H22 cell apoptosis.

5.Old femoral neck fractures with senile osteoporosis: choice of femoral prosthesis according to femoral calcar-medullary cavity ratio

BACKGROUND:Old femoral neck fractures with senile osteoporosis have more surgical difficulties.There are a lot of intraoperative and postoperative complications and long-term effects are poor.OBJECTIVE:To explore the clinical outcomes of hip arthroplasty in patients with old femoral neck fractures with senile osteoporosis.METHODS:From October 2012 to July 2014,20 patients with old femoral neck fractures and senile osteoporosis were enroled in this study.Posterolateral approach was used to perform hip arthroplasty.Of them,14 patients received primary total hip arthroplasty with biological prosthesis,and 6 patients were subjected to semi-hip arthroplasty with straight shank cylindrical renovated biological prosthesis.RESULTS AND CONCLUSION: The patients were folowed up for 3 to 24 months.Harris score was apparently increased after arthroplasty in patients with old femoral neck fractures and senile osteoporosis compared with pre-arthroplasty.Postoperative radiographs revealed that femoral stem biological fixation was good.Bone fixation was visible in radiographs at 3 months after arthroplasty.These findings suggested that old femoral neck fractures and senile osteoporosis could be treated with hip arthroplasty.To select the type of prosthesis and surgical methods according to the femoral calcar-medulary cavity ratio in patients with old femoral neck fractures and senile osteoporosis can effectively restore the function of hip joint.
6.Enhancement MRI evaluation of neuroblastoma staging in children

Objective To evaluate the value and limitation of Gd-DTPA enhanced MRI for neuroblastoma staging in children. Methods Twelve cases of neuroblastoma proved by operation or bone marrow aspiration were examined by gadolinium-enhanced MRI. The age ranged from seven months to five years, mean 3.7 years. Eight tumors originated from adrenal, and four from posterior mediastinum. Conventional sequences, double dose gadolinium-enhanced MRI, and 3D CEMRA were used in all patients. Six cases were examined by CT in same time. Imaging staging on surgic-histopathological-based International Neuroblastoma Staging System (INSS) was performed. Results Six patients were staged by CT, including stage Ⅰ-Ⅱ in 2 cases, stage Ⅲ in 4 cases, and stage Ⅳ in none. Twelve patients were staged by conventional MRI, including stage Ⅰ-Ⅱ in 2 cases, stage Ⅲ in 9 cases, and stage Ⅳ in 1 case. Twelve patients were staged by double dose gadolinium-enhanced MRI, including stage Ⅰ-Ⅱ in 1 case, stage Ⅲ in 1 case, and stage Ⅳ in 10 cases. Conclusion Gadolinium-enhanced MRI was a single best imaging modality for neuroblastoma, most useful for distal to diaphragm metastasis, dumbbell tumor intraspinal extension, and bone marrow metastasis that was not detected by aspirate examination. Enhancement MRI was important in evaluating the therapy and was also helpful in assessing the therapeutic efficacy and relapse. 3D CEMRA helps demonstrate large vascular encasement and tumor erosion into important organs, and it is useful in assessing the respectability. Long examination time and lack in showing the characteristic calcium were the limitations.
7.Effect of fascia parotideomasseterica reconstruction in the prevention of Frey syndrome

Objective To study the effect of fascia parotideomasseterica reconstruction in the prevention of Frey syndrome.Methods 45 patients who suffered parotidectomy with parotid fascia maintained and reconstructed were followed-up,the OccurrenCe rate of Frey syndrome wasobserved.Results The occurrence rate of the subjective symptom8 with Frey syndrome Was 6.7%(3/45)6 montes after surgery;the occurrence rate of Frey syndrome was 11.1%(5/45)by Minor test review 1 year after surgery;the remaining 41 cases were non-induced reactlons.Condusion Fascia parotidcomassetefica could reduce the development of Frey syndrome in the parotid gland surgery in pafients with benign tumors.
8.Minimally Invasive Perventricular Device Closure of Ventricular Septal Defect:a Comparative Study in 80 Patients

Objective To evaluate the efficacy of minimally invasive perventricular device closure of ventricular septal defect (VSD).
Methods Between September 2011 and February 2013, we collected 40 patients who underwent perventricular closure via a small lower sternal incision (minimally invasive group), aged 15.5±3.5 years (12 months to 32 years) with a body weight of 24.2±7.5 kg (10.8-58.0 kg). The mean size of VSD was 5.6±0.5 mm (2-14 mm). Another 40 patients were included as the surgical group, receiving the conventional surgical repair of VSD. The device of the minimally invasive group was released under the guidance of transesophageal echocardiography. Success rate, cardiac indicators, and clinical outcomes of the 2 groups were compared.
Results The patients in the surgical group and those in the minimally invasive group showed similar results in success rate (both 97.5%). The procedure time, intensive care unit stay, hospital stay, and postoperative recovery time in the minimally invasive group were significantly shorter than those in the surgical group (58±21 minutes versus 145±26 minutes, 2±1 days versus 8±3 days, 5±1 days versus 16±6 days, 3±1 days versus 90±20 days, all P<0.05). The minimally invasive group had a higher incidence of conduction anomalies (17.5%versus 2.5%, P<0.05). In the follow-up period of 3-12 months, there was no new residual shunt, noticeable aortic regurgitation, significant arrhythmias, or device failure except for new complications in the surgical group.
Conclusions The success rate of minimally invasive perventricular device closure of VSD under transesophageal echocardiography guidance is similar to that of conventional surgical repair, but the short-term outcomes of the minimally invasive approach is much better. Long-term follow-up is necessary to confirm the effectiveness of this technique.
10.Effect of Taohong Qinlian Decoction on HMGB1 in Septic Rat Cardiac Muscle.

OBJECTIVETo observe the levels of high mobility group box-1 protein (HMGB1), tumor necrosis factor-alpha (TNF-α), IL-6, troponin I (Tn I) release in septic rats, and to explore themechanism of Taohong Qinlian Decoction (TQD) in the treatment of septic myocardial injury.
METHODSA total of 48 healthy male Wistar rats of clean grade were randomly divided into the sham-operation group (Sham), the sepsis model group (CLP), and the TQD treatment group (ZY), 16 in each group. Concen-trations of TNF-α, IL-6, Tn I, and HMGB1 expression were detected in each group at 24 and 48 h after operation. Pathological changes of cardiac muscle were observed under light microscope.
RESULTSConcentrations of TNF-α, IL-6, Tn I and HMGB1 at 24 and 48 h after operation were significantly higher in the CLP group than in the Sham group (P < 0.01). Concentrations of TNF-α, IL-6, Tn I, and HMGB1 at 24 and 48 h after operation were significantly lower in the ZY group than in the CLP group (P < 0.05). Myocardial injury occurred in the CLP and the ZY group under light microscope. And this injury was more severe in the CLP group than in the ZY group.
CONCLUSIONTQL could reduce the level of sepsis-related inflammatory cytokines and protect myocardium in septic rats.
