1.Treatment of fresh subtrochanteric fracture combined with old femoral neck fracture with hemiarthoplasty through anterolateral approach.

OBJECTIVETo explore the clinical effects of one-stage hemiarthroplasty in treating fresh subtrochanteric fracture combined with old femoral neck fracture.
METHODSFrom October 2003 to October 2013, 7 patients with fresh subtrochanteric fracture and old femoral neck fracture were treated with hemiarthroplasty in one stage. There were 5 males and 2 females, aged from 69 to 80 years old with an average of 74.5 years. According to the Garden classification, 2 cases were type IV and 5 cases were type V. According to the A0 classification of subtrochanteric fracture, all cases were type A1 (long oblique femoral subtrochanteric fractures). Biological coating long handle femoral prosthesis was used in the patients. The hip joint functions were assessed according to Charnley scoring system after all hemiarthroplasty.
RESULTSSeven patients were followed up from 12 to 24 months with an average of 18 months. There were 1 case with urinary system infection and 1 case with deep venous thrombosis. Of all, 5 cases recovered the walk ability at 6 months after operation and 2 cases could walk with the help of the crutch at 1 year after operation. According to Charnley scoring system to assess the hip joint function, the total Charnley scores reached 15.0±2.5, and 4 cases got an excellent results, 2 good, 1 fair.
CONCLUSIONArthroplasty through anterolateral approach can obtain satisfactory clinical effects in treating fresh subtrochanteric fracture combined with old femoral neck fracture, it has advantages of good joint stability, little pain, less complication, quickly recover, early activities on fields.

4.Biomechanical characteristics of external fixation for tibial plateau fractures

BACKGROUND:Scholars have studied the three-dimensional finite element models of tibial plateau fractures with different fixation methods, but the research on three-dimensional finite element model of tibial plateau fracture fixation was not much. OBJECTIVE:To analyze the biomechanical characteristics of external fixator for tibial plateau fracture by using the three-dimensional finite element method. METHODS:The tibial plateau fracture models treated with locking plate fixation and external fixator were established. Three-dimensional finite element analysis was used to analyze the stress distribution and displacement of two kinds of models. RESULTS AND CONCLUSION:(1) The average displacement value and the maximum displacement value of tibial plateau fracture external fixator model group were smal er than that of tibial plateau fracture locking plate fixation model group, but no significant difference was found (P>0.05). (2) X axial displacement value and Y axial displacement value were smal er in the tibial plateau fracture external fixator model group than in the tibial plateau fracture locking plate fixation model group, but no significant difference was found (P>0.05). (3) The stress of tibial plateau fracture locking plate fixation model focused on the junction of the screw and bone tissue as wel as the junction of plates and screws. The maximum stress value of tibial plateau fracture locking plate fixation model (173 MPa) was greater than that of tibial plateau fracture external fixator model (86 MPa). The stress of tibial plateau fracture external fixator model was scattered. Obvious stress concentration did not occur. The stress at the junction of the clamp and the nail and the junction of the connecting rod and screw was higher than that at other regions, and was a weak link in the clinical mechanics. (4) Results showed that external fixation for tibial plateau fractures had better stability and fixing strength as compared with the internal fixation.

8.The deacclimatization symptom scores of 159 youth returning to the same plain from different plateau.

OBJECTIVETo analysis deacclimatization symptom scores of 159 youth from Karakoram and Tibet Ali area, and provide the basis for the development of relevant prevention and control measures.
METHODSUsing the method of epidemiological symptoms questionnaire, 18 symptoms of 190 youth who returned to the plain area from the different plateau were investigated. The symptom scores of different altitude, age, the time of staying, different units, continuous or intermittent stage and education were surveyed.
RESULTSDeacclimatization symptom scores among 5,000 meter groups were significantly higher than those of 4,300 meter and 3,700 meter group (P < 0.05, P < 0.01). There was no significant difference between the 4,300 meter group and the 3,700 meter group (P > 0.05). There were significant differences among the stayed personnel (different age, position, unit, education, time, continuous or intermittent) (P < 0.01). There was significant difference between the continuous defended the group and intermittent group (P < 0.01).
CONCLUSIONDeacclimatization symptom scores were related to the plateau exposure time, altitude, workload, plateau continued exposure. The older, the longer exposure, the higher altitude, the greater workload at plateau were showed higher deacclimatization symptom score.

9.Mice islet allograft tolerance induced by blockade of OX40/OX40L and CD40/CD154 costimulation pathway

Objective To investigate the effects of blockade of OX40/OX40L costimulation pathway on mice islet allograft tolerance in CD40/CD154 costimulation pathway blockade mice.Methods C57BL/6 mice were induced into diabetes mellitus as recipients,and were transplanted with DBA/2 mice islets.The recipients were divided into four groups,(1) treated with IgG as controls,(2) anti-OX40L mAb,(3) anti-CD154,(4) combined treatment of anti-OX40L mAb and anti CD154mAb.The mean survival time (MST) of islet allograft was observed.The expression of OX40 in activated T cells of CD154 deficient mice was detected.Effector T cells were obtained from the spleen of CD154 deficient mice cultured with or without anti-OX40L mAb for 3 days.The proliferation of T cells was assayed.Results The MST in the control group,anti-OX40L mAb group,anti-CD154 mAb group and anti OX40L mAb + anti-CD154 mAb group was 19,22,48,and >150 days respectively (P <0.05).The OX40 expression was readily induced in the 66% activated T effector cells.CD154 deficient T effector cells proliferation was inhibited by the addition of anti-OX40L mAb in the culture in a dose-dependent fashion.Conclusion The blockade of OX40/OX40L costimulation pathway can promote islet allograft tolerance in CD40/CD154 costimulation pathway blockade mice by inhibiting the proliferation of T cells.
10.Experimental study on cross-resistance of Culex pipiens pallens to 3 kinds of chemical pesticides

Objective To understand the cross-resistance of Culex pipiens pallens to common pesticides,so as to provide the evidence for improving the application of chemical pesticides. Methods The IV instar larvae of DDVP-resistant,propoxur-resistant and cypermethrin-resistant strains as well as the sensitive strain of Culex pipiens pallens were collected to detect the re-sistance to DDVP,propoxur and cypermethrin based on the WHO bioassay method. Results The resistance coefficients of DDVP-resistant strain to DDVP,propoxur and cypermethrin were 14.47,8.96 and 207.27 respectively. The resistance coeffi-cients of propoxur-resistant strain to DDVP,propoxur and cypermethrin were 3.27,6.93 and 8.65 respectively. The resistance coefficients of cypermethrin-resistant strain to DDVP,propoxur and cypermethrin were 2.93,1.61 and 501.11 respectively. Con-clusion The resistance and cross-resistance could be generated during the long-term application of a single kind of chemical pesticide,and we should pay more attention to the varieties and dosages of them.
